Primary hepatic leiomyosarcoma:A case report and literature review

BACKGROUND Primary hepatic leiomyosarcoma(PHL)is a rare tumor with a very low incidence of about 0.2%.CASE SUMMARY A 48-year-old diabetic,hypertensive,and morbidly obese female patient presented with a history of abdominal pain and weight loss for 2 *** had no history of fever,jaundice,or other liver disease(s).Clinical examination revealed a palpable mass in the *** evaluation with a contrast-enhanced computed tomography(CT)scan of the abdomen and pelvis revealed an illdefined enhancing hyper vascular hepatic mass of 9.9 cm×7.8 cm occupying the left hepatic lobe with evidence of central necrosis,compression effect on the left hepatic vein,and partial wash-out on delayed *** further workup,the maximum standardized uptake value on positron emission computed tomography scan was 6.4,which was suggestive of *** remaining part of the liver was normal without any evidence of ***-guided biopsy of the mass showed smooth muscle neoplasm suggestive of *** optimization for co-morbidities,an extended left hepatectomy was planned in a multidisciplinary team *** intraoperative ultrasound,the left hepatic lobe was entirely replaced by a large tumor extending to the caudate lobe with a compression effect on the middle and left hepatic *** histopathology showed nodular and whorled white tumor comprised of spind-led/fascicular cells with moderate to severe pleomorphism and focal *** mitotic index was greater than 20 mitoses per 10 high-power *** resection margins were free of ***(IHC)depicted a desmin-positive/caldesmon-negative/discovered on gastrointestinal stromal tumor 1-negative/cluster of differentiation 117-negative profile,confirming the definitive diagnosis as *** This case report highlights the rare malignant mesenchymal hepatic *** confirm PHL diagnosis,one requires peculiar histopathological findings with ancillary IHC confirmation.M

BACKGROUND Liver resection is the mainstay for a curative treatment for patients with resectable hepatocellular carcinoma(HCC),also in elderly *** this,the evaluation of patient condition,liver function and extent of disease remains a demanding process with the aim to reduce postoperative morbidity and *** To identify new perioperative risk factors that could be associated with higher 90-and 180-d mortality in elderly patients eligible for liver resection for HCC considering traditional perioperative risk scores and to develop a risk *** A multicentric,retrospective study was performed by reviewing the medical records of patients aged 70 years or older who electively underwent liver resection for HCC;several independent variables correlated with death from all causes at 90 and 180 d were *** coefficients of Cox regression proportional-hazards model for sixmonth mortality were rounded to the nearest integer to assign risk factors'weights and derive the scoring *** Multivariate analysis found variables(American Society of Anesthesiology score,high rate of comorbidities,Mayo end stage liver disease score and size of biggest lesion)that had independent correlations with increased 90-and 180-d mortality.A clinical risk score was developed with survival *** This score can aid in stratifying this population in order to assess who can benefit from surgical treatment in terms of postoperative mortality.

BACKGROUND Primary hepatic leiomyosarcoma is a very rare entity that originates from smooth *** diagnosis requires a high degree of suspicion due to atypical clinical presentation and non-specific imaging *** SUMMARY We report the case of a 42-year-old man,with no relevant past medical history,accidentally diagnosed with a nodular liver lesion on a routine abdominal *** function tests and hematology parameters as well as tumor markers were normal.A contrast-enhanced abdominal computed tomography scan revealed a heterogenous hepatic lesion measuring 40 mm 30 mm,adjacent to the left branch of the portal vein and the round *** to the unclear diagnosis,the patient underwent surgical *** and immunohistochemical examinations confirmed complete(R0)resection of a hepatic *** patient remains without any signs of tumor recurrence for more than 2 *** We report a rare case of accidentally diagnosed primary hepatic leiomyosarcoma originating from the portal vein or the round *** this tumor has aggressive metastatic potential,a tumor-free resection margin is essential to improve survival.

Primary liver cancer is amongst the commonest tumors worldwide,particularly in parts of the developing world,and is increasing in incidence. Over the past three decades,surgical hepatic resection has evolved from a high risk,resource intensive procedure with limited application,to a safe and commonly performed operation with a range of indications. This article reviews the approach to surgical resection for malignancies such as hepatocellular cancer,metastatic liver de-posits and neuroendocrine tumors. Survival data after resection is also reviewed,as well as indications for curative resection.

