Characteristics and utility of combined duplex color-coded Ultrasonography in the cervical rotational position with head MRI and MRA in the differential diagnosis of apogeotropic direction-changing positional nystagmus

Objective:To determine whether cervical Ultrasonography,magnetic resonance imaging(MRI),and magnetic resonance angiography(MRA)are useful in the differential diagnosis of etiology and understanding the pathophysiology in cases of apogeotropic direction-changing positional nystagmus(DCPN).Methods:Thirty patients with apogeotropic DCPN were classified into 11 cases of central disease,seven cases of mixed central and peripheral disease,and 12 cases of peripheral disease by differential diagnosis based on various balance function,neuro-otological,and imaging tests.Cervical Ultrasonography using the cervical rotation method and MRI and MRA of the head and neck were performed in most patients with apogeotropic DCPN.We reviewed the presence of abnormal imaging findings according to the disease etiology.Results:Of the 30 patients with apogeotropic DCPN,23 showed vascular abnormalities or central lesions on imaging.Vascular lesions were found in six of the 12 patients with peripheral disease.Cervical Ultrasonography with cervical rotation detected blood flow disturbance in the vertebral artery in eight patients in whom the disturbance could not be detected by MRI or MRA of the head and neck.Discussion:We hypothesize that the causative disease of apogeotropic DCPN may be strongly associated with circulatory insufficiency of the vertebrobasilar and carotid arteries,and that impaired blood flow in these vessels may affect peripheral vestibular and central function.In patients with apogeotropic DCPN,examinations of vestibular function,central nervous system symptoms,and brain hemodynamics are valuable for differential diagnosis.

Reappraising the Value of Fetal First-Trimester Ultrasonography

In the last few years,the introduction of cell-free DNA has rapidly altered prenatal screening regimens and is increasingly offered as the second- or,at times,even the first-tier screening test.Should an early anomaly scan also be part of an up-to-date screening policy? This paper reappraises the value of fetal first-trimester Ultrasonography.The primary aims of the first-trimester scan are to establish gestational age based on the measurement of fetal crown-rump length,to detect multiple pregnancy and chorionicity,and to measure fetal nuchal translucency thickness as part of a combined screening test for chromosomal abnormalities.With recent advancements in ultrasound technology,there is compelling evidence that a majority of fetuses with major structural abnormalities and almost half of them without chromosomal abnormalities can be detected in the first trimester.We focused on the first-trimester screening of fetal major defects,especially including fetal congenital heart disease and cleft lip and palate by ultrasound markers and views.Moreover,it is critical to highlight that after a detailed anomaly scan in the first trimester without major structural anomalies and positive genetic tests,the residual chance of favorable outcome in fetuses with isolated increased nuchal translucency is relatively high.The discussion on the role of cell-free DNA in prenatal screening is still ongoing.Even in the event of it becoming a first-line screening test for aneuploidies,the importance of a first-trimester fetal scan,including assessment of markers for other anomalies,remains undisputed.

Do specific Ultrasonography features identified at the time of early pregnancy loss predict fetal chromosomal abnormality? e A systematic review and meta-analysis

To investigate the association of specific Ultrasonography features identified during the diagnosis of early pregnancy loss(EPL)and abnormal karyotype.This was a systematic review and meta-analysis conducted in accordance with PRISMA criteria.We searched PubMed,Cochrane and Ovid MEDLINE from 1977 to Jan 2017 to identify the articles that described EPL with karyotype and Ultrasonography features.Risk differences were pooled to estimate the chromosomal abnormality rates in Ultrasonography features,including pre-embryonic,enlarged yolk sac(YS),short crown rump length(CRL),small gestational sac(GS),symmetrical arrested growth embryo,or gestational sac with only a YS.Quality assessment of included studies was performed using Strengthening the Reporting of Observational Studies in Epidemiology(STROBE)checklists for Observational Studies(2007 version).Thirteen studies were included in the meta-analysis.Chromosomal abnormality was more likely to occur in embryonic EPL and enlarged YS.On the other hand,short CRL,small GS,symmetrical arrested growth embryo,or gestational sac with only a YS,were not associated with an increased risk of fetal chromosomal abnormality.Ultrasonography features at the time of diagnosis of EPL have limited predictive value of fetal chromosomal abnormality.

BACKGROUND Breast non-mass-like lesions(NMLs)account for 9.2%of all breast lesions.The specificity of the ultrasound diagnosis of NMLs is low,and it cannot be objectively classified according to the 5th Edition of the Breast Imaging Reporting and Data System(BI-RADS).Contrast-enhanced ultrasound(CEUS)can help to differentiate and classify breast lesions but there are few studies on NMLs alone.AIM To analyze the features of benign and malignant breast NMLs in grayscale Ultrasonography(US),color Doppler flow imaging(CDFI)and CEUS,and to explore the efficacy of the combined diagnosis of NMLs and the effect of CEUS on the BI-RADS classification of NMLs.METHODS A total of 51 breast NMLs verified by pathology were analyzed in our hospital from January 2017 to April 2019.All lesions were examined by US,CDFI and CEUS,and their features from those examinations were analyzed.With pathology as the gold standard,binary logic regression was used to analyze the independent risk factors for malignant breast NMLs,and a regression equation was established to calculate the efficiency of combined diagnosis.Based on the regression equation,the combined diagnostic efficiency of US combined with CEUS(US+CEUS)was determined.The initial BI-RADS-US classification of NMLs was adjusted according to the independent risk factors identified by CEUS,and the diagnostic efficiency of CEUS combined with BI-RADS(CEUS+BI-RADS)was calculated based on the results.ROC curves were drawn to compare the diagnostic values of the three methods,including US,US+CEUS,and CEUS+BI-RADS,for benign and malignant NMLs.RESULTS Microcalcification,enhancement time,enhancement intensity,lesion scope,and peripheral blood vessels were significantly different between benign and malignant NMLs.Among these features,microcalcification,higher enhancement,and lesion scope were identified as independent risk factors for malignant breast NMLs.When US,US+CEUS,and CEUS+BI-RADS were used to identify the benign and malignant breast NMLs,their sensitivity rates were 82.6%,91.3%,and

