Efficacy and safety of Buyang Huanwu decoction(补阳还五汤)for diabetic peripheral neuropathy:a systematic review and Meta-analysis

OBJECTIVE:To evaluate the efficacy and safety of Buyang Huanwu decoction(BYHWD)in treating diabetic peripheral neuropathy(DPN).METHODS:Eight electronic databases,including China National Knowledge Infrastructure Database,Wanfang Database,China Science and Technology Journal Database,Chinese Biomedical Literature Database,Cochrane Library,Embase,Web of Science,and PubM ed,were searched for randomized controlled trials(RCTs)of BYHWD to treat DPN.We identified all RCTs related to BYHWD and those on the treatment of DPN with the combination of mecobalamin.RevMan software was used for the statistical analysis.RESULTS:Twenty-one RCTs with a total of 1945 patients were included.The methodological quality of the literature included was low.Meta-analysis showed that the efficacy of the treatment group was significantly better than that of the control group in the treatment of DPN with BYHWD[risk ratio(RR)=0.33,95%CI(0.27,0.40),Z=11.25,Ptreatment group.The MNCV in the common peroneal nerve[(MD=1.63,95%CI(0.39,2.87)]and SNCV[(MD=4.56,95%CI(3.16,5.97)]were significantly higher than those in the control group(Ptreatment.There was no significant difference in fasting blood glucose[(MD=-0.42,95%CI(-0.89,0.05),Z=1.76,P=0.08)]between the treatment and control groups;postprandial blood glucose[(MD=-0.62,95%CI(-1.19,0.05),Z=2.12,P=0.03)]decreased significantly.No significant difference was found in the blood lipid levels between the treatment and control groups,including triglycerides[(MD=-0.21,95%CI(-0.52,0.10),Z=1.34,P=0.18)]and cholesterol[(MD=-0.13,95%CI(-0.27,0.00),Z=1.92,P=0.06)].Of the 21 RCTs,only five

OBJECTIVE:To evaluate the effectiveness and safety of Xiangsha Liujun pills(香砂六君丸) on the decreased digestive function in patients in the recovery phase of the Coronavirus disease 2019(COVID-19).METHODS:A randomized,double blind,placebo controlled clinical trial was conducted.A total of 200 COVID-19 patients in the recovery phase were included in our study in Ezhou Hospital of Traditional Chinese Medicine.Totally 200 subjects were randomly divided into a treatment group(Xiangsha Liujun pills) and a control group(placebo),with 100 in each group.Subjects took Xiangsha Liujun pills or placebo orally three times a day for two weeks.Three visits were scheduled at week 0(baseline),week 1(the middle of the intervention) and week 2(the end of the intervention) for each eligible patient.The total efficacy rates for improving the Traditional Chinese Medicine(TCM) symptoms(fatigue,poor appetite,abdominal distension and loose stools) and the disappearance rates of symptoms were observed and compared in the treatment and control groups.Adverse events were recorded during the study period.SAS 9.4 was used to analyze the data.RESULTS:A total of 200 patients were included in this study,among which 4 participants withdrew because the drugs did not work.Three patients were excluded for age.Before the treatment,there was no significant difference between the TCM symptoms scores of subjects.After 1 week of treatment,the full analysis set(FAS) showed that the efficacy rates for abdominal distension and loose stools in the treatment group were significantly higher than the control group(P 0.05).In addition,the disappearance rate of fatigue in the treatment group was significantly higher than the control group(P treatment in the rates of poor appetite,abdominal distension,and loose stools(P > 0.05).After 2 weeks of treatment,the efficacy r

Short and long-term outcomes of two drug eluting stents in bifurcation lesions

Background Because no large prospective studies are available, this study evaluated the clinical outcomes of two drug eluting stents in bifurcation lesions. Methods Lesions with diameter of side branch ≥2.5 mm were selected. From October 2003 to June 2005, 112 patients with 113 bifurcation lesions were treated by two drug eluting stents （DESs）, technique. The location of bifurcation lesions were left anterior descending coronary artery/diagonal in 62 patients, left main distal bifurcation in 32, left circumflex coronary artery/obtute marginal branch in 18 and right coronary artery distal bifurcation in 1. Procedures for bifurcation lesions were crush or modified crush technique in 64, ＂T＂ stenting technique in 27, modified ＂Y＂ stenting, kiss stenting, ＂V＂ stenting as well as culotte stenting technique in 11, 5, 3 and 3, respectively. Among 226 lesions, 91 Cypher or Cypher select stents, 74 TAXUS and 67 Firebird were used. Final kiss balloon dilation was performed in 60 （93.7%） with crush technique after stenting. Results Success rate of percutaneous coronary intervention for the bifurcation lesions was 100%. One patient, who developed inhospital acute myocardial infarction due to subacute thrombosis, was successfully treated by a second intervention. Major adverse cardiac events rate in-hospital was 0.89% （1/112） and during followup was 7.14% （8/112）, No death occurred during the followup of all patients. Angiographic followup was effected for 46 patients, restenosis for eight, coronary artery bypass grafting for 1 and a repeat intervention for 5. Restenosis involving TAXUS, Cypher and Firebird was 5 （5/18, 27.8%）, 2 （2/17, 11.8%） and 1 （1/11, 9.1%）, respectively （P〉0.05）. Total restenotic rate was 17.4% （8/46）. Conclusions When ostium of side branch has severe stenosis and 〉12.5 mm in diameter, two-stent strategy in this bifurcation lesion is safe and effective, and the outcomes are satisfactory. Restenotic rates were not different between TAXUS, Cypher and Firebird DESs.

