Background:The programmed intermittent epidural bolus(PIEB)technique is widely used in labor analgesia,but the parameter settings of PIEB have not yet been standardized.We designed a study to identify the optimal interval duration for PIEB using 10 mL of ropivacaine 0.08%and sufentanyl 0.3 mg/mL,a regimen commonly used to control labor pain in China,to provide effective analgesia in 90%of women during the first stage of labor without breakthrough pain.Methods:We conducted a double-blind sequential allocation trial to obtain the effective interval 90%(EI90%)during the first stage of labor between April 2019 and May 2019.This study included the American Society of Anesthesiologists physical status II–III nulliparous parturients at term,who requested epidural analgesia.The bolus volume was fixed at 10 mL of ropivacaine 0.08%with sufentanyl 0.3 mg/mL.Participants were divided into four groups(groups 60,50,40,and 30)according to the PIEB intervals(60,50,40,and 30 min,respectively).The interval duration of the first parturient was set at 60 min and that of subsequent parturients varied according to a biased-coin design.The truncated Dixon and Mood method and the isotonic regression analysis method were used to estimate the EI90%and its 95%confidence intervals(CIs).Results:Forty-four women were enrolled in this study.The estimated optimal interval was 44.1 min(95%CI 41.7–46.5 min)and 39.5 min(95%CI 32.5–50.0 min),using the truncated Dixon and Mood method and isotonic regression analysis,respectively.The maximum sensory block level above T6 was in nearly 20%of parturients in group 30;however,5.3%,0%,and 0%of the parturients presented with sensory block level above T6 in groups 40,50,and 60,respectively.There were no cases of hypotension and only one parturient complained of motor block.Conclusion:With a fixed 10 mL dose of ropivacaine 0.08%with sufentanyl 0.3 mg/mL,the optimal PIEB interval is about 42 min.Further studies are warranted to define the efficacy of this regimen throughout all stages of labor.

Background：To compare the diagnostic values of magnetic resonance elastography （MRE） and diffusion-weighted imaging （DWI） in staging hepatic fibrosis （HF） in an animal model.Methods：This study consisted of 44 rabbits served as HF group and 9 normal rabbits.HF group was divided into two subgroups：Group A （n =32） and Group B （n =12）.Rabbits in Group B were served as a complementary group when rabbits in Group A suddenly died during the study.Rabbits from control and Group A underwent abdominal MR imaging （MRI）,MRE,and DWI.In Group A,random eight rabbits underwent MRI examinations at 4,5,6,l0 weeks after carbon tetrachloride oil subcutaneous injection.Liver stiffness （LS） and apparent diffusion coefficient （ADC） values of liver parenchyma were measured.The diagnostic performance of MRE and DWI for staging HF was compared using the receiver operating characteristic curve analysis on the basis of the histopathological analysis of HF.Results：Significant differences of LS and DWI values were present among HF stages （P 〈 0.005）.The LS values measured on MRE （r =0.838,P 〈 0.001） were more strongly correlated with the HF stages than with ADC values （r =-0.527,P 〈 0.001）.The area under the receiver operating characteristic curve values of LS were significantly larger than those of DWI were for discriminating two stages of HF （0.979 vs.0.712 for ≥ S 1,0.922 vs.0.699 for ≥ S2）.MRE showed higher specificity for predicting all stages of HF compared to DWI.Conclusions：MRE more strongly correlated with the HF stages than DWI and is more specific in predicting all HF stages.

Objective: The recurrence and progression of ameloblastoma are unpredictable. Therefore, we examined the influence of clinical factors on recurrence time and analyzed the clinical factors associated with early recurrence and cancerization. We then developed a staging system to predict early recurrence and cancerization. Methods: All of the primary craniofacial ameloblastoma patients treated in Shanghai Ninth People's Hospital, Shanghai Jiao Tong University School of Medicine were recorded. There were 87 recurrent cases used to create a staging system and tested in a Cox regression analysis for risk factors associated with early recurrence or cancerization following surgery. Results: There were 890 craniofacial ameloblastoma patients, and 72 cases had recurrence. There were also 15 cases with cancerous recurrence. The overall recurrence rate was 9.78%, and the cancer rate was 1.69%. The primary cases were classified into the following 3 stages based on clinicopathological features: stage I, the maximum tumor diameter stage II, the maximum diameter of tumor >6 cm or tumor invasion to the maxilla sinus/orbital floor/soft tissue; and stage III, tumor invasion of the skull base or metastasis into regional lymph nodes. When the method of surgery was controlled by partial correlation, the staging had significance with recurrence time (P=0.004). The Cox analysis showed the tumor stage was correlated with recurrence time (P=0.027) and cancerization time (P=0.002). However, the surgical method did not influence the recurrence time when adjusted for cofounding variables. Conclusions: Tumor larger than 6 cm and invasion to soft tissues or adjacent anatomical structures are associated with early recurrence. This staging system can be used to predict the risk factors of early recurrence and cancerization in ameloblastoma patients.

