Recognition of Association Between Blood Stasis Syndrome and traumatic injury among Doctors of Korean Medicine:A Cross-Sectional Observation Study

Objective： To investigate the diagnostic indicators and herbal treatments for blood stasis syndrome（BSS） patients with traumatic injuries and to identify the association between BSS and traumatic injury in Korea. Methods： Two-hundred and four patients with traumatic injury were recruited from the Gangnam and Daejeon branches of Jaseng Hospital of Oriental Medicine between June 2014 and December 2014. Two independent doctors of Korean medicine（DKMs） determined the diagnosis of BSS or non-BSS based on the subjects＇ signs and symptoms. The scores assigned to BSS symptoms and DKMs＇ reasons for diagnosing BSS in patients with traumatic injury were investigated. Both medication and herbal prescription records from a 3-month period were collected for all patients diagnosed with BSS by both DKMs. Results： A total of 169 of 204（82.8%） patients received consistent diagnosis related to BSS by two DKMs. Among them, 54.4%（92 cases） were diagnosed with BSS, and 45.6%（77 cases） were not diagnosed with BSS. DKMs most frequently cited symptoms of recent traumatic injury as justifications for BSS diagnoses, and also selected pain-related indicators such as abdominal pain, sharp pain and nocturnal pain as important reasons in diagnosing BSS. In addition, an inconsistency in the pattern identification theory with respect to traumatic injury was observed. Although only 92 cases（54.4%） of patients were diagnosed with BSS, 77.6% of them were prescribed decoctions for BSS. Conclusions： DKMs considered traumatic injury could cause BSS, and utilized decoction for BSS in patients with traumatic injury without confirming a diagnosis of BSS because they assumed the main symptoms or pathologies of traumatic injury to be closely related to BSS.

Acute traumatic extratemporal facial nerve injury: A 5-year review

Background: traumatic facial nerve injury is the second leading cause of facial paralysis, presenting significant challenges such as difficulties with eating, speaking, impaired vision, and loss of facial expression. Understanding these injuries is critical for plastic surgeons to optimize recovery and functional outcomes. This study examined our experience with acute traumatic extratemporal facial nerve injuries, focusing on their characteristics, the frequency of affected nerve branches, and identification using Seckel's facial danger zones as anatomical landmarks. We also explored the implications of surgical management based on the current literature and our ***: We reviewed 50 patients with acute traumatic extratemporal facial nerve injuries treated at Hospital General Dr. Manuel Gea Gonz alez in Mexico City from January 2019 to January 2024. The collected data included demographics(age, sex, severity, and time to medical attention), injury mechanism, affected nerve branches using Seckel's zones, and treatment ***: The majority of the patients were male(82%), with an average age of 29 ± 16 years. Sharp trauma from assault was the most common cause(66%). The buccal and frontal branches were most frequently affected, with Seckel zones IV and II involved in 46.86% and 33.33% of cases, respectively. Primary neurorrhaphy was performed in 96% of the ***: The effective management of traumatic facial nerve injuries relies on understanding the characteristics of the injury and using anatomical landmarks for prompt localization. Primary neurorrhaphy is the preferred surgical approach, with nerve grafting being an alternative. Ideally, early intervention within 72 h is crucial for optimal nerve recovery.

Endovascular repair of traumatic thoracic aortic injury:10-year single-center experience

