Several monocentric studies have noted alterations in Spontaneous brain activity in Alzheimer's disease (AD), although there is no consensus on the altered amplitude of low-frequency fluctuations in AD patients. The main aim of the present study was to identify a reliable and reproducible abnormal brain activity pattern in AD. The amplitude of local brain activity (AM), which can provide fast mapping of Spontaneous brain activity across the whole brain, was evaluated based on multisite rs-fMRI data for 688 subjects (215 normal controls (NCs), 221 amnestic mild cognitive impairment (aMCI) 252 AD). Two-sample t-tests were used to detect group differences between AD patients and NCs from the same site. Differences in the AM maps were statistically analyzed via the Stouffer's meta-analysis. Consistent regions of lower Spontaneous brain activity in the default mode network and increased activity in the bilateral hippocampus/parahippocampus, thalamus, caudate nucleus, orbital part of the middle frontal gyrus and left fusiform were observed in the AD patients compared with those in NCs. Statistical analysis to establish an ignition scenario based on extrinsic and intrinsic variables of coal seams that affect Spontaneous combustion

Spontaneous combustion of coal seams in open pit mines is a problem that occurs in coal mines around the world.Theories have been developed that express several factors which generate self-heating in the coal and favor combustion.The synergy of these factors contributes to the increase in the temperature of oxidation reactions,facilitating the culmination of the activation energy of the system,in which the ignition develops and the reaction is Spontaneous.Currently,the incidence of each variable’s susceptibility to coal combustion is unknown,so in this work a methodology was developed to determine statistically which of the variables has the greatest contribution in the phenomenon.Descriptive statistics,atypical values detection,principal component determination,cluster analysis,and logistic regression were used to determine how many indicator variables are necessary to describe susceptibility to coal combustion.For the analysis,21,000 data from different seams of two open-pit coal mines in endwalls and abandoned pits were processed with consideration given to extrinsic and intrinsic factors.Seam temperature,atmospheric pressure,wind speed,oxygen,methane,height,ash,volatile matter,heat power,vitrinite,liptinite and the Hardgrove Index are the most influential variables in the phenomenon.

Recurrent primary Spontaneous pneumothorax in a large Chinese family: a clinical and genetic investigation

Background:Primary Spontaneous pneumothorax(PSP)is a common manifestation of Birt-Hogg-Dubé(BHD)syndrome,which is an autosomal dominant disorder caused by mutation of the folliculin(FLCN)gene.This study was established to investigate the mutation of the FLCN gene and the phenotype in a family with PSP.Methods:We investigated the clinical and genetic characteristics of a large Chinese family with recurrent Spontaneous pneumothorax.Genetic testing was performed by Sanger sequencing of the coding exons(4-14 exons)of the FLCN gene.Results:Among ten affected members in a multi-generational PSP kindred,with a total of 18 episodes of Spontaneous pneumothorax,the median age for the initial onset of pneumothorax was 42.5 years(interquartile range:28.8-57.2 years).Chest computed tomography scan of the proband showed pulmonary cysts and pneumothorax.A novel nonsense mutation(c.1273C>T)in exon 11 of FLCN gene that leads to a pre-mature stop codon(p.Gln425*)was identified in the family.The genetic analysis confirmed the diagnosis of BHD syndrome in this family in the absence of skin lesions or renal tumors.Conclusions:A novel nonsense mutation of FLCN gene was found in a large family with PSP in China.Our results expand the mutational spectrum of FLCN gene in patients with BHD syndrome.

