Surface Treated Catheters for Vascular Access—Useful?

Background: Catheter-related infections (CRI), thrombosis, and stenosis are among the most frequent complications associated with catheters which are inserted in vessels as vascular access. These problems are usually related to the handling of the staff, the catheter materials, and the surface properties of the catheter. To mitigate such complications surface treatment process of the outer surface, such as ion beam assisted deposition is investigated in a retrospective study from 1992 to 2007, to prove if the surface treatment of the catheters is a sufficient solution. Methods: This study (1992-2007) evaluated silver coated and non-coated implanted large-bore catheters used for extracorporeal detoxification. In 159 patients, 54 patients received a silver coated catheter (Spi-Argent, Spire Corporation, Bedford, MA, USA) and 105 patients, an untreated catheter served as controls. The catheters were inserted into the internal jugular or subclavian veins. After removal, the catheters were cultured for bacterial colonization using standard microbiologic assays. They were also examined using a scanning electron microscope (SEM). Results: The silver coated catheters showed a tendency towards longer in situ time. The microbiologic examinations of the catheter tips were in both catheter types high positive, but not significant. Conclusion: The silver coated catheters showed no significant reduction in infection rate by evaluation of all collected data in this retrospective study. There was no association between both catheters in significantly reducing patient discomfort. Other surface treatments which include the outer and inner surface are necessary. New developed catheter materials such as the microdomain structured inner and outer surface, as an example, are considered more biocompatible because they mimic the structure of natural biological surface.

Management of difficult wounds can be a complex,challenging and expensive task,especially for wounds showing a slow healing process.Topical negative pressure(TNP)therapy has greatly improved difficult wounds treatment.It allows to treat patient on an outpatient management,to reduce the complication rate with shorter hospital stay,to avoid frequent dressings with expensive advanced materials and allow a lower commitment of health professionals.Vacuum assisted Closure(VAC)system is a therapeutic device based on the administration of a controlled TNP introduced by Morykwas and Argenta in 1997.It is indicated in different kinds of wound,but clinical evidences are present only for few of them.In this work we summarize indications and recommendations for VAC therapy and we analyze the actual better choice of treatment based on evidences and personal experience in order to stimulate further studies.Finally we introduce recent applications of VAC system such as Prevena,VAC Instill and VAC Via Prevena is a system based on TNP indicated in the management of closed wounds that present risk factors for dehiscence.VAC Instill is a system that allows to associate TNP and topical administration of solutions,such as antibiotics or disinfectants,to treat specific type of wounds.VAC Via is a device based on TNP,characterized by little dimen-sion and a preset system that allow the treatment of little wounds for 7 d,with no impairment for the patient.The aim of our paper is to describe a report of VAC therapy use in order to stimulate further studies and to define the level of evidence of VAC therapy.

Implantation of the embryo into the endometrium is the first step in the establishment of pregnancy. This process is complex, and depends on many factors. Recurrent implantation failure is a source of distress to patients and specialists. It is defined as failure to achieve a viable pregnancy, following “>3 embryo transfers with high quality embryos or the transfer of ≥ 10 embryos in multiple transfers”. Thrombophilic conditions that contribute to recurrent implantation failure are the main concern in this review. The mechanism of implantation failure is believed to be due to decreased blood flow to the endometrium and placenta which can hinder normal endometrial receptivity leading to miscarriage. Defects in early placentation resulting in pregnancy failure, have focused attention on the therapeutic potential of low molecular weight heparin in the implantation process. Heparin has a role at all stages of implantation to improve pregnancy outcomes. There are controversies in literature regarding the association between thrombophilia and recurrent implantation failure and available literature regarding this issue is very heterogeneous. Various investigators, have shown that women with RIF are more likely to have a thrombophilia disorder, yet a clear cause cannot be acknowledged from these studies. Heparin treatment has been evaluated in several studies, showing conflicting evidence. However, several studies have pointed out that it may play a role in a subset of patients who presents a thrombophilia mutation, thus the group of patients that might benefit is needed to be identified. This review is dedicated to evaluate the published literature about the role of low molecular weight heparin in case of recurrent implantation failure with or without the presence of thrombophilia.

Detecting Dementia-Related Wandering Locomotion of Elders by Leveraging Active Infrared Sensors

For elders with dementia, wandering is among the most problematic, frequent and dangerous behavior. Managing wandering behavior has become increasingly imperative due to its high prevalence, negative outcomes and burden on caregivers. We study to propose an active infrared-based method to identify wandering locomotion by monitoring rhythmical repetition of an elder’s indoor motion events. Specifically, we utilize our customized active infrared sensors to collect human indoor motions that will be converted into motion events by using hardware redundancy technique. Each motion event is a directed motion obtained via introducing temporal and dimensions into the spatial motion data. Based on the most cited spatial-temporal patterns of wandering locomotion, a spatiotemporal model is then proposed to identify wandering locomotion from an ongoing sequence of motion events. Experimental evaluation on eight individuals’ real-world motion datasets has shown that our proposed method is able to effectively identify wandering locomotion from repetitive events collected from active infrared sensors with a value over 98% for both accuracy and precision based on properly chosen parameters. Wandering in elders with dementia that follow specific spatiotemporal patterns can be reliably identified by analyzing repetitive motion events collected from active infrared sensors based on the well-known spatiotemporal patterns of wandering locomotion.

