Assessment of hindlimb motor recovery affer severe Thoracic spinal cord injury in rats: classification of CatWalk XT■ gait analysis parameters

Assessment of locomotion recovery in preclinical studies of experimental spinal cord injury remains challenging. We studied the CatWalk XT■gait analysis for evaluating hindlimb functional recovery in a widely used and clinically relevant Thoracic contusion/compression spinal cord injury model in rats. Rats were randomly assigned to either a T9 spinal cord injury or sham laminectomy. Locomotion recovery was assessed using the Basso, Beattie, and Bresnahan open field rating scale and the CatWalk XT■gait analysis. To determine the potential bias from weight changes, corrected hindlimb(H) values(divided by the unaffected forelimb(F) values) were calculated. Six weeks after injury, cyst formation, astrogliosis, and the deposition of chondroitin sulfate glycosaminoglycans were assessed by immunohistochemistry staining. Compared with the baseline, a significant spontaneous recovery could be observed in the CatWalk XT■parameters max intensity, mean intensity, max intensity at%, and max contact mean intensity from 4 weeks after injury onwards. Of note, corrected values(H/F) of CatWalk XT■parameters showed a significantly less vulnerability to the weight changes than absolute values, specifically in static parameters. The corrected CatWalk XT■parameters were positively correlated with the Basso, Beattie, and Bresnahan rating scale scores, cyst formation, the immunointensity of astrogliosis and chondroitin sulfate glycosaminoglycan deposition. The CatWalk XT■gait analysis and especially its static parameters, therefore, seem to be highly useful in assessing spontaneous recovery of hindlimb function after severe Thoracic spinal cord injury. Because many CatWalk XT■parameters of the hindlimbs seem to be affected by body weight changes, using their corrected values might be a valuable option to improve this dependency.

To the Editor:The morbidity of Thoracic lesions is very high during the treatment of malignant hematological diseases,and tumor recurrence or invasion is common,in addition to infections.Percutaneous lung biopsy or bronchoscopy could result in severe hemoptysis,pneumothorax,or infection because of low platelets and hypo-immunity,and treatment is often ineffective due to the ambiguous diagnosis.Even when a patient is diagnosed with a fungal disease,it is difficult to eliminate the lesion with only antifungal medicine.

Biomaterial bridging provides physical substrates to guide axonal growth across the lesion.To achieve efficient directional guidance,combinatory strategies using permissive matrix,cells and trophic factors are necessary.In the present study,we evaluated permissive effect of poly(acrylonitrile-co-vinyl chloride)guidance channels filled by different densities of laminin-precoated unidirectional polypropylene filaments combined with Schwann cells,and glial cell line-derived neurotrophic factor for axonal regeneration through a T10 hemisected spinal cord gap in adult rats.We found that channels with filaments significantly reduced the lesion cavity,astrocytic gliosis,and inflammatory responses at the graft-host boundaries.The laminin coated low density filament provided the most favorable directional guidance for axonal regeneration which was enhanced by co-grafting of Schwann cells and glial cell line-derived neurotrophic factor.These results demonstrate that the combinatorial strategy of filament-filled guiding scaffold,adhesive molecular laminin,Schwann cells,and glial cell line-derived neurotrophic factor,provides optimal topographical cues in stimulating directional axonal regeneration following spinal cord injury.This study was approved by Indiana University Institutional Animal Care and Use Committees(IACUC#:11011)on October 29,2015.

Effects of obesity on breast size,Thoracic spine structure and function,upper torso musculoskeletal pain and physical activity in women

Purpose:This study investigated the effects of obesity on breast size,Thoracic spine structure and function,upper torso musculoskeletal pain and physical activity participation in women living independently in the community.Methods:A total of 378 women were divided into 3 groups(Not Overweight:body mass index(BMI)=22.5±0.2 kg/m^2(mean±SE);Overweight:BMI=27.4±0.3 kg/m^2;Obese:BMI=35.4±0.3 kg/m^2).Outcome variables of breast volume(mL),Thoracic flexion torque(N·m),Thoracic kyphosis(degrees),upper torso musculoskeletal pain(score) and time spent in physical activity(min) were calculated and compared among the 3 groups,adjusting for between-group differences in age.Results:There was a significant main effect of BMI on all outcome variables.Participants classified as Obese displayed significantly larger breasts,had greater Thoracic flexion torques and reported less time participating in physical activity relative to the participants who were classified as Not Overweight and Overweight.Participants in the Obese group also displayed significantly more Thoracic kyphosis and reported significantly more upper torso musculoskeletal pain compared to their counterparts who were classified as Not Overweight.Conclusion:This study is the first to demonstrate that increased obesity levels were associated with compromised kyphosis and loading of the Thoracic spine,as well as increased symptoms of upper torso musculoskeletal pain and reduced time spent in physical activity in women living in the community.We recommend further research to determine whether evidence-based interventions designed to reduce the flexion torque generated on the Thoracic spine can improve these symptoms of upper torso musculoskeletal pain and the ability of women with obesity to participate in physical activity.

