Injury to the supraaortic artery is a rare event, with poor prognosis and high mortality.1 Improvement of the outcome may lie on the combination of several aspects, including better pre-hospital care, use of emergency cardiopulmonary bypass （CPB）, improved surgical techniques and facilities, and advanced postoperative intensive care. Some researchers emphasized the importance of emergency CPB in the treatment and thought that it was responsible mainly for the improved outcome.2-6 However, there exists controversies about it.7-9 In this article, we reported that a patient with life-threatening hemorrhage due to traumatic transection of the left common carotid artery, who was admitted to our hospital in July 2003, was treated successfully with operations with help of emergency CPB and systemic hypothermia.

AIM： To evaluate the impact of splenectomy on longterm survival, postoperative morbidity and mortality of patients with gastric cancer by performing a metaanalysis METHODS： A search of electronic databases to identify randomized controlled trials in The Cochrane Library trials register, Mediine, CBMdisc （Chinese Biomedical Database） and J-STAGE, etc was performed. Data was extracted from the studies by 2 independent reviewers. outcome measures were survival, postoperative morbidity and mortality and operation-related events. The meta-analyses were performed by RevMan 4.3. RESULTS： Three studies comprising 466 patients were available for analysis, with 231 patients treated by gastrectomy plus splenectomy. Splenectomy could not increase the 5-year overall survival rate [RR = 1.17, 95% confidence interval （CI） 0.97-1.41]. The postoperative morbidity （RR = 1.76, 95% CI 0.82-3.80） or mortality （RR = 1.58, 95% CI 0.45-5.50） did not suggest any significant differences between the 2 groups. No significant differences were noted in terms of number of harvested lymph nodes, operation time, length of hospital stay and reoperation rate. Subgroup analyses showed splenectomy did not increase the survival rate for proximal and whole gastric cancer. No obvious differences were observed between the 2 groups when stratified by stage. Sensitivity analyses indicated no significant differences regarding the survival rates （P 〉 0.05）. CONCLUSION： Splenectomy did not show a beneficial effect on survival rates compared to splenic preservation. Routinely performing splenectomy should not be recommended.

OBJECTIVE： To evaluate the effect and safety of Zuogui pill and Yougui pill, classic Y/n and Yang tonic formula （CYYTF）, in the treatment of osteoporosis and the underlying mechanism. METHODS： Participants aged 55 to 75 with osteoporosis and Kidney deficiency in Traditional Chi-nese Medicine （TCM） will be included and randomly allocated into two groups： treatment group and control group. Participants in the treatment group were treated with Zuogui pill or Yougui pill TCM formula granule, while the control group received pla- cebo. Primary outcomes are the lumbar spine on bone mineral density （BMD）（L1-4） and femoral BMD. Secondary outcomes include pain intensity, health-related quality of life （HRQoL）, bone turn- over markers and safety. RESULTS： Totally 200 patients were enrolled from December 2014 to April 2016 from four hospitals. There were no statistically significant differences between the two groups at baseline （P 〉 0.05） and it was good to comparability. Statistically signifi- cant differences between the two groups were ob- served for the lumbar BMD （L1-4）, pain VAS scores and HRQoL at six months and twelve months and femoral BMD at twelve months （P 〈 0.05）, but no significant differences for femoral BMD and bone turnover markers at six months （P 〉 0.05）. More- over, significant difference was observed at differ- ent time before and after treatment in terms of lum- bar spine （L1-4） BMD, femoral BMD, pain VAS scores and health-related quality of life, and there was an crossover effect between the time and groups be- fore and after treatment. In additional, in the treat- ment group, 8 patients lost to follow-up and 3 patients had adverse events （AEs） and in the control group, 10 patients lost to follow-up and 2 patients had AEs. No remarkable differences were observed between the two groups with regard to AEs, lost rate and safety （P 〉 0.05）. There are several studies on the impact of extreme weather on the incidence and mortality of stroke, but the research on the effect of extreme weather events on severity and outcome of ischemic stroke is scarce. This study aimed to evaluate the impact of cold spells on the severity and clinical outcomes of patients with initial acute ischemic stroke in a hospital-based study. We enrolled 553 patients with initial ischemic stroke during the cold seasons between 2016 and 2019. Patients were separated into the mild stroke group and moderate-to-severe stroke group according to the National Institutes of Health Stroke Scale(NIHSS) of the U.S., and good outcome group and poor outcome group according to modified Rankin Scale(mRS) scores. There are nine different definitions of cold spells according to intensities and duration time. After adjustment for climate factors, air quality index, and common risk factors, it is found that cold spells were associated with moderate-to-severe neurological deficits and clinical outcomes in overall ischemic stroke patients. Furthermore, the delay effect for stroke severity started at the beginning of the cold wave(Lag 0) and lasted up to 14 days(Lags 0–14). In addition, when a cold spell was set as a daily mean temperature outcomes, and there was a definite delay effect of at least 7 days(Lags 0–7) and it lasted up to 14days(Lags 0–14). At last, we concluded that cold spells may be a contributory factor for more severe neurological deficits and worse outcomes in patients with initial ischemic stroke.

