The association between admission systolic blood pressure and 1-year mortality in patients with non-ST-segment elevation acute coronary syndrome

Background In patients with acute coronary syndrome（ACS）, lower admission systolic blood pressure（SBP）levels infer a worse prognosis. However, the predictive potential of admission SBP on 1-year mortality has not fully elucidated in patients with non-ST-segment elevation ACS（NSTEACS）. Methods We enrolled 1325 patients to investigate the association between admission SBP in patients hospitalized for NSTEACS. We analyzed the association between admission SBP and 1-year mortality. Admission SBP was categorized as low（〈110 mm Hg）, normal（110-140 mm Hg）, high（141-160 mm Hg）, and very high（〉160 mm Hg）. Results Compared with patients with normal admission SBP, those with low SBP had a significantly increased hazard ratios（HRs） for 1-year mortality of 3.03（P〈0.05）, while patients with high and very high admission SBP had no significantly increased HRs for 1-year mortality. Conclusion Low admission SBP, but not elevated admission SBP, is a strong independent predictor of 1-year mortality in patients with NSTEACS.

Background There were few studies to explore the relationship between postoperative mean systolic blood pressure （PMSBP） within 6 h after cardiac catheterization and contrast-induced nephropathy （CIN）. Methods From June 2010 to February 2013, 299 consecutive patients undergoing cardiac catheterization were recruited. Patients were classified into quartiles based on PMSBP （〈 112, 112-120, 121-131, and 〉 132 mmHg）. Baseline data, CIN incidence and in-hospital outcomes were compared between the groups. Logistic regression was used to assess the relationship between PMSBP and CIN. Results CIN occurred in 26（8.7%） patients. The incidence of CIN in PMSBP quartiles were 15.3% （11/72）, 15.1% （11/73）, 2.4% （2/82） and 2.8% （2/72）（P = 0.001） respectively. There were no significant difference in in-hospital death, renal replacement therapy and intra-aortic balloon pump （IABP） support between the groups （P 〉 0.05）. Univariate logistic regression analysis showed that PMSBP was significantly associated with CIN （OR = 0.956, 95% CI： 0.928-0.986, P = 0.004）. Multivariate logistic regression analysis found that after adjusting baseline estimate glomerular filtration rate, age 〉 75 years and acute myocardial infarction, PMSBP 〈 120 mmHg was still an independent risk factor for CIN （OR = 5.049, 95% CI： 1.820-14.009, P = 0.002）. Conclusions Lower PMSBP was significantly associated with an increased risk of CIN. PMSBP 〈 120 mmHg was an independent risk factor for CIN. Intensive blood-pressure control after cardiac catheterization might increase the risk of CIN.

Objective:A positive association between hypertension or high-normal blood pressure(BP) and risk of non-alcoholic fatty liver disease(NAFLD) is well-known,however,no data have been generated exploring the risk of NAFLD within the normal range of *** aimed to assess the association between normal systolic blood pressure(SBP) and risk of ***:A total of 27769 subjects from two separate medical centers were *** were divided into four groups(G1 to G4) by SBP levels:G1:90-99 mmHg,G2:100-109 mmHg,G3:110-119 mmHg and G4:120-129 *** prevalence,hazard ratios(HRs) and 95%confidence intervals(CIs) for NAFLD were calculated across each group,using the G1 as ***:Higher SBP was observed in subjects with NAFLD than those without *** prevalence of NAFLD in a cross-sectional population from G1 to G4 was 6.1%,13.6%,19.6%and 25.8%,*** HRs for NAFLD in the longitudinal population were 2.17(95%CI 1.60-2.93),3.87(95%CI 2.89-5.16),5.81(95%CI 4.32-7.81) for G2,G3,and G4,*** adjusting for known confounding variables,HRs of G2 to G4 were 1.44(95%CI 1.06-1.96),1.94(95%CI 1.44-2.61),2.38(95%CI 1.75-3.23),***:This is the first study to demonstrate that increased levels of SBP within the normal range are associated with significantly elevated risks of NAFLD,independent of other confounding factors.