Background:The results of the Trial Assigning IndividuaLized Options for Treatment(TAILORx)suggested that approximately 70%of T1-2N0M0,hormone receptor-positive,human epidermal growth factor receptor 2(HER2)-negative breast cancer patients can avoid chemotherapy and receive only adjuvant endocrine therapy.We conducted a retrospective analysis of the clinicopathologic features and prognostic factors of patients with breast cancer who met the inclusion criteria of the TAILORx trial.Methods:According to the enrollment criteria of the TAILORx trial,a retrospective analysis was performed on patients with breast cancer who were treated from January 2008 to December 2015 at Peking University First Hospital.The clinicopathologic characteristics of all patients were analyzed,and prognoses were calculated using the Kaplan-Meier method and a Cox proportionate hazards model.Results:A total of 2430 patients with early stage breast cancer who were admitted at our hospital had complete clinicopathologic data and follow-up information.Of these patients,722 met the inclusion criteria and were enrolled in the present study,accounting for 29.7%of all patients.Among them,417(57.8%)patients received only adjuvant endocrine therapy(the non-chemo group),and 305(42.2%)patients received adjuvant chemotherapy followed by adjuvant endocrine therapy(the chemo group).No statistically significant difference was observed in overall survival(OS)between the two groups(non-chemo vs.chemo:5-year OS:97.9%vs.97.9%,χ^2=1.00,P=0.995;hazard ratio[HR]=1.00,95%confidence interval[CI]:0.46–2.21).A significant difference was observed in disease-free survival(DFS)between the two groups(non-chemo vs.chemo:5-year DFS:97.9%vs.94.7%,χ^2=8.65,P=0.003;HR=3.05,95%CI:1.40–6.67).The choice of adjuvant therapy was associated with clinicopathologic factors,such as the age at diagnosis,T stage,histologic grade,the Ki67 index,the presence of intravascular tumor thrombus(P<0.001),pathologic type,and menstrual status(P=0.014).Conclusions:In the absence of internationally

The roots of Paeoniae lactiflora Pall.are widely consumed as crude drugs in Asian countries due to their remarkable beneficial health effects.The present research was undertaken to illuminate the dynamic changes in metabolites and enzymes and facilitate selection of the harvesting time when the herb can provide optimum health benefits.P.lactiflora roots were analyzed at 12 stages of growth for monoterpenoid glycosides,phenols,nucleosides,nucleobases,amino acids,and polysaccharides by high-performance liquid chromatography with photodiode array detector,ultra-high pressure liquid chromatography coupled with tandem mass spectrometry,and UV spectrophotometry.The enzyme activities of plant β-glucosidases and esterases were determined by UV methods.The total content of monoterpenoid glycosides and phenols peaked in December.For nucleosides and nucleobases,the highest content appeared in April.The maximum phasic accumulation of the total amino acids took place in March,and the content of total polysaccharides reached a peak value in September.December,April,and March were selected as the appropriate harvesting times for producing natural medicinal or health food products.Plant β-glucosidases and esterases showed the highest activity in December and May,respectively.When the activity of β-glucosidase increased,esterase activity decreased,while the contents of oxypaeoniflora and paeoniflorin increased.When esterase activity increased,the contents of benzoylpaeoniflorin,paeoniflorin,and gallic acid decreased.In conclusion,the results from the present study would be useful in determination of the suitable time for harvesting P.lactiflora roots for medicinal purposes.

BACKGROUND： Combined hepatocellular carcinoma and cholangiocarcinoma （cHCC-CC） is a rare subtype of primary liver cancer consisting of both hepatocellular carcinoma （HCC） and cholangiocarcinoma （CC）. Because of the rarity of this tumor, its feature is poorly understood. The present study aimed to evaluate the clinicopathological features and long-term prognosis of patients with cHCC-CC after surgery and to compare with those of the patients with stage-matched HCC and CC. METHODS： The dinicopathological features of the patients who underwent surgery for cHCC-CC at our center during the period of 2001-2010 were retrospectively analyzed and compared with those of stage-matched HCC and CC patients. Cancer staging was performed according to the AJCC Cancer Staging Manual （6th ed.）. Overall survival and disease-free survival were compared among the groups and prognostic factors of cHCC-CC were evaluated. RESULTS： Significant differences were observed in clinico- pathological features among 42 patients with cHCC-CC, 90 patients with HCC and 45 patients with CC. Similar to HCC patients, cHCC-CC patients had frequent hepatitis B virus antigen positivity, microscopic vessel invasion, cirrhosis and high level of serum alpha-fetoprotein. Similar to CC patients, cHCC-CC patients showed increased bile duct invasion and decreased capsule. The 1-, 3-, and 5-year overall survival and disease- free survival of patients with cHCC-CC were not significantly different from those with stage-matched patients with CC;but significantly poorer than those with HCC. In subanalysis of patients with stage Ⅱ, the overall survival in patients with cHCC-CC or CC was significantly poorer than that in patients with HCC. We did not find the difference in patients with other stages. Univariate analysis of overall and disease-free survival of patients with cHCC-CC showed that the vascular invasion and intrahepatic metastasis were the significant predictive factors.
