Blastocystis incidence, spontaneous clearance, persistence and risk factors in a rural community in Thailand: A prospective cohort study

Objective: To investigate the incidence, spontaneous clearance, persistence and risk factors of Blastocystis infection in a rural community in Thailand.Methods: In February 2016, a cohort study was conducted in a rural community of Thailand. Baseline information of intestinal parasitic infections and follow-up visits were conducted. Blastocystis infection was detected in stool samples examined with short-term in vitro cultivation using Jones’ medium supplemented with 10% horse serum. Participants were tested for Blastocystis infection at every study visit. Cox proportional hazard regression analysis was used to evaluate co-variates to determine the incidence of Blastocystis infection. Results: Of 733 enrolled participants, 57(7.78%) were positive and 676(92.22%) were negative for Blastocystis infection. All 676 participants were followed-up for Blastocystis infection between December 2016 and January 2018. In all, 52 of 482 cases(10.79%) comprised individual Blastocystis infection. The incidence density rates of Blastocystis infection, of spontaneous clearance of the disease and of persistent infection were 8.13, 63.14 and 38.70 per 1 000 person-months of follow-up, respectively. Our multivariate analysis revealed that opened defecation in fields or bushes(HR=4.8;95% CI=2.1-10.4, P<0.001), raising cows(HR=2.8;95% CI=1.1-7.1, P=0.032), not washing hands after animal contact(HR=2.1;95% CI=1.0-4.2, P=0.044) and presenting symptoms of nausea or vomiting(HR=2.3;95% CI=1.0-5.3, P=0.047) were predictive associated risks for the incidence of Blastocystis infection. Conclusions: Our study indicates that the best way to prevent Blastocystis infection can be by maintaining good personal hygiene, hand washing after animal contact and improving sanitary facilities.

Permanent Pacemaker Implantation in a Patient with Ventricular Fibrillation due to spontaneous Coronary Artery Dissection: A Case Report

We present here the case of a 43-year-old female without any known past medical history who was brought into the emergency department (ED) by the emergency medical services (EMS) after receiving cardiopulmonary resuscitation (CPR) on the field. Per EMS, on arrival patient was found to be on ventricular fibrillation and was shocked 3 times. Patient had no past medical history. In the ED, EKG showed sinus rhythm and troponin was 23 which is slightly high compared to upper limit. Patient was taken to the cath lab immediately which showed small to medium caliber septal 1 branch severe mid segment disease with distal tapering suggestive of spontaneous coronary artery dissection. No stent was placed. Electrophysiology was consulted and an Implantable Cardioverter Defibrillator was placed. spontaneous coronary artery dissection (SCAD) is a phenomenon where an epicardial coronary artery dissection occurs that is not related to atherosclerosis, trauma or iatrogenia. Patients with SCAD presenting with ventricular arrhythmias are not very common. In a Canadian registry analyzing 1056 patients with SCAD, only 84 of them presented with ventricular fibrillation (VF) or ventricular tachycardia (VT) and only 8 underwent ICD placement. They followed up the patients for 5 years and 8 patients suffered VT/VF. 5 of those 8 patients had VT/VF on initial SCAD presentation, and only 1 of them had undergone ICD insertion. There are no specific guidelines regarding ICD placement on patients with coronary artery dissection, but the AHA/ACC/HRS guidelines can help us make decisions. Our case underscores the importance of more prospective or retrospective studies to identify those patients with SCAD who would benefit from ICD placement for secondary prevention. The current guidelines for ventricular arrhythmias are an excellent tool for the electrophysiologist regarding the management of these arrhythmias in other specific scenarios but guidance on SCAD is still lacking.

