Percutaneous nephrostomy versus ureteral stent in hydronephrosis secondary to obstructive urolithiasis:A systematic review and meta-analysis

Objective: To assess if there is a preferable intervention between retrograde ureteral stent (RUS) and percutaneous nephrostomy (PCN) tube, in cases of upper urinary tract stone obstruction with complications requiring urgent drainage, by evaluating outcomes regarding urinary symptoms, quality of life (QoL), spontaneous stone passage, and length of hospital stays, since there is no literature stating the superiority of one modality over the other.Methods: We searched MEDLINE and other sources for relevant articles in June 2019 without any date restrictions or filters applied. The selection was done first by the title and abstract screening and then by full-text assessment for eligibility. Only randomized controlled trials or cohort studies in patients with hydronephrosis secondary to obstructive urolithiasis that presented comparative data between PCN and RUS placement concerning at least one of the defined outcome measures were included. Lastly, MEDLINE database and PubMed platform were screened again using the same terms, from June 2019 until November 2022.Results: Of 556 initial articles, seven were included in this review. Most works were considered of moderate-to-high quality. Three studies regarding QoL showed a tendency against stenting, even though only one demonstrated statistically significant negative impact on overall health state. Two works reported significantly more post-intervention urinary symptoms in stenting patients. One article found that PCN is a significant predictor of spontaneous stone passage, when adjusted for stone size and location. Findings on length of hospital stays were not consistent among articles.Conclusion: PCN appears to be the intervention better tolerated, with less impact on the patient’s perceived QoL and less post-operative urinary symptoms, in comparison with RUS. Nevertheless, further studies with larger samples and a randomized controlled design are suggested.

Polyurethane(PU)ureteral stents are used in clinics to maintain the ureteral patency.Due to biofilm formation and encrustation complications,long-term clinical usage has been limited.It is therefore necessary to develop an effective response to this unmet medical need.A heparin/poly-L-lysine/copper(NPs)coating was developed in our previous work that showed the effect of preventing infection and encrustation in vitro.In this work,a further study was conducted by grafting NPs on clinical ureteral stents that then were implanted into the infectious bladders of Wistar rats to investigate the effects of nanoparticles on bacterial growth and crystal deposition in vivo.It was found that decreased numbers of adherent microbes,urease amount splitting by bacteria,and deposited crystals were observed on the NPs stents with significant differences in comparison with PU stents.Besides,histological analysis showed that the NPs stents decreased the host tissue inflammation in close relation to the decrease biofilm formation and encrustation after 28 days of implantation.

Ultrathin,elastic,and self-adhesive nanofiber bio-tape:An intraoperative drug-loading module for ureteral stents with localized and controlled drug delivery properties for customized therapy

During the postoperative management of urinary diseases,oral or intravenous administration of drugs and implanting ureteral stents are usually required,making localized drug delivery by ureteral stent a precise and effective medication strategy.In the traditional drug loading method,the drug was premixed in the implants in production lines and the versatility of drugs was restricted.However,the complex situation in the urinary system fails the possibility of finding a“one fits all”medication plan,and the intraoperative drug-loading of implants is highly desired to support customized therapy.Here,we designed an ultrathin(8μm),elastic,and self-adhesive nanofiber bio-tape(NFBT)that can easily encapsulate drugs on the stent surface for controllable localized drug delivery.The NFBT exhibited high binding strength to a ureteral stent,a sustained release over 7 d in PBS for hydrophilic drug,and a zero-order release curve over 28 days for the hydrophobic drug nitrofurantoin(NFT).Further in vivo experiments using a porcine ureteral tract infection model demonstrated that NFBT loaded with NFT could significantly reduce the bacterial concentration in urine.The total amount of NFT delivered by the NFBT was about 2.68 wt%of the recommended dose for the systemic administration.

Ever since the ureteral stent design was fitted with a curl on both sides to prevent it from migrating up or down the ureter some 40 years ago,its use has gained tremendous momentum,aiding in the rise and evolution of endourology and has confidently kept its place in modern time urology.Over the past four decades,several designs,coating and biomaterials have been developed,trying to reduce infection,encrustation and other stent related symptoms.As the ideal stent has not yet been discovered,different ways of helping patients with their complaints have been researched.This review will cover these aspects of stent use in urolithiasis.

