Clinical validation of Global Coagulation Tests to Guide Blood Component Transfusions in Cirrhosis and ACLF

Background and Aims:Patients with cirrhosis and acuteon-chronic liver failure(ACLF)may have bleeding complications and need for invasive procedures.Point-of-care(POC)coagulation tests like thromboelastography(TEG)and Sonoclot may be better for guiding patient management than the standard coagulation tests(SCTs),like prothrombin time,platelet count and international normalized ratio.Methods:We prospectively compared and validated the POC tests and SCTs in 70 persons with ACLF and 72 persons with decompensated cirrhosis who had clinical bleeding and checked for episodes of re-bleeding and transfusion requirements.We assessed pre-procedure requirement of blood components when correction was done based on an SCT or POC strategy.Results:Episodes of bleeding were seen in 45%and 28%of ACLF and cirrhosis patient,respectively(p=0.036),with the major site of bleeding being gastrointestinal(31%and 16%,respectively).Platelet counts correlated with TEG-maximum amplitude in cirrhosis(p=0.045)and prothrombin time correlated positively with TEG-reaction(R)time(p=0.032),TEG-Clot kinetics(K)time(p=0.042),Son-activated clotting time(p=0.038)and negatively with clot rate(p=0.043)in ACLF,making these correctable target variables in POC transfusion algorithms.Of 223 procedures,transfusion of fresh frozen plasma and platelet concentrate was reduced by 25%(p=0.035)and 20.8%(p=0.045)by using a POC strategy in 76 patients.Correction of deranged Son-activated clotting time and TEG-reaction time was noted in 68%and 72%after 24 h of fresh frozen plasma transfusion in ACLF and 85%and 80%in cirrhosis,respectively.Conclusions:Our study clinically validates that POC tests can better detect coagulation defects and transfusion thresholds in ACLF and cirrhosis,whereas use of conventional tests appear to be less suitable in patients with clinical bleeding.Trial Registration:NCT04332484.

A Bayesian validation approach to practical boson sampling

Boson sampling is a promising candidate for demonstrating quantum supremacy. The validation that involves judging whether a quantum setup outputs photons following the boson sampling model is an essential task in the experiments. However, the current validation methods may result in an incorrect conclusion being reached in realistic experiments, in which no ideally identical photons exist. Accordingly, this study proposes a slope-based approach, which is an extended Bayesian validation, to model the degree of photon indistinguishability. Through numerical simulations and performance evaluations, we demonstrate that the proposed approach can correctly validate boson sampling against the distribution of classical particles. In addition to offering a useful approach for validation, our research indicates that physicists should pay more attention to the quality of photon indistinguishability in boson sampling experiments.

Verification and validation of detonation modeling

The mathematical model used to describe the detonation multi-physics phenomenon is usually given by highly coupled nonlinear partial differential equations. Numerical simulation and the computer aided engineering (CAE) technique has become the third pillar of detonation research, along with theory and experiment, due to the detonation phenomenon is difficult to explain by the theoretical analysis, and the cost required to accredit the reliability of detonation products is very high, even some physical experiments of detonation are impossible. The numerical simulation technique can solve these complex problems in the real situation repeatedly and reduce the design cost and time stunningly. But the reliability of numerical simulation software and the serviceability of the computational result seriously hinders the extension, application and the self-restoration of the simulation software, restricts its independently innovational ability. This article deals with the physical modeling, numerical simulation, and software development of detonation in a unified way. Verification and validation and uncertainty quantification (V&V&UQ) is an important approach in ensuring the credibility of the modeling and simulation of detonation. V&V of detonation is based on our independently developed detonation multiphysics software-LAD2D. We propose the verification method based on mathematical theory and program function as well as availability of its program execution. validation is executed by comparing with the experiment data. At last, we propose the future prospect of numerical simulation software and the CAE technique, and we also pay attention to the research direction of V&V&UQ.

