Medicinals with properties of warming Yang and tonifying Qi in terms of Traditional Chinese Medicine: their effects on left ventricular ejection fraction and aldosterone in rats with induced failure heart

OBJECTIVE: To investigate on Left ventricular ejection fraction value and aldosterone of two medicinals of Traditional Chinese Medicine(TCM) with the properties of warming Yang and tonifying Qi in terms of TCM ***: An animal model of coronary ligation of heart failure after myocardial infarction was employed to study the influence of these two kinds of drugs on three batches of rats. On the basis of the average score of left ventricle ejection fraction during the investigation, there were some different groups, including Wen Yang(the warming Yang)group, Yi Qi(tonifying Qi) group, Wen Yang and Yi Qi group, captopril group, digoxin group. In additional, an artificial operation group was set for compari-son. The systemic intervention using these medicinal and drugs was taken effects on the 2nd day after the operation of myocardial infraction(MI) with once a day. At week one, two, and four after the MI treatment, evaluated were EF values, and ferritin,angiotensin-II and aldosterone in the rats' ***: At week one and week two, the medicinal of Wen Yang, Yi Qi, Wen Yang pluse Yi Qi, and digoxin could improve left ventricular ejection fraction in rats with heart failure; Compared to the model group, captopril Left ventricular EF value increased, but there was not significant. At week four,heart failure and left ventricular EF values was improved in the intervention group and the other four captopril drug intervention. At week one in the rats with drug intervention, the medicinals of Yi Qi, Wen Yang plus Yi Qi, and captopril could inhibit activation in vivo hormone aldosterone in heart failure rats; aldosterone in Wen Yang group and digoxin group were not different from that in the model group at week two and ***: Chinese medicinals with properties of Wen Yang, Yi Qi have significant effect on improving left ventricular EF in rats with heart failure; compared to Yi Qi medicinals, Wen Yang medicinals that inhibit the effectiveness of the time

Effect of trabeculated myocardial mass on left ventricle global and regional functions in noncompaction cardiomyopathy

Cardiovascular Function during First 24 Hours after Off-Pump and On-Pump CABG—A Prospective Observational Comparative Study

Purpose: Myocardial revascularization by coronary artery bypass grafting (CABG) in ischemic heart disease patients has direct impact on hemodynamic parameters in the immediate post-operative period. The peri-operative cardiovascular functioning is an important determinant for outcome of surgery. In On-Pump CABG (ONCAB), the cardiopulmonary bypass has a negative effect on myocardium. Off-Pump CABG (OPCAB) avoids the effect of CPB but complete revascularization with difficult positioning of heart is technically demanding. This study is aimed to compare the cardiovascular functioning in the immediate post-operative period after OPCAB and ONCAB. Methods: Total 106 patients were operated for CABG from January 2015 to June 2016, of which 75 patients were operated for OPCAB and 31 patients were operated for ONCAB. For the comparison, hemodynamic parameters were measured during anesthesia before surgery, postoperatively after 1 and 4 hours (h) in the ICU, and in the morning after surgery, approximately after 20 h. Results: The time-dependent rise of hemodynamic parameters like Cardiac Output (CO), Cardiac Index (CI), stroke volume (SV) and Left Ventricular stroke Work Index (LVSWI) in the immediate post-operative hours (1 h and 4 h) are more predominant in OPCAB group than ONCAB group although the difference is eliminated mostly at 20 h. The better peripheral vasodilation after OPCAB causes immediate fall of Systemic Vascular Resistance Index (SVRI) after OPCAB. Conclusion: Better cardiovascular functioning immediately after OPCAB than ONCAB may be important for better hemodynamic stability. The difference is however eliminated after 24 hours indicating little significance in long term outcome.

