Effect of precise nursing service mode on postoperative urinary incontinence prevention in patients with prostate disease

BACKGROUND Patients with benign prostatic disease often experience detrusor morphological changes and dysfunction.In severe cases,it leads to bladder detrusor dysfunction,resulting in dysuria,frequent urination,urgent urination,incomplete urination,and other symptoms including renal function injury.An operation to restore normal urination function and to control postoperative complications,as far as possible,is the most common method for benign prostatic disease.AIM To observe the effect of precise nursing service mode on postoperative urinary incontinence prevention in patients with prostate disease.METHODS In total,130 patients diagnosed with benign prostatic disease,from January 2018 to June 2021,in our hospital,were selected and divided into observation and control groups according to their treatment options.Sixty-five cases in the control group were given routine nursing mode intervention and 65 cases in the observation group received precise nursing service mode intervention.The intervention with the observation group included psychological counseling about negative emotions,pelvic floor exercises,and post-hospital discharge care.The complications of the two groups were counted,and the general postoperative conditions of the two groups were recorded.The urinary flow dynamics indexes of the two groups were detected,and differences in clinical international prostate system score(IPSS)and urinary incontinence quality of life questionnaire(I-QOL)scores were evaluated.RESULTS Postoperative exhaust time(18.65±3.23 h and 24.63±4.51 h),the time of indwelling catheter(4.85±1.08 d and 5.63±1.24 d),and hospitalization time(8.78±2.03 d and 10.23±2.28 d)in the observation group were lower than in the control group.The difference was statistically significant(Purinary flow rate(Qmax)increased(P0.05)compared to pre-operation.The Qmax of the observat

The prevalence,associated factors,and impact of urinary incontinence in pregnant and postpartum women in Nanjing,China:A cross-sectional study

Objective:urinary incontinence(UI)is highly prevalent in antenatal and postnatal women while the prevalence of UI varied largely from 3.84%to 38.65%.This study was to assess the prevalence of UI,the associated factors,and the impact of UI on daily life in pregnant and postpartum women in Nanjing,China.Methods:The prevalence of UI and the impact of UI on life were assessed by the validated Chinese version of International Consultation on incontinence Questionnaire-urinary incontinence-short form and the validated Chinese version of urinary incontinence quality of life.The associated factors were estimated by using logistic regression analysis.Results:UI affected 37.80%of pregnant women and 16.41%of postpartum women of the study population.Among the pregnant participants,the prevalence rates of stress UI,urge UI,and mixed UI were 25.77%,4.47%,and 7.10%,respectively.Among the postpartum women,the prevalence rates of stress UI,urge UI,and mixed UI were 11.15%,1.92%,and 2.69%,respectively.In both pregnant women and postpartum women,vaginal delivery had significantly increased the odds of reporting UI(p=0.007,p=0.003,respectively).The impact of UI on daily life was significantly greater in postpartum women compared to pregnant women especially in social embarrassment(p=0.000).Conclusion:The prevalence rates of UI were high in pregnant women in Nanjing,China.Vaginal delivery significantly increased odds of reporting UI.UI has a great impact on pregnant and postpartum women’s life,especially in social embarrassment.

Prevalence and risk factors of urinary incontinence in Fuzhou Chinese women

Background We randomly sampled a healthy community to evaluate the prevalence and associated risk factors of urinary incontinence. Methods The survey was performed in Fuzhou, China. Of women over twenty years of age in the city, 3.0% were randomly selected and 4684 evaluated by Bristol Female urinary Tract Symptoms Questionnaire. Results Of the women in Fuzhou, 19.0% had urinary incontinence. The prevalence of stress incontinence, urge incontinence and mixed incontinence was 16.6% (n=777), 10.0% (n=468), 7.7% (n=360) respectively. The prevalence of the three types of urinary incontinence increased significantly with age (P2 (2.1, 1.5-2.9), hypertension (2.7, 1.4-5.6), constipation (2.6, 1.8-3.8), alcohol consumption (4.7, 1.1-20.2), episiotomy (1.7, 1.4-2.0), higher body mass index (BMI, 1.8, 1.5-2.2) and unskilled worker (0.7, 0.5-0.8) were potential risk factors for stress incontinence. Urge incontinence was associated with age (OR, 1.3, 95%CI, 0.9-1.3), menopause (1.6, 1.1-2.4), Caesarean delivery (0.2, 0.1-0.5), parity >2 (2.6, 1.8-3.8), constipation (2.3, 1.4-3.7), foetal birthweight (1.7, 1.1-2.4), episiotomy (1.4, 1.1-1.8), higher BMI (1.5, 1.2-2.0) and unskilled worker (0.7, 0.5-0.9). Conclusions The prevalence of urinary incontinence and its subtypes in Chinese women is lower than that of occidental women. In China, age, vaginal delivery, parity, hypertension, constipation, alcohol consumption, episiotomy, higher BMI are potential risk factors for stress incontinence. Urge incontinence is associated with age, menopause, Caesarean delivery, parity, constipation, foetal birthweight, episiotomy, higher body mass index.