Objective assessment of bradykinesia in Parkinson’s disease using evolutionary algorithms:clinical validation

Background:There is an urgent need for developing objective,effective and convenient measurements to help clinicians accurately identify bradykinesia.The purpose of this study is to evaluate the accuracy of an objective approach assessing bradykinesia in finger tapping(FT)that uses evolutionary algorithms(EAs)and explore whether it can be used to identify early stage Parkinson’s disease(PD).Methods:One hundred and seven PD,41 essential tremor(ET)patients and 49 normal controls(NC)were recruited.Participants performed a standard FT task with two electromagnetic tracking sensors attached to the thumb and index finger.Readings from the sensors were transmitted to a tablet computer and subsequently analyzed by using EAs.The output from the device(referred to as"PD-Monitor")scaled from−1 to+1(where higher scores indicate greater severity of bradykinesia).Meanwhile,the bradykinesia was rated clinically using the Movement Disorder Society-Sponsored Revision of the Unified Parkinson’s Disease Rating Scale(MDS-UPDRS)FT item.Results:With an increasing MDS-UPDRS FT score,the PD-Monitor score from the same hand side increased correspondingly.PD-Monitor score correlated well with MDS-UPDRS FT score(right side:r=0.819,P=0.000;left side:r=0.783,P=0.000).Moreover,PD-Monitor scores in 97 PD patients with MDS-UPDRS FT bradykinesia and each PD subgroup(FT bradykinesia scored from 1 to 3)were all higher than that in NC.Receiver operating characteristic(ROC)curves revealed that PD-Monitor FT scores could detect different severity of bradykinesia with high accuracy(≥89.7%)in the right dominant hand.Furthermore,PD-Monitor scores could discriminate early stage PD from NC,with area under the ROC curve greater than or equal to 0.899.Additionally,ET without bradykinesia could be differentiated from PD by PD-Monitor scores.A positive correlation of PD-Monitor scores with modified Hoehn and Yahr stage was found in the left hand sides.Conclusions:Our study demonstrated that a simple to use device employing classifiers derived from EAs could n

Rasch validation of the Chinese parent-child interaction scale (CPCIS)

Background Proper parent-child interaction is crucial for child development, but an assessment tool in Chinese is currently lacking. This study aimed to develop and validate a parent-reported parent-child interaction scale for Chinese preschool children. Methods The Chinese parent-child interaction scale (CPCIS) was designed by an expert panel based on the literature and clinical observations in the Chinese context. The initial CPCIS had 14 parent-child interactive activity items. Psychometric properties of the CPCIS were examined using the Rasch model and confirmatory factor analysis (CFA). Convergent validity was investigated by the associations between CPCIS and family income, maternal education level, and children's school readiness. Results The study recruited 567 Chinese parent-child pairs from diverse socioeconomic backgrounds, who completed the CPCIS. Six out of the 14 items in the initial CPCIS were dropped due to suboptimal fi t values. The refined 8-item CPCIS was shown to be valid and reliable by Rasch models and CFA. The person separation reliability and Cronbach's α of the CPCIS were 0.81 and 0.82, respectively. The CPCIS scores were positively associated with family's socioeconomic status (η2 = 0.05,P < 0.001), maternal education level (η2 = 0.08,P < 0.001), and children's school readiness (η2 = 0.01,P < 0.01). Conclusion CPCIS is an easily administered, valid, and reliable tool for the assessment of parent-child interactions in Chinese families.

Dynamic simulation of pilot thickener operation using phenomenological model with results validation for continuous and discontinuous tests

The phenomenological theory of sedimentation-thickening processes predicts the settling behavior of a flocculated suspension in dependence of two functions, the batch flux density function and the effective solid stress. These functions were determined using batch settling tests. The governing equations for sedimentation were then solved numerically for these functions and the predictions were compared to the experimental results from pilot scale thickener tests. Firstly, the continuous tests were performed in the plexiglass pilot thickener at different feed flow rates and discharge rates and the solid volume fraction of discharge, the bed height and the time were recorded for each condition. These tests were also simulated and it was observed that there is a good agreement between the results of continuous tests and the results of dynamic simulation. Secondly, the discontinuous tests were performed in the plexiglass pilot thickener at different feed flow rates with a discharge rate of zero. The bed formation rate was determined for each condition. These tests were also simulated and it was observed that there is a good agreement between the results of discontinuous tests and the results of simulation.

