BACKGROUND Mean age of patients with valves diseases is significantly increasing,and,in the near future,cardiac surgeons will have to deal with a considerable number of patients aged more than 80 years.The remarkable results gained by the minimally invasive approach have encouraged its application in more complex and fragile patients,such as older people.This study aimed to identify the rate of early mortality and major complications,and independent predictors for mid-term mortality in octogenarians undergoing minimally invasive valve Surgery.METHODS Octogenarian patients undergoing right mini-thoracotomy mitral and/or tricuspid valve Surgery between 2006 and2020 were included.Primary endpoint was to identify independent predictors for mid-term mortality,and secondary endpoints were operative morality,stroke,independent predictors for early composite outcome,and quality of life at follow-up.RESULTS Analysis was performed on 130 patients.Stroke occurred in one patient(0.8%),while operative mortality was 6%(eight patients).One-year and five-year survival were 86% and 64%,respectively.Logistic regression identified age and creatinine level as independent predictors of mid-term mortality,survival analysis showed that age ≥ 84 years and creatinine level ≥ 1.22 mg/dL were the cut-off points for worst prognosis.Female gender and hypertension were found to be independent predictors of early composite outcome.CONCLUSIONS Results of the present study show that age alone should not be considered a contraindication for minimally invasive valve Surgery.Identifying patients who are most likely to have survival and functional benefits after Surgery is decisive to achieve optimal health outcomes and prevent futile procedures.

Introduction Albumin,which is a 65-kDa liver-synthesized protein,accounts for nearly 50%of total plasma protein and contributes to approximately 80%of intravascular oncotic pressure.Albumin helps maintain microvascular integrity,functions as an antioxidant,and transports hormones,fatty acid,bile salts,bilirubin,electrolytes(e.g.,calcium,magnesium,copper,zinc,et al.),and drugs.[1–3]Human serum albumin(HSA)is a sterile,liquid albumin product derived from large pools of human plasma by fractionation and pasteurization.The medical use of HAS could date back to approximately the time of World War II.

To improve the diagnosis and treatment of inflammatory breast cancer(IBC),the Chinese Society of Breast Surgery(CSBrS)of the Chinese Medical Association identified the key clinical issues for clinical practice guidelines through literature research and evaluated the relevant evidence with reference to the Grading of Recommendations Assessment,Development and Evaluation(GRADE).[1]Level of evidence standard and recommendation strength criteria are shown in previous publication.[2]Recommendation strength review committee was comprised of 31 members and all the members are breast surgeons.The target audience are Chinese breast disease specialists.The recommendation is shown in Table 1.

The primary treatment modality for tumors affecting the oral and maxillofacial region is Surgery.Surgical treatment of tumors affecting this region is a challenge and poses relatively high surgical risk,especially in lesions affecting the deep regions of the head and the neck due to the complex anatomy of this region including structures of great importance such as the internal carotid artery,the internal jugular vein,and the cavernous sinus at the skull base,the cranial nerves and the orbital contents with the eye globes,extraocular muscles,and the lacrimal gland.Oncologic resection often results in devastating defects in the oral and maxillofacial region,which adversely affect patients’facial appearance and important functions such as speech,chewing,deglutition,or vision and significantly impact patients’quality of life.In order to obtain a good result concerning tumor excision in the oral and cranio-maxillofacial region as well as a correct aesthetic and functional reconstruction,an adequate presurgical evaluation of the patient and accurate planning of the surgical resection and the reconstructive approach must be carefully designed.

To the Editor:Natural orifice specimen extraction Surgery(NOSES)is an actively developing approach for extracting specimens and performing non-incisional colorectal cancer Surgery.Previous studies have shown that its effectiveness and safety in the surgical treatment of rectal cancer are similar to those of conventional laparoscopic Surgery.NOSES was demonstrated to be a possible alternative approach to conventional laparoscopy,for which a mini-laparotomy is used to extract the specimen,as there were no differences in surgical characteristics and short-term outcomes.[1]Recently,new promising approaches have been demonstrated for rectal tumors extraction with bowel eversion and extra-abdominal resection.

Objective:To scrutinize the definitions of minimal invasive surgical therapy(MIST)and to investigate urologists’knowledge,attitudes,and practices for benign prostatic obstruction surgeries.Methods:A 36-item survey was developed with a Delphi method.Questions on definitions of MIST and attitudes and practices of benign prostatic obstruction surgeries were included.Urologists were invited globally to complete the online survey.Consensus was achieved when more than or equal to 70%responses were“agree or strongly agree”and less than or equal to 15%responses were“disagree or strongly disagree”(consensus agree),or when more than or equal to 70%responses were“disagree or strongly disagree”and less than or equal to 15%responses were“agree or strongly agree”(consensus disagree).Results:The top three qualities for defining MIST were minimal blood loss(n=466,80.3%),fast post-operative recovery(n=431,74.3%),and short hospital stay(n=425,73.3%).The top three surgeries that were regarded as MIST were Urolift®(n=361,62.2%),Rezum®(n=351,60.5%),and endoscopic enucleation of the prostate(EEP)(n=332,57.2%).Consensus in the knowledge section was achieved for the superiority of Urolift®,Rezum®,and iTIND®over transurethral resection of the prostate with regard to blood loss,recovery,day Surgery feasibility,and post-operative continence.Consensus in the attitudes section was achieved for the superiority of Urolift®,Rezum®,and iTIND®over transurethral resection of the prostate with regard to blood loss,recovery,and day Surgery feasibility.Consensus on both sections was achieved for EEP as the option with the better symptoms and flow improvement,lower retreatment rate,and better suitable for prostate more than 80 mL.Conclusion:Minimal blood loss,fast post-operative recovery,and short hospital stay were the most important qualities for defining MIST.Urolift®,Rezum®,and EEP were regarded as MIST by most urologists.

Introduction:Machine learning (ML)‐based facial nerve injury (FNI) forecasting grounded on multicentric data has not been released up to now.Three distinct ML models,random forest (RF),K‐nearest neighbor,and artificial neural network (ANN),for the prediction of FNI were evaluated in this mode.Methods:A retrospective,longitudinal,multicentric study was performed,including patients who went through parotid gland Surgery for benign tumors at three different university hospitals.Results:Seven hundred and thirty‐six patients were included.The most compelling aspects related to risk escalation of FNI were as follows:(1) location,in the mid‐portion of the gland,near to or above the main trunk of the facial nerve and at the top part,over the frontal or the orbital branch of the facial nerve;(2) tumor volume in the anteroposterior axis;(3) the necessity to simultaneously dissect more than one level;and (4) the requirement of an extended resection compared to a lesser extended resection.By contrast,in accordance with the ML analysis,the size of the tumor (>3 cm),as well as gender and age did not result in a determining favor in relation to the risk of FNI.Discussion:The findings of this research conclude that ML models such as RF and ANN may serve evidence‐based predictions from multicentric data regarding the risk of FNI.Conclusion:Along with the advent of ML technology,an improvement of the information regarding the potential risks of FNI associated with patients before each procedure may be achieved with the implementation of clinical,radiological,histological,and/or cytological data.