Objective To evaluate the association between diabetic retinopathy(DR) and mean ocular perfusion pressure(MOPP) in patients with type 2 diabetes mellitus(T2 DM).Methods Patients from the Fushun Diabetic Retinopathy Cohort Study(FS-DIRECT), a communitybased prospective cohort study conducted in northeast China, were included in this study. The presence and severity of DR were determined by grading fundus photographs according to the Early Treatment Diabetic Retinopathy Study(ETDRS) retinopathy scale. systolic and diastolic blood pressure(SBP and DBP) were recorded using an electronic sphygmomanometer. Intraocular pressure(IOP) was measured using an iCare rebound tonometer. MOPP was calculated using the formula MOPP = 2/3 [DBP + 1/3(SBP-DBP)]-*** In total, 1,857 patients who had gradable fundus photography and MOPP data were enrolled in this study. Male patients had a higher MOPP than female patients(52.25 ± 8.75 vs. 50.96 ± 8.74 mmHg, P = 0.002). Overall, both male and female patients with any type of DR, non-proliferative DR(NPDR), or non-sight-threatening DR(non-STDR) had significantly higher MOPP relative to patients without DR. Increased MOPP(per 1 mmHg) was in turn associated with the presence of any type of DR[odds ratio(OR) = 1.03, 95% confidence interval(CI) : 1.02–1.04], NPDR(OR = 1.03 95% CI: 1.02–1.04),and non-STDR(OR = 1.03, 95% CI: 1.01–1.04) after adjusting for confounders. Increased MOPP(per 1 mmHg) was also associated with an increased likelihood of macular edema(OR = 1.02, 95% CI:1.01–1.04).Conclusions The results suggest that increased MOPP was associated with DR and macular edema in northeastern Chinese patients with T2 DM.

Little is known about early coincidental changes in bone and vascular properties, particularly in the context of skeletal anabolism （puberty） versus relative equilibrium （young adulthood）. We aimed to determine if subclinical markers of vascular function were associated with bone mineral content （BMC） and to evaluate the contribution of systemic factors in healthy females ages 14-42 years. Endothelial function was assessed by flow mediated dilatation （FMD）, arterial stiffness by pulse wave velocity （PWV） and augmentation index （AIx）, blood pressure （BP） by sphygmomanometer, BMC by DXA, and systemic factors by fasting blood draw. General linear models controlled for age, race and height indicated a positive association between systolic BP （SBP） and BMC independent of systemic factors. When stratified by age using 19 years as a cut-point, there was an inverse relationship between AIx75 in adolescents with insulin （P〈0.10） or inflammatory markers （P〈0.10） in statistical models. Conversely, there was a positive relationship between BMC and both PWV and AIx75 in young adults （P〈0.05）. The link between bone and the vasculature may be life stage-dependent. In the context of a less dynamic microenvironment in young adult females, metabolic factors appear to moderate less of an effect of hemodynamic properties on the skeleton relative to adolescents.

Background Debates exist about the preference for single-or multiple-set resistance exercise(RE)on achieving similar training outcome,however,no study examined their effects on the heart,thus,this study was to examine the effect of volume-matched single-and multiple-set RE on the myocardial response through examining rate pressure product(RPP).Methods Fourteen healthy untrained college students performed two leg press exercises(4×10 and 1×40 at 150%body mass)on different days,and each was performed twice in a counterbalanced crossover ***-and post-exercise SBP and HR were measured by auscultatory method,RPP was calculated as SBP×HR×10^(-3),RPP difference(RPPdiff)was calcu-lated by post-exercise RPP subtracting pre-exercise *** Single-set post-exercise RPP(17.48±3.16 mmHg·beats/min),HR(123.86±20.25 beats/min)was significantly greater than multiple-set post-exercise RPP(13.66±3.04 mmHg·beats/min),HR(101.61±19.14 beats/min),*** was no significant difference of SBP(141.07±9.37 vs.134.29±15.16 mmHg)between exercise protocols(P=0.13).The RPPdiff of single-set(6.74±2.86 mmHg·beats/min)was significantly greater than that of multiple-set(3.62±2.90 mmHg·beats/min)(P<0.001).Conclusions Single-set RE exerts a greater myocardial oxygenation response as compared to multiple-set,HR contributed more than SBP to the increase of RPP in *** results indicate that single-set RE can be warranted when wanting to train the heart,while multiple-set RE would appear best for those who should avoid heart stress.

Virtual reality(VR)seems to have the potential to provide opportunities to promote physical activity(PA)in a fun *** paper aimed to examine the acute effects of three different virtual reality-based exercise bikes on young adults’blood pressure(BP)and feelings compared to a traditional exercise cycling *** exercise sessions(immersive VR cycling,two non-immersive VR cycling,and traditional cycling)were completed by 36 young adults(22 females;Mage=23.6 years).BP was measured immediately before and after each session using a BP cuff and exercise-induced feelings were assessed via an established survey immediately after each ***-cipants’previous experience with VR was used as the covariate in the ANCOVA with repeated ***-icant main effects were observed across cycling sessions for systolic blood pressure[F(2,29)=3.04,p=0.02,(η^(2)=0.38)]and feelings[F(3,32)=7.74,psystolic blood pressure compared to the two non-immersive VR ***,immersive VR significantly increased feelings compared to the two non-immersive VR sessions,whereas these two non-immersive VR exercises had significantly greater increased feelings compared to traditional cycling,*** suggest immersive VR-based exercise cycling may lead to higher exercise intensities compared to non-immersive VR ***,immersive VR cycling yielded higher feelings compared to non-immersive VR and traditional ***,immersive VR-based exercise can be a fun and physically active health promotion tool among young adults.
