tcm diagnosis:INTER RATER RELIABILITY AND THE EFFECTS OF NORMALISATION

Design: Five experienced practitioners each defined the health of 42 subjects using fourteen tcm diagnostic descriptors and a Likert scale of ***: The data were divided into three groups: most well,intermediate and least well based upon the subjects total pathogenic score as derived from the total score accrued from the fourteen *** normalizations to remove bias, total score bias, individual descriptor bias and a combination of score and descriptor bias were applied to the *** agreement and Gwet's AC1 were reported in raw and normalized ***: Agreement was 60% and AC1 of 45 agreement within the three wellness groups was as follows:(1) Most well 72%,AC1 0.64;(2) Intermediate 58%, AC1 0.44;(3) Least well 50%,AC1 *** poor performance of the least well group's agreement was *** was most effective in the least well group with descriptor normalization, where a 17% improvement,while a combination of descriptor and score normalization elicited a 14% *** and Recommendations: Normalization improved inter-rater agreement in pathogenic diagnosis of *** practitioners removed scoring bias should have thesame effect, and may therefore significantly improve diagnostic *** is recommended that if further investigations confirm the indications of this preliminary study, a ‘true score' training program will be employed from the ‘top down' in our profession and improve agreement between researchers, teaching staff, practitioners and students.
