Increased P wave duration in patients with depression or anxiety disorder

BACKGROUND： Activation of the sympathetic nervous system plays an important role in regulating cardiovascular actions. P wave parameters can provide general information on central cardiovascular autonomic regulatory responses, which are altered in patients with anxiety disorders and depression. In particular, there are no reports addressing changes in P wave duration and dispersion. OBJECTIVE： To compare the differences in P wave duration and P wave dispersion between patients with anxiety disorders and depression, because patients with anxiety disorders and depression develop abnormal electrocardiograms. DESIGN, TIME AND SETTING： A non-randomized concurrent controlled study was performed. Patients with depression and general anxiety disorders were admitted at the psychiatry outpatient clinics of the Medical Faculty of Duezce University of Turkey between May 2005 and October 2006. PARTICIPANTS： A total of 71 consecutive patients with depression and anxiety disorders, as well as 50 physically and mentally healthy age- and gender-matched controls were selected. METHODS： Electrocardiogram records were obtained at the time of admission to the outpatient clinics. MAIN OUTCOME MEASURES： P wave duration and P wave dispersion were measured. RESULTS： Both the maximum （Prnax） and minimum （Pmin） P wave duration were greater in patients with psychiatric disorders than in healthy controls. Pmax was significantly greater in patients with depression or anxiety disorders （Bonferroni test, P 〈 0.017）. The P wave dispersion was similar between patients and controls （P 〉 0.017）. P waves were similar between panic patients and other anxiety patients. Beck depression results were positively correlated with Prawn and Prnax （r= 0.374, 0.302, P = 0.013, 0.049, respectively）, and not associated with P wave dispersion （P 〉 0.05）. CONCLUSION： Psychiatric disorders are associated with increases in Prnax, but not with P wave dispersion. The P wave changes were associated with the degree of depression.

Prevalence of Pre-Operative anxiety and Predictors among Elective Surgical Patients and Their Pre-Operative Hemodynamic Changes at Muhimbili National Hospital

Background: Surgery is a traumatic process that may subject patients to physiological and psychological responses leading to pre and post-operative effects. Preoperative anxiety, if severe may be associated with several complications including cancellation of surgery, poor intra- and post-operative analgesic control, altered post-operative healing, and longer hospital stays to mention a few. Objective: The main purpose was to determine the prevalence and predictors of pre-operative anxiety among elective surgical patients and their hemodynamic changes in these patients in the Surgical department in MNH. Methodology: This was a prospective descriptive and analytical study performed at MNH which involved patients admitted for elective surgical procedures from June 2021 to February 2022. Information was gathered in a structured questionnaire along with APAIS scores. Results: 169 patients for elective surgery in General Surgery and Urology units were included in the study. Among them, 94 males and 74 females. The overall pre-operative anxiety was 11.8%, 80.5% had moderate to severe Information-Related anxiety, while 26% and 17.2% had moderate-to-severe surgery-related and anesthesia-related types of anxiety respectively. Conclusion: Pre-operative anxiety was comparatively lower in our settings as well no factor was found with a significant relation to pre-operative anxiety, hence further and broader evaluation is recommended to result in the assessment and management of patients before elective surgery.

anxiety is a common disease in the modern society which significantly affects people’s daily lives and function,thus it has become an increasingly highlighted issue.anxiety is regulated by neural circuits in the brain.Therefore,the basal mechanism of anxiety has been studied,especially research based on the related neural circuits.For a long time,due to the limitations of science and technology,there was no breakthrough in research regarding anxiety.However,in recent years,due to the progress of technology,the research on anxiety neural circuits has made great progress.For example,the interaction among various brain regions,such as the central nucleus of the amygdala(CeA),the ventral tegmental area(VTA),the ventral hippocampus(vHPC),and so on.This article focuses on three brain regions:including BLA,BNST,and VTA,and illustrate their different roles and mechanisms in regulating anxiety.On this basis,this intensive study of anxiety will further promote the progress of anxiety research and provide therapeutic targets for the related treatment.

