Background The superiority of balloon angioplasty plus aggressive medical management(AMM)to AMM alone for symptomatic intracranial artery stenosis(sICAS)on efficacy and safety profiles still lacks evidence from randomised controlled trials(RCTs).Aim To demonstrate the design of an RCT on balloon angioplasty plus AMM for sICAS.Design balloon Angioplasty for Symptomatic Intracranial Artery Stenosis(BASIS)trial is a multicentre,prospective,randomised,open-label,blinded end-point trial to investigate whether balloon angioplasty plus AMM could improve clinical outcome compared with AMM alone in patients with sICAS.Patients eligible in BASIS were 35–80 years old,with a recent transient ischaemic attack within the past 90 days or ischaemic stroke between 14 days and 90 days prior to enrolment due to severe atherosclerotic stenosis(70%–99%)of a major intracranial artery.The eligible patients were randomly assigned to receive balloon angioplasty plus AMM or AMM alone at a 1:1 ratio.Both groups will receive identical AMM,including standard dual antiplatelet therapy for 90 days followed by long-term single antiplatelet therapy,intensive risk factor management and life-style modification.All participants will be followed up for 3years.Study outcomes Stroke or death in the next 30 days after enrolment or after balloon angioplasty procedure of the qualifying lesion during follow-up,or any ischaemic stroke or revascularisation from the qualifying artery after 30 days but before 12 months of enrolment,is the primary outcome.Discussion BASIS trail is the first RCT to compare the efficacy and safety of balloon angioplasty plus AMM to AMM alone in sICAS patients,which may provide an alternative perspective for treating sICAS.Trial registration number NCT03703635;https://www.***.gov.Table 1 Continued Inclusion criteria BASIS,balloon Angioplasty for Symptomatic Intracranial Artery Stenosis;Hb,haemoglobin;INR,international normalized ratio;sICAS,symptomatic intracranial artery stenosis.

Resuscitative endovascular balloon occlusion of the aorta in the treatment of severe hemorrhagic shock caused by upper gastrointestinal bleeding

Hemorrhagic shock is a life-threatening disease often encountered in emergency departments(EDs).Hemorrhagic shock caused by extensive bleeding from multiple sites is often associated with high mortality and morbidity.In recent years,resuscitative endovascular balloon occlusion of the aorta(REBOA)has been widely used in traumatic hemorrhagic shock and is considered to be an effective resuscitation measure.[1]Some studies reported that REBOA was also effective for non-traumatic hemorrhage.[2,3]In this study,we report a case of hemorrhagic shock caused by acute upper gastrointestinal bleeding that was successfully treated and received REBOA to obtain a transition time.This report may provide feasible options for emergency physicians,gastroenterologists,or surgeons to more actively treat refractory gastrointestinal bleeding.

With high rates of morbidity and mortality,posterior circulation stroke accounts for 20% of ischemic strokes,and approximately 25% occurs in patients with stenosis in the vertebral and/or basilar arteries.liiOn the other hand,the risk of posterior circulation ischemic stroke reaches 30% in patients with symptomatic vertebral artery(VA)stenosis afterSyears,[2]subclavian artery(SA)occlusion,although with a low prevalence in the general population,is also an important causeof posterior circulation stroke.

To the Editor:Chronic thromboembolic pulmonary hypertension(CTEPH)is a chronic progressive disease in which pulmonary artery pressure(PAP)increases and eventually leads to right heart failure and death.balloon pulmonary angioplasty(BPA)is a new and effective treatment for CTEPH patients who are unsuitable for surgery or who develop recurrent or persistent pulmonary hypertension after pulmonary endarterectomy(PEA).In recent years,major medical centers at home and abroad have been gradually developing and improving BPA,but overall,a unified treatment process and complication control strategy is lacking.