Application of ventilation simulation to Spontaneous combustion control in underground coal mine:A case study from Bulianta colliery

Spontaneous combustion of residual coal in longwall goaf is a long standing hazard. Airflow leakage into goaf is a major driver to the hazard and this issue deteriorates where longwalls are operating in multiple seams and shallow covers because mining-induced cracks are very likely to draw fresh airflow into goaf due to presence of pressure differential between longwall face and surface. To study the problem more critically, a ventilation simulation package ‘‘Ventsim" is used to conduct a case study from Bulianta colliery. It was found that isolating and pressurizing active longwall panel can mitigate the problem and the pressure differential can be adjusted by varying performance of auxiliary fan and resistance of ventilation regulator. A booster ventilation system can also mitigate the problem by adjusting fan duties. Ventilation simulation is a powerful tool to study Spontaneous combustion control in underground coal mine.

Left atrial enlargement and non-paroxysmal atrial fibrillation as risk factors for left atrial thrombus/Spontaneous Echo contrast in patients with atrial fibrillation and low CHA2DS2-VASc score

Objective To determine the risk factors for thromboembolism in lower risk patients with non-valvular atrial fibrillation(AF)and low CHA2DS2-VASc scores,which remain undefined.Methods We retrospectively analyzed the baseline clinical characteristics,routine laboratory parameters,and echocardiographic measurements of 705 patients(71.1%male;mean age:52.10±9.64 years)with low CHA2DS2-VASc score(0 or 1;1 point for female sex)out of 1346 consecutive patients with non-valvular AF who underwent transesophageal echocardiography(TEE)at Guangdong Cardiovascular Institute between January 2013 and December 2015.Results Patients with left atrial thrombus(LAT)or Spontaneous echo contrast(SEC)on TEE(24/705,4%)showed a higher incidence rate of vascular disease(54.2%vs.32.9%,P=0.045)and non-paroxysmal AF(79.2%vs.29.4%,P<0.001),larger left atrial diameter(43.08±4.59 vs.36.02±5.53 mm,P<0.001),and lower left ventricular ejection fraction(58.23±8.82%vs.64.15±7.14%,P<0.001)than those without.Multivariate logistic regression analysis identified left atrial diameter[odds ratio(OR)=1.171,95%confidence interval(CI):1.084–1.265,P<0.001]and non-paroxysmal AF(OR=3.766,95%CI:1.282–11.061,P=0.016)as independent risk factors for LAT/SEC.In ROC curve analysis,a left atrial dimeter cutoff of 37.5 mm yielded 95.0%sensitivity and 62.7%specificity(AUC:0.847,P<0.0001,95%CI:0.793–0.914).Conclusion In patients with non-valvular AF with low CHA2DS2-VASc score,the presence of LAT or SEC was associated with left atrial enlargement,which had moderate predictive value,and non-paroxysmal AF.

●AIM:To use the voxel-wise degree centrality(DC)method to explore the underlying functional network brain-activity in patients with ophthalmectomy.●METHODS:A total of 32 ophthalmic surgery patients(10 women and 22 men),and 32 healthy subjects(10 women and 22 men)highly matched in gender,age,and the same operation method.Everyone experienced a restingstate functional magnetic resonance imaging scan.The Spontaneous brain activity could be assessed by DC.Correlation analysis was used to explore the relationships between the average DC signal values and behavior performance in different regions.Receiver operating characteristic(ROC)cur ve analysis was utilized to differentiate between ophthalmectomy patients and healthy controls(HCs).●RESULTS:Compared with HCs,ophthalmectomy patients had greatly reduced DC values in left lingual gyrus,bilateral lingual lobe,left cingulate gyrus,and increased DC values of left cerebellum posterior lobe,left middle frontal gyrus1,right supramarginal gyrus,left middle frontal gyrus2,right middle frontal gyrus.However,we did not find that there was a correlation between the average DC values from various brain regions and clinical manifestations.●CONCLUSION:Dysfunction may be caused by ophthalmectomy in lots of cerebral areas,which may show the potential pathological mechanism of ophthalmectomy and it is beneficial to clinical diagnosis.