Effectiveness of Virtual Reality for Pediatric Pain and anxiety Management during Skin Prick Testing

This study investigated the effectiveness of virtual reality (VR) distraction, compared to comic book distraction and no distraction, in reducing pain and anxiety during a medical procedure in a pediatric population: the skin prick test. Although this test has many advantages and is considered to be minimally invasive, it causes anxiety and painful discomfort in children. Ninety-two children aged 7 to 17 years consulting for an allergic test received VR distraction, comic book distraction, or no distraction. Outcome measures included pain score, level of anxiety, and VR measures. The results showed that there were no significant differences between the three groups regarding sex, age, and preprocedural anxiety level. In the distraction groups (VR and comic book), children reported significantly lower pain and procedural anxiety scores than children with no distraction;VR distraction had a more significant effect than comic book distraction. A decrease in anxiety before and during the skin prick test is significantly more significant in VR distraction. This study suggested the effectiveness and feasibility of VR to reduce pain and anxiety during the pediatric skin prick test.

Prevalence and Predictors of anxiety and Depression among Gynaecological Cancer Patients at a Tertiary Health Facility in Southern Nigeria

Background: Cancer patients frequently suffer from mental health problems because of their reactions to their cancer diagnosis, cancer type, treatment effects, recurrence, fear of the end-of-life, survival, and financial burden. Some hospitals have integrated mental health assessments into cancer care, but our centre has little experience with this practice. Aim and Objectives: To ascertain the prevalence and predictors of anxiety and depression in patients with gynaecological cancer at a tertiary health facility in southern Nigeria. Materials and Methods: This was a facility-based cross-sectional descriptive study of 75 women with histologically confirmed gynaecological cancer managed at the University of Port Harcourt Teaching Hospital, Port Harcourt, from January 1, 2022, to December 31, 2022. Individuals having a history of drug addiction, severe cognitive impairment, non-consenting patients, and those with communication difficulties were excluded from the study. A data collection form was used to obtain socio-demographic, reproductive, and clinical characteristics, while the Hospital anxiety and Depression Scale (HADS) was used to assess for anxiety and depression. Data was analyzed using descriptive statistics to determine the association of variables with anxiety and depression. Results: Most 27 (36.0%) of the respondents were in 40 - 49 age group with a mean age of 50.4 ± 12.3 years. The study showed that 39 (52.0%) of the respondents exhibited symptoms of anxiety, of which 14 (35.9%) were mild, 20 (51.3%) had moderate anxiety and 5 (12.8%) experienced severe anxiety. In addition, 46 (61.3%) of them showed symptoms of depression, of which 17 (37.0%) were mild, 15 (32.6%) were moderate and 14 (30.4%) experienced severe depression. Effects of the Treatment of Carpal Tunnel Syndrome with Surgery and Injections on the Hospital anxiety and Depression Scale (HADS)

Background: The prevalence of carpal tunnel syndrome (CTS) and of anxiety and depression in primary care practice are high. Different studies had shown an increased prevalence of anxiety and depression in CTS patients. Nevertheless, few papers had been published studying the anxiety and depression scales in the treatment of CTS, either with corticosteroid injections (I) or with surgical decompression (S). Objective: To assess whether clinical improvement observed after the treatment of CTS either with I or with S correlates with an improvement in the punctuations of the Hospital anxiety and Depression scales (HADS), at 3, 6 and 12-month follow-up. Methods: Randomized and open-label clinical trial, comparing I and S. Patients with symptoms suggestive of CTS (nocturnal paraesthesias) of at least 3 months duration and neurophysiological confirmation were included. Patients with clinically apparent motor impairment were excluded. The subjective evaluation of symptoms was carried out using the visual-analogue scale of pain (VAS-p). Clinical reviews were performed 3, 6 and 12 months after treatment. Each patient completed the HADS questionnaire and a VAS-p at 0, 3, 6, and 12 months. Statistical significance was established using the Student’s t test and the Mann-Whitney U test when necessary. A linear regression analysis was used to know the effect of the treatment adjusted for the initial score of both scales. Results: 65 patients were included (30 in group I and 35 in group S). There was no statistical difference between both groups in terms of age, gender distribution, disease duration, VAS-p, neurophysiological testing severity of CTS or the 8 subscales of HADS. Both groups improved significantly in relation to the baseline VAS-p values, in the reviews at 3, 6 and 12 months, with no significant differences between I and S. At 6 months, the reduction in the anxiety scale was around 3 points for both treatments (S = 3.6 and I = 3.2), without reaching significant differences. At 12 months, it was somewhat higher for t

High Fat Diet Alteration of Gut Microbiota Impacts Learning, Memory and anxiety Response in Mice: Cannabidiol and Omega 3 Possible Remedies

