Objective: The prevalence of sport-related concussion(SRC) is high and results in a number of serious health consequences. One area that has received minimal research is the relationship between SRC and sleep. The literature shows that sleep deficiency is a frequent negative consequence of SRC. At the same time, sleep deficiency delays recovery from SRC and contributes added risk of symptom recurrence.A 2014 study of chronic pain patients who learned to apply the complementary and alternative medicine intervention hand self-shiatsu(HSS) had promising, sleep-promoting results that warrant further investigation with other populations. This proof-of-concept study explored the feasibility of HSS as an intervention to promote sleep onset and continuity for young adults with SRC.Methods: This study employed a prospective case-series design, where participants act as their own controls. Baseline and follow-up data included standardized self-reported assessment tools and sleep actigraphy.Results: Seven athletes, aged between 18 and 25 years, participated in the study. Although statistically significant improvement in actigraphy sleep scores between baseline and follow-up was not achieved,metrics for sleep quality and daytime fatigue showed significant improvement.Conclusion: These findings support the hypothesis that HSS has the potential to improve sleep and reduce daytime fatigue in young postconcussion athletes. This pilot study provides guidance to refine research protocols and lays a foundation for further, large-sample, controlled studies.

Background:Developing appropriate concussion prevention and management paradigms in middle school(MS)settings requires understanding parents’general levels of concussion-related knowledge and attitudes.This study examined factors associated with concussion-symptom knowledge and care-seeking attitudes among parents of MS children(aged 10-15 years).Methods:A panel of 1224 randomly selected U.S.residents,aged ≥18 years and identifying as parents of MS children,completed an online questionnaire capturing parental and child characteristics.The parents’concussion-symptom knowledge was measured using 25 questions,with possible answers being“yes”,“maybe”,and“no”.Correct answers earned 2 points,“maybe”answers earned 1 point,and incorrect answers earned 0 point(range:0-50;higher scores=better knowledge).Concussion care-seeking attitudes were also collected using five 7-point scale items(range:5-35;higher scores=more positive attitudes).Multivariable ordinal logistic regression models identified predictors of higher scores.Models met proportional odds assumptions.Adjusted odds ratios(aORs)with 95%confidence intervals(95%CIs)(excluding 1.00)were deemed statistically significant.Results:Median scores were 39(interquartile range:32-44)for symptom knowledge and 32(interquartile range:28-35)for care-seeking attitude.In multivariable models,odds of better symptom knowledge were higher in women vs.men(aOR=2.28;95%CI:1.71-3.05),white/non-Hispanics vs.other racial or ethnic groups(aOR=1.88;95%CI:1.42-2.49),higher parental age(10-year-increase aOR=1.47;95%CI:1.26-1.71),and greater competitiveness(10%-scale-increase aOR=1.24;95%CI:1.13-1.36).Odds of more positive care-seeking attitudes were higher in white/non-Hispanics vs.other racial or ethnic groups(aOR=1.45;95%CI:1.06-1.99)and in older parental age(10-year-increase aOR=1.24;95%CI:1.05-1.47).Conclusion:Characteristics of middle school children’s parents(e.g.,sex,race or ethnicity,age)are associated with their concussion-symptom knowledge and care-seeking attitudes.Parents’variation

