Administration of Levothyroxine 45 - 60 Minutes before Breakfast Improves Biochemical Availability as Evidenced by Reduced thyrotropin Levels

Introduction: Serum thyrotropin (TSH) level is used to assess adequacy of levothyroxine dosing for patients with hypothyroidism. Some patients have raised TSH levels despite being on an adequate dose of levothyroxine (100 mcg/day - 200 mcg/day). Aim: To evaluated the effect of advising patients to take their levothyroxine 45 - 60 minutes before breakfast on raised serum TSH levels. Patients and Methods: Rather than increase the dose, patients with raised TSH levels were asked to take their levothyroxine at least 45 - 60 minutes before breakfast and other oral medications. Thyroid Function Tests were assessed at base line and repeated after two months. Results: Data from ten patients who presented between 2008 and 2010 were analyzed (9 females, 1 male): With median (IQR) age: 39 (33 - 49) years and duration of hypothyroidism: 6 (3 - 7.8) years. Median (IQR) levothyroxine dose was 175 (144 - 250) mcg, serum free-Thyroxine (free-T4): 13 (10.5 - 17.1) pmol/L and serum TSH: 12.63 (6.2 - 48.3) mIU/L. After two months all patients demonstrated biochemical improvement;a decrease in serum TSH to 3.15 (0.4 - 6.1) mIU/L accompanied by an increase in serum free-T4 to 17.7 (14.8 - 21.3) pmol/L. Both changes were statistically significant (p < 0.05 and p < 0.01, respectively). The median (IQR) percentage TSH reduction was 83.5 (40.3 - 95.8) mIU/L and this bore no significant correlation with the initial TSH level (rs = 0.2, p = 0.58). Conclusion: Changing levothyroxine administration to 45 - 60 minutes before breakfast and other oral medications reduced TSH levels by 40% - 96% in all patients. We recommend this advice for all patients with hypothyroidism on adequate doses of levothyroxine but still appear biochemically under-replaced.

To get the hybridoma cell lines secreting anti-thyrotropin monoclonal antibodies with high affinity and specificity. Methods: BALB/c mice were immunized with extract of human pituitaries. The spleen cells of one immunized mouse were fused with mouse myeloma cells in polyethylene glycol and the positive clones were subcloned 3 times. Results: Two hybridoma cell lines which secrete anti-thyrotropin monoclonal antibodies with high affinity and specificity have been collected. The antibodies were of the IgG1 subclass and their maximum binding with thyrotropin was 60% and 45. 1% respectively. Using competitive binding assay,the antibodies were found to direct against different epitopes of human thyrotropin. Conclusion: The extract of human pituitaries could be used to produce monoclonal anti-pituitary hormone antibodies. The two anti-thyrotropin monoclonal antibodies produced in this study could be used in the establishment of a sensitive measurement of human thyrotropin.

Recently, we surveyed thyroid function and TSH concentration of villagers in an endemic goiter area where iodized salt had been supplied for 25 years; it was found that the serum FT3 and TSH levels determined with immunoradiometric analysis (IRMA) were higher and the FT4 level was lower than that of the controls. It was shown that the inhabitants of the endemic goiter area had subclinical hypothyroidism based on the "ultrasensitive" method for TSH assay. We suggest that the best biochemical techniques for monitoring the iodized salt prophylaxis program and the physiological response of the villagers to iodine should be the periodical measurement of serum TSH level using ultrasensitivc assay and determination of FT4 level.

