OBJECTIVE: To provide an objective experimental basis for the gastric mucosa pathological evolution and the transformation of different Traditional Chinese Medicine(TCM) syndromes in helicobacter pylori(H. pylori)-related gastric diseases(HPGD) patients, based on the combination of TCM syndrome differentiation, molecular biology and ***: A total of 203 participants were enrolled in this study. The expressions of miR-499/miR-149 and *** infection in the gastric tissues from all participants were detected. The genotyping for miR-499 rs3746444 and miR-149 rs2292832 was ***: In H. pylori positive subjects, the proportion of precancerous gastric lesions(PGL) in liver-stomach disharmony syndrome(LSDS) group was higher than in spleen Qi deficiency syndrome(SQDS) group(P stomach damp-heat syndrome(SSDHS) group and LSDS group(all P < 0.001). We also found C allele of miR-149 rs2292832 was linked to lower risk of gastric atrophy [miR-149 rs2292832 C vs T:adjusted odds ratio = 0.207; 95% confidence interval(0.043-0.989); P = 0.048]. Compared with healthy control(HC) group, the expression of miR-499 was significantly increased in GC group, while the expression of miR-149was significantly decreased in chronic inflammation group,PGL group and GC group(all P < 0.05). Test for trend showed that GC risk was on a rising trend with the increasing expression of miR-499 and decreasing expression of miR-149(both P for trend < 0.05).CONCLUSION: The C allele of miR-149 rs2292832 may be a protective factor for gastric mucosal atrophy. *** may participate in the evolution of benign to malignant gastric mucosa lesions by inducing the overexpression of miR-499 and down regulation of miR-149. In addition, patients with H. pylori infection combined SQDS or LSDS may have higher risk of gastric mucosal malignant lesions.

INTRODUCTIONGastric epithelial dysplasia (GED) hypothetically is a straight-forward concept: dysplastic epithelium replacing the normal gastric epithelium of the stomach [1].In the stomach ,like any other segment of the gut ,it is defined as an unequivocal non-invasive epithelial change[2,3].The observation of gastric dysplasia as a cancerous lesion was recognized over a century ago ,but it is only after the advent of gastroscopy that its clinical significance has been stressed[4-7].

Gastric pseudolipomatosis, usual or unusual? Re-evaluation of 909 endoscopic gastric biopsies

Microvesicular pneumatosis intestinalis, also called 'pseudolipomatosis' for resembling fatty infiltration, is characterized by the presence of small gas voids in the gastrointestinal wall, especially in mucosa. These voids are not lined with epithelia. There are few reported cases about colon, duodenum and skin. Because there is only one case report about pseudolipomatosis in the stomach, we reevaluated 909 endoscopic biopsies taken from gastric corpus to check the presence of pseudolipomatosis. We determined pseudolipomatosis foci in 3 percent (n=27) of biopsies. In two cases there were pseudolipomatosis foci in endoscopic biopsies having otherwise normal histologic findings, while there were pseudolipomatosis foci in endoscopic biopsies of 25 patients with gastritis. H pylori was found in 85 % of biopsies having pseudolipomatosis foci. In this study, we presented some histopathologic characteristics of pseudolipomatosis seen in gastric mucosa.

INTRODUCTIONThe field of gastrointestinal hormones has expanded at a dizzying rate[1-4].Gastrointestinal hormones as regulatory peptides that appear to be major components of bodily integration and have important regulatory actions on physioligical function of the gastrointestinal tract .The successful isolation of some gastrointestinal hormones and the development of sensitive methods for their detection have led to the unexpected finding that they also exist in the brain .

AIM: To study the relationship between Helicobacter pylori (H. pylori) and gastric carcinoma and its possible pathogenesis by H. pylori. METHODS: DNEL technique and immunohistochemical technique were used to study the state of apoptosis, proliferation and p53 gene expression. A total of 100 gastric mucosal biopsy specimens, including 20 normal mucosa, 30 H. pylori-negative and 30 H. pylori-positive gastric precancerous lesions along with 20 gastric carcinomas were studied. RESULTS: There were several apoptotic cells in the superficial epithelium and a few proliferative cells within the neck of gastric glands, and no p53 protein expression in normal mucosa. In gastric carcinoma, there were few apoptotic cells, while there were a large number of proliferative cells, and expression of p53 protein significantly was increased. In the phase of metaplasia, the apoptotic index (AI, 4.36%+/-1.95%), proliferative index (PI, 19.11%+/-6.79%) and positivity of p53 expression (46.7%) in H. pylori-positive group were higher than those in normal mucosa (P<0.01). AI in H. pylori-positive group was higher than that in H. pylori-negative group (3.81%+/-1.76%), PI in H. pylori-positive group was higher than that in H. pylori-negative group (12.25%+/-5.63%, P<0.01). In the phase of dysplasia, AI (2.31%+/-1.10%) in H. pylori-positive group was lower (3.05%+/-1.29%) than that in H. pylori-negative group, but PI (33.89%+/-11.65%) was significantly higher (22.09+/-8018%, P<0.01). In phases of metaplasia, dysplasia and gastric cancer in the H. pylori-positive group, AIs had an evidently graduall decreasing trend (P<0.01), while PIs had an evidently gradual increasing trend (P<0.05 or P<0.01), and there was also a trend of gradual increase in the expression of p53 gene. CONCLUSION: In the course of the formation of gastric carcinoma, proliferation of gastric mucosa can be greatly increased by H. pylori, and H. pylori can induce apoptosis in the phase of metaplasia, but in th