EYA4 gene functions as a prognostic marker and inhibits the growth of intrahepatic cholangiocarcinoma

Background:The molecular prognostic markers and carcinogenesis of intrahepatic cholangiocarcinoma(ICC) have not been well *** purpose of this study was to investigate the prognostic value of the eyes absent homolog 4(EYA4) gene in ICC and its biological effects on ICC growth in vitro and in ***:One hundred twelve patients with ICC who underwent hepatectomy were enrolled in the ***4 mRNA and EYA4 protein levels in ICC and adjacent non-tumoral tissues were evaluated using real-time quantitative polymerase chain reaction and immunohistochemical staining,***4 protein levels in ICC cells were determined using western blot *** associations between EYA4 expression and clinicopathologic features of ICC were *** identify independent prognostic factors,univariate and multivariate analyses were *** biological effects of EYA4 on ICC cells were evaluated by establishing stable EYA4-overexpressing transfectants in vitro,and EYA4's effects on tumor growth were evaluated by intra-tumoral injection of EVA4-expressing plasmids in a NOD/SCID murine model of xenograft ***:ICC tissues had significantly lower EYA4 mRNA and protein levels compared with adjacent non-tumoral tissues(both P < 0.001).Univariate and multivariate analyses showed that EYA4 protein level,tumor number,adjacent organ invasion,lymph node metastasis,and tumor differentiation were independent prognostic factors for diseasefree survival and overall survival(all P < 0.05).In vitro,EYA4 overexpression inhibited tumor cell growth,foci formation,and cell *** vivo,intra-tumoral injection of EYA4-expressing plasmids significantly inhibited ICC growth in the murine xenograft model compared with the control group(P < 0.05).Conclusion:EYA4 gene functioned as a molecular prognostic marker in ICC,and its overexpression inhibited tumor growth in vitro and in vivo.

Prognostic analysis of patients with gastrointestinal stromal tumors： a single unit experience with surgical treatment of primary disease

Background Gastrointestinal stromal tumor （GIST）, the most common type of mesenchymal tumors of the gastrointestinal tract, is a recently recognized tumor. The biological behavior of GIST is highly variable. surgical resection remains the major treatment for GIST. In this study we retrospectively analyzed our surgical experience with 181 GIST patients to determine the effects of the treatment and the pathological features and prognosis factors of these GIST patients. Methods The clinicopathological features and follow-up data of the 181 patients with GIST who had received surgical resection between January 1999 and December 2007 at Ren Ji Hospital were retrospectively reviewed. Immunohistochemical stains including CDl17 （KIT）, CD34, and other markers were used. Tumor size, mitotic index and other pathological parameters were recorded. According to the consensus of NIH risk-group stratification system based on maximum tumor size and mitotic index （per 50 high power field）, tumors were classified into very-low-risk group （15 tumors, 8.3%）, low-risk group （48, 26.5%）, intermediate-risk group （52, 28.7%） and high-risk group （66, 36.5%）. Prognostic factors were analyzed by Cox analysis including age, sex, tumor size, tumor site, mitotic index, NIH categories and surgical procedures. Results One hundred and seven （59.1%） of the 181 tumors were located in the stomach, 51 （28.2%） in the small intestine, 9 （5.0%） in the colon and rectum, and 14 （7.7%） in other sites including the omentum and mesentery. The median age of the patients was 58 （range, 24-84） years, and 102 patients （56.4%） were male. Tumor size ranged from 0.5 to 30 cm, while the mean size was 7.02 cm. Metastasis was found in 7 patients. One hundred and seventy-six （97.2%） of the 181 patients underwent radical resection, and among them 26 patients received extensive resection with the adjacent organ adherent to the tumors. Inflammatory fibroid polyps (IFP) are a rare benign entity that arises from the submucosa of the gastrointestinal tract and protrudes into the lumen. The clinical presentation depends on the size and localization. They are particularly found in the ileum, where they are frequently present as an ileal intussusception. We report the case of a 56-year-old female patient who presented with periumbilical pain along with multiple episodes of vomiting and chronic constipation. An abdominal CT scan revealed an ileo-ileal intussusception of an endoluminal hypodense lesion with mesenteric lymphadenopathy. surgical treatment consisted of segmental ileal resection with primary anastomosis. The histopathological analysis revealed an inflammatory fibroid polyp in the ileum. Although these tumors have no malignant potential, surgery is always indicated in ileo-ileal intussusception to ascertain the histological nature of the lesion.

Perivascular epithelioid cell tumor of male pelvic cavity:a case report and literature review

Perivascular epithelioid cell tumors （PEComas） are a family of rare mesenchymal neoplasms. The PEComas, composed of epithelioid and spindle cells, have the same cellular and immtmohistocherrtical features but are found in different visceral and soft tissue sites. Here, we report the histological and immunohistochemical features of one case of PEComa restricted in the pelvic visceral peritoneum of a male patient. The patient was treated with radical surgery, and was well and on follow-up visits without tumor recurrence.
