Persistent increase and improved survival of stage I lung cancer based on a large-scale real-world sample of 26,226 cases

Background:Lung cancer prevails and induces high mortality around the *** study provided real-world information on the evolution of clinicopathological profiles and survival outcomes of lung cancer,and provided survival information within stage I ***:Patients pathologically confirmed with lung cancer between January 2009 and December 2018 were identified with complete clinicopathological information,molecular testing results,and follow-up *** in clinical characteristics were evaluated usingχ^(2)*** survival(OS)was calculated through the Kaplan-Meier ***:A total of 26,226 eligible lung cancer patients were included,among whom 62.55%were male and 52.89%were ***-smokers and elderly patients took increasingly larger proportions in the whole patient *** proportion of adenocarcinoma increased from 51.63%to 71.80%,while that of squamous carcinoma decreased from 28.43%to 17.60%.Gene mutations including EGFR(52.14%),KRAS(12.14%),and ALK(8.12%)were ***,younger,non-smoking,adenocarcinoma patients and those with mutated EGFR had better survival ***,this study validated that early detection of early-stage lung cancer patients had contributed to pronounced survival benefits during the *** with stage I lung cancer,accounted for an increasingly considerable proportion,increasing from 15.28%to 40.25%,coinciding with the surgery rate increasing from 38.14%to 54.25%.Overall,period survival analyses found that 42.69%of patients survived 5 years,and stage I patients had a 5-year OS of 84.20%.Compared with that in 2009-2013,the prognosis of stage I patients in 2014-2018 was dramatically better,with 5-year OS increasing from 73.26%to 87.68%.Regarding the specific survival benefits among stage I patients,the 5-year survival rates were 95.28%,93.25%,82.08%,and 74.50%for stage IA1,IA2,IA3,and IB,respectively,far more promising than previous ***:Crucial clinical and pa

Secondary sentinel lymph node tracing technique: a new method for tracing lymph nodes in radical gastrectomy for advanced gastric cancer

Objective： To explore the feasibility and clinical value of secondary sentinel lymph node （SSLN） tracing technique in radical gastrectomy for advanced gastric cancer （AGC）. Methods： From January 2009 to June 2011,247 patients who suffered from gastric angle cancer with metastasis in No. 3 group lymph nodes were divided randomly into groups A and B. Methylthioninium chloride was injected into the peripheral tissue of the metastatic No. 3 group lymph nodes of 138 patients in group A before tumor resections. SSLNs were traced and individual lymphadenectomies were carried out based on the biopsy results of the SSLNs. Standard D2 radical gastrectomies were carried out directly on 109 patients in group B. Postoperative follow-up and survival analysis were carried out for patients in both groups. Results： SSLNs were found in 114 （82.6%） patients in group A. Ninety of those patients （78.9%） demonstrated existing metastasis in SSLNs. According to Kaplan-Meier＇s method, the postoperative 3-year cumulative survival rates were 63.5% and 47.5%, and the median survival time were 40 and 36 months for the patients of groups A and B, respectively （P〈0.05）. Conclusions： The SSLN tracing technique is feasible in radical gastrectomy for AGC. It gives surgeons important information about the terminal status of lymph node metastasis and provides some scientific basis for individual lymphadenectomy.

Prognostic factors for primary superficial transitional cell carcinoma of the bladder:a retrospective cohort study

Background Previous studies showed that the prognostic factors for superficial transitional cell carcinoma of the bladder varied with the findings of different cohorts. Few multivariate analyses of prognostic factors for superficial bladder tumors have been reported in China and bladder preservation as a prognostic index of superficial bladder tumors is limited and scarce in Chinese patients. This study was conducted to analyze a group of risk factors for prognostic outcomes for patients with primary superficial transitional cell carcinoma of the bladder. Methods Between January 1980 to December 2000, 198 patients [172 men and 26 women; mean age （52.98± 11.28） years] with primary superficial transitional cell carcinoma who were pathologically classified as Ta or T1 in Hunan Provincial Tumor Hospital （Changsha, China） were enrolled in this study. Surgical methods included local resection and electric coagulation of bladder tumors, transurethral resection of bladder tumors and partial cystectomy. After initial surgical treatment, patients were followed through a cystoscopy every three months during the first two years and every six months thereafter in the design of retrospective cohort. survival analysis was performed to analyze risk factors of the prognostic outcomes for transitional cell carcinoma of the bladder. Canonical correlation analysis was conducted to present and interpret synthetically the multi-correlation between all kinds of prognostic outcomes and risk factor in multiply dimensions. Results The average follow-up period was （6.65±4.74） years. Assessments at three, five, and 10 years showed recurrence rates, respectively, of （28.32±3.45）%, （35.31±3.83）%, and （42.48±4.40）%; progression rates of （8.89±2.14）%, （15.16±2.94）%, and （23.88±4.19）%; bladder-preservation rates of （94.68± 1.74）%, （93.87± 1.91）%, and （91.51±2.49）%; metastasis rates of （8.25＋2.05）%, （11.24±2.47）%, and （28.94±4.93）%; and cancer-related survival rates of （95.02± 1.62）%, （90.70±2.45）%,