Hemangioma is the most common benign hepatic tumor.Although spontaneous rupture is rare,the mortality rate ranges from 60 to 75%.Only 34 cases have been reported in the literature,with only one report using transcatheter arterial embolization(TAE) alone as treatment.We report a case of spontaneous rupture with "flowering sign" of a giant hepatic hemangioma,presenting with acute abdominal pain and shock,while the volume of the hemangioma and blood loss were similar.The patient was successfully managed by transarterial chemoembolization(TACE) alone,which has an operative mortality rate of up to 36.4%.

spontaneous bacterial and fungal peritonitis in patients with liver cirrhosis: A literature review

spontaneous bacterial(SBP) and spontaneous fungal peritonitis(SFP) can be a life-threatening infection in patients with liver cirrhosis(LC) and ascites. One of the possible mechanisms of developing SBP is bacterial translocation. Although the number of polymorphonuclear cells in the culture of ascitic fluid is diagnostic for SBP, secondary bacterial peritonitis is necessary to exclude. The severity of underlying liver dysfunction is predictive of developing SBP; moreover, renal impairment and infections caused by multidrug-resistant(MDR) organism are associated with a fatal prognosis of SBP. SBP is treated by antimicrobials, but initial empirical treatment may not succeed because of the presence of MDR organisms, particularly in nosocomial infections. Antibiotic prophylaxis is recommended for patients with LC at a high risk of developing SBP, gastrointestinal bleeding, or a previous episode of SBP, but the increase in the risk of developing an infection caused by MDR organisms is a serious concern globally. Less is known about SFP in patients with LC, but the severity of underlying liver dysfunction may increase the hospital mortality. SFP mortality has been reported to be higher than that of SBP partially because the difficulty of early differentiation between SFP and SBP induces delayed antifungal therapy for SFP.

The Kinetics and Mechanism of spontaneous Dispersion of Ionic Compounds Onto Surfaces of Supports

The dispersion kinetics of ionic compounds on the surfaces of supports is studied by using atemperature-programmed X-ray diffractometer.It is observed that for a given ionic compound/support mix-ture,there exists a critical dispersion temperature T_c,at which the ionic compound starts to disperse ontothe surface of the support at an appreciable rate.The dispersion process has 2 steps:the ions first leave thesurface of the ionic compound for the external surface of the support,and then the ions diffuse to the wholesurface of the support.For the dispersion of ionic compounds with high melting points,the first step is rate-determining.Their critical dispersion temperatures and dispersion activation energies depend on the ioniccompounds themselves and increase with their melting points,but are independent of supports.For the dis-persion of ionic compounds with low melting points,the second step is rate-determining.Their critical dis-persion temperatures and dispersion activation energies depend on both the ionic compounds and the sup-ports.

Menstrual cycle characteristics as an indicator of fertility outcomes:evidence from prospective birth cohort study in China

OBJECTIVE:To evidently assess the applicability of regulate menstrual cycle(MC)characteristics in Traditional Chinese Medicine(TCM)as an indicator for fertility.METHODS:A community-based prospective cohort study was conducted in China.Between January 2010 and December 2012,women who were willing to conceive within 2 years were enrolled in the study.Other than the MC length pattern,a well-adopted MC characteristic,menstrual blood color and clots were specifically concerned for women enrolled.All participants were followed up in 2 years by trained nurses.Pregnancy rate,fecundability odds ratio(FOR)and risk of miscarriage were assessed as fertility outcomes.RESULTS:A total of 2109 women were effectively included in this cohort for analysis.Results show that women with irregular MC length were less likely to achieve conception(FOR_(irregular)=0.59;95%CI=0.45-0.77,P<0.001).Menstrual blood in bright red color was also associated with decline in likelihood of conception(FOR=0.79;95%CI=0.63-0.98,P=0.04).Women with menstrual blood in light red were at higher risk of miscarriage(OR=2.39;95%CI=0.91-6.28,P=0.08).No significant impact was found between menstrual blood clots and fertility outcomes(FOR=1.02,95%CI=0.83-1.25,P=0.88;OR=1.2695%CI=0.77-2.07,P=0.35).CONCLUSIONS:MC characteristics can be an effective and simple indicator for women’s fertility.Increasing the knowledge of MC characteristics for women in reproductive ages would bring great benefits to their preconception health conditions.

spontaneous bacterial peritonitis is a complication of ascitic patients with end-stage liver disease(ESLD); spontaneous fungal peritonitis(SFP) is a complication of ESLD less known and described. ESLD is associated to immunodepression and the resulting increased susceptibility to infections. Recent perspectives of the management of the critically ill patient with ESLD do not specify the rate of isolation of fungi in critically ill patients,not even the antifungals used for the prophylaxis,neither optimal treatment. We reviewed,in order to focus the epidemiology,characteristics,and,considering the high mortality rate of SFP,the use of optimal empirical antifungal therapy the current literature.
