Food allergy in irritable bowel syndrome:The case of non-celiac wheat sensitivity

Irritable bowel syndrome(IBS) is one of the most common gastrointestinal disorders, having a prevalence of 12%-30% in the general population. Most patientswith IBS attribute their symptoms to adverse food reactions. We review the role of diet in the pathogenesis of IBS and the importance of dietary factors in the management of these patients. The MEDLINE electronic database(1966 to Jan 2015) was searched using the following keywords: "food", "diet", "food allergy", "food hypersensitivity", "food intolerance", "IBS", "epidemiology", "pathogenesis", "pathophysiology", "diagnosis", "treatment". We found 153 eligible papers; 80 were excluded because: not written in English, exclusive biochemical and experimental research, case reports, reviews, and research otherwise not relevant to our specific interest. We selected 73 papers: 43 original papers, 26 reviews and 4 letters to the editor. These papers focused on IBS pathogenesis, the association between IBS and atopy, and between IBS and food allergy, the relationship between IBS and non-celiac wheat sensitivity, the role of diet in IBS. Pending further scientific evidence, a cautious approach is advisable but the concept of food allergy should be included as a possible cause of IBS, and a dietary approach may have a place in the routine clinical management of IBS.

In recent years,increasing attention has been paid to bioengineered bacteria as vectors for the treatment of allergic diseases.The methods for preparing bioengineered bacteria that can express exogenous genes are improving.Research has focused mainly on application of bioengineered bacteria expressing recombinant allergens,hypoallergenic derivatives of allergens,T-cell epitope derivatives,cytokines,or as mucosal adjuvants to enhance immunotherapy effects.This strategy offers new ideas for the treatment of allergic diseases.This review summarizes recent advances in use of live bioengineered bacteria in allergic diseases as well as the challenges of using microorganisms(or their components)in immunotherapy.

Indoor microorganisms impact asthma and allergic rhinitis(AR),but the associated microbial taxa often vary extensively due to climate and geographical variations.To provide more consistent environmental assessments,new perspectives on microbial exposure for asthma and AR are needed.Home dust from 97 cases(32 asthma alone,37 AR alone,28 comorbidity)and 52 age-and gender-matched controls in Shanghai,China,were analyzed using high-throughput shotgun metagenomic sequencing and liquid chromatography-mass spectrometry.Homes of healthy children were enriched with environmental microbes,including Paracoccus,Pseudomonas,and Psychrobacter,and metabolites like keto acids,indoles,pyridines,and flavonoids(astragalin,hesperidin)(False Discovery Rate<0.05).A neural network co-occurrence probability analysis revealed that environmental microorganisms were involved in producing these keto acids,indoles,and pyridines.Conversely,homes of diseased children were enriched with mycotoxins and synthetic chemicals,including herbicides,insecticides,and food/cosmetic additives.Using a random forest model,characteristic metabolites and microorganisms in Shanghai homes were used to classify high and low prevalence of asthma/AR in an independent dataset in Malaysian schools(N=1290).Indoor metabolites achieved an average accuracy of 74.9%and 77.1%in differentiating schools with high and low prevalence of asthma and AR,respectively,whereas indoor microorganisms only achieved 51.0%and 59.5%,respectively.These results suggest that indoor metabolites and chemicals rather than indoor microbiome are potentially superior environmental indicators for childhood asthma and AR.This study extends the traditional risk assessment focusing on allergens or air pollutants in childhood asthma and AR,thereby revealing potential novel intervention strategies for these diseases.

Extensive research of respiratory physiology and diseases of the airways and lungs provides better understanding of mechanisms of diseases and it is a source of new data with potential clinical application. The majority of the data in the field of airway defensive reflexes were obtained from guinea pig model, as the most convenient one. Neurophysiology and neuropharma-cology of the cough reflex—which is exclusively mediated by the vagus nerve, share close similarities between guinea pigs and humans. The models used to study pathological processes and their influence on airway-defensive reflexes use sensitization with ovalbumin—the protein from chicken egg, which does not mimic allergies within human respiratory system. House dust mites (HDM) (Dermatophagoides pteronyssius and Dermatophagoides farinae) represent frequent human aeroallergens, however the HDM models are used considerably less than ovalbumin models. The primary objective of this review is to focus on already validated models of HDM-induced airway diseases to see, whether some of them are suitable to study mechanisms of peripheral cough plasticity in a condition of HDM-induced pathological processes. The main purpose of future use and validation of HDM model is to produce higher ability to translate the results obtained in animal models to human cough research.

Role of allergy in Sinonasal Polyposis

Objectives: The exact role of allergy in sinonasal polyposis is not yet clearly elucidated and is undoubtedly a contro-versial subject. The study focussed on the association of allergy in nasal polyposis. We aim to determine whether there was a correlation between Serum IgE, absolute eosinophil count, eosinophilic inflammation, and nasal polyps. Methods: A study group of fifty two consecutive patients of nasal polyposis were evaluated prospectively and compared with 26 controls who underwent septoplasty and mimimal FESS or Endoscopic Sphenopalatine artery ligation. Patients were evaluated for presence of allergy with regard to absolute eosinophil count, total serum IGE and tissue eosinophilia and correlations were established. All patients were categorized based on histological evidence of tissue eosinophilia. Re-sults: The incidence of asthma was 5.8% and positive history of allergy was obtained in 40.4% of patients in study group and 7.7% of patients in control group. The statistically significant association was not seen with absolute eosino-phil count, Serum IgE and tissue eosinophilia. Tissue eosinophilia was observed in more number of patients with nasal polyposis compared to controls. So clinical significance might be established. Conclusions: allergy is parameter that is frequently associated with this disease, irrespective of the type of polyp and the age at presentation. Unrecognized and untreated allergy adds to the morbidity of the disease and generally results in poor treatment outcome.

Prevalence of Peanut allergy in Offspring of Peanut Farmers

Introduction: While exposure to environmental peanut during infancy appears to promote sensitization by the epicutaneous route, early and frequent peanut ingestion during infancy may prevent peanut allergy through oral tolerance. Due to parental occupation, the offspring of peanut farmers are likely exposed to peanut protein in their environment, but they may also have early introduction into their diet due to ready access. The purpose of this study was to determine the prevalence of peanut allergy in offspring of peanut farmers. Methods: A survey was mailed by the National Peanut Board to peanut farmers in the United States collecting information on peanut consumption, peanut reactions, physician-confirmed peanut allergy (PA), epinephrine auto-injector prescription and history of other allergies in offspring. Egg allergy (EA) served as a comparator to peanut allergy. Results: Of 10,349 households surveyed, 1050 responses were received (2493 offspring, participation rate 10%). The self-reported prevalence of peanut allergy in the general population in the USA is 0.9%, 1.3%, and 0.7% for children 0 - 5 years, 6 - 10 years, and 11 - 17 years, respectively. In offspring of peanut farmers, PA vs EA was reported in 3.69% (10/271) vs 2.6% (7/271), 2.8% (14/493) vs 2.0% (10/493), and 1.48% (37/2493) vs 1.4% (35/2493) of offspring ≤10 years, offspring ≤15 years and all offspring, respectively. Physician-confirmed PA was reported in 3.3% (9/271), 2.6% (13/493), and 1.24% (31/2493) in the three groups, respectively. Physician-confirmed PA with epinephrine auto-injector prescription was reported in1.8% (5/271), 1.8% (9/493), and 0.68% (17/2493) in the three groups, respectively. Conclusions: We found that the self-reported prevalence of peanut allergy in offspring of peanut farmers was similar to that reported in the general population. This is a unique population that may provide insight into factors that influence development of peanut allergy.