Mid-term effects of lung volume reduction surgery on pulmonary function in patients with chronic obstructive pulmonary disease

Background Now lung volume reduction surgery （LVRS） has become one of the most effective methods for the management of some cases of severe chronic obstructive pulmonary disease （COPD）. We evaluated the mid-term effects of LVRS on pulmonary function in patients with severe COPD. Methods Ten male patients with severe COPD aged 38-70 years underwent LVRS and their pulmonary function was assessed before, 3 months and 3 years after surgery. The spirometric and gas exchange parameters included residual volume, total lung capacity, inspiratory capacity, forced vital capacity, forced expiratory volume in one second, diffusion capacity for CO, and arterial blood gas. A 6-minute walk distance （6MWD） test was performed.Results As to preoperative assessment, most spirometric parameters and 6MWD were significantly improved after 3 months and slightly 3 years after LVRS. Gas exchange parameters were significantly improved 3 months after surgery, but returned to the preoperative levels after 3 years. Conclusions LVRS may significantly improve pulmonary function in patients with severe COPD indicating for LVRS. Mid-term pulmonary function 3 years after surgery can be decreased to the level at 3 months after surgery. Three years after LVRS, lung volume and pulmonary ventilation function can be significantly improved, but the improvement in gas exchange function was not significant.

Preliminary evaluation of posterior dynamic lumbar stabilization in lumbar degenerative disease in Chinese patients

Background There has been some controversy related to the use of the Wallis system,rather than disc fusion in the treatment of patients with degenerative spine disease.Furthermore,there are no reports concerning the application of this dynamic stabilization system in Chinese patients,who have a slightly different lifestyle with Western patients.The aim of this study was to assess the safety and efficacy of the dynamic stabilization system in the treatment of degenerative spinal diseases in Chinese patients.Methods The clinical outcomes of 20 patients with lumbar degenerative disease treated by posterior decompression with the Wallis posterior dynamic lumbar stabilization implant were studied.All of the patients completed the visual analogue scale and the Chinese version of the Oswestry Disability Index.The following radiologic parameters were measured in all patients：global lordotic angles and segmental lordotic angles （stabilized segments,above and below adjacent segments）.The range of motion was then calculated.Results Nineteen patients （95%） were available for follow-up.The mean follow-up period was （27.25±5.16） months （range 16-35 months）.The visual analogue scale decreased from 8.55±1.21 to 2.20±1.70 （P 〈0.001）,and the mean score on the Chinese version of the Oswestry Disability Index was improved from 79.58%±15.93% to 22.17%±17.24% （P 〈0.001）.No significant changes were seen in the range of motion at the stabilized segments （P=0.502） and adjacent segments （above,P=0.453; below,P=0.062）.The good to excellent result was 94.4% at the latest follow-up.No complications related to the use of the Wallis posterior dynamic lumbar stabilization occurred.Conclusions It was found to be both easy and safe to use the Wallis posterior dynamic lumbar stabilization implant in the treatment of degenerative lumbar disease,and the early therapeutic effectiveness is good.The Wallis system provides an alternative method for the treatment of lumbar degenerative disease.

Minimally invasive two-incision total hip arthroplasty:a short-term retrospective report of 27 cases

Background Total hip arthroplasty （THA） is widely applied for the treatment of end-stage painful hip arthrosis. Traditional THA needed a long incision and caused significant soft tissue trauma. Patients usually required long recovery time after the operation. In this research we aimed to study the feasibility and clinical outcomes of minimally invasive two-incision THA. Methods From February 2004 to March 2005, 27 patients, 12 males and 15 females with a mean age of 71 years （55-76）, underwent minimally invasive two-incision THA in our department. The patients included 9 cases of osteoarthritis, 10 cases of osteonecrosis, and 8 cases of femoral neck fracture. The operations were done with VerSys cementless prosthesis and minimally invasive instruments from Zimmer China. Operation time, blood loss, length of fncision, postoperative hospital stay, and complications were observed. Results The mean operation time was 90 minutes （80-170 min）. The mean blood loss was 260 ml （170-450 ml） and blood transfusion was carried out in 4 cases of femoral neck fracture （average 400 ml）. The average length of the anterior incision was 5.0 cm （4.6-6.5 cm） and of the posterior incision 3.7 cm （3.0-4.2 cm）. All of the patients were ambulant the day after surgery. Nineteen patients were discharged 5 days post-operatively and 8 patients 7 days post-operatively. The patients were followed for 18 months （13-25 months）. One patient was complicated by a proximal femoral fracture intraoperatively. No other complications, including infections, dislocations, and vascular injuries, occurred. The mean Harris score was 94.5 （92-96）. Conclusions Two-incision THA has the advantage of being muscle sparing and minimally invasive with less blood loss and rapid recovery. However, this technique is time consuming, technically demanding, and requires fluoroscopy.