Cartilage tympanoplasty in the treatment of adhesive otitis media with and without Eustachian tube balloon dilatation

Purpose:To compare cartilage tympanoplasty(CT)combined with eustachian tube balloon dilatation(ETBD)and cartilage tympanoplasty alone as a surgical treatment modality for adhesive otitis media(AdOM)in terms of graft healing,audiological outcomes,and impact on life style,using Chronic Otitis Media Outcome Test 15(COMOT-15).Methods:50 patients with AdOM were randomly classified into 2 groups:25 patients for cartilage tympanoplasty only(CT group)and 25 patients for cartilage tympanoplasty combined with eustachian tube balloon dilatation(CT t ETBD group).Clinical outcomes in both groups were compared at 3 and 6 months of follow up.Results:There was no significant difference in graft healing between the two groups.Postoperative COMOT-15 scores significantly decreased in both groups with a significant difference between the groups with regard to the decrease in COMOT-15 scores at 3 and 6 months of follow-up(P<0.05).Hearing improvement was achieved,as the mean preoperative ABG was 26.5±5.4 and 27.1±4.6 dB,and the mean postoperative ABG at 6 months was 19.4±4.4 and 14.6±3.9 dB in the CT and the CT t ETBD groups,respectively.The difference in the magnitude of ABG reduction in the two groups was significant at 3 and 6 months of postoperative follow-up(P<0.05)in favour of the CT t ETBD group.Conclusion:ETBD can increase the success rate of cartilage tympanoplasty in patients with AdOM by enhancing the audiological outcome and quality of life.

balloon dilation of Eustachian tube combined with tympanostomy tube insertion and middle ear pressure equalization therapy for recurrent secretory otitis media

Objective: To report outcomes of balloon dilation Eustachian tuboplasty combined with tympanostomy tube insertion and middle ear pressure equalization therapy in treatment of recurrent secretory otitis media. Methods: Fifty one patients with recurrent secretory otitis media (62 ears) underwent balloon dilation of Eustachian tube and tympanic tube insertion under general anesthesia, followed by long term middle ear pressure equalization therapies. The Eustachian tube score (ETS) and Eustachian tube function questionnaire (ETDQ-7) were used for pre- and postoperative (up to 12 months) evaluation of Eustachian tube functions. Results: The mean ETS score was 2.34 ± 0.97 preoperatively, and 6.17 ± 1.54, 7.23 ± 1.62, 8.24 ± 1.97, and 7.63 ± 1.86 at 1, 3, 6 and 12 months postoperatively, respectively (P balloon dilation of Eustachian tube combined with tympanostomy and catheterization resulted in significant improvement of subjective symptoms and objective evaluation of Eustachian tube functions in most patients with recurrent secretory otitis media, as indicated by the ETS and ETDQ-7 scores, demonstrating high levels of efficacy and patient satisfaction.

Introduction Meckel’s diverticulum(MD),which usually occurs in the distal ileum,is the most common congenital malformation of the gastrointestinal tract[1].MD is generally asymptomatic;however,-2%–6%of affected patients develop complications[2]such as bleeding,obstruction,or perforation.Intestinal hemorrhage is the most common complication,accounting for -30%of all complications[3].Laparoscopic surgery is usually recommended for MD with hemorrhagic complications.Inverted MD is very rare.MD was reportedly found by double-balloon enteroscopy and surgical treatment was performed.

To the Editor:Postpartum hemorrhage(PPH)is a serious complication of childbirth and a leading cause of maternal morbidity and mortality worldwide.Therefore,the correct and proactive treatment of PPH is key to reducing maternal mortality rates.The Bakri balloon is one treatment method for PPH.Here,we investigated the therapeutic effect,advantages,and disadvantages of the Bakri balloon to provide a theoretical basis for seeking appropriate pathological occasions and optimal timing for placing the Bakri balloon(Cook Inc.,Spencer,IN,USA)and develop a more reasonable and effective method for treating PPH.

balloon dilator controls massive bleeding during endoscopic ultrasound-guided drainage for pancreatic pseudocyst:A case report and review of literature

Pancreatic pseudocyst(PPC), a common sequela of acute or chronic pancreatitis, was defined by the revised Atlanta classification as "a collection." Endoscopic ultrasound(EUS)-guided drainage is often considered a standard first-line therapy for patients with symptomatic PPC. This effective approach exhibits 90%-100% technical success and 85%-98% clinical success. Bleeding is a deadly adverse event associated with EUS-guided drainage procedures, and the bleeding rate ranges from 3% to 14%. Hemostasis involves conservative treatment, endoscopy, interventional radiology-guided embolization and surgery. However, few studies have reported on EUSguided drainage with massive, multiple hemorrhages related to severe pancreatogenic portal hypertension(PPH). Thus, the aim of this case report was to present a case using a balloon dilator to achieve successful hemostasis for PPH-related massive bleeding in EUSguided drainage of PPC. To our knowledge, this method has not been previously reported.