Background Left ventricular hypertrophy （LVH） is an independent predictor of morbidity and mortality in chronic hemodialysis （CHD） patients. It remains unclear whether efforts to correct fluid overload in young CHD can reverse LVH. This prospective single-center cohort study evaluated left ventricular masses index （LVMI） evolution in fluid overloaded young CHD patients with or without water-sodium control. Methods A total of 106 young patients aged between 22 and 44 years on CHD were enrolled in this prospective, control study. Patients were divided into three groups according to the percentage of interdialytic weight gain （PIDWG （%） = 100% x （predialysis weight - dry weight）/dry weight） at baseline. Thirty-six patients with PIDWG more than 5% received health education to restrict water-sodium intake strictly （Group I）. Other 36 patients with comparable fluid status levels （the PIDWG more than 5%） did not receive health education to restrict water-sodium intake strictly （Group Ⅱ） and those with PIDWG less than 5% （Group Ⅲ, n=34） were sewed as controls. Echocardiographic studies were performed to evaluate LVMI at baseline and then after 1, 3 and 6 months. The total follow-up time was 6 months. Results There was no significant difference in clinical data （such as age, gender, aetiology of renal failure and nutritional state, etc.） among the three groups at baseline. At baseline, the prevalence of LVH in Group I, Ⅱ and Ⅲ was 75.0%, 72.2% and 55.9%, respectively. LVMI was directly correlated with PIDWG （r = 0.779, P 〈0.01）. After 6 months, the PIDWG in Group I decreased in mean by （3.77±1.09）%, and LVMI decreased in mean by （27.59±12.15） g/m2. The prevalence of LVH decreased in mean by 25.0%, and the blood pressure decreased in mean by （14.69±11.50） mmHg/ （7.14±7.51） mmHg. The medication category and total medication frequency of hypotensive drugs significantly decreased in Group I at 6 months. Background:Daily moderate-to-vigorous physical activity(MVPA)is vital to the physical,mental,and social well-being of children.Early restrictions during the coronavirus disease 2019(COVID-19)pandemic included the closure of schools and physical activity(PA)amenities across the US.This study aimed to examine the impact of the pandemic on the PA and play behavior of U.S.children and to provide evidencebased recommendations to improve their PA.Methods:A cross-sectional,online,parent-reported survey was conducted of children aged 318 years between April and June 2020 to assess light PA and MVPA using a modified Godin Leisure-Time Exercise Questionnaire.Additional items included family/child socioeconomic demographics,child adaptability to the pandemic,and community access.The survey was shared through social media and snowball sampling distribution.Results:Analysis of 1310 surveys indicated child PA scores declined significantly during the pandemic(from 56.6 to 44.6,max 119,p<0.001).Specifically,MVPA score decreased(from 46.7 to 34.7,max 98,p<0.001)while light PA remained the same.Age-based changes were seen in the quantity,variety,and intensity of PA,with the lowest pandemic-related impact seen in preschoolers and the highest in high schoolers(4.7 vs.17.2,p<0.001).Community-based peer PA decreased across all age groups.Conclusion:This study shows decreased PA levels in U.S.children,according to parent reporting,during the COVID-19 pandemic.Recommendations for community leaders,educators,and parents to improve PA in children are provided.With continued spread of COVID-19,these results and recommendations may be imperative to the physical well-being of U.S.children.

Purpose: We aimed to examine the longitudinal associations between parents' and youth's participation in physical activity(PA).Methods-: One hundred and ninety youth cuompleted self-admitiistered questionnaires 3 times per year from 2011 to 2015, and their parents completed an interviewer-administered questionnaire during a telephone interview once in2011-2012. Data on youth's and parents' activities were classified as interdependent or coactive/imdependent.Results: youth with one or both parents who participated in interdependent activities were more likely to maintain participation in interdependent activities(hazard ratio(HR) = 3.63: 95% confidence interval(CI) = 1.30-10.17). youth's sustained participation in coactive/independent activities was not associated with parents' participation in coactive/independent activities(HR= 0.97; 95%CI=0.46-2.06).Conclusion: Longitudinal associations between parents' and youth's participation in PA differed across type of PA. Encouraging parents' participation in interdependent activities may promote sustained participation in interdependent activities in youth.

Background:Children and adolescents can be distinguished by different typologies(clusters) of physical activity and sedentary behavior.How physical activity and sedentary behaviors change over time within different typologies is not known.This study examined longitudinal changes in physical activity and sedentary time among children and adolescents with different baseline typologies of activity-related behavior.Methods:In this longitudinal study(3 annual time points) of children(n=600,age=9.2±0.4 years(mean±SD),50.3% girls) and adolescents(n=1037,age=13.6±1.7 years,48.4% girls),participants were recruited in Spain in 2011-2012.Latent class analyses identified typologies based on self-reported screen,educational,social and relaxing sedentary behaviors,active travel,muscle strengthening activity,and sport at baseline.Within each typology,linear mixed growth models explored longitudinal changes in accelerometer-derived moderate-to-vigorous physical activity and sedentary time,as well as time by class interactions.Results:Three typologies were identified among children( "social screenies",12.8%;"exercisers",61.5%;and "non-sporty active commuters",25.7%) and among adolescents "active screenies",43.5%;"active academics",35.0%;and "non-sporty active commuters",21.5%) at baseline.Sedentary time increased within each typology among children and adolescents,with no significant differences between typologies.No changes in physical activity were found in any typology among children.In adolescents,physical activity declined within all typologies,with "non-sporty active commuters" declining significantly more than "active screenies" over 3 years.Conclusion:These results support the need for interveation to promote physical activity and prevent increases in sedentary time during childhood and adolescence.Adolescents characterized as "non-sporty active commuters" may require specific interventions to maintain their physical activity over time.