Management of urinary incontinence before and after Total Pelvic Reconstruction for Advanced Pelvic Organ Prolapse with and without incontinence

Background： The effectiveness of an anti-incontinence procedure concomitant with prolapse reconstruction for pelvic organ prolapse （POP） in preventing urinary incontinence （UI） after surgery remains controversial. Our study aimed to describe the incidence of pre- and postoperative UI for pelvic reconstructive surgery and evaluate the management of POP associated with U1. Methods： A total of 329 patients who underwent total pelvic reconstruction between June 2009 and February 2015 at a single institution were identified. These patients were divided into two groups （Group A [Prolift reconstruction]： n = 190 and Group B [modified total pelvic reconstruction]： n = 139）. Data regarding surgical procedures and patient demographic variables were recorded. Chi-square and Student＇s t-tests were used for two independent samples. Results： A total of 115 patients presented with UI preoperatively. The average follow-up time was 46.5 months, with 20 patients lost to tbllow-up （6.1%）. The cure rates of stress UI （SUI）, urgency UI （UUI）, and mixed UI （MUI） were 51% （30/59）, 80% （16/20）, and 48% （14/29）, respectively. The cure rate of UUI after total pelvic reconstruction （80% [16/20]） was higher than that of SUI （50.8% [30/59], χ2 = 5.219, P = 0.03）, and the cure rate of MUl （48%, 14/29） was the lowest. The cure rate of patients with UI symptoms postoperatively was lower than that of those with symptoms preoperatively （9.1% [28/309] vs. 16.2% [50/309], χ2 = 7.101, P = 0.01）. There was no difference in the incidence of Ul postoperatively between Groups A and B （P 〉 0.05）. The cure rate of SUl in patients undergoing tension-free vaginal tape-ohturator was not higher than that in those who did not undergo the procedure （42.9% [6/14] vs. 53.3% [24/45], χ2 = 0.469, P = 0.49）. There were no differences in the cure rate for POP or U1 between these two types of reconstructions （P 〉 0.05）. Conclusions： No correlation between the incidence of UI and POP was identified. Role of different childbirth strategies on pelvic organ prolapse and stress urinary incontinence： a prospective study

Background Traumatic damage to fascial and muscular support structures during childbirth may be a major factor of the development of stress urinary incontinence （SUI） and pelvic organ prolapse （POP）. The aim of this study was to prospectively investigate the role of different childbirth strategies on POP and SUI. Methods A total of 120 selected pregnant women were recruited and divided into two groups： vaginal delivery group （n=72） and selective cesarean delivery group （n=48）. The women were questioned with SUI sheet and estimated with POP quantification （POP-Q） within 6-8 weeks after delivery. The correlations of SUI and results of POP-Q with the strategies of delivery were analyzed, ttest, X2 test, and the Mann-Witney test were used for statistical analysis. Results Among the 120 women, SUI was developed during 29-30 gestational weeks in 20 patients （16.6%）, of whom 14 （70%） had symptoms of SUI till 6 weeks postpartum. Totally 20 （16.6%） women had SUI symptoms after delivery. The prevalence of postpartum SUI was positively correlated with the occurrence of SUI during 29-30 gestational weeks （P〈0.001）. In the vaginal delivery group, 100% of the women suffered form urinary prolapse 6 weeks postpartum, while 87.5% of those in the selective cesarean delivery group developed POP （P〈0.01）. The percentages of the first- and second-degree urinary prolapse in the vaginal delivery group were 20.8% and 79.2% respectively, which were significantly different from those in the selective cesarean delivery group （64.6% and 22.9%; both P=0.000）. Conclusions Pregnancy and delivery play an etiologic role in the development of SUI and POP. The onset of SUI during late pregnancy implies a significant risk of postpartum SUI symptoms. The prevalence of POP is significantly higher after vaginal delivery than after cesarean delivery.