AIM: To investigate the presence of *** DNA within gastric epithelial cells in patients with *** infection and its possible carcinogenic mechanism. METHODS: Total 112 patients, with pathologically confirmed chronic superficial gastritis, chronic atrophic gastritis, intestinal metaplasia, atypical hyperplasia or gastric cancer were studied. Among them, 28 were *** negative and 84 *** positive. *** DNA in gastric epithelial cells was detected by GenPoint catalyzed signal amplification system for in situ hybridization. RESULTS: In the *** positive group, zero out of 24 chronic superficial gastritis (0.0%), four out of 25 precancerous changes (16.0%) and thirteen out of 35 gastric cancers (37.1%) showed *** DNA in the nucleus of gastric epithelial cells, the positive rates of *** DNA in the nucleus of gastric epithelial cells were progressively increased in chronic superficial gastritis, precancerous changes and gastric cancer groups (chi(2)=12.56, P=0.002); One out of 24 chronic superficial gastritis (4.2%), eleven out of 25 precancerous changes (44.0%) and thirteen out of 35 gastric cancers (37.1%) showed *** DNA in the cytoplasm of gastric epithelial cells (chi(2)=10.86, P=0.004). In the *** negative group, only one patient with gastric cancer was found *** DNA in the nucleus of gastric epithelial cells; Only two patients, one patient with precancerous changes and another with gastric cancer, showed *** DNA in the cytoplasm of gastric epithelial cells. Furthermore, *** DNA must have been in the cytoplasm as long as it existed in the nucleus of gastric epithelial cells. CONCLUSION: *** DNA exists both in the nucleus and the cytoplasm of gastric epithelial cells in patients with *** infections. AIM:Inflammatory fibroid polyp (IFP) is a rare benign lesion that may occur throughout the digestive *** is more commonly found in the antrum of the stomach in particular. It mostly affects adults at the average age of 60 *** polyps are able to cause abdominal pain,gastrointestinal bleeding,intestinal obstruction or *** this paper we report a case of gastric TIP with unusual presenting features. METHODS:A child with gastric IFP was described and the literature was reviewed, RESULTS:A 4-year-old girl presented with fever for 2 months,arthralgia of knees and ankles,iron defidency anemia, and *** stool examination was positive for occult *** upper gastrointestinal study demonstrated a large Iobulated mass at the upper part of gastric *** gastrectomy en blocwith this 5 cm×8 cm mass was subsequently *** examination was consistent with *** the mass excision,her fever abruptly declined and disappeared together with anemia and *** remained asymptomatic and the abdominal ultrasonography performed at the 24-month follow-up demonstrated no recurrence of the tumor. CONCLUSION:The etiopathogenesis of IFP still remains unclear,The presence of IFP throughout the gastrointestinal tract and its variable clinical appearances make it difficult to diagnose,The inflammatory symptoms found in this patient support the hypothesis of inflammatory benign lesions of IFP.

Self-propelling micro-and nano-motors(MNMs)have been extensively investigated as an emerging oral drug delivery carrier for gastrointestinal(GI)tract ***,the propulsion of current MNMs reported so far is mostly based on the redox reaction of metals(such as Zn and Mg)with severe propulsion gas generation,remaining non-degradable residue in the GI ***,we develop a bioinspired enzyme-powered biopolymer micromotor mimicking the mucin penetrating behavior of Helicobacter pylori in the *** converts urea to ammonia and the subsequent increase of pH induces local gel-sol transition of the mucin layer facilitating the penetration into the stomach tissue *** successful fabrication of micromotors is confirmed by high-resolution transmission electron microscopy,electron energy loss spectroscopy,dynamic light scattering analysis,zeta-potential *** acidic condition,the immobilized urease can efficiently converted urea to ammonia,comparable with that of neutral condition because of the increase of surrounding pH during *** administration into the stomach,the micromotors show enhanced penetration and prolonged retention in the stomach for 24 ***,histological analysis shows that the micromotors are cleared within 3 days without causing any toxicity in the GI *** enhanced penetration and retention of the micromotors as an active oral delivery carrier in the stomach would be successfully harnessed for the treatment of various GI tract diseases.
