Cyclopexy versus vitrectomy combined with intraocular tamponade for treatment of cyclodialysis

AIM:Tocomparetheeffectsandcomplicationsof direc cyclopexy (DC) versus vitrectomy, endophotocoagulation, and gas/silicone oil endotamponade (VEE) treatment in patients with cyclodialysis and persistent ***: This is a prospective, comparative, non-randomized clinical trial which includes 52 patients with cyclodialysis and persisting hypotony. Fifty-two patients suffering from cyclodialysis and persistent hypotony in one eye were divided into 2 groups (groups DC and VEE) and treated, respectively, with direct cyclopexy or vitrectomy, endophotocoagulation, and gas/silicone oil endotamponade. The patients were followed up for 12 months. Assessments included best corrected visual acuity (BCVA), intraocular pressure (IOP), anterior chamber depth (ACD), anterior chamber volume (ACV) and subjective rating of the pain caused by the ***: After a follow-up of 12 months, significan improvement was seen in postoperative mean BCVA, IOP, ACD and ACV in both treatment groups (which were not significantly different from each other). The success rates for the treatments were not significantly different (DC: 50.0% vs VEE: 62.5% , P =0.383). Postoperative morbidity of cataract and subjective pain rating were significantly higher in the VEE group vs the DC group (P =0.003 and P <0.001 respectively).CONCLUSION: DC and VEE were effective surgica procedures in treating patients with cyclodialysis and persistent hypotony. Patients had better tolerance to DC treatment and VEE was more likely lead to cataract complications. Taking into consideration the ease of the operation, success rate, and patient comfort, DC treatment seems preferable to VEE treatment in patients with simple cyclodialysis. While VEE has the advantage of treating patients with cyclodialysis combined with vitreous hemorrhage.

Effect of intravitreal conbercept treatment before vitrectomy in proliferative diabetic retinopathy

AIM： To evaluate the safety and efficacy of intravitreal conbercept（IVC） injections as pretreatment for pars plana vitrectomy（PPV） in severe proliferative diabetic retinopathy（PDR）. METHODS： This was a retrospective chart review of all patients who underwent PPV for PDR from January 2014 to October 2016. Patients who underwent IVC injection before PPV were assigned to the IVC group; the others were assigned to the control group. The IVC was performed 3-7 d before surgery in the IVC group. All the eyes in the two groups were operated by the same doctor to complete the vitrectomy. Intraoperative complications and the changes in best-corrected visual acuity（BCVA） before and after surgery were compared between the two groups. RESULTS： A total of 68 eyes of 63 patients（22 eyes in the IVC group and 46 eyes in the control group） were examined. The risk of intraoperative bleeding was lower in the IVC group（2/22） than in the control group（25/46, P=0.000）. Furthermore, the use of endodiathermy was significantly lower in the IVC group（1/22） than in the control group（12/46, P=0.047）. The surgical time in the IVC group（112.64±34.52 min） was significantly shorter than in the control group（132.85±40.04 min, P〈0.05）. Compared to the BCVA before surgery, the mean BCVA was significantly improved after surgery for both groups（P〈0.05）. CONCLUSION： PPV is an effective treatment and can improve vision in patients with PDR. Preoperative intravitreal injection of conbercept could reduce the chances of intraoperative bleeding and the use of endodiathermy and shorten the operative time, which are beneficial in the management of PDR.

Long-term outcome of pathologic myopic foveoschisis treated with posterior scleral reinforcement followed by vitrectomy

AIM: To report the long-term outcome of posterior scleral reinforcement(PSR) followed by vitrectomy for pathologic myopic foveoschisis(MF).METHODS: The records of 27 patients(44 eyes) treated with posterior scleral reinforcement(PSR) followed by vitrectomy for pathologic MF were retrospectively *** best-corrected visual acuity(BCVA), refractive error, axial length, and spectral-domain optical coherence tomography findings and complications were ***: Forty-four eyes of 27 patients were included in this study. The follow-up period was 47.98±18.23mo(24-83mo). The mean preoperative BCVA(logMAR) was 1.13±0.63, and the mean postoperative BCVA was 0.30±0.33at the last visit. There showed a significant improvement in BCVA postoperatively(Pvitrectomy is an effective procedure to improve the visual acuity and the anatomical structure of macula. It can also stabilize the axial length for a long time.

