The choledochal cyst(CC)can be better termed as biliary tract malformation because of the close association of embryology and etiology in the causation of *** to Babbitt's postulation of reflux,damage and dilatation,reflux was not demonstrable as the causative factor in all varieties of *** pressure in the biliary system,otherwise termed ductal hypertension,is put forth as an alternative to explain the evolution of *** forme fruste type,which does not find a place in the standard classification,typifies the ductal hypertension *** a closer,in-depth review would be able to highlight this apt terminology of biliary tract malformation.

Schwannoma is a tumor derived from Schwann cells which usually arises in the upper extremities, trunk, head and neck, retroperitoneum, mediastinum, pelvis, and peritoneum. However, it can arise in the gastro-intestinal tract, including biliary tract. We present a 24-year-old male patient with obstructive jaundice, whose investigation with computed tomography abdomen showed focal wall thickening in the common hepatic duct, difficult to differentiate with hilar adenocarcinoma. He was diagnosed intraoperatively schwannoma of common bile duct and treated with local resection. The patient recovered well without signs of recurrence of the lesion after 12 mo. We also reviewed the common bile duct schwannoma related in the literature and evaluated the difficulty in pre and intraoperative differential diagnosis with adenocarcinoma hilar. Resection is the treatment of choice for such cases and the tumordid not recur in any of the resected cases.

Value of T-tube in biliary tract reconstruction during orthotopic liver transplantation:a meta-analysis

Objective:To compare biliary complications after biliary tract reconstruction with or without T-tube in orthotopic liver ***:Randomized control trials(RCTs) and comparative studies were identified by a computerized literature search of the Cochrane Library,MEDLINE(1966/1-2010/4),Scopus(1980/1-2010/4),***(2010/4),the Cochrane Hepato-biliary Group Controlled Trials Register,and the Cochrane Central Register of Controlled *** and data were extracted and assessed *** outcomes were reported as odds ratios(ORs) and weighted mean difference with 95% confidence intervals(CI).Results:Five RCTs and eight comparative studies with a total of 1 608 subjects were *** data showed that the operation with T-tube had better outcomes for duct stenosis(P=0.01,OR=0.45,95% CI 0.24-0.85).The operations with or without T-tube had equivalent outcomes as follows:overall biliary complications(P=0.85,OR=1.15,95% CI 0.28-4.72),bile leaks(P=0.38,OR=0.75,95% CI 0.39-1.42),and cholangitis(P=0.24,OR=4.64,95% CI 0.36-60.62).These results were strengthened by the analysis of all thirteen non-randomized and randomized ***:Our systematic review and meta-analysis suggest that the insertion of a T-tube reduces the incidence of biliary stenosis without increasing the incidence of other biliary complications.

In this study 12 Japanese long ear rabbits were used as models of acute cholangitis of severe type (ACST), and also an increasing pressure apparatus of selfmade caecus to form high pressure of the biliary tract. The animals were observed for changes in blood dynamics in an attempt to explore the effect and relation of high pressure of biliary tract and infective element in pathogenesis of ACST. It was found that when the biliary pressure was increased within 120 min in the 20 kpa group,the blood endotoxin level showed no obvious increment (P>0.05),but the decreased range of average MAP (mean artery pressure) was over 4 kpa,and the cardiac output also decreased evidently (Pbiliary pressure was decreased,MAP and cardiac output were restored to normal gradually. Of these animals 3 didn't restore their normal condition when the blood pressure decreased to zero and died *** the electric discharge frequency of the right greater splanchnic nerves increased (Pbiliary tract pressure is the important factor leading to a decrease in blood pressure of ACST,and even bringing about irreversible shock , which is involved in the activity of splanchnic nerves.

Diffusion-weighted magnetic resonance imaging(DWI)is a well established method for the evaluation of intracranial diseases,such as acute *** for extracranial application is more difficult due to physiological motion artifacts and the heterogeneous composition of the ***,thanks to the newer technical development of DWI,DWI has become increasingly used over the past few years in extracranial organs including the abdomen and *** previous studies of DWI have been limited to the evaluation of diffuse parenchymal abnormalities and focal lesions in abdominal organs,whereas there are few studies about DWI for the evaluation of the biliopancreatic *** further studies are needed to determine its performance in evaluating bile duct,gallbladder and pancreas diseases,DWI has potential in the assessment of the functional information on the biliopancreatic tract concerning the status of tissue cellularity,because increased cellularity is associated with impeded diffusion,as indicated by a reduction in the apparent diffusion *** detection of malignant lesions and their differentiation from benign tumor-like lesions in the biliopancreatic tract could be improved using DWI in conjunction with findings obtained with conventional magnetic resonance ***,DWI can be useful for the assessment of the biliopancreatic tract in patients with renal impairment because contrast-enhanced computed tomography or magnetic resonance scans should be avoided in these patients.

Schwannoma is a myelin sheath tumor complicated with neurofibroma, neurofibromatosis and neurogenic sarcoma. Peripheral nerve sheath tumors represent 2%-6% of gastrointestinal tract stromal tumors (GIST), but there are deficient data about location of neurogenic tumors in the biliary system and only nine cases of schwannoma of the extrahepatic biliary tract have been reported. These tumors are clinically non-specific. They are usually symptomatic by compressing the close or adjacent structures when being retroperitoneal, and their preoperative diagnosis is extremely difficult. This paper reviews the literature data and describes a case of schwannoma of the common bile duct associated with cholestasis in a healthy young woman, diagnosed and treated in our department. This case is of interest on account of the complexity of its diagnosis and the atypical macroscopic growth pattern of the tumor.

Placement of percutaneous transhepatic biliary stent using a silicone drain with channels

This report describes a method for percutaneous transhepatic biliary stenting with a BLAKE Silicone Drain, and discusses the usefulness of placement of the drain connected to a J-VAC Suction Reservoir for the treatment of stenotic hepaticojejunostomy. Percutaneous transhepatic biliary drainage was performed under ultrasonographic guidance in a patient with stenotic hepaticojejunostomy after hepatectomy for hepatic hilum malignancy. The technique used was as follows. After dilatation of the drainage root, an 11-FF tube with several side holes was passed through the stenosis of the hepaticojejunostomy. A 10-Fr BLAKE Silicone Drain is flexible, which precludes onestep insertion. One week after insertion of the 11-Fr tube, a 0.035-inch guidewire was inserted into the tube. After removal of the 11-Fr tube, the guidewire was put into the channel of a 10-Fr BLAKE Silicone Drain. The drain was inserted into the jejunal limb through the intrahepatic bile duct and was connected to a J-VAC Suction Reservoir. Low-pressure continued suction was applied. Patients can be discharged after insertion of the 10-Fr BLAKE Silicone Drain connected to the J-VAC Suction Reservoir. Placement of a percutaneous transhepaUc biliary stent using a 10-Fr BLAKE Silicone Drain connected to a J-VAC Suction Reservoir is useful for the treatment of stenotic hepaticojejunostomy.

Biloma is an encapsulated bile collection outside the biliary tree due to a bile leak. It is occasionally found following traumatic liver injury or iatrogenic injury to the biliary tract, induced either during an endoscopic or surgical procedure. It is a rare complication of transcatheter arterial embolization (TAE). Although biloma can be shrunk by appropriate aspiration or drainage in majority of cases,we report a case of intrahepatic biloma following repeated TAE for hepatocellular carcinoma (HCC) and complicated by infection and intrahepatic stones. This particular constellation of problems has not been reported before and the intrahepatic stones need to be removed by percutaneous procedure.
