Multidimensional instabilities always develop with time during the process of radiation pressure acceleration,and are detrimental to the generation of monoenergetic proton beams.In this paper,a sharp-front laser is proposed to irradiate a triple-layer target(the proton layer is set between two carbon ion layers)and studied in theory and simulations.It is found that the thin proton layer can be accelerated once to hundreds of MeV with monoenergetic spectra only during the hole-boring(HB)stage.The carbon ions move behind the proton layer in the light-sail(LS)stage,which can shield any further interaction between the rear part of the laser and the proton layer.In this way,proton beam instabilities can be reduced to a certain extent during the entire acceleration process.It is hoped such a mechanism can provide a feasible way to improve the beam quality for proton therapy and other applications.

Neurotoxicity is an infrequent adverse reaction to iodinated contrast agents. Contrast induced neurotoxicity following coronary angiogram is very rare. Renal disease is a risk factor for contrast induced neurotoxicity. We report a case of contrast induced neurotoxicity following coronary angiogram and intervention using Iohexol(Omnipaque 350) in an end stage renal disease patient on peritoneal dialysis who had prior exposure to iodinated contrast without any adverse reaction. Hemodialysis had to be initiated for rapid removal of the contrast agent with subsequent complete resolution of neurological deficits. This case highlights the need for interventionalists to be aware of an important adverse reaction to iodinated contrast agents, especially in individuals with renal dysfunction, and that neurotoxicity is a possibility even with prior uneventful exposures. The role and timing of hemodialysis in contrast induced neurotoxicity in patients with chronic kidney disease and in those without chronic kidney disease needs further deliberation.

Bacterial translocation(BT) refers to the passage of viable bacteria or bacterial products from the intestinal lumen, through the intestinal epithelium, into the systemic circulation and extraintestinal locations. The three principal mechanisms that are thought to be involved in BT include bacterial overgrowth, disruption of the gut mucosal barrier and an impaired host defence.BT is commonly observed in liver cirrhosis and has been shown to play an important role in the pathogenesis of the complications of end stage liver disease, including infections as well as hepatic encephalopathy and hepatorenal syndrome. Due to the importance of BT in the natural history of cirrhosis, there is intense interest for the discovery of biomarkers of BT. To date, several such candidates have been proposed, which include bacterial DNA, soluble CD14, lipopolysaccharides endotoxin, lipopolysaccharide-binding protein, calprotectin and procalcitonin. Studies on the association of these markers with BT have demonstrated not only promising data but, oftentimes, contradictory results. As a consequence, currently, there is no optimal marker that may be used in clinical practice as a surrogate for the presence of BT.

Late stage thermal history of the Songliao Basin and its tectonic implications: Evidence from apatite fission track (AFT) analyses

Apatite Fission Track (AFT) data from the Songliao Basin indicates that the late stage tectonic movements in the Songliao Basin have zoning in space and episodes in time. The late stage tectonic movements started from the east part of the basin and migrated westward. AFT ages in the east part of the basin are older than those in the west part of the basin, suggesting that the uplift occurred earlier in the east than in the west. The denudation thickness in the east part of the basin is significantly greater than that in the centre and west. The thermal history evolved two episodes of rapid cooling and subsequent slow cooling processes. Age-depth relationship derived from the AFT data indicates a four-episode denudation history. Further Monte Carlo random simulation of the AFT data reveals the four changing points of the thermal evolution at 65 Ma, 43.5 Ma, 28 Ma and 15 Ma, respectively. The uplifting and denudation rates from different episodes of evolution are proportional to the plate convergence rate. Based on the above analyses and the regional geologic background, it is concluded that the late stage thermal events in the Songliao Basin are the far field response to the subduction of the Pacific Plate under the Eurasian Plate. The first episode of the rapid cooling probably started at the end of the Nenjiang Formation, climaxed at the end of the Cretaceous and ceased at the Late Eocene. The subsequent slow cooling lasts another 15 Ma. The first episode of the evolution is the far field response to the major episode of the Yanshan Movement and subsequent series of the tectonic reorganization, especially the directional change of the Pacific Movement and also the subduction of the Indian Plate underneath the Eurasian Plate. While the second episode of the evolution is the far field response to the extension and closure of the Sea of Japan. Extension led to the migration and converging of the mantle heat flow to the Sea of Japan and resulted in the rapid cooling of the Songliao Basin.