In this paper, a simple and facile surfactant assisted combustion synthesis is reported for the ZnO nanoparticles. The synthesis of ZnO-NPs has been done with the assistance of non-ionic surfactant TWEEN 80. The effect of fuel variations and comparative study of fuel urea and glycine have been studied by using characterization techniques like X-ray diffraction (XRD), transmission electron microscope (TEM) and particle size analyzer. From XRD, it indicates the presence of hexagonal wurtzite structure for ZnO-NPs. Using X-ray broadening, crystallite sizes and lattice strain on the peak broadening of ZnO-NPs were studied by using Williamson-Hall (W-H) analysis and size-strain plot. Strain, stress and energy density parameters were calculated for the XRD peaks of all the samples using (UDM), uniform stress deformation model (USDM), uniform deformation energy density model (UDEDM) and by the size-strain plot method (SSP). The results of mean particle size showed an inter correlation with W-H analysis, SSP, particle analyzer and TEM results.

Cervical pregnancy is rare and heterotopic cervical pregnancy is more uncommon ever. assisted Reproduction Technology enhances these pregnancies. The diagnosis and the treatment which are in emergency can be sometimes difficult. This article is a case report of a triplet heterotopic cervical pregnancy after embryo transfer, and a scientist review of the literature found in MEDLINE. To date, in the English literature, we found only 7 cases report of triplet heterotopic cervical pregnancies.

Optimization of Extraction Process of Total Flavonoids and Chlorogenic Acid from Flos Lonicerae by Orthogonal Experiment

[Objectives]To explore the process of simultaneous extracting total flavonoids and chlorogenic acid from Flos Lonicerae by pressure assisted extraction.[Methods]Based on the single factor experiment,orthogonal experiments were carried out to investigate the effects of solvent concentration,solid-to-liquid ratio,extraction pressure,extraction time and extraction temperature on the extraction rate of total flavonoids and chlorogenic acid.[Results]The results showed that the extraction pressure had a significant effect on the extraction of two kinds of chemical constituents from F.Lonicerae.The optimum process conditions were as follows:solvent concentration was 60%,solid-liquid ratio was 1∶20(g/mL),extraction pressure was 2 MPa,extraction time was 30 min,extraction temperature was 70℃.The process conditions are stable,and the extraction efficiency is higher than that of ultrasonic assisted extraction method.Under the test conditions,the extraction rates of total flavonoids and chlorogenic acids were respectively 15.66%and 3.89%.[Conclusions]This study provides a new theoretical basis for the development and utilization of F.Lonicerae.

The effect of administrating hyoscine bromide on pregnancy rate before embryo transfer in ART cycles

assisted reproductive technology methods i.e., IVF and ICSI were extensively used for treatment of infertility. Although, these methods have many advantages but ART pregnancy rate remains low (30% - 35%). It seems that uterine peristalsis occurred after rhCG administration in turn resulting to impair implantation of the transferred embryos. Therefore this study was conducted to possess of whether hyoscine bromide can effectively alleviate uterine contraction and then have any effect on the reproductive outcome. Healthy women who underwent IVF or ICSI cycles (n = 142) were randomly assigned to one of two groups at the start of the cycle. Group control (n = 71) without applied hyoscine and group experiment (n = 71) applied hyoscine (25 mg). Controlled ovarian hyperstimulation was induced with follicle stimulating hormone (FSH). Sperm preparation and ovarian puncture for in vitro maturation and fertilization (IVF) of oocyte were carried out for 42 hours after rhCG injection. Two weeks after rhCG injection, bhCG pregnancy test to determine pregnancy outcome in both groups was done and the outcome results, along with other information were recorded separately. The pregnancy rate in experimental group was significantly higher than that of control group. when all of the cycles with or without giving 25 mg of hyosine were stratified by the mean total follicles (more/less than 2 follicles) and embryos (more/less than25 kg/m2), the Patients’ age, the quality grade of embryos and infertility duration (more/less than 5 years), the reproductive outcome, in overall, in the subjects with more than 10 oocytes, 5 embryos, more than quality A and B grades of embryos, less than 35 years age and also less than 5 years infertility duration was more pronounced than the other patients. But the other parameter was not affected by the reproductive outcome. Clinical or statistical improvement of the reproductive outcome could be demonstrated in the hyosine treated group especially with increasing the mean total follicles, embryo, quality

In the present work, one-dimensional nanostructures of silicon oxide (SiOx) have been synthesized by thermal annealing method with and without chromium thin film on silicon substrate. The synthesis was carried out at different process temperatures ranging from 1000°C to 1100°C by using gold/chromium (Au/Cr) catalysts stack layer on the Si substrate in nitrogen (N2) ambience. The as-synthesized SiOx nanostructures have tetragonal rutile structure and show polycrystalline nature. The SEM images reveal wire-like nanostructures on the substrate with and without chromium thin film. Under the catalytic reaction of the gold/chromium metal, the density of SiOx nanowires is enhanced, since the Cr layer serves as a diffusion barrier for the diffusion of the gold downwards into the Si substrate. The vapor-liquid solid (VLS) growth mechanism is found to be dominant in the growth of SiOx nanowires. Furthermore, X-Ray diffraction microscopy (XRD) and Photoluminescence spectroscopy (PL) analysis conclude the defect free growth of the SiOx nanowires on gold/chrome/silicon substrate.