OBJECTIVE: To study the effect of early comprehensive therapy of Traditional Chinese Medicine(TCM) on motor function of in patients with spinal cord ***: Fifty-one standard spinal cord injury patients with paraplegia were randomly assigned to an experimental or control group. The experimental group received TCM comprehensive therapy, and the control group received modern Western Medicine(WM) treatment for 4 weeks. The motor score(MS), Barthel Index(BI) and American Spinal Injury Association(ASIA) grading were measured in both groups before and after ***: After treatment, the MS and BI scores of the TCM comprehensive therapy group improved significantly(P 0.05). The differences between the experimental and control groups after treatment were not significant(P > 0.05).CONCLUSION: Early TCM comprehensive therapy is an effective method for improving motor function in patients with spinal cord injury.

OBJECTIVE:To assess the efficacy and safety of gingerindirect moxibustion for chronic fatigue syndrome(CFS).METHODS:In this central randomized,controlled trial,290 CFS participants were recruited and randomly allocated to group A(ginger-indirect moxibustion plus acupuncture)or group B(acupuncture alone).The study consisted of a treatment period of 8 weeks with a total of 24 treatments(3 sessions per week,every other day),and a follow-up period of 12 *** outcome was measured by Fatigue Severity Scale(FSS),Psychological Health Report(SPHERE),the Self-rating depression scale(SDS)and the Hamilton anxiety scale(HAMA)at baseline,2,4,6,8,12 and 20 ***:With the treatment undergoing,the changes of FSS,SPHERE,SDS and HAMA scores in both groups increased gradually,and the effect maintained at the 12th *** groups,significantly higher score changes were seen in group A in FSS after 4 weeks treatment(11.94 vs 9.12,95%CI:0.94,4.7)and in SPHERE after 2 weeks treatment(3.7 vs 2.27,95%CI:0.56,2.31).But for SDS and HAMA,the improvement did not differ significantly between *** severe adverse events were ***:Ginger-indirect moxibustion is a safe and effective intervention to relieve fatigue and accompanying physical symptoms of CFS.

OBJECTIVE:To evaluate the clinical efficacy and safety of external application of Chinese herbal medicine(exCHM)for psoriasis vulgaris(PV).METHODS:Different search portals,including the China National Knowledge Infrastructure Database,China Science and Technology Journal Database,Wanfang,Sino Med,clinicaltrials,Pub Med,Cochrane Library,and Cochrane Central Register of Controlled Trials,were searched for randomized controlled trials(RCTs)that compared the ex-CHM intervention with other treatment protocols for PV,with available data as of November 25,*** Review Manager 5.3 software was used for ***:Nineteen RCTs involving 1988 participants were included,of which twelve RCTs qualified for the *** results showed that the addition of CHM bath to narrow-band ultraviolet B(NB-UVB)had a higher response rate[RR=1.29;95%CI(1.19,1.40);P<0.00001];lower psoriasis area severity index(PASI)[MD=-3.15;95%CI(-4.79,-1.52);P=0.0002],adverse reactions rate[RR=0.32;95%CI(0.15,0.66);P=0.002],and recurrence rate[RR=0.48;95%CI(0.30,0.79);P=0.004]than NB-UVB *** addition of CHM fumigation to NB-UVB also showed a higher response rate[RR=1.29;95%CI(1.11,1.49);P=0.0007]and lower adverse reactions rate[RR=0.44;95%CI(0.24,0.79);P=0.006].In addition,CHM bath could reduce the adverse reactions induced by 308 nm excimer laser and improve patients’quality of life better than *** fumigation could improve the efficacy of calcipotriol ointment and reduce the adverse *** fumigation combined with external washing plus acitretin showed better results than using acitretin *** statistical difference was observed between CHM external washing and calcipotriol ointment or CHM ointment and retinoic ***:Current evidence showed that CHM bath and fumigation appeared to be efficient and safe for PV ***,no definite conclusion could be drawn due to the low quality of included studies and thus more well-designed studies are ne