Risk assessment of gas control and Spontaneous combustion of coal under gas drainage of an upper tunnel

The adjustment of the gas drainage rate has an immediate impact on air leakage in gob,thus resulting in the change of self-heating of coal.While regulating the gas drainage parameters,the risk of Spontaneous combustion of coal should be considered.The risk assessment of gas control and Spontaneous combustion of coal under gas drainage in a tunnel was investigated at different gas drainage rates.The distributions of the air volume along the working face,the gas management effects and the width of the oxidation zone were subjected to risk analysis.As the simulation results showed,with increasing gas drainage rate,although the safety of gas dilution by ventilation was assured,the intensifying air leakage caused the oxidation zone to move into the deeper gob and led to an increase in the width of the oxidation zone.A risk assessment method was proposed to determine a suitable gas drainage rate for the upper tunnel.The correctness of the risk assessment and the validity of the numerical modelling were confirmed by the field measurements.

To editor:Spontaneous spinal epidural hematoma(SSEH)is uncom-mon,especially during pregnancy.However,SSEH is a seriousemergency that must be managed immediately to prevent per-manent neurological deficit of the mother and stillbirth.Theetiology and pathogenesis of SEEH remain unknown,al-though multiple theories and precipitating factors are de-scribed in the literature.In this report,we describe the caseof a pregnant female who developed SSEH.

OBJECTIVE:To assess the effects of Bushenantai(BSAT)granule(补肾安胎颗粒)on angiogenesis-related factors[E2,P,and vascular endothelial growth factor(VEGF)]at the maternal-fetal interface of recurrent Spontaneous abortion(RSA)mice,and to evaluate the role of BSAT in promoting angiogenesis at the maternal-fetal interface by influencing the expression of sex hormones,and VEGF.METHODS:A mouse model with normal pregnancy and another with Clark’s classic RSA were established.The RSA mice were randomly assigned to six groups:normal,model,progesterone,high-doseBSAT granule(BSAT-H),medium-dose-BSAT granule(BSAT-M),and low-dose-BSAT granule(BSAT-L)(n=10 for each group).The embryo loss rate and the histopathological changes in the decidual tissues were measured.Serum levels of estrogen(E2),progesterone(P),and VEGF were detected by enzyme-linked immunosorbent assay.The m RNA and protein expressions of estradiol receptor(ER),progesterone receptor(PR),VEGF,and vascular endothelial growth factor receptor 2(VEGFR2)in the decidual tissues were identified by immunohistochemistry,Western blotting,and quantitative reverse transcription polymerase chain reaction.RESULTS:The embryo loss rate in all groups that received BSAT treatment was reduced,while the number of blood vessels at decidual tissues was increased.The serum levels of E2,P and VEGF were elevated,and the m RNA and protein expressions of ER,PR,VEGF,and VEGFR2 in the decidual tissues were enhanced.CONCLUSION:BSAT can improve angiogenesis at the maternal-fetal interface and reduce the embryo loss rate,which may be associated with its ability to increase the serum levels of estrogen,progesterone,and VEGF,in addition to up-regulation of m RNA and protein expression of ER,PR,VEGF,and VEGFR2 in the decidual tissues.

Role of primary repair in Spontaneous esophageal rupture management:decades of experience from a single center

Spontaneous esophageal rupture,or Boerhaave syndrome,is a rare but potentially fatal condition with a mortality rate ranging from 20%to 30%in the literature.[1]In some European countries,the incidence is below 3 to 6/1,000,000 persons.[2]Nowadays,the treatment of this disease remains a challenge to physicians due to the variable clinical manifestations and lack of consensus or guidelines.[3]To better understand this disease and summarize our experience in this field,we searched the medical files at our hospital using both electronic and manual methods.Herein,we report a series of 73 patients with Spontaneous esophageal rupture in a single center over two decades.Between April 1983 and June 2013,a total of 73 consecutive patients were diagnosed with Spontaneous esophageal rupture at the Fourth Hospital of Hebei Medical University.The clinical data were collected retrospectively with all of the patients’informed consent and the study was conducted in accordance with the Declaration of Helsinki.
