supportive care needs and health-related quality of life among Chinese lung cancer survivors

Purpose: The purpose of this study was to explore the supportive care needs and health-related quality of life (HRQoL) of lung cancer survivors, and to identify the association between patient characteristics, HRQoL and supportive care needs. Method: This was a sub-study of a larger scale survey of cancer survivors’ supportive care needs. A total of 48 lung cancer survivors were recruited from a regional teaching hospital, and a three-part structured questionnaire was used to collect 1) socio-demographic and clinical characteristics, 2) supportive care needs and 3) health-related quality of life data. Results: The three most commonly reported unmet needs were all in the health-system information domain: 1) being informed about your results as soon as possible (58%), 2) being informed about cancer which is under control or diminishing (50%), and 3) being informed about things you can do to help yourself get well (46%). The second most common unmet need domain was access to healthcare and ancillary support services. The survivors generally reported satisfactory HRQoL. However, household income and unmet physical and psychological needs were independently associated with HRQoL among these survivors. Conclusion: The high unmet needs in the health-system information area call for a review of the content and amount of information provided to lung cancer survivors. In addition, more attention should be given to lung cancer survivors with low incomes but high physical and psychological unmet needs, who require appropriate follow-up and long-term care of a physical, social and psychological kind.

How Parenting Affects Children＇s Futures：Empirical Study in Japan

In this study, we conducted a survey on Japanese adults about the way they were raised in their childhood and analyzed the influence of parenting method on children＇s futures. Using the major factor method, we conducted principal factor analysis of the responses fi＇om a 20-question survey of Japanese adults to assess the parent-child relationship during childhood. Four factors （＂interest＂, ＂trust＂, ＂norm＂, and ＂independence＂） are derived. We added ＂time spent together＂ with parent（s） and parent＇s ＂strict discipline＂ to the indices and classified the parenting methods into five types：（1） supportive, （2） tiger, （3） indulgent, （4） uninvolved, and （5） abusive. Then we compared the average earnings, sense of well-being, and educational career of the respondents classified according to the parenting type. Among the five types, the ＂supportive＂ type showed the highest achievement in all the three items, and the ＂abusive＂ type the lowest in all the items.

Psychotherapy in anorexia nervosa: What does the absence of evidence mean?

Psychological treatment in anorexia nervosa(AN) is disheartening. Psychotherapy is the "treatment of choice" for adults though this recommendation is grounded on the absence of good quality clinical studies. This paper seeks to address the question of why improvements in the psychological treatment of AN have been thwarted, and why one of the best treatments available for adult patients is specialist supportive clinical management that has entered the stage through the backdoor of nonspecific supportive treatments originally serving as a placebo treatment assigned in randomized clinical trials to control for non-specific aspects of true psychosocial treatments. The possibility that most of the psychopathological features that characterise the AN symptoms profile could be best understood as the direct consequences of emaciation would enhance the utility of research with animal models for generating new hypothesis to improve AN treatment.

A Study on Locations of Death and Factors Associated with Death among Cancer Patients in South Korea

Background: Location of death has been used to examine an indicator for good death. This study aims to examine location of death among patients with three major cancers (gastric, liver, and lung) and other factors associated with location of death in South Korea. Methods: We selected the medical and pharmacy claims data for health services and location of death among the 42,596 decedents with cancer (lung 16,632, liver 15,872, gastric 10,092) from 2009 to 2013. We used logistic regressions to identify factors associated with home death. Outcome measures are locations of death (hospital, outpatient clinics or emergency room and home). Results: Only 8.9% died at home whereas 46.5% died in hospital as inpatients. Patients with more than one comorbid cancer or receivers for any supportive care were significantly more likely to die in hospital. Female and younger than 55 years old liver cancer patients were associated with home death. Patients living in metropolitan area, or paying more insurance premium, or being public aid beneficiaries, were associated with home death. Conclusions: The supportive care service use prior to death was significantly associated with increasing odds to hospital death. Being older than 75, or having multiple cancers was significant factors associated with hospital death, whereas living in metropolitan area, lower income or emergency visit were significant factors with home death. These findings are opposite to what is found, as the palliative care and hospice is predominantly hospital-centered. The findings emphasize a need to available end-of-life care in community for dying patients.

Identification and Calculation Method of the Financial Benefits of IT Projects for Better Financial Evaluation

To financially evaluate an IT projects is to assess its financial feasibility, while the financial benefits are the core parameters of financial evaluation of IT projects. Therefore, correctly identifying the financial benefits of IT projects is the precondition to ensure the validity of the financial evaluation. Essentially, IT projects can be divided into productive IT projects and supportive IT projects. The paper analyzes the importance of identifying the financial benefits and introduces the meaning, characteristics and classification of financial benefits of IT projects. On this basis, the paper identifies the financial benefits of both productive IT projects and supportive IT projects and emphasizes the formation and specific calculation method of the two types of IT projects. For productive IT projects and supportive IT projects, the calculation of financial benefits should employ different methods.

