Chinese preparation xuesaitong promotes the mobilization of bone marrow mesenchymal stem cells in rats with cerebral infarction

After cerebral ischemia, bone marrow mesenchymal stem cells are mobilized and travel from the bone marrow through peripheral circulation to the focal point of ischemia to initiate tissue regeneration. However, the number of bone marrow mesenchymal stem cells mobilized into peripheral circulation is not enough to exert therapeutic effects, and the method by which blood circulation is promoted to remove blood stasis influences stem cell homing. The main ingredient of xuesaitong capsules is Panax notoginseng saponins, and xuesaitong is one of the main drugs used for promoting blood circulation and removing blood stasis. We established rat models of cerebral infarction by occlusion of the middle cerebral artery and then intragastrically administered xuesaitong capsules（20, 40 and 60 mg/kg per day） for 28 successive days. Enzyme-linked immunosorbent assay showed that in rats with cerebral infarction, middle- and high-dose xuesaitong significantly increased the level of stem cell factors and the number of CD117-positive cells in plasma and bone marrow and significantly decreased the number of CD54-and CD106-positive cells in plasma and bone marrow. The effect of low-dose xuesaitong on these factors was not obvious. These findings demonstrate that middle- and high-dose xuesaitong and hence Panax notoginseng saponins promote and increase the level and mobilization of bone marrow mesenchymal stem cells in peripheral blood.

Evaluating the consistency of herb injectable formulations could improve their product quality and clinical safety,particularly concerning the composition and content levels of trace ingredients.Panax notoginseng Saponins Injection(PNSI),widely used in China for treating acute cardiovascular diseases,contains low-abundance(10%-25%)and trace saponins in addition to its five main constituents(notoginsenoside R1,ginsenoside Rg1,ginsenoside Re,ginsenoside Rb1,and ginsenoside Rd).This study aimed to establish a robust analytical method and assess the variability in trace saponin levels within PNSI from different vendors and formulation types.To achieve this,a liquid chromatography-triple quadrupole mass spectrometry(LC-MS/MS)method employing multiple ions monitoring(MIM)was developed.A“post-column valve switching”strategy was implemented to eliminate highly abundant peaks(NR_(1),Rg_(1),and Re)at 26 min.A total of 51 saponins in PNSI were quantified or relatively quantified using 18 saponin standards,with digoxin as the internal standard.This study evaluated 119 batches of PNSI from seven vendors,revealing significant variability in trace saponin levels among different vendors and formulation types.These findings highlight the importance of consistent content in low-abundance and trace saponins to ensure product control and clinical safety.Standardization of these ingredients is crucial for maintaining the quality and effectiveness of PNSI in treating acute cardiovascular diseases.

Role of xuesaitong in Amelioration of Neural Function and Alteration of Bax Expression in Rats with Brain Trauma

Traumatic brain injury (TBI) is one of the most common diseases in clinical neurosurgery characterized with high incidence rate, mortality and many complications. Objectives: The purpose of this study is to explore the roles of xuesaitong in the therapeutic effect of brain trauma and alteration of expression in Bax, a kind of promoting apoptosis factor. Methods: The rat traumatic brain injury models were established by using modified free falling body impact method. Thereafter, xuesaitong was employed to be administered to TBI rats, and NSS Score Rating Scale was used to evaluate the effect of xuesaitong. Moreover, real-time PCR was used to detect the Bax expression changes before and after the xuesaitong administration. Results: xuesaitong could accelerate the neurofunctional recovery of TBI rats, accompanied by NSS Scores significant decrease. Simultaneously, it also could inhibit the expression of Bax factor. Conclusions: Xusaitong could markedly ameliorate TBI restoration, in which it promotes the neurofunctional recovery, and at the same time it inhibits the expression of Bax.

The study on the mechanism of Sodium aescinate combined with xuesaitong for cerebral thrombosis treatment

Objective:To observe the mechanism of Sodium aescinate combined with xuesaitong for cerebral thrombosis treatment and offer clinical help to cerebral thrombosis treatment. Methods:A total of 92 patients with cerebral thrombosis were selected and randomly divided into groups: the observation group (46 people) and the control group (46 people).The patient in the control group were treated with conventional therapy and the patients in the observation group were treated with Sodium aescinate combined with xuesaitong on the basis of conventional therapy. Inflammatory factors (IL-6, IL-10, TNF-α and CRP), Nerve cell factor (NSE, NGF and NTF) and blood rheology indexes (Fg, BVH, BVL and PV) were detected and analyzed before and after treatment.Results:The comparison of inflammatory factors, Nerve cell factor and blood rheology indexes in the two groups before treatment were not statistically significant. Inflammatory factors (IL-6, TNF-α and CRP), Nerve cell factor (NSE) and blood rheology indexes (Fg, BVH, BVL and PV) in both groups after treatment significantly decreased compared with that before treatment. Inflammatory factors (IL-10), Nerve cell factor (GF and NTF) in both groups after treatment significantly increased compared with that before treatment. The changes were statistically significant. Inflammatory factors (IL-6, TNF-α and CRP), Nerve cell factor (NSE) and blood rheology indexes (Fg, BVH, BVL and PV) in observation group after treatment decreased more significantly than that in control group, and inflammatory factors (IL-10), Nerve cell factor (GF and NTF) increased more significantly than that in control group. The difference between two groups was considered statistically significant.Conclusion:Sodium aescinate combined with xuesaitong could inhibit the inflammatory reaction, improve the blood flow condition and promote rehabilitation in patients with cerebral thrombosis. So it has a very important clinical significance of the treatment to cerebral hemorrhage.

Effect of xuesaitong Combined with Autologous Platelet-rich Gel on Blood Glucose,AT-III,TNF-αand 24-h Urinary Protein Level in Patients with Diabetic Foot

[Objectives]To investigate the clinical effect of different doses of xuesaitong combined with autologous platelet-rich gel(APG)on patients with diabetic foot(DF).[Methods]90 patients with diabetic foot admitted to our hospital from February 2017 to February 2019 were enrolled in the study.According to the random number table method,the subjects were divided into study group A and B and control group C.Group A was given a low dose of xuesaitong combined with APG,while group B was given high-dose xuesaitong combined with APG and group C was treated only with APG.Patients in the three groups were observed and the changes of related indexes were detected.[Results]After treatment,with regard to the three groups,the fasting blood glucose,2-h postprandial blood glucose level,HbA1c,TNF-α,Hcy,blood urea nitrogen(BUN),creatinine(Cr)and 24-h urine protein levels were all decreased,yet AT-III level was increased,and granulation tissue coverage and thickness,wound clearance rate were increased,while the repair time of ulcer surface was significantly reduced.The above indexes were all significantly different(P0.05).[Conclusions]xuesaitong combined with APG could effectively reduce the blood sugar level of DF patients,improve the clinical indexes,promote wound healing,and the high-dose group had more significant advantages and was worthy of promotion.