Effect of Acupuncture at Acupoints Neiguan(PC 6)and Ximen(PC 4)on the Left Cardiac Function in the Patients with Coronary Heart Disease

Purpose. This study is to investigate the influence of acupuncture at acupoints Neiguan （PC 6） and Ximen （PC 4） on the stroke volume and the flow rate of peak value of left ventricle ejection in the patients with coronary heart disease. Methods. 60 cases of coronary heart disease were divided into acupoint Neiguan （PC 6） group and acupoint Ximen （PC 4） group, and the stroke volume and the flow rate of peak value of left ventricle ejection were detected 3 min after manipulating needles and 20 rain after retaining needles respectively, then were compared with those before acupuncture. Results. 3 min after manipulating needles, the stroke volume and the flow rate of peak value of left ventricle ejection were enhanced in 46 cases, and 20 min after retaining needles, both the stroke volume andthe flow rate of peak value of left ventricle ejection were all enhanced in 34 cases. Conclusion. Puncturing acupoints Neiguan （PC 6） and Ximen （PC 4） could enhance the contractility of left ventricle wall, increase the stroke volume of the heart and improve the myocardial ischemia in the patients with coronary heart disease.

Electrical cardiometry(EC), as a type of thoracic electrical bioimpedance, is a continuous noninvasive method for measuring cardiac output(CO). Although the accuracy of EC has been verified by compared to transthoracic echocardiography(via trans-aortic valve flow velocity time integral [EVVTI]), there are rare data on investigating the agreement with transthoracic echocardiography(via M-mode [EVMM]) as a widely used measurement method. This study aimed to investigate the agreement in CO measurements between EC and EVMM in the neonate. In this prospective observational study, simultaneous CO measurements carried by EC and by EVMM in neonatal intensive care patients. Data were compared by correlation, the coefficient of variation(CV) to evaluate the consistency of measurements, Bland–Altman analysis by calculated the bias(mean difference), precision(1.96×SD of the difference) and limits of agreement(bias± 1.96 SD). We enrolled 100 neonates(median GA: 33.79 weeks, IQR 29.79-38.96 weeks) and performed 109 paired between EC and EVMM. There was an excellent correlation for CO(r=0.86, p < 0.001) measured by EC and EVMM. These correlations remained significant under different ventilation patterns and hemodynamic significance of the patent ducts arteriosus(PDA) in various birth weight infants. Coefficient of variation of CO was 53% and 43% for EC and EVMM, respectively. The level of agreement was little wide(mean bias=-0.15 L/min, precision=0.27 L/min). In conclusions, although the wide agreement of measurements of CO, EC has a good correlation with EVMM which not interfered by PDA, various respiratory conditions and different birth weight in neonates. Our results indicate that EC, as the non-invasive and continuous monitoring tools, has the potential to trend CO in the neonatal intensive care unit(NICU).

Real-time Three-dimensional Echocardiography in Assessment of Left Ventricular and Right Ventricular volumes

Real-time three-dimensional echocardiography (RT3DE)is a new ultrasound technique that enables dynamic threedimensional visualization and quantification of the heart in real time. Investigation of feasibility and methodology of RT3DE in determining left ventricular (LV) and right ventricular (RV) volumes, RT3DE was performed in 35 normal adults using Philips SONOS 7500 system with a 2-4 MHz matrix array transducer. The 60°×60° 'pyramid' volume database was obtained and analyzed on a TomTec echo workstation. Both LV and RV volumes were calculated with four 3DE methods (i.e. apical 2, 4, 8, and 16-plane) through manually tracing ventricular endocardial borders in end diastole and end systole. stroke volumes were then calculated. LV volume was also measured by 2DE Simpson's rule using GE VIVID 7 ultrasound machine.

本文应用心室起搏方法,在12只清醒犬研究了不同刺激部位(左室前、后壁及右室流出道)和连结间隔的室性早搏对左室功能的影响。发现,室性早搏与对照比较,左室收缩压峰值(LVSP)在各刺激部位和连结间隔基本上没有变化;左室压变化速率(LV dp/dt)和每搏输出量(SV)在各部位短连结间隔显著降低;左室舒张期末压(LVEDP)仅在刺激左室前、后壁的短连结间隔显著减少。室性早搏与早搏后代偿收缩相比,LVSP 在各剌激部位的短连结间隔显著降低;LVEDP和 dp/dt,在各刺激部位和连结间隔都显著降低;SV,除右室流出道的长连结间隔外,均显著降低。并且,这些变化与刺激部位和连结间隔密切相关。总之,实验证明,室性早搏可使左室心肌收缩性能和泵功能降低,而且,右室功能状态也可影响左室机能。