Communication Skills Training for Residents: Which Variables Predict Learning of Skills?

Objective: This study aimed to determine variables associated (predictors and correlates) with the learning of assessment and supportive skills in the context of a communication skills training for medical residents. Methods: Learning was measured by comparing residents’ communication skills in a simulated consultation before and after a communication skills training. Communication skills were transcribed and tagged with a computer-assisted program. Potential variables associated with learning (residents’ characteristics, contextual characteristics and pre-training communication skills) were measured before the training and entered in regression analysis. Results: Fifty-six residents followed the training between 2002 and 2006. Poor pre-training assessment and supportive skills predicted the respective learning of these skills. Better assessment skills’ learning was predicted by copings (i.e. lower level of emotional coping), lower levels of self-efficacy and depersonalization. Better supportive skills’ learning was predicted by a lower work experience and associated with a higher training attendance rate. Conclusions: Predictors and correlates of assessment and supportive skills learning were different. Trainers needed to detect certain residents’ characteristics (i.e. depersonalization) in order to optimize assessment skills learning. Trainers needed to be aware that supportive skills are difficult to learn and to teach and may need more training hours.

Multicenter cooperative observational study of idiopathic pulmonary fibrosis with non-small cell lung cancer

AIM: To research the natural course of idiopathic pulmonary fibrosis(IPF) with advanced non-small cell lung cancer(NSCLC) and the association between acuteMETHODS: From May 2007 through April 2011, 17 CT naive patients with IPF and advanced NSCLC were enrolled. Patients were classified into best supportive care(BSC) group or CT group based on the patient's preference. Patients in the CT group received carboplatin(CBDCA)(AUC 5-6) plus paclitaxel(PTX)(175-200 mg/m2) on day 1 of each 21-d cycle as first-line ***: All patients but one chose the CT group. In the CT group, the objective response rate was 38%. The most frequent toxicity ≥ grade 3 was neutropenia(88%). Two patients(12.5%) developed AE-IPF. The median progression-free survival, the median survival time and the 1-year survival rate were 4.1 mo, 8.7 mo and 35%, respectively. Second-line CT-related AE and CT-unrelated AE occurred in 2 and 3 patients(1: BSC group; 2: CT group), respectively. Seven(41%) of all patients developed AE-IPF throughout the clinical course, and 6 of 7 patients with AE-IPF died within one ***: The incidence of AE-IPF was higher among IPF patients with advanced NSCLC than among those without NSCLC. CBDCA plus PTX regimen was tolerable and effective. However, AE-IPF has a fatal toxicity with or without CT in IPF patients with advanced NSCLC.

Recent advances in technology provide countless innovative solutions and applications for supporting children with autism in educational learning and personal development. This result is an increasingly recognized need to deal with new and unexpected risks and issues such as social exclusion. Diverse advanced technologies were aimed to support learning activity from different perspectives with multiple strategies. However, despite the significant amount of work and explored technology, several common risks pop up due to user’s vulnerability to diverse risks such as negative screen effects. Accordingly, there is still plenty of room for improvement in this regard. To address these vulnerabilities and gaps, this paper aims at identifying issues and challenges improving the technology applied for Autism Spectrum Disorders and autistics dedicated applications. It put forward requirements and design decisions supporting safe autistic dedicated interaction with regards to ISO31000 risk management process.

Effective Ways to Promote Language Learners'Motivation

Motivationis a term both teachers and learners talk about most when they refer to language learning success or failure ( D?rnyei,2001) since there is a strong co-relationship between motivation and language learning ( Horwitz,2013) . Although re-search and theories on the importance and types of motivation are productive, educators still feel having little guidance on how to mo-tivate their students in schooling ( Shernoff,2013) . Here, I would like to introduce some motivational teaching practices, such as providing authentic learning opportunities, fostering student autonomy and creating an emotionally supportive environment, to our language teachers who wish to create engaging learning experiences for ESOL or EFL learners.

More Tolerance and Freedom for Speaking Classes

In the practice of English Language Teaching, speaking can be the most difficult part for many learners, especially in China. This paper firstly reveals the necessity of giving more freedom and tolerance on speaking classes, based on the nature of teaching as well as speaking. Then there is more exploration for the Chinese ELT (English Language Teaching) approaches, explaining the difficulties from the aspects of Chinese culture and philosophy. Afterwards implications are showed on how to give a supportive class in different circumstances, such as dealing with silence, error correction and L1 (native language) interferences.