Assessing Efficient Risk Ratios: An Application to Surgical stage Prediction in Cervical Cancer

Background:?Cervical cancer remains the second most commonly diagnosed cancer and the third leading cause of cancer death in developing countries. Improving clinicians’ knowledge and understanding of surgical staging is critical?in the fight against the disease. However, a systematic evaluation of different ordinal regression models based on diverse predicted outcomes has not been given its due share in literature.?Objective:?To systematically assess the flexibility of odds ratios for three popular ordinal regression models i.e.?the Multinomial Logistic (ML) model, the Continuation Ratio (CR) model and Adjacent Category Logistic (ACL) model when applying cervical cancer data in surgical stage prediction.?Method:?We systematically, compared the performance of CR, ML and the ACL as the predictive mechanisms, and evaluate the most appropriate model in the cervical cancer setting. The study considered women who visited the Oncology department at the Moi Teaching and Referral Hospital’s Chandaria Cancer and Chronic Diseases Center and were diagnosed and surgically treated for cervical cancer from January 2014?to December 2018.?Results and Conclusion:?We presented the comparison between?3?different regression models for ordinal data within the cervical cancer setting. We found that the CR model without proportional odds yielded better results?comparing Akaike Information Criterion (AIC), log likelihood ratio and residual deviance. In addition, the key prognostic factor associated with invasive cervical cancer was the (International Federation of Gynecology and Obstetrics) FIGO clinical stage which in particular, had a higher influence on the surgical stage 2 outcomes compared to the lesser surgical stage categories. All the 5?independent features selected for classifying the patients into surgical stages were the FIGO clinical stage and partly, the presence or absence of?symptomatic vaginal discharge.

Early stage Triple Negative Breast Cancer Has Significantly Better Outcomes than More Advanced Disease: A Single Centre Retrospective Review

A retrospective, serial analysis of 181 triple negative breast cancer (TNBC) patients was undertaken at a regional cancer centre in Canada. The primary focus of the analysis was to investigate the effect of presenting stage in patients with TNBC on progression free and overall survival. We were able to demonstrate that patients presenting with an earlier stage breast cancer had a significantly superior progression free and overall survival when compared to more advanced stage. The adjusted multivariate cox-regression analyses for the overall and progression free survival suggest that the hazard of death was significantly lower for patients with stages I (HR = 0.09;95% CI 0.03 - 0.24) and II (HR = 0.29;95% CI 0.16 - 0.54) than for patients with stage III. The only other predictor of progression free survival besides stage, was receipt of radiotherapy (HR = 0.39;95% CI 0.22 - 0.69) in the adjusted cox regression analysis. Less than 2% of patients presented with stage IV disease. The small numbers presenting with stage IV disease may have impact on the development of clinical and translational trials. Certainly there may be stage migration if staging included more standardized or more sensitive investigations such as PET scans, and this might an important consideration in developing clinical trials. Twenty-five percent of patients presented with stage I disease. It is important for patients with TNBC presenting with earlier stages of disease that they are aware that they will have a better prognosis than their counterparts with more advanced disease. It is important that we are aware of this patient population, as their treatment recommendations are unclear and a source of a fair amount of controversy currently.

A Method to Predict Banana Pseudostem’s Strength at Seedling stage

Banana industry played important roles in Chinese economics. Banana production was always affected heavily by typhoon. Selecting new cultivars with high strength was the basic way to resolve this problem. If the selecting work was performed at mature stage, it will need large area and much labor. If this work can be done at seedling stage, this process will be simplified significantly. We found that banana plants that had high strength at seedling stage always had high strength at mature stage. The physical strength of seedling pseudostem was closely related with pseudostem diameter, lignin content and the expression of 4-coumarate:CoA ligase gene Mu4CL15. Those seedlings whose pseudostem had bigger diameter, more lignin content and higher expression of Mu4CL15 always had higher strength to resist lodging. Through screening plants whose pseudostem had bigger diameter, more lignin content and higher expression of Mu4CL15, plants with high-strength pseudostem can be selected. Using this method, thousands of seedlings can be identified in short time. This process does not need large area and much labor. If this method was applied, the process of breeding new cultivars of banana with high resistance to lodging will be accelerated.

An Interactive Method for Validating stage Configuration

Software product Line (SPL) is an emerging methodology for developing software products. stage-configuration is one the important processes applying to the SPL. In stage-configuration, different groups and different people make configuration choices in different stages. Therefore, a successful software product is highly dependent on the validity of stage-configuration process. In this paper, a rule-based method is proposed for validating stage-configuration in SPL. A logical representation of variability using First Order Logic (FOL) is provided. Five operations: validation rules, explanation and corrective explanation, propagation and delete-cascade, filtering and cardinality test are studied as proposed operations for validating stage-configuration. The relevant contributions of this paper are: implementing automated consistency checking among constraints during stage-configuration process based on three levels (Variant- to-variant, variant-to-variation point, and variation point-to-variation point), define interactive explanation and corrective explanation, define a filtering operation to guide the user within stage-configuration, and define (explicitly) delete-cascade validation.