Background:Clinical outcome of adrenocortical carcinoma(ACC)varies because of its heterogeneous nature and reliable prognostic prediction model for adult ACC patients is limited.The objective of this study was to develop and externally validate a nomogram for overall survival(OS)prediction in adult patients with ACC after surgery.Methods:Based on the data from the Surveillance Epidemiology,and End Results(SEER)database,adults patients diagnosed with ACC between January 1988 and December 2015 were identified and classified into a training set,comprised of 404 patients diagnosed between January 2007 and December 2015,and an internal validation set,com-prised of 318 patients diagnosed between January 1988 and December 2006.The endpoint of this study was OS.The nomogram was developed using a multivariate Cox proportional hazards regression algorithm in the training set and its performance was evaluated in terms of its discriminative ability,calibration,and clinical usefulness.The nomogram was then validated using the internal SEER validation,also externally validated using the Cancer Genome Atlas set(TCGA,82 patients diagnosed between 1998 and 2012)and a Chinese multicenter cohort dataset(82 patients diag-nosed between December 2002 and May 2018),respectively.Results:Age at diagnosis,T stage,N stage,and M stage were identified as independent predictors for OS.A nomo-gram incorporating these four predictors was constructed using the training set and demonstrated good calibration and discrimination(C-index 95%confidence interval[CI],0.715[0.679-0.751]),which was validated in the internal validation set(C-index[95%CI],0.672[0.637-0.707]),the TCGA set(C-index[95%CI],0.810[0.732-0.888])and the Chi-nese multicenter set(C-index[95%CI],0.726[0.633-0.819]),respectively.Encouragingly,the nomogram was able to successfully distinguished patients with a high-risk of mortality in all enrolled patients and in the subgroup analyses.Decision curve analysis indicated that the nomogram was clinically useful and applicable.Conclusions:T

In parallel real-time database systems, concurrency control protocols must satisfy time constraints as well as the integrity constraints. The authors present a validation concurrency control(VCC) protocol, which can enhance the performance of real-time concurrency control mechanism by reducing the number of transactions that might miss their deadlines, and compare the performance of validation concurrency control protocol with that of HP2PL(High priority two phase locking) protocol and OCC-TI-WAIT-50(Optimistic concurrency control-time interval-wait-50) protocol under shared-disk architecture by simulation. The simulation results reveal that the protocol the author presented can effectively reduce the number of transactions restarting which might miss their deadlines and performs better than HP2PL and OCC-TI-WAIT-50. It works well when arrival rate of transaction is lesser than threshold. However, due to resource contention the percentage of missing deadline increases sharply when arrival rate is greater than the threshold.

Development and validation of RP-HPLC and RP-UPLC methods for quantification of parathyroid hormones (1-34) in medicinal product formulated with meta-cresol

Rapid and sensitive reversed phase high performance liquid chromatography (RP-HPLC) and ultra performance liquid chromatography (RP-UPLC) method with UV detection has been developed and validated for quantification of parathyroid hormone (PTH) in presence of meta-cresol as a stabilizer in a pharmaceutical formulation.Chromatography was performed with mobile phase containing 0.1% Trifluoroacetic acid (TFA) in MilliQ water and 0.1% TFA in acetonitrile with gradient program and flow rate at 0.3 mL/min for HPLC and 0.4 mL/min for UPLC.Quantification was accomplished with internal reference standard (qualified against innovator product and National Institute for Biological Standards and Control (NIBSC) standard).The methods were validated for linearity (correlation coefficient 0.99),range,accuracy,precision and robustness.Robustness was confirmed by considering three factors;mobile phase composition,column temperature and flow rate/age of mobile phase.Intermediate precision was confirmed on different equipments,different columns and on different days.The relative standard deviation (RSD) (<2% for RP-HPLC and <1% for UPLC,n=30) indicated a good precision.Retention time was found about 17 min and 2 min by HPLC and UPLC methods,respectively.Both methods are simple,highly sensitive,precise and accurate and have the potential of being useful for routine quality control.

Comparison and validation of SRTM and ASTER GDEM for a subtropical landscape in Southeastern China

This paper evaluates the quality characteristics of existing versions of 3-arc-second SRTM and 1-arc-second GDEM over Anji County,Zhejiang,China using reference elevations from a high-quality 1:10 k topographic map.Results show that SRTM has higher accuracy(RMSE=12.44 m)than GDEM(RMSE=14.20 m for Version 1,12.76 m for Version 2);however,unsatisfactory void filling and an overall 1/2 pixel shift exits in SRTM version 4.1.Although spurious elevations over omitted water bodies still persist,GDEM Version 2 demonstrated significant improvement over Version 1.Accuracies of both SRTM and GDEM decrease on steeper slopes.Aspect also influences both the magnitude and the sign of errors.DEM accuracy in non-forested areas is considerably higher than that in forested areas.SRTM version 4.1 and GDEM version 2 possessed actual spatial resolutions of 90 m,despite both of them failed to match their nominal accuracies.SRTM Version 4.1 could be the first choice,while ASTER GDEM Version 2 would be a good alternative for areas where extensive voids exist in SRTM.