Background traumatic thoracic aortic injury （TAI） is an uncommon but fatal injury. The purpose of this study was to present our experience and results of thoracic endovascular aortic repair （TEVAR） in patients with TAI in single center. Methods From February 2007 to March 2016, the medical records of 21 patients with TAI admitted to our institution were retrospectively reviewed. Among these patients, 20 patients were treated with TE- VAR and 1 was excluded for posterolateral thoracotomy. The demographics, injury characteristics, procedural de- tails, and clinical outcomes of these patients were evaluated. Results In the 20 patients with TEVAR, 14 pa- tients were man （70.0%） and the mean age was 34.3_＋10.8 years （range, 14-55 years）. The causes of injury were motor vehicle accident （18/20, 90.0%） and fall from height （ 2/20, 10.0%）. All patients had concomitant injuries and a mean ISS was 36.0 ± 15.0 （20-75）. All patients received TEVAR with the aortic injuries located at aortic isthmus （11/20, 55.0%） and the remaining at descending aorta. Nine patients received intentional coverage of LSA during TEVAR. Of which, 6 cases were partial coverage. The primary technical success of TEVAR was 100% and a single stent graft was sufficed for the TAI repair in all cases. The average cover length was 152.3± 20.0 mm （range, 100-200 mm）. No patient died or had serious postoperative complications in hospital. The rate of absorption period of aortic injuries after TEVAR for ≤3 months, 3-6 months, and ≥6 months was 65.0%, 30.0%, and 5.0% respectively. There was no late explanation or device failures identified although 2 patients were detected poor apposition in the 12 months after TEVAR. Conclusions TEVAR can be performed safely with a relatively low mortality and morbidity and should be the preferred treatment for patients with TAI. But the durability of endograft needs long follow-up.

Severe injuries are the major cause of death in those aged under 40,mainly due to road traffic ***,metabolic and immune pathways respond to limit the tissue damage sustained and initiate wound healing,repair and regeneration ***,depending on age and sex,the response to injury and patient prognosis differ *** are catabolic and immunosuppressive and are produced as part of the stress response to injury leading to an intra-adrenal shift in steroid biosynthesis at the expense of the anabolic and immune enhancing steroid hormone dehydroepiandrosterone(DHEA)and its sulphated metabolite dehydroepiandrosterone sulphate(DHEAS).The balance of these steroids after injury appears to influence outcomes in injured humans,with high cortisol:DHEAS ratio associated with increased morbidity and *** models of trauma,sepsis,wound healing,neuroprotection and burns have all shown a reduction in proinflammatory cytokines,improved survival and increased resistance to pathological challenges with DHEA *** supplementation studies,which have focused on post-menopausal females,older adults,or adrenal insufficiency have shown that restoring the cortisol:DHEAS ratio improves wound healing,mood,bone remodelling and psychological ***,there are no DHEA or DHEAS supplementation studies in trauma patients,but we review here the evidence for this potential therapeutic agent in the treatment and rehabilitation of the severely injured patient.

BACKGROUND Dilaceration is a rare dental developmental anomaly characterized by an abrupt deviation along the longitudinal axis of the root in which an angulation forms between the root and the ***,we report on dilacerated bilateral maxi-llary central incisors in mixed *** SUMMARY A 10-year-old girl presented with a chief complaint of unerupted central *** oral examination and radiography provided the basis for a diagnosis of dilaceration of the maxillary central *** surgical exposure of the impacted teeth,a button with an attached chain was applied to the palatal surface of teeth 11 and *** 8 mo,a button was bonded to the labial surface of the crown to fix an elastic chain and move the teeth toward the maxillary ***,a fixed appliance was applied to tooth alignment to Class 1 malocclusion using a 0.019×0.025-inch nickel-titanium *** 3 years of follow-up,the clinical findings and radiographic assessment showed that the roots had developed with vital dental pulp and healthy periodontium,were acceptable aesthetically,and showed no *** The rare occurrences of dilacerated bilateral maxillary central incisors can be successfully treated through surgical exposure and orthodontics.

Coagulation complications following trauma

traumatic injury is one of the leading causes of death, with uncontrolled hemorrhage from coagulation dysfunction as one of the main potentially preventable causes of the mortality. Hypothermia, acidosis, and resuscitative hemodilution have been considered as the significant contributors to coagulation manifestations following trauma, known as the lethal triad. Over the past decade, clinical observations showed that coagulopathy may be present as early as hospital admission in some severely injured trauma patients. The hemostatic dysfunction is associated with higher blood transfusion requirements, longer hospital stay, and higher mortality. The recognition of this early coagulopathy has initiated tremendous interest and effort in the trauma community to expand our understanding of the underlying pathophysiology and improve clinical treatments. This review discusses the current knowledge of coagulation complications following trauma.
