Background:Currently,there is no formal consensus regarding a standard classification for gastric cancer(GC)patients withadequate set)***:All patients in this study underwent R0 *** overall survival(OS)difference between the Limited and adequate set from a large Chinese multicenter dataset was *** the 8th American Joint Committee on Cancer(AJCC)pathological nodal classification(pN)for GC as base,a modified nodal classification(N’)resembling similar analogy as the 8th AJCC pN classification was *** performance of the proposed and 8th AJCC GC subgroups was compared and validated using the Surveillance,Epidemiology,and End Results(SEER)dataset comprising of 10,208 multi-ethnic GC ***:Significant difference in OS between the Limited and adequate set(corresponding N0–N3a)using the 8th AJCC system was observed but the OS of N0_(limited)vs.N1_(adequate),N1_(limited)vs.N2_(adequate),N2_(limited)vs.N3_(aadequate),and N3_(alimited)vs.N3_(badequate)subgroups was almost similar in the Chinese ***,we formulated an N’classification whereby only the nodal subgroups of the Limited set,except for pT1N0M0 cases as they underwent less extensive surgeries(D1 or D1+gastrectomy),were re-classified to one higher nodal subgroup,while those of the adequate set remained unchanged(N’0=N0_(adequate)+pT1N0M0_(limited),N’1=N1_(adequate)+N0_(limited)(excluding pT1N0M0_(limited)),N’2=N2_(adequate)+N1_(limited),N’3a=N3_(aadequate)+N2_(limited),and N’3b=N3_(badequate)+N3_(alimited)).This N’classification demonstrated less heterogeneity in OS between the Limited and adequate *** analyses demonstrated superior statistical performance of the pTN’M system over the 8th AJCC edition and was successfully validated using the SEER

INVERSION CALCULATION OF THE WESTERN PACIFIC BOUNDARY CURRENT DURINGOCTOBER, 1988

The inverse method developed by Wunsch (1978) is widely used to determine circulation in the oceans (Fu , 1980; Wunsch , Hu and Grant, 1983 ; Joyce and Wunsch , 1986; Rintoul, 1988). However, for inversion manipulation, measurements down to the bottom are required, and so is a certain solid boundary. In the present paper an equation of potential condition satisfied by the velocity field at the reference level is added to Wunsch’s underdetermined matrix equation in order to make the method adequate for the case where measurements do not touch bottom and when a solid boundary does not exist. The western boundary current in the Pacific is calculated and examined in terms of the velocity field and transport with CTD data gathered by the R/V Science I in October 1988 .

Supporting and retaining competent primary care workforce in low-resource settings:lessons learned from a prospective cohort study

Objective Assess whether local health facilities can adequately support the performance of general practitioners(GPs)trained by China’s national compulsory services programme(CSP).Design Prospective cohort *** Health facilities in middle and western rural areas in China,2015–*** Cohorts of CSP graduates from 2015 to 2019 in four major medical *** outcomes Job performance measured by a 12-item Job Performance Scale;productivity measured by outpatient volume per day;turnover measured by ever changing jobs within the past *** 91.2%,92.0%and 90.5%GPs working in township health centres reported inadequate medication,equipment and external assistance from higher level hospitals,while CSP graduates working in secondary or tertiary hospitals reported a lower rate of less than 60%.The top three tests reported as lacking were blood gases(67.7%),microbiology(61.6%)and cancer biomarkers(49.7%);the top three lacked procedures were CT scan(64.8%),MRI scan(58.1%)and ambulatory BP monitoring(55.8%);and the top three lacked drugs were drugs for cardiovascular diseases(23.3%),systematic hormonal preparations(17.7%)and traditional Chinese medicines(13.0%).Multivariable analysis showed that facility support was positively associated with job performance—adequate medication increased job performance by 2.2 points(95%CI 0.7to 3.8),and adequate external assistance increased job performance by 3.3 points(95%CI 1.8to 4.8).Facility support was also positively associated with productivity—adequate medication increased outpatients seen per day by 20%(95%CI 0.1to 0.3),and adequate equipment increased outpatients seen per day by 12%(95%CI 0.0to 0.2).Facility support did not have significant impact on turnover,but GPs who changed jobs in the past year were 1.9–2.3 times more likely to report adequate facility *** GPs in township health centres experienced a high prevalence of shortage in facility *** identification of a positiv

A model adequacy test should be carried out on the basis of accurate aprioristic ideas about a class of adequate models, as in solving of practical problems this class is final. In article, the quadratic sums entering into the equation of the dispersive analysis are considered and their independence is proved. Necessary and sufficient conditions of existence of adequate models are resulted. It is shown that the class of adequate models is infinite.

As Could We Assure Safety in Large-Scale Manufacturing of Nanoparticles for the Biomedical Use

Nanoparticles provide great advantages but also great risks. Risks associating with nanoparticles are the problem of all technologies, but they increase in many times in nanotechnologies. adequate methods of outgoing production inspection are necessary to solve the problem of risks, and the inspection must be based on the safety standard. Existing safety standard results from a principle of “maximum permissible concentrations or MPC”. This principle is not applicable to nanoparticles, but a safety standard reflecting risks inherent in nanoparticles doesn’t exist. Essence of the risks is illustrated by the example from pharmacology, since its safety assurance is conceptually based on MPC and it has already come against this problem. Possible formula of safety standard for nanoparticles is reflected in many publications, but conventional inspection methods cannot provide its realization, and this gap is an obstacle to assumption of similar formulas. Therefore the development of nanoparticle industry as a whole (also development of the pharmacology in particular) is impossible without the creation of an adequate inspection method. There are suggested new inspection methods founded on the new physical principle and satisfying to the adequate safety standard for nanoparticles. These methods demonstrate that creation of the adequate safety standard and the outgoing production inspection in a large-scale manufacturing of nanoparticles are the solvable problems. However there is a great distance between the physical principle and its hardware realization, and a transition from the principle to the hardware demands great intellectual and material costs. Therefore it is desirable to call attention of the public at large to the necessity of urgent expansions of investigations associated with outgoing inspections in nanoparticles technologies.