The life-saving emergency Thoracic endovascular aorta repair management on suspected aortoesophageal foreign body injury

BACKGROUND:Fatal aortic rupture caused by esophageal foreign body(EFB),is associated with a high mortality,but can be prevented by Thoracic endovascular aorta repair(TEVAR)that performed increasingly as technology improves.This study aims to investigate the cause,management and prognosis of suspected penetrating aortoesophageal foreign body injury.METHODS:Twelve cases who met the criteria were enrolled in this study.The demographic and clinical data were reviewed for evaluating the characteristics of EFB.RESULTS:Among 12 cases enrolled,7 were males and 5 were females,with an age 27–86 years.The distance of EFB from aorta(DFA)of 7 cases were less than or equal to 0 mm,5 cases were 0–2 mm.Eleven cases were managed with TEVAR,only one case was with open surgery standby but finally treated by flexible endoscopy(FE)successfully,without TEVAR.In group with TEVAR,EFB of 7 cases were successfully removed by rigid endoscopy(RE),and one of them was failed at the first RE treatment.EFB of 2 cases were successfully removed by open surgery with TEVAR,and other 9 cases were managed by endoscopies with TEVAR.The mean length of stay of hospitalization(LOS)and length of ICU stay of patients treated by open surgery with TEVAR(18.50±2.12 days and 5.50±0.71 days)was significantly longer than those of patients treated by endoscopy with TEVAR(7.00±2.74 days and 1.33±1.12 days,P<0.001 and P=0.001,respectively).Five cases had severe complications.CONCLUSION:Rational application of TEVAR can be a life-saving management for aortoesophageal foreign body injury,and jointed with endoscopy is safe and effective with a shorter length of ICU or total hospital stay.

SMARCA4-deficient undifferentiated Thoracic tumor:A case report

BACKGROUND SMARCA4-deficient undifferentiated tumors(SMARCA4-DUTs)present with diverse clinical manifestations and progress to metastasis and even cause death within a few months.This novel subset of undifferentiated tumors occurs in the middle-aged population and is strongly associated with a smoking history.Distin-guishing it from other malignancies is challenging.CASE SUMMARY A 62-year-old man presented with chest pain for 7 d.The patient had no respiratory symptoms and normal pulmonary function test results.The patient had been a smoker for 8 years and quit smoking 2 years ago.Chest computed tomography revealed a huge mass involving the left upper and lower lung lobes with pericardial invasion and multiple metastases.Tumor samples were obtained using open frozen biopsy,after several unsuccessful attempts.The tumor was composed of sheets of undifferentiated disclosive cells with vesicular nuclei and prominent nucleoli.The differential diagnosis included high-grade lymphoma,germ cell tumor,NUT carcinoma,undifferentiated carcinoma,and sarcoma.The tumor cells were large,arranged in sheets,and did not exhibit glandular or squamous differentiation.Frequent foci of necrosis were noted.There was no evidence of epithelial differentiation on immunohistochemical staining.The SMARCA4 stain showed complete loss of expression of SMARCA4,which is diagnostic.CONCLUSION In the present case,Thoracic SMARCA4-DUT was diagnosed based on clinical features,absence of epithelial differentiation,and negative SMARCA4 expression.

Physician-modified fenestrated stent-graft for Thoracic endovascular aortic repair(TEVAR)has economic advantages and can be used in acute situations for aortic dissection and aneurysm involving aortic arch.[1]However,accurate alignment of fenestrations and branch arteries of the aortic arch is challenging,especially for multi-fenestration.[2]Once misaligned,complications such as branch occlusion or cerebral ischemia will occur.Details about how to align fenestrations with branch arteries of the aortic arch in TEVAR with physician-modified triple fenestrated stent-graft are elaborated in this article.

Dear Editor,We would like to present a novel system for monitoring double-lumen tube(DTL)positions in Thoracic surgery by respiratory sounds at the trachea and the bronchi,based on our previously described methodology,algorithms,and patented technology[1-2].Most Thoracic surgeries require one-lung ventilation(OLV).A DLT or a bronchial blocker(BB)is the most common means for managing OLV.Intraoperative DLT or BB malposition is a critical issue that needs to be accurately and promptly detected to prevent the incidence of hypoxemia and unintended re-expansion of operated lung during OLV[3-4].A multi-center study of 2127 patients requiring OLV during Thoracic surgery revealed that DLTs were used in 95%of patients with a malposition rate of 14%,and BBs were used in 5%of patients with a malposition rate of 33%[5].

To the Editor:Continuous monitoring of hemodynamic status can be used to detect early abnormal circulation and is very helpful for the management of the critically ill.Today,different minimally invasive and non-invasive monitoring systems can measure functional hemodynamic parameters to guide treatment planning and improve patient care.Among them,the Thoracic electrical bioimpedance(TEB)technique is widely available and is accurate for most patients,especially after open-heart surgery.However,there are ethnicity-related differences among hemodynamic characteristics and the hemodynamic response to the same cardiovascular disease differs among racial groups.Currently,no nationwide hemodynamic parameter reference ranges can be used for healthy Chinese Han adults.A large-scale,forward-looking research investigation of the hemodynamic state markers in China has not been wholly reported.Therefore,it is necessary to establish ethnicity-specific hemodynamic reference ranges for the Chinese Han population and evaluate the relationship between these parameters and age/sex,in order to promote efficient and accurate judgment in clinical practice.From July 2012 to September 2017,a prospective,nationwide study led by the Ministry of Science and Technology of the People’s Republic of China was designed,organized,and performed to establish normal reference ranges of hemodynamic parameters for the Chinese Han population and to confirm the impacts of age and sex on these parameters.

To the Editor:Bronchial artery aneurysm(BAA)is a rare but potentially fatal disease.BAA is reported in less than 1%of all selective bronchial arteriographies.[1]The primary treatment is endovascular therapy,including transcatheter arterial embolization or aortic stent-graft placement.Surgical treatment can be considered for patients with treatment failure,recurrence or contraindications to endovascular therapy.Herein,we present a rare case of an ectopic mediastinal bronchial artery aneurysm that was successfully resected via video-assisted Thoracic surgery(VATS).