Long-term results and radiographic findings of percutanous vertebroplasties with polymethylmethacrylate for vertebral osteoporotic fractures

Background Percutaneous vertebroplasty （PVP） has become a popular procedure for painful vertebral osteoporotic fracture （VOF）, with immediate pain relief and improved mobility; however, polymethylmethacrylate （PMMA） injected into the vertebral body is not absorbable and little information is available concerning the long-term results. In this retrospective study, we evaluated the long-term clinical results and radiological changes after PVPs for VOFs. Methods Fifty-one patients with VOFs were treated by PVPs with PMMA between 2000 and 2004. After 〉7 years of follow-up, eight patients had died from causes unrelated to the intervention and 12 patients were lost to follow-up, thus leaving 31 patients available for evaluation with an average length of follow-up of 9.2 years （follow-up rate, 72.1%）. Among these 31 patients, the PMMA was injected at 43 levels with a mean volume of 4.3 ml per level （range, 2-6 ml）. The pain was assessed with a visual analog scale （VAS）, and the mobility was graded as walking without difficulty （grade 1）, walking with assistance （grade 2）, and bedridden （grade 3）. Plain radiographs and computed tomography （CT） were obtained and assessed pre-operatively, immediately post-operatively, and after 7 years of follow-up. The PMMA, vertebral height, and Cobb angle were assessed and compared. Results All of the patients experienced pain relief and improved mobility after intervention and during the follow-up period. Cement leakage was detected in post-operative CT scans in 9 of 51 patients, but without neurological compromise. For the 31 patients followed up over 7 years, the VAS decreased from 8.3±2.6 pre-operatively, to 2.1±1.6 immediately post-operatively, and 1.0±0.9 at the final follow-up evaluation, with significantly improved mobility. Additional compression fractures occurred at adjacent levels in three patients, and there were no new fractures at the augmented vertebrae. Background Few data on ankle fractures in China from large multicenter epidemiological and clinical studies are available.The aim of this research was to evaluate the epidemiological features and surgical outcomes of ankle fractures by reviewing 235 patients who underwent ankle fracture surgery at five hospitals in China.Methods This study included patients who underwent ankle fracture surgery at five Chinese hospitals from January 2000 to July 2009.Age,gender,mechanism of injury,Arbeitsgemeinschaft für Osteosynthesefragen （AO） fracture type,fracture pattern,length of hospital stay and treatment outcome were recorded.Statistical analyses were conducted using SPSS software.The American Orthopedic Foot and Ankle Society （AOFAS） ankle-hindfoot scale,visual analogue scale （VAS）,and arthritis scale were used to evaluate outcome.Results Of 235 patients with ankle fractures,105 were male with an average age of 37.8 years and 130 were female with an average age of 47.3 years.The average follow-up period was 55.7 months.There were significant differences in the ratios of patients in different age groups between males and females,and in mechanisms of injury among different age groups.There were also significant differences in the length of hospital stay among different fracture types and mechanisms of injury.In healed fractures,the average AOFAS ankle-hindfoot score was 95.5,with an excellence rate of 99.6%,the average VAS score was 0.17,and the average arthritis score was 0.18.Movement of the injured ankle was significantly different to that of the uninjured ankle.There were no significant differences between AO fracture types,fracture patterns or follow-up periods and AOFAS score,but there were some significant differences between these parameters and ankle joint movements,pain VAS score and arthritis score.Conclusions Ankle fractures occur most commonly in middle-aged and young males aged 20-39 years and in elderly females aged 50-69 years.The most common mechanisms of injury are twisting injuries and falls from a stand

Efficacy and Safety of Glecaprevir/Pibrentasvir in Patients with Chronic HCV Infection

Hepatitis C virus(HCV)infection is a major cause of end-stage liver disease,including decompensated cirrhosis and hepatocellular carcinoma.Over 95%of patients with HCV infection have achieved sustained virologic response at 12 weeks under the treatment of several pan-genotypic regimens approved for patients with HCV infection.The glecaprevir/pibrentasvir(G/P)regimen has some features that distinguish it from others and is the only 8-week regimen approved for treatment-naive patients and patients experienced in regimens containing(peg)interferon,ribavirin,and/or sofosbuvir,without an HCV NS3/4A protease inhibitor or NS5A inhibitor(except those with genotype 3).This review aims to summarize the efficacy and safety of G/P in HCV-infected patients from clinic trials and real-world studies,including those who have historically been considered difficult to cure.
