Left atrial appendage aneurysm （LAAA） is an extremely rare cardiac anomaly which is mainly characterized by localized or diffuse dilatation of left atrial appendage.[1] This condition with diverse symptoms including arrhythmia,thromboembolic events,and heart failure confuses physician and is easily misdiagnosed.[2] We herein report a case of huge LAAA which is misdiagnosed originally.

Background Anti-N-methyl-D-aspartate receptor(NMDAR)encephalitis accounts for the vast majority of cases of autoimmune *** common clinical symptoms are psychiatric symptoms,behavioral dysfunction,seizures,speech impairment,cognitive impairment,movement disorders,decreased consciousness,and autonomic *** report was aimed to provide evidence for anti-NMDAR encephalitis with seizure-like activity,based on the clinical presentations,brain magnetic resonance imaging(MRI),and electroencephalogram(EEG)*** presentation We report a 11-year-old girl who suffered epigastric pain after vigorous physical *** felt weakness of the *** had syncope only once and urinary incontinence on the way to the hospital and vomited at the *** entire procedure lasted approximately 15 *** next night she had recurrent headache attacks along with irritability,like someone“tapping”to the right occipital *** experienced numbness in the left upper limbs and shallowing of the frontal lines on the left *** monitoring showed normal findings,but brain MRI,EEG recording,and cerebrospinal fluid(CSF)antibody testing showed abnormal *** patient was finally diagnosed with anti-NMDAR *** treatment with glucocorticoids and acyclovir,she fully recovered and was discharged from the *** Although the patient presented with pain and impaired limb movements,she did not have respiratory or(and)circulatory failure,and the symptoms entirely *** accurate diagnosis and timely treatment are essential for patients with anti-NMDAR encephalitis.

We presented a case of anomalous single-coronary artery detected incidentally during routine coronary angiography. A 32-year-old male Chinese patient presented with recurrent pre-syncope and six episodes of syncope. Coronary angiography and coronary-computed tomography (CT)-angiography performed by a dual-source computed tomography (DSCT) revealed that the patient had a single large right coronary artery. A moderately large branch originated from the proximal part of the single right coronary artery and extended to the left,passing the anterior to the pulmonary artery,and divided into the anterior descending artery branch and circumflex branch at the base of the left auricular appendage. The episodes of the syncope were suspected to be caused by coronary arterial spasm,so this patient was on a regimen of 30 mg of diltiazem every 6 h and had no recurrence of syncope during follow-up.

Venepuncture is the commonest procedure performed in health care settings. A review of literature and search on complications of venepuncture and blood collection. This procedure is without complications, which sometime can be fatal. Complications that can arise from venepuncture include haematoma formation, nerve damage, pain, haemaconcentration, extravasation, iatrogenic anaemia, arterial puncture, petechiae, allergies, fear and phobia, infection, syncope and fainting, excessive bleeding, edema and thrombus.

We report a 43-year-old man who had been experiencing recurrent attacks of syncope for several years, and a search for the causes behind these lapses in consciousness was fruitless for many years. The patient was admitted to our department of internal medicine because of the symptoms flushing, hypotension and episodes of syncope. After a careful anamnesis and then a comprehensive investigation, we reached the definitive diagnosis of the infrequent disease “systemic mastocytosis”.
