Background: Melasma and post-inflammatory hyperpigmentation provide a significant source of psychosocial morbidity, especially in those with Fitzpatrick skin types III-VI. In Europe, a proprietary product aimed at treating these conditions, similar to Kligman’s formula but with a longer expiry date, has become available. Objectives: To assess patient satisfaction with a newly available combination de-pigmenting preparation. Methods: We conducted a small study to see if patients felt that this new product affected their quality of life and skin symptoms from hyperpigmentation. 41 subjects, who had all been prescribed a 15 g tube to use sparingly at night for 90 days within the last 12 months were telephoned to rate the effect the cream had on their quality of life and skin symptom improvement. Each patient also had their Dermatology Life Quality Index (DLQI) score assessed. Results: Out of the 29 patients who responded to the study, 22 had melasma and 7 had post-inflammatory hyperpigmentation from acne. 21 subjects felt that the cream made either a marked or moderate improvement on their quality of life and 23 subjects felt that the cream made either a marked or moderate improvement on their skin symptoms. Conclusion: Patients reported improvement in both hyperpigmentation and quality of life, suggesting a high level of satisfaction with treatment. The long shelf life of the product may also promote compliance and reduce health-care costs.

Aim In vitro dissolution test and pharmacokinetics in beagle dogs wereconducted to assess the formulation of tretinoin in self-emulsifying systems. Methods Theconcentrations of tretinoin were determined by HPLC. A crossover study was performed in four fastingbeagle dogs with the formulation of self-emulsifying systems and commercial capsules. Results Theresults showed that the dissolution rate in 15 min of tretinoin in self-emulsifying systems washigher than 80% while that of the commercial capsules was lower than 5% . The area under the plasmaconcentration-time curve (AUC) of the self-emulsifying formulation was significantly higher andC_(max) was approximately two times greater than those of commercial capsule, respectively, Inaddition, the time taken to reach peak was shorter (2 h to 1.25 h) for self-emulsifying formulationof tretinoin. Conclusion The self-emulsifying drug delivery *** significantly increasetretinoin in vitro dissolution and in vivo absorption.

OBJECTIVE: To investigate the response of multiple myeloma (MM) cells to arsenic trioxide (As2O3) and their possible mechanisms. METHODS: Two MM-derived cell lines RPMI8226 and U266 cells were used as in vitro models. Cell apoptosis was assessed by morphology, flow cytometry, and DNA gel electrophoresis. Mitochondrial transmembrane potentials (delta psi m) were evaluated by measuring cellular Rhodamine 123 staining intensity. Protein expression was analyzed using Western blot. RESULTS: Zero point one to 0.5 mumol/L As2O3 inhibited cell proliferation and 2.0 mumol/L As2O3 induced cell apoptosis, while 1.0 mumol/L As2O3 inhibited proliferation with a weak degree of apoptosis induction in RPMI8226 and U266 cell lines. As2O3-induced apoptosis was accompanied by mitochondrial transmembrane potentials (delta psi m) collapse and caspase-3 activation in the presence of intact membrane. Glutathione depleter buthionine sulfoximine enhanced, while disulfide bond-reducing agent dithiothreitol partially antagonized As2O3-induced delta psi m collapse and apoptosis in MM cells. All-trans retinoic acid (ATRA) could also induce apoptosis in RPMI8226 cells, but it did not show any cooperative effects with As2O3. CONCLUSION: As2O3 exerts apoptosis-inducing and growth-inhibiting effects on MM cells, and mitochondrium is a pivotal and common target of As2O3 for apoptosis induction.

Background: Acne vulgaris is a common skin disease. Its pathogenesis is multifactorial, and therapy can now be directed at many of these factors. Among these factors are androgen hormones which play an important role in the pathogenesis of acne. Accordingly,many antiandrogens have been developed to treat acne by systemic and topical ways. Five-Alpha Avocuta 2% cream is a new topical 5-alpha reductase inhibitor extracted from the tropical fruit “avocado”. Objectives: To evaluate the effectiveness and safety of 5-Alpha Avocuta cream and compare it to tretinoin 0.025% cream, in the treatment of acne vulgaris. Patients& Methods: This single blinded comparative study was done in Department of Dermatology– Baghdad Teaching Hospital - from July 2009 through October 2010. All demographic points related to the disease were obtained from each patient to evaluate severity of acne. Sixty-eight patients with acne were divided into two groups and instructed to apply the drugs for three months. Group A uses 5-Alpha avocuta 2% (Teen-derm K) cream and Group B uses tretinoin (Acretin) 0.025% cream. Severe acne was excluded from the study. The clinical improvement was evaluated by counting the number of inflammatory lesions before and after treatment and compare between them by paired t-test. Results: Fifty-eight patients completed the study, their age ranged from 12 - 30 years with a mean of 18.21 ± 4.64 years. In Group A there was a significant reduction of the mean number of papules from 29.7 before treatment to 15.5 after treatment (p < 0.0001), while the mean number of pustules decreased from 4.9 to 0.6 (p = 0.019). In Group B the mean number of papules was reduced from 28.5 to 14.9 (p < 0.0001), while the mean number of pustules dropped from 5.3 before treatment to 0.7 after treatment (p = 0.009). The mean difference of papules after 14 weeks in Group A was 15.55 ± 6.95 while in Group B, the mean difference was 14.86 ± 6.99. This result was statistically not significant (p = 0.7

Treatment options for ichthyosis and ichthyosiform disorders are limited and often ***-four patients used combination creams of 2%cholesterol with 2%lovastatin,and 10%glycolic acid with 0.025% tretinoin and 2%ketoconazole *** one month(n=20),the average percent reduction in severity scores was 40%,and at three months(n=10),it was 60.3%.Side effects were mainly mild *** findings suggest that these two combination creams could be beneficial in the treatment of ichthyosiform disorders.
