Aneurysmal subarachnoid hemorrhage(aSAH)is a typical neurosurgical emergency.The patient's prognosis is related to the severity of the initial illness and post-operative complications,especially delayed cerebral ischemia(DCI).DCI,which occurs in up to 30%of patients with subarachnoid hemorrhage(SAH),usually 2 weeks after hemorrhage,is one of the leading causes of death and disability in aSAH.

To the Editor:The septate uterus is the most common uterine anomaly and is associated with an increased risk of adverse reproductive outcomes.Transcervical resection of septum(TCRS)is widely performed in patients with adverse reproductive outcomes.The incidence of de novo intrauterine adhesions(IUAs)after TCRS is approximately 5%to 25%.[1]Various anti-adhesion methods are widely used in the clinical management of patients with septate uteri,including intrauterine devices,Foley balloons,estrogen,antibiotics,and hyaluronic acid gel.However,insufficient evidence supports or opposes any of these methods(Level C),[2]lacking high-grade evidence.

Radical operation for hilar cholangiocarcinoma(HCCA)is the most effective treatment,but high rates of severe postoperative complication and death remain concerns due to the complexity of biliary and vascular anatomy of the hepatobiliary region.De-layed arterial hemorrhage(DAH)occurring>24 h postoperatively usually causes life-threatening bleeding,and relaparotomy for DAH would be difficult and hazardous because of postoperative adhe-sions and critical general condition.As endovascular technique is less invasive and has improved over the recent decades,clini-cians have begun to prefer endovascular treatment(EVT)to surgi-cal treatment of DAH after hepatobiliary pancreatic surgery.How-ever,to our knowledge,a relatively large series of patients dealing with the outcomes of EVT of DAH after surgery for HCCA has not been reported.Herein,we analyzed the technical and clinical out-comes of EVT in 17 patients to evaluate the efficacy and safety of EVT.

Background:Enhanced recovery after surgery(ERAS)has shown effectiveness in terms of reducing the hospital stay and cost.However,the benefit of ERAS in patients undergoing hepatectomy for benign liver lesions is still unclear.Methods:ERAS was implemented in our center since March 1st,2018.From September 2016 to Febru-ary 2018,109 patients were enrolled into the control group,and from March 2018 to June 2019,124 patients were enrolled into the ERAS group.All the indicators related to operation,liver functions,and postoperative outcomes were included in the analysis.Results:The clinicopathologic baselines were similar in these two groups.A significantly higher propor-tion of patients underwent laparoscopic surgery in the ERAS group.On the whole,intraoperative blood loss(100.00 mL vs.200.00 mL,Pafter stratified by operation methods,ERAS group showed significantly shorter postoperative hospital stay in both open and laparoscopic operation(both P<0.001).In patients underwent open surgery,ERAS group demonstrated significantly shorter operative duration(131.76±8.75 min vs.160.73±7.23 min,P=0.016),less intra-operative blood loss(200.00 mL vs.450.00 mL,P=0.008)and less postoperative morbidity(16.00%vs.44.44%,P=0.040).Conclusions:ERAS program may be safe and effective for the patients underwent hepatectomy,especially open surgery,for benign liver lesions.

Enhanced recovery after cesarean(ERAC)delivery is an evidence-based,multi-disciplinary approach throughout pre-,intra-,postoperative period.The ultimate goal of ERAC is to enhance recovery and improve the maternal and neonatal outcomes.This review highlights the role of anesthesiologist in ERAC protocols.This review provided a general introduction of ERAC including the purposes and the essential elements of ERAC protocols.The tool used for evaluating the quality of ERAC(ObsQoR-11)was discussed.The role of anesthesiologist in ERAC should cover the areas including management of peri-operative hypotension,prevention and treatment of intra-and post-operative nausea and vomiting,prevention of hypothermia and multi-modal perioperative pain management,and active pre-operative management of unplanned conversion of labor analgesia to cesarean delivery anesthesia.Although some concerns still remain,ERAC implementation should not be delayed.Regular assessment and process improvement should be imbedded into the protocol.Further high-quality studies are warranted to demonstrate the effectiveness and efficacy of the ERAC protocol.