Treatment of benign ureteral stricture by double J stents using high-pressure balloon angioplasty

Background Balloon dilatation angioplasty is a minimally invasive surgery for treating benign ureteral stricture. The aim of this study was to investigate the effect of placing double J （D-J） stents using high-pressure balloon angioplasty in treating benign ureteral stricture.Methods A total of 42 patients （48 cases） with benign ureteral stricture （42 had benign ureteral stricture） were investigated by inserting dual D-J stents using high-pressure balloon angioplasty. The control group contained 50 patients （57 cases） employing the conventional balloon angioplasty with a single D-J stent inserted for comparison.Results The overall effective rate of the treated and control groups was 87.8% （36/41） and 62.7% （32/51）, respectively （P 〈0.05）.Conclusion This new approach produces a better curative effect than the conventional balloon angioplasty with a single D-J stent insertion in treating benign ureteral stricture.

stents for malignant ureteral obstruction

Malignant ureteral obstruction can result in renal dysfunction or urosepsis and can limit the physician’s ability to treat the underlying cancer.There are multiple methods to deal with ureteral obstruction including regular polymeric double J stents(DJS),tandem DJS,nephrostomy tubes,and then more specialized products such as solid metal stents(e.g.,Resonance stent,Cook Medical)and polyurethane stents reinforced with nickel-titanium(e.g.,UVENTA stents,TaeWoong Medical).In patients who require long-term stenting,a nephrostomy tube could be transformed subcutaneously into an extra-anatomic stent that is then inserted into the bladder subcutaneously.We outline the most recent developments published since 2012 and report on identifiable risk factors that predict for failure of urinary drainage.These failures are typically a sign of cancer progression and the natural history of the disease rather than the individual type of drainage device.Factors that were identified to predict drainage failure included low serum albumin,bilateral hydronephrosis,elevated C-reactive protein,and the presence of pleural effusion.Head-to-head studies show that metal stents are superior to polymeric DJS in terms of maintaining patency.Discussions with the patient should take into consideration the frequency that exchanges will be needed,the need for externalized hardware(with nephrostomy tubes),or severe urinary symptoms in the case of internal DJS.This review will highlight the current state of diversions in the setting of malignant ureteral obstruction.

Estimation of successful capping with complete aspiration of bladder via nephroureterostomy tube

BACKGROUND ureteral stent and nephroureterostomy tube(NUT)are treatments of ureteral obstruction.ureteral stent provides better quality of life.Internalization of NUT is desired whenever possible.AIM To assess outcomes of capping trial among cancer patients with complete aspiration of retained contrast from bladder via NUT.METHODS Our Institutional Review Board approved retrospective review of all NUT placement,NUT exchange and conversion of nephrostomy catheter into NUT performed during June 2013 to June 2015(n=578).Cases were excluded due to lack of imaging of bladder(n=37),incomplete aspiration of bladder(n=324),no attempt at capping NUT(n=166),and patients with confounding factors interfering with results of capping trial including non-compliant bladder,bladder outlet obstruction and catheter malposition(n=14).Study group consisted of 37 procedures in 34 patients(male 19,female 15,age 2-83 years,average 58,median 61)most with cancer(prostate 8,endometrial 5,bladder 4,colorectal 4,breast 2,gastric 2,neuroblastoma 2,cervical 1,ovarian 1,renal 1,sarcoma 1,urothelial 1 and testicular 1)and one with Crohn’s disease.Medical records were reviewed to assess outcomes of capping trial.Exact 95%confidence intervals(95%CI)were calculated.RESULTS Among patients with complete aspiration of retained contrast,30(81%,95%CI:0.65-0.92)catheters were successfully capped(range 12-94 d,average 40,median 24.5)until planned conversion to internal stent(23),routine exchange(5),removal(1)or death unrelated to catheter(1).Seven capping trials(19%,95%CI:0.08-0.35)were unsuccessful(range 2-22 d,average 12,median 10)due to leakage(3),elevated creatinine(2),fever/hematuria(1)and nausea/vomiting(1).CONCLUSION Capping trial success among patients with complete aspiration of retained contrast/urine from bladder via NUT appears high.

Forgotten ureteral stents in a Tertiary Hospital in Accra and a Review of Endourological Management of Upper Urinary Tract Pathologies in the West Africa Sub-Region

Background: Forgotten ureteral stent is defined as prolong indwelling ureteral stent whose function is no longer desired. ureteral stents are used in the management of upper urinary tract pathologies. Prolonged indwelling ureteral stents may be complicated by urosepsis or renal failure, encrustation, stone formation, spontaneous fracture which may either be retained or voided in the urine (stenturia). Hitherto, these complications were managed by open procedures alone in our center. We report our recent experience in endourology with the management of three cases of forgotten ureteral stents with durations of ten years and two years (two cases) and review endourological practice in West Africa. Conclusion: Although encrusted stents can be managed successfully by minimally invasive approaches in the majority of cases, the best treatment is prevention. Urology units should have preferably an electronic stent register such that when the time for removal is due, the patient's name and details are flagged red. If electronic register is not available, then a hard paper/book register should be made to prevent situations of forgotten stents. Also, efforts must be made to improve endourological services in the West Africa subregion to allow patients to have the benefit of endourology in the management of upper urinary tract pathologies including that of stones originating from an encrusted or fractured forgotten ureteral stent.
