Background:The Bismuth-Corlette(BC)classification is used to categorize hilar cholangiocarcinoma by proximal extension along the biliary tree.As the right hepatic artery crosses just behind the left bile duct,we hypothesized that BCⅢb tumors would have a higher likelihood of local unresectability due to involvement of the contralateral artery.Methods:A retrospective review of a prospectively maintained database identified patients with hilar cholangiocarcinoma taken to the operating room for intended curative resection between April 2008 and September 2016.Cases were assigned BC Stages based on preoperative imaging.Results:Sixty-eight patients were included in the study.All underwent staging laparoscopy after which 16 cases were aborted for metastatic disease.Of the remaining 52 cases,14 cases were explored and aborted for locally advanced disease.Thirty-eight underwent attempt at curative resection.After exclud-ing cases aborted for metastatic disease,the chance of proceeding with resection was 55.6%for BCⅢb Staged lesions compared to 80.0%of BCⅢa lesions and to 82.4%for BCⅠ-Ⅲa Staged lesions(PStages.Conclusions:When hilar cholangiocarcinoma is preoperatively Staged as BCⅢb,surgeons should antici-pate higher rates of locally unresectable disease,likely involving the right hepatic artery.

In 2017,American College of Cardiology(ACC)/American Heart Association(AHA)et al.jointly released the latest guidelines for adult hypertension,exactly including prevention,diagnosis,assess and treatment,in which blood pressure levels greater than 130/80 mm Hg were defined as hypertension[1].Based on these modified guidelines,the morbidity of hypertension in US increased from 32%to 46%.

BACKGROUND Cutaneous involvement in Hodgkin lymphoma (HL) is a rare finding.Few cases have been reported in literature,most describing paraneoplastic manifestations.Only very few papers have described primary HL skin infiltration,reporting a wide range of clinical presentations that frequently include ulcers;plaques,nodules and papules have also been noticed.CASE SUMMARY We report the case of a 56-year-old man who presented fever,multiple adenomegalies of neck and axilla and thick serpiginous skin lesions involving bilateral pectoral regions.After an initial diagnostic workup for a suspected active infectious disease,a lymph node biopsy was performed,which showed a neoplastic invasion from a mixed cellularity classical HL.The same histological pattern was described in a cutaneous biopsy of the chest lesions.The other staging procedures performed revealed an advanced disease,with unfavourable clinical prognostic features.The patient was prescribed 6 cycles of ABVD chemotherapy scheme (doxorubicin,bleomycin,vinblastine,dacarbazine),a regiment that requires demonstration of metabolic response achievement at the interim PET/CT scan to confirm continuation or to change therapeutic strategy.CONCLUSION Skin involvement in HL is a rare finding and may represent a challenging clinical presentation due to extremely various types of lesions observed.

AIM To investigate the incidence and risk factors of portosplenomesenteric vein thrombosis(PSMVT) in the early Stage of severe acute pancreatitis(SAP).METHODS Patients with SAP in a tertiary care setting from January 2014 to December 2016 were retrospectively reviewed. All contrast-enhanced computed tomography(CT) studies were reassessed and reviewed. Clinical outcome measures were compared between SAP patients with and without PSMVT in the early Stage of the disease. Univariate and multivariate logistic regression analyses were sequentially performed to assess potential risk factors for the development of PSMVT in SAP patients. A receiver operating characteristic(ROC) curve was generated for the qualifying independent risk factors.RESULTS Twenty-five of the one hundred and forty(17.86%) SAP patients developed PSMVT 6.19 ± 2.43 d after acute pancreatitis(AP) onset. PSMVT was confirmed by contrast-enhanced CT. Multivariate stepwise logistic regression analyses showed that Balthazar's CT severity index(CTSI) scores [odds ratio(OR): 2.742; 95% confidence interval(CI): 1.664-4.519; P = 0.000], hypoalbuminemia(serum albumin level Stage of SAP.

Physiological Responses of Contrasting Rice Genotypes to Salt Stress at Reproductive Stage

Salinity is a major abiotic stress affecting plant growth and productivity. Considerable genetic variation is present in rice in response to salt stress, with higher sensitivity during early seedling and reproductive Stage. In this study, physiological changes in leaves and developing panicles of rice genotypes (IR686, Sadri, Rc222, CSR28, IR670 and Pokkali) contrasting in salt tolerance at the reproductive Stage were evaluated in greenhouse experiment under salt stress. The results showed that IR670 and the tolerant-check Pokkali maintained lower Na^+/K^+ ratio, less reduction in chlorophyll concentration, lower malondialdehyde (MDA) production, higher concentrations of reduced ascorbate (reduced AsA), higher proline accumulation and lower percentage reduction in pollen viability than the salt-sensitive genotypes under salt stress. The higher concentration of reduced AsA suggests an efficient ROS-scavenging system. Physiological measurements and pollen viability analysis revealed that Sadri (moderately tolerant at the seedling Stage) is sensitive to salt stress at the flowering Stage. The findings will be useful in breeding salt tolerant varieties at both seedling and reproductive Stages by selecting appropriate genotypes and phenotypes.

The temperature field in unsteady phase greatly affects the quality of friction plug welding(FPW).An analytical model is put forward to correlate the process parameters and the temperature field in unsteady phase of FPW.Applying the von Mises criterion for plastic deformation and linearizing the heat flux,the model is achieved by Laplace transformation.The predicated peak temperature and peak time agree with the experiment data,with errors of about 4%and 8%,of AA7075-T6 FPW.