Chronic intake of High Fat Diet (HFD) has the potential of causing a number of metabolic disorders. It is also theorized to be involved in perturbation of gut microbiota. Cannabididol (CBD) and omega 3 are known to possess a number of medicinal usefulness. Their combined use in experimental interventions is quite limited. A total of 15 mice were used for this research divided into three groups of five animals each. Group 1 was administered water and normal chow ad libidum. Group 2 had HFD and water ad libidum. Group 3 had HFD plus CBD (10 mg/kg) and omega 3 (200 mg/kg) all for a total of 12 weeks. They were tested on the elevated plus maze (EPM) and average entry time into the closed arm was recorded. They were also tested on the Y-maze and spontaneous alternations were measured. Thereafter animals were sacrificed and faecal content in the caecum was collected in sterile bottles and cultured for E. coli count. It was found that HFD group at p value E. coli count (2.4 × 106 ± 4.5) compared to group 1 (1.4 × 106 ± 5.6) and group 3 (1.42 × 106 ± 6.3). The findings revealed that HFD enhanced gut E. coli overgrowth which was reduced by CBD and Omega 3. The memory impairment and anxiety induction by HFD was also significantly ameliorated by CBD and omega 3. E. coli known to be implicated in dementia induction was suppressed by the interventions. Possible mechanisms proposed are actions of CBD and omega 3 on CB1, TRVP and 5HT receptors in reducing anxiety and their antioxidant/anti-inflammatory actions in mitigating the neuro-inflammatory effect of HFD and immune hyperstimulation of E. coli via the gutbrain-axis.

Level of Insomnia, Symptoms of Depression, and anxiety among College Students with Mobile Phone Addiction: Basis for Guidance and Counseling Program Enhancement

Mobile phone addiction is a growing concern among college students, with significant implications for their mental health. This study examines the complex relationship between mobile phone addiction and its impact on college students’ insomnia, symptoms of depression, and anxiety. A sample of 399 college students from traditional Chinese medicine colleges in Guangxi, China, completed the Mobile Phone Addiction Tendency Scale questionnaire. Among them, 155 students with high Mobile Phone Addiction Tendency scores were selected for the correlation study. The analysis revealed a strong association between mobile phone addiction and insomnia. Furthermore, mobile phone addiction was significantly associated with symptoms of depression, including feelings of sadness, worthlessness, and hopelessness, as well as heightened anxiety symptoms, such as excessive worry and restlessness. These findings emphasized the importance of targeted interventions and preventive measures to address the negative impact of mobile phone addiction on college students’ mental health.

Effect of Sacubitril-Valsartan Combined with Zhenyuan Capsule in the Treatment of Chronic Heart Failure Comorbid anxiety and Depression and Its Effect on Inflammatory Factors

Objective: To investigate the effect of sacubitril-valsartan combined with Zhenyuan capsule in the treatment of chronic heart failure comorbid anxiety and depression and its effect on the level of inflammatory factors. Methods: A total of 106 patients with chronic heart failure comorbid anxiety and depression from February 2020 to March 2022 were continuously enrolled and divided into control group (36 cases), observation group A (36 cases) and observation group B (34 cases) according to treatment methods. All groups were given conventional treatment. On the basis of routine treatment, the control group, observation group A and observation group B were given valsartan, sacubitril-valsartan and sacubitril-valsartan plus Zhenyuan Capsules for the treatment of consecutive 8 weeks. The patients in the 3 groups were evaluated by the Self-rating anxiety Scale (SAS) and Self-rating Depression Scale (SDS) before and after treatment, and the clinical efficacy of heart failure was evaluated, and the detection of left ventricular ejection fraction (LVEF), left ventricular end systolic diameter (LVESD), left ventricular end-diastolic diameter (LVEDD), N terminal brain natriuretic peptide (NT-proBNP), tumor necrosis factor alpha (TNF alpha), interleukin 6 (IL-6), c-reactive protein (CRP) was conducted. Results: The clinical efficacy rate and total effective rate of heart failure in observation group A and observation group B were significantly higher than those in the control group (P < 0.05), and the observation group B was higher than the observation group A (P < 0.05);SAS and SDS scores in observation group A and observation group B were significantly lower than the control group (P < 0.05), and observation group B was lower than observation group A (P < 0.05);The LVEF in the three groups was all increased compared with those before treatment, and the levels of LVESD, LVEDD, NT-proBNP, TNF-α, IL-6, and hs-CRP were all decreased compared with those before treatment;The changes of above indexes in observation group A and obs