Obejctive To investigate the cellular immune mechanism of thyroid associated ophthalmopathy (TAO) and provide a basis for treating TAO with cytokine or anti cytokine agents, we determined whether interferon (IFN) γ and interleukin (IL) 4, representative cytokines of Th1 and Th2 cells, may have some effect on the development and progression of TAO Method Retroorbital fibroblasts (RF) proliferation and the synthesis of hyaluronic acid (HA) and type Ⅳ collagen were measured with liquid scintillation and radioimmunoassay Results IFN γ stimulated RF proliferation and HA synthesis and had a significant inhibitory effect on type Ⅳ collagen synthesis IL 4 stimulated proliferation and type Ⅳ collagen synthesis in RF and had inhibitory effect on HA synthesis When IFN γ (100?U/ml) and IL 4 (1?μg/L) were incubated together with RF, they antagonized their respective stimulatory or inhibitory effect on the proliferation and synthesis of HA and type Ⅳ collagen thyrotropin (TSH) stimulated RF proliferation and type Ⅳ collagen synthesis in a dose dependent manner, while only at high dose (100?U/L and 200?U/L), stimulated RF to synthesize HA Conclusions IFN γ, a representative cytokine of Th1 cells, is responsible for the inflammatory process of TAO; whereas IL 4, a representative cytokine of Th2 cells, has some effect on the repair process IL 4 could antagonize the inflammatory effect of IFN γ on RF TSH may have aggravating effect on the pathogenesis of TAO

Objective:To investigate thyroid-stimulating hormone(TSH)levels and thyroid peroxidase antibody(TPOAb)in early pregnancy and to estimate the risk of development of gestational diabetes mellitus(GDM).Methods:The levels of TSH,free thyroxine,free triiodothyronine,and TPOAb were retrospectively evaluated in 2333 pregnant women.All women recruited were divided into groups of TSH high(H),TSH low(L),TSH(H)TPOAb positive(+),TSH(H)TPOAb negative(-),TSH normal(N)TPOAb(+),TSH(L)TPOAb(+),TSH(L)TPOAb(-),and estimated the risk of GDM.Results:TSH level(X^(2)=13.0024,P=0.0015)and TSH combined TPOAb(X^(2)=20.0038,P=0.0012)were correlated to development of GDM.The level of TSH in 95 women was increased,and in 216 women were decreased.Of the 2333 pregnant women,546 patients were diagnosed with GDM,with a prevalence of 23.40%.The prevalence of GDM was 35.78%and 28.70%for the TSH high(H)and TSH low(L)pregnant women,respectively.The increased TSH(P=0.0013,odds ratio:2.09,confidence interval:1.34-3.28)was correlated to increase GDM incidence.TSH(H)TPOAb(+)(n=29),TSH(H)TPOAb(-)(n=58),TSH normal(N)TPOAb(+)(n=265),TSH(L)TPOAb(+)(n=30),TSH(L)TPOAb(-)(n=154)were in this study.The prevalence of GDM was 51.72%,29.31%,23.02%,26.67%,and 29.87%for the TSH(H)TPOAb(+),TSH(H)TPOAb(-),TSH(N)TPOAb(+),TSH(L)TPOAb(+),and TSH(L)TPOAb(-)pregnant women,respectively.Only the TSH(H)TPOAb(+)pregnant women had a significant higher incidence of GDM(P=0.0018,odds ratio:3.63,confidence interval:1.62-8.16).Trimester specific results showed that only second trimester had higher GDM risk either analyze TSH or the combination of TSH and TPOAb.Conclusion:Elevated TSH level and TPOAb positive in the second trimester of pregnancy were correlated to an increased risk of GDM.

The effects of khat on the hormonal levels have been established;however, the effects on human beings are controversial. The aim of our study was to investigate the possible effects of khat on the levels of serum thyroid hormones, testosterone, estradiol (E2), prolactin and cortisol in men. A total of 50 blood samples were collected from healthy males who referred to chew khat for more than 10 years and analyzed for the above hormones. The results were compared to the hormonal levels of 35 non khat chewers. Chewing khat causes significant increases in the testosterone (P < 0.03), prolactin (P < 0.05), E2 (P < 0.00005), FT3 (P < 0.04), and TSH (P < 0.05) levels. No significant differences were found in the serum level of FT4 between the two groups. The level of cortisol were significantly lower (P < 0.001) in the khat chewers group compared to the control group. This study suggests that khat chewing can cause reduction in the cortisol level, which may cause increases of testosterone, prolactin and E2. In addition, chewing khat increases the level of TSH and FT3 serum levels. Therefore, khat may contribute to the relevant disorders caused by abnormal levels of the studied hormones in the people who are chewing