Combination therapy for male erectile dysfunction and urinary incontinence

urinary incontinence （UI） and erectile dysfunction （ED） are both very prevalent conditions. Insertion of an artificial urinary sphincter （AUS） and penile prosthesis （PP） is an effective and proven method of treatment for both conditions. With advancing age, as well as with increasing populations of patients radically treated for prostate cancer, the occurrence of both conditions found in the same patient is increasing. The purpose of this article was to analyze the available evidence for simultaneous surgical management of male ED and UI using prosthetic devices. The existing literature pertaining to dual implantation of AUS and PP was reviewed. The concomitant insertion of the PP with the male perineal sling was also considered. Concurrent ED and UI are increasingly seen in the post radical prostatectomy population, who are often younger and less willing to suffer with these conditions. Insertion of an AUS and PP, either simultaneously or as a two-stage procedure, appears to be a safe, efficacious and long-lasting method of treatment. The improvements in design of both the AUS and PP as well as the development of the single transverse scrotal incision have made simultaneous insertion of these prostheses possible. Dual implantation of the PP and male sling looks promising in a selected population. In conclusion, the insertion of the AUS and PP for the treatment of concurrent UI and ED is safe and effective. Simultaneous insertion of these prostheses in the same patient offers potential advantages in operative and recovery time and is associated with high patient satisfaction. Combination therapy should therefore be included in the arsenal of treatment of these conditions.

INTRODUCTIONRadical cystectomy for bladder and urinary diversion is considered the gold standard for treatment of muscular invasive bladder cancer or high-risk nonmuscular invasive bladder cancer. urinary diversion is usually classified into three types： orthotopic neobladder, ileal conduit or sigmoid conduit, and cutaneous ureterostomy Orthotopic neobladder is a better choice for urinary diversion than the other types because of its nonurinary diversion and better quality of life when this method is feasible for patients. One of the complications after creating an orthotopic neobladder is urinary incontinence. With an increasing amount of orthotopic neobladder procedures, more patients are suffering from urinary incontinence. Therefore, an article is required for diagnosis and treatment of urinary incontinence after the orthotopic ileal neobladder procedure.

Pressure assessment in intercavernous embedding of bulboperineal urethra for treatment of urinary incontinence after prostatic operation

Aim: To put forward criteria for the pressure assessment in the operation of intercavernous embedding of bulboper-ineal urethra for the treatment of urinary incontinence after prostatic operation. Methods: A F14 urethral catheter isinserted during the operation and upon suturing the corpora cavernosa centrally, the catheter is slowly pushed in andpulled out in order that the operator feels a certain degree of close-fit resistance. The degree of tightness of the stitches,which regulate the compression pressure, is adjusted in accordance with this close-fit sensation. To further ascertain theadequacy of the force of compression, the bladder is filled with 300 ml physiological saline and observe the appropriate-ness (size and continuity) of the outflow stream when the lower abdomen is depressed with a pressure of 80 - 90 cmH_2O. The operation was given to six patients suffered from urinary incontinence for 20 or more months after prostaticoperation. Results: Five cases achieved complete recovery, while the therapeutic effect of the 6th one was not sat-isfactory . A second stage operation was carried out 3 months later with the addition of one more stitch both proximallyand distally to reinforce the compression force. The condition was improved dramatically. The follow-up period aver-aged 3.5 years. Conclusion; The adequacy of the compression pressure exerted by the juxtaposed corpora cavernosais the key point determining the outcome of the operation. The measures for assessing the compression pressure suggest-ed by the authors are helpful in obtaining the good results of the present paper (6/6 success) as compared with 25/34success in the previous report. Contemporary diagnostics and treatment options for female stress urinary incontinence

Stress urinary incontinence is not a deadly disease,but for the large population of women suffering from it,it is a very important issue.Especially in the continuously aging population all over the world,there is more and more need for treatment of this serious medical condition.Treatment of female stress urinary incontinence exists already for ages.In the 20th century invasive treatments like Burch colposuspension and pubovaginal slings were the mainstay of surgical treatments.The introduction of the midurethral sling made the procedure less invasive and accessible for more caregivers.Luckily there are many options available and the field is developing quickly.In recent years many new medical devices have been developed,that increase the number of treatment options available and make it possible to find a suitable solution for the individual patient based on subjective and objective results and the chances of complications.This manuscript provides an introduction to the therapeutical options that are available nowadays for female stress urinary incontinence.