Dear editor,Pancreatic carcinoma is highly malignant and is mainly characterized by a low rate of eligibility for curative-intent resection,rapid metastases and/or local relapses even after surgery.[1-3]Imaging examinations like enhanced computed tomography(CT)/magnetic resonance imaging(MRI),and positron emission tomography-computed tomography(PET-CT),play an important role in detecting tumor recurrence or residual after radical resection of pancreatic cancer.However,these detection platforms are increasingly reported to have insufficient sensitivity and specificity.[4,5]Here we report a pancreatic cancer patient who had progressively increased carbohydrate antigen 19-9(CA199)level one year and three months after surgery;however,none of the above imaging methods were able to identify the cause.After our investigations,we discovered mediastinal metastasis using endoscopic Ultrasonography(EUS)and confi rmed by EUS-guided fi ne needle aspiration(EUS-FNA).

Dear Editor,Priapism is an uncommon pathological erection that mainly occurs in adult males between the ages of 20 years and 50 years.1 It is defined as an involuntary erection that persists more than 4 h without any sexual stimulation.2 This article reports two patients with high-flow priapism,which is relatively rare.We propose some new ultrasound features to diagnose this disease.After embolization,we assessed the embolization location and monitored whether there was recanalization in the embolization treatment site following therapy through Ultrasonography.These have not been mentioned in the previous literature.

Using Ultrasonography to Evaluate the Relationship between Capsular Invasion or Extracapsular Extension and Lymph Node Metastasis in Papillary Thyroid Carcinomas

Background： Previous studies have demonstrated that Ultrasonography is the recommended imaging modality for preoperative staging of papillary thyroid carcinomas （PTCs）. However, only a few studies have kept watch on preoperative evaluation of capsular invasion （CI） or extracapsular extension （ECE） and cervical lymph node metastasis using preoperative Ultrasonography. This study aimed to investigate the relationship between the CI or ECE and the cervical lymph node metastasis in PTCs using preoperative Ultrasonography and postoperative pathology in Chinese patients. Methods： The data of preoperative Ultrasonography and postoperative pathology of 166 patients who had a defnitive diagnosis of PTCs from October 2011 to July 2014 at Xuanwu Hospital, Beijing were collected and reviewed. Preoperative ultrasonic parameters of thyroid nodules were compared with those of postoperative pathological diagnoses. All the patients were divided into bilateral PTCs group （n - 42, 78 nodules） and unilateral PTCs group （n = 124, 124 nodules）, and the data of the nodnle sizes, CI or ECE, and cervical lymph node metastasis by preoperative Ultrasonography were compared between two groups. Results： A total of 202 nodules of 166 patients which were confimled by preoperative Ultrasonography and postoperative pathology were analyzed. Hypoechogenicity （n = 201, 99.5%） and inegular margins （tl = 167, 82.7%） were the main ultrasonic characteristics of PTCs. A significant moderate agreement between preoperative ultrasonic examination and postoperative pathology fbr CI or ECE （K - 0.622, P 〈 0.001 ） was observed. The diagnostic sensitivity was 92.0%~ and specificity was 7 I. 1%. In bilateral PTCs group, 81.0% had CI or ECE, and 61.9% had cervical lymph node metastasis. In unilateral PTCs group, 76.6% had CI or ECE, and 58.1% had cervical lymph node metastasis. New horizons in the endoscopic Ultrasonography-based diagnosis of pancreatic cystic lesions

Pancreatic cystic lesions(PCLs) are increasingly being identified because of the widespread use of highresolution abdominal imaging. These cysts encompass a spectrum from malignant disease to benign lesions, and therefore, accurate diagnosis is crucial to determine the best management strategy, either surgical resection or surveillance. However, the current standard of diagnosis is not accurate enough due to limitations of imaging and tissue sampling techniques, which entail the risk of unnecessary burdensome surgery for benign lesions or missed opportunities of prophylactic surgery for potentially malignant PCLs. In the last decade, endoscopic innovations based on endoscopic Ultrasonography(EUS) imaging have emerged, aiming to overcome the present limitations. These new EUS-based technologies are contrast harmonic EUS, needle-based confocal endomicroscopy, through-the-needle cystoscopy and through-the needle intracystic biopsy. Here, we present a comprehensive and critical review of these emerging endoscopic tools for the diagnosis of PCLs, with a special emphasis on feasibility, safety and diagnostic performance.

To the Editor: We would like to congratulate Tian and colleagues on their recent article: Diagnostic value of contrast-enhanced Ultrasonography for intrahepatic cholangiocarcinoma with tumor diameter larger than 5 cm [1]. They argued that, despite not being included in the American Association for the Study of Liver Diseases(AASLD) guidelines for the diagnosis of liver diseases, contrast-enhanced Ultrasonography(CEUS) can be of use for diagnosis of large(>5 cm), mass-forming intrahepatic cholangiocarcinoma(ICC).