Primary rhegmatogenous retinal detachment:evaluation of a minimally restricted face-down positioning after pars plana vitrectomy and gas tamponade

AIM:To evaluate the safety and efficacy of a minimally restricted face-down postoperative positioning following pars plana vitrectomy(PPV)with gas tamponade for primary rhegmatogenous retinal detachment(RRD).METHODS:Patients with primary RRD treated with PPV and gas tamponade and followed up for at least 6 mo were selected for the *** phakic eyes underwent simultaneous cataract *** patients were required to be in a postoperative position that prevented downward flow of retinal *** with macular detachment were positioned face-down for only a couple of *** patients were assessed for preoperative and postoperative best-corrected visual acuity(BCVA),anatomical retinal reattachment rate,and postoperative ***:In total,40 eyes of 39 patients with primary RRD were included in the study.A single tear was present in 30 eyes(75.0%),multiple retinal tears were present in nine eyes(22.5%),and oral dialysis was present in one eye(2.5%).The anatomical success rate was 90.0%(36 cases)after the primary surgery,and the final anatomical success rate was 100%.The BCVA improved significantly(P<0.001)from 0.75 logarithm angle of resolution(log MAR)preoperatively to 0.12 log MAR at the final *** complications included intraocular pressure elevation(≥25 mm Hg)in 11 patients(27.5%),fibrin formation in two patients(5.0%),pupillary capture of the intraocular lens in two patients(5.0%),and posterior synechia in one patient(2.5%).CONCLUSION:A minimally restricted face-down and flexible postoperative positioning after PPV and gas tamponade for primary RRD is effective and safe.

Non-buckled vitrectomy for retinal detachment with inferior breaks and proliferative vitreoretinophathy

AIM: To investigate the efficacy of non-buckled vitrectomy with classical endotamponade agents in the treatment of primary retinal detachment (RD) complicated by inferior breaks and proliferative vitreoretinophathy (PVR). METHODS: A retrospective, consecutive and case series study of 40 patients with inferior break RD and PVR >= C1 was conducted. All patients underwent a standard 3-port 20-gauge pars plana vitrectomy (PPV) with gas or silicone oil tamponade without supplementary scleral buckling. The vitreous and all proliferative membrane were completely removed, and retinectomy was performed when necessary. The mean follow-up was 12.5 months. The primary and final anatomic success rate, visual acuity and complications were recorded and analyzed. RESULTS: Primary anatomic success rate was achieved in 35 of 40 eyes (87.5%) and the final anatomic success rate was 100%. The most common cause of redetachment was recurrent PVR. The best-corrected visual acuity (BCVA) at final follow-up was improved in 34 eyes (85%), remained stable in 1 eye (2.5%), and worsened in 5 eyes (12.5%). The mean visual acuity at final follow-up was improved significantly (P=0.000). CONCLUSION: This retrospective study provides evidence that vitrectomy without scleral buckling seemed to be an effective treatment for inferior break RD with PVR. With complete removal of vitreous and proliferative membranes and timing of retinectomy, the inferior breaks which complicated with PVR could be dosed successfully without additional sclera! buckling.

Concurrent removal of intravitreal lens fragments after phacoemulsification with pars plana vitrectomy prevents development of retinal detachment

AIM: To evaluate the outcomes of "concurrent vitrectomy" to retrieve dislocated lens fragment during ***: In a retrospective, observational case series, data of patients who underwent "concurrent" pars plana vitrectomy(PPV) for dislocated lens fragments between the period 2000 and 2008 were reviewed. Data collected included patient demographics, pre-operative visual acuity, intra-operative occurrence of retinal breaks, duration of follow up, post-operative intraocular pressure, final best-corrected visual acuity(BCVA),presence of cystoid macular edema(CME) and occurrence of rhegmatogenous retinal detachment(RRD).· RESULTS: A total of 58 eyes of 58 patients were included in the study. At 12 mo the mean postoperative BCVA was log MAR 0.17(20/30) with a range of log MAR0 to 0.69(20/20 to 20/100), with 96.6%(56/58) of patients showing post- operative improvement in visual acuity(P =0.005). None of the patients developed postoperative retinal detachment, endophthalmitis or non-resolving uveitis at 12 ***: Our study results suggest concurrent PPV for retained lens fragments after cataract surgery is beneficial and may decrease the risk of glaucoma and prevent development of RRD.