Research Progress on Heat Stress of Rice at Flowering Stage

Global warming has caused frequent occurrence of heat stress at the flowering Stage of single-season rice in the Yangtze River region of China, which results in declines of spikelet fertility and yield in rice. Rice flowering Stage is the most sensitive period to high temperatures, and therefore, the key for heat stress happening is the flowering Stage coinciding with high temperature, which causes spikelet fertility decreasing in heat-sensitive varieties, and is the major factor for heat injury differences among various rice planting regions. With the development of rice breeding, temperature indexes for heat stress has been converted from daily maximum temperature of 35 oC to 38 oC with the stress duration of more than 3 d. During the flowering Stage, anther dehiscence inhibition and low pollen shedding onto the stigma are two main reasons for spikelet fertility reduction under high temperatures. At panicle initiation Stage, high temperatures aggravate spikelet degeneration, and destroy floral organ development. Various types of rice varieties coexist in production, and indica-japonica hybrid rice demonstrates the highest heat resistance in general, followed by indica and japonica rice varieties. In production, avoiding high temperature is the main strategy of preventing heat stress, and planting suitable cultivars and adjustment of sowing date are the most effective measures. Irrigation is an effective real-time cultivation measure to decline the canopy temperature during the rice flowering Stage. We suggested that further study should be focused on exploring heat injury differences among different rice variety types, and innovating rice-planting methods according to planting system changes in rice planting regions with extreme heat stress. Meanwhile, high temperature monitor and warning systems should be improved to achieve optimal heat stress management efficiencies.

AIM To explore the features and prognostic value of lymph node metastasis in patients with T1-Stage colorectal cancer(CRC).METHODS In all,321 cases of T1-Stage CRC were selected from 10132 patients with CRC who received surgical therapy in six large-scale hospitals in China and were retrospectively analyzed. Univariate and multivariate analyses were performed to analyze the risk factors for lymphatic metastasis. A survival analysis was then performed to analyze the prognostic value of lymph node metastasis.RESULTS The occurrence rate of T1 Stage was 3.17%(321/10132);of these patients,the lymph node metastasis rate was 8.41%(27/321),and the non-lymph node metastasis rate was 91.59%(294/321). Univariate analysis showed that preoperative serum CEA,preoperative serum CA199,preoperative serum CA724,vascular invasion,and degree of differentiation were associated with lymph node metastasis in T1-Stage CRC(P Stage CRC(P Stage CRC was 3.17% for all CRC cases. Preoperative serum CA724,vascular invasion,and degree of differentiation are independent risk factors for lymph node metastasis. Lymph node metastasis is an independent prognostic factor for OS in patients with T1-Stage CRC.

The critical Stage for inducing oviparity and embryonic diapause in parthenogenetic Artemia(Crustacea: Anostraca): an experimental study

The brine shrimp Artemia exhibits two reproductive modes: 1) oviparity, producing diapause embryos;and 2) ovoviviparity, producing free-swimming nauplii. Previous studies have suggested the existence of a critical Stage that determines the reproductive mode. Physicochemical factors, such as photoperiod, temperature, and salinity, have been suggested to irreversibly aff ect the reproductive mode of oocytes during this critical Stage. In this study, experiments were carried out using a photoperiod and temperature-sensitive parthenogenetic Artemia clone where maternal Artemia were shifted bidirectionally between ovoviviparity (18 h L:6 h D, 27℃) and oviparity (6 h L:18 h D, 19℃) culture conditions. In the main experiment ( Artemia shifted at six diff erent Stages including the post-larva Ⅱ to adult Ⅱ), the reproductive mode of fi rst brood was converted when shifting was performed on post-larva Ⅱ and Ⅲ but was not converted when females were shifted after post-larva Ⅲ. A supplementary experiment further revealed that the reproductive mode of fi rst brood could be altered when shifting females at an "early phase of postlarva Ⅳ", characterized by a developing ovisac reaching the middle of the third abdominal segment, ventral spines, and some oocytes growing larger than the others. In both experiments, reproductive modes of the second brood were signifi cantly aff ected when the shifting was performed on post-larva Ⅳ. These results suggest that the critical Stage for inducing oviparity and embryonic diapause is at the previtellogenic Stage of oocytes, or at maternal "early phase of post-larva Ⅳ" for the first-brood off spring. During this Stage, diff erential gene expression patterns of the two destined oocytes may be triggered by the token stimuli signals received by the oocytes.

Introduction: Brain metastasis is common in relapsed neuroblastoma patients, but the characteristics of brain metastasis remain largely unknown. This study aimed to investigate the status of brain metastasis with neuroblastoma in South China.Methods: In this retrospective case?based study, 106 patients with Stage 4 neuroblastoma from the Department of Pediatric Oncology in Sun Yat?sen University Cancer Center between January 2004 and May 2013 were included. The incidence, risk factors, and survival status of these patients were reviewed and analyzed.Results: Of the 106 patients, 11(10.4%) developed brain metastasis, accounting for 20.0% of 55 patients with relapse or progression. The age at initial diagnosis of the 11 patients ranged from 2 to 10 years(median 4 years), which was younger than that of the patients without brain metastasis(median 5 years, range 1–10 years, P = 0.073). The male to female ratio of the 11 patients was 8:3, which was not signiicantly diferent from that of the patients with?out brain metastasis(P = 0.86). Patients with brain metastasis had higher lactate dehydrogenase levels than those without brain metastasis, but the diferences were not signiicant(P initial diagnosis to the develo= 0.076). Eight patients died, and 3 patients survived. The median interval from thepment of brain metastasis was 18 months(range 6–32 months). The median survival was 4 months(range 1 day to 29 months) after the diagnosis of brain metastasis. The median interval from the manifestation of brain metastasis to death was 3 months(range 1 day to 11 months).Conclusions: High?risk factors for brain metastasis in cases of neuroblastoma include bone marrow involvement and a younger age at initial diagnosis. Nevertheless, multiple treatment modalities can improve disease?free survival.