To the Editor:We read with great interest the recent article by Zhu et al.[1].In the study,the authors analyzed the outcomes of 26 patients diagnosed with hepatic artery occlusion(HAO)and treated with an endovascular approach(EVT)within the first 30 days after ortho-topic liver transplantation(LT).The median interval from LT to EVT was 7 days,most patients were treated with angioplasty and only two(7.7%)needed stent placement.The authors should be congrat-ulated as they achieved a 100%of success rate with an 80.8%of 1-year survival rate.

Background:Enhanced recovery after surgery (ERAS) protocols are a series of perioperative care to optimize preoperative preparation, prevent postoperative complications, minimize stress, and speed up recovery. This study aimed to assess the impact of ERAS protocols for functional endoscopic sinus surgery (FESS) in patients with chronic rhinosinusitis with nasal polyps (CRSwNP).Methods:One hundred and two patients with CRSwNP undergoing FESS were randomly divided into the ERAS group and the control group. The outcomes of the Self-Rating Anxiety Scale (SAS), Visual Analogue Scale (VAS), Medical Outcomes Study Sleep Scale (MOS-SS) and Kolcaba Comfort Scale Questionnaire (GCQ) were determined in both groups. The serum levels of C-reactive protein (CRP) were compared preoperatively and 24 hours postoperatively.Results:The ERAS group had a significantly better SAS scores than did the control group (28 [24, 35] vs. 43 [42, 47], Z=5.968, P 0.05);However, the CRP level in 24 hours postoperatively was significantly lower in the ERAS group than that in the control group (2.5 [1.4, 3.9] vs. 6.6 [3.8, 9.0], Z = 5.027, P 0.05), hemorrhage, aspiration and tumble, were not increased in the ERAS group compared with those in the control group. The ERAS group had a significantly shorter length of hospital stay (5 [4, 5] days vs. 8 [8,9] days, Z=8.939, P<0.001) and hospitalization expenses ($ 2670 [2375, 2740] vs. $3129 [3116, 3456], Z=8.514, P<0.001).Conclusions:ERAS pro

Background For endovascular rescue therapy(ERT)of cerebral vasospasm(CVS)due to spontaneous subarachnoid haemorrhage(sSAH),non-compliant(NCB)and compliant(CB)balloons are used with both balloon types bearing the risk of vessel injury due to specific mechanical properties.Although severe delayed arterial narrowing after transluminal balloon angioplasty(TBA)for CVS has sporadically been described,valid data concerning incidence and relevance are missing.Our aim was to analyse the radiological follow-up(RFU)of differently TBA-treated arteries(CB or NCB).Methods Twelve patients with utilisation of either NCB or CB for CVS were retrospectively analysed for clinical characteristics,ERT,functional outcome after 3 months and RFU.Compared with the initial angiogram,we classified delayed arterial narrowing as mild,moderate and severe(60% calibre reduction).Results Twenty-three arteries were treated with CB,seven with NCB.The median first RFU was 11 months after TBA with CB and 10 after NCB.RFU was performed with catheter angiography in 18 arteries(78%)treated with CB and in five(71%)after NCB;magnetic resonance angiography was acquired in five vessels(22%)treated with CB and in two(29%)after NCB.Mild arterial narrowing was detected in three arteries(13%)after CB and in one(14%)after NCB.Moderate or severe findings were neither detected after use of CB nor NCB.Conclusion We found no relevant delayed arterial narrowing after TBA for CVS after sSAH.Despite previous assumptions that CB provides for more dilatation in segments adjacent to CVS,we observed no disadvantages concerning long-term adverse effects.Our data support TBA as a low-risk treatment option.

Stroke is the second leading cause of death worldwide with about 50% of survivors being chronically disabled(Donkor,2018).The behavioral improvement seen in stroke patients in the first weeks after a stroke is contributed by behavioral compensation, reorganization in somatotopic maps and activity in peri-infarct but also distant regions which are connected to the stroke area as supported by animal studies.