Endotamponades in pars plana vitrectomy for metallic intraocular foreign body associated with endophthalmitis

AIM: To study the criterion-reference of endotamponades in pars plana vitrectomy for metallic intraocular foreign body (MIOFD) associated with endophthalmitis. METHODS:Thirty-six patients of MIOFD with endophthalmitis accorded with Exclusion and inclusion criteria were retrospectively analyzed. A detailed analysis of the patients' natural factors, preoperative examinations, intraoperative endotamponades choice, postoperative complications and therapeutic effects was performed. RESULTS: BSS was used in 4 eyes without obvious retinal damage. There was no postoperative complication and their visual acuity (VA) was improved. Sixteen eyes that had mild retinal damage filled with C3F8 gas. The postoperative VA improved in 10 eyes (62.5%), 4 eyes (25.0%) remained unchanged and 2 eyes (12.5%) decreased. Only 2 cases occurred postoperative retinal detachment in gas group. Another 16 eyes with serious retinal damage were treated with silicone oil. Postoperative VA of 9 eyes (56.3%) improved, 3 eyes (18.8%) remained unchanged and 4 eyes (25.0%) decreased. The silicone oil group had higher incidence of postoperative complications, but the incidence of secondary treatment had no significant different between silicone oil and gas group. CONCLUSION: An appropriate choice of endotamponades in vitrectomy surgery for MIOFB with endophthalmitis is important for prognosis.

Ocular trauma treated with pars plana vitrectomy: early outcome report

AIM： To evaluate demographic variables and visual outcomes, among patients with ocular injuries involving the posterior segment, managed with pars plana vitrectomy.· METHODS： The records of patients were studied retrospectively from March to September 2010, to determine the age, gender, place of occurrence of trauma, visual acuity, anatomical site, nature of injury,wound length, the presence of an afferent pupillary defect, and the timing of vitrectomy. The Ocular Trauma Score was measured. The minimum follow-up from presentation was 6mo.·RESULTS： Ninety patients（77 males, 13 females）, with a mean age of 32.7 ±15.8y were included over the 6-month period. The majority of cases occurred in the workplace（47 patients）, followed by home（14 patients）.The mean visual acuity（log MAR） of patients significantly improved from 2.36 ±0.72 preoperatively to 1.50 ±1.14 postoperatively. Twenty-three patients had preoperative vision better than 2.0 log MAR, the postoperative visual acuity was significantly better among these patients than patients with worse than 2.0 log MAR（P 〈0.001）. Visual improvement between groups with early vitrectomy（〉7d）and delayed vitrectomy（〉7d） was not significantly different（P =0.66）. Postoperative visual acuity was not significantly different between patients with injury in Zone I and II（P =0.64）, but patients with injury in Zone III had significantly poorer visual acuity（P =0.02）. Patients with relative afferent pupillary defect had significantly poorer postoperative visual acuity（P =0.02）. Preoperative visual acuity, the difference of preoperative and postoperative visual acuity, and postoperative visualacuity were significantly different between groups with different ocular trauma scores（P〈 0.001）.·CONCLUSION： Trauma is more likely to occur in men under 40 y of age and in the workplace. AIM: To evaluate serum concentrations of angiogenesis-related cytokines in proliferative diabetic retinopathy (PDR) before and after vitrectomy. METHODS: Serum samples were collected from 30 PDR patients with varying severity before and after vitrectomy. Serum concentrations of vascular endothelial growth factor (VEGF), pigment epithelium-derived factor (PEDF), interleukin-8 (IL-8) and interferon-inducible protein-10 (IP-10) were determined by enzyme-linked immunosorbent assays(ELISA). RESULTS: Serum concentrations of VEGF, PEDF, IL-8 and IP-10 were significantly higher in PDR patients than that in controls, respectively (Pvitrectomy. CONCLUSION: Elevated cytokines levels in serum may be diagnostically useful in PDR. Angiogenesis-related cytokines play important roles in the development of PDR, and would instruct the risk assessment of pathogenetic condition in PDR patients.