Paclitaxel-etoposide-carboplatin/cisplatin versus etoposidecarboplatin/cisplatin as first-line treatment for combined small-cell lung cancer: a retrospective analysis of 62 cases

Objective: To compare the efficacy and adverse effects of paclitaxel-etoposide-carboplatin/cisplatin(TEP/TCE) regimen with those of etoposide-carboplatin/cisplatin(EP/CE) regimen as first-line treatment for combined small-cell lung cancer(CSCLC).Methods: A retrospective study was conducted on 62 CSCLC patients who were treated at Tianjin Medical University Cancer Institute and Hospital from July 2000 to April 2013 and administered with TEP/TCE regimen(n=19) or EP/CE regimen(n=43) as first-line CSCLC treatment. All patients received more than two cycles of chemotherapy, and the response was evaluated every two cycles. The primary endpoint was overall survival(OS), and the secondary endpoints were progression-free survival(PFS), objective response rate(ORR), disease control rate(DCR), and adverse effects. Results: ORR between the TEP/TCE and EP/CE groups showed a statistical difference(90% vs. 53%, P=0.033). Both groups failed to reach a statistical difference in DCR(100% vs. 86%, P=0.212). The median PFS and OS of the TEP/TCE group were slightly longer than those of the EP/CE group, although both groups failed to reach a statistical difference(10.5 vs. 8.9 months, P=0.484; 24.0 vs. 17.5 months, P=0.457). However, stratified analysis indicated that the PFS of patients with stages III and IV CSCLC showed marginally significant difference between the TEP/TCE and EP/CE groups(19.5 vs. 7.6 months; P=0.071). Both rates of grade IV bone marrow depression and termination of chemotherapy in the TEP/TCE group were significantly higher than those in the EP/CE group(26.3% vs. 7.0%, P=0.036; 31.6% vs. 14.7%, P=0.004). Conclusion: The TEP/TCE regimen may not be preferred for CSCLC, and this three-drug regimen requires further exploration and research. To date, the EP/CE regimen remains the standard treatment for CSCLC patients.

Overexpression of polo-like kinase1 predicts a poor prognosis in hepatocellular carcinoma patients

AIM： To elucidate the role of overexpressed polo-like kinasel （PLK1）in hepatocellular carcinoma （HCC）. METHODS： We prospectively collected clinicopathological, immunohistochemical and semi-quantitative reverse transcription-polymerase chain reaction （RT-PCR） data from 135 HCC patients undergoing successful hepatectomy. The correlations between PLK1 mRNA expression and clinicopathologic variables were analyzed by Mann-Whitney U test. Prognostic factors were identified by univariate and multivariate analyses. RESULTS： Immunohistochemical results showed overexpression of PLK1 was mainly found in tumor tissues compared with tumor-free tissue. A similar mRNA result was obtained by semi-quantitative RT-PCR. A total of 111 samples were positive for PLK1 mRNA expression. The positive expression was correlated with venous invasion, tumor nodules and Edmondson grade. Furthermore, 1, 3, 5-year survival rates in the positive expression group were significantly lower than the negative control group. Multivariate analysis showed that positive PLK1 expression was an independent risk factor for HCC. CONCLUSION： PLK1 could be a potential biomarker for diagnosis and therapy for HCC.

Combined treatment of oxaliplatin and capecitabine in patients with metastatic esophageal squamous cell cancer

AIM:To investigate the efficacy and side effects of the combined therapy of oxaliplatin and capecitabine in patients with metastatic esophageal squamous cell cancer(ESCC) and the survival of the ***:Sixty-four patients(median age of 63 years) with histological or cytological confirmation of ESCC received oxaliplatin 120 mg/m2 intravenously on day 1 and capecitabine 1000 mg/m2 orally twice daily on days 1 to 14 in a 21-d treatment cycle as palliative *** patient received at least two cycles of *** efficacy,side effects and patient survival were ***:The partial response(PR) rate was 43.8%(28/64).Stable disease(SD) rate was 47.9%(26/64),and disease progression rate was 15.6%(10/64).The clinical benefit rate(PR + SD) was 84.4%.The main toxicities were leukopenia(50.0%),nausea and vomiting(51.6%),diarrhea(50.0%),stomatitis(39.1%),polyneuropathy(37.5%) and hand-foot syndrome(37.5%).No grade 4 event in the entire cohort was *** median progression-free survival was 4 mo,median overall survival was 10 mo(95% CI:8.3-11.7 mo),and the 1-and 2-year survival rates were 38.1% and 8.2%,*** Karnofsky index,single metastatic lesion and response to the regimen indicated respectively good ***:Oxaliplatin plus capecitabine regimen is effective and tolerable in metastatic ESCC *** regimen has improved the survival moderately and merits further studies.

Comparisons of multiple characteristics between young and old lung cancer patients
