Background Passive diagnosis of human African trypanosomiasis(HAT)at the health facility level is a major component of HAT control in Guinea.We examined which clinical signs and symptoms are associated with HAT,and assessed the performance of selected clinical presentations,of rapid diagnostic tests(RDT),and of reference laboratory tests on dried blood spots(DBS)for diagnosing HAT in Guinea.Method The study took place in 14 health facilities in Guinea,where 2345 clinical suspects were tested with RDTs(HAT Sero-K-Set,rHAT Sero-Strip,and SD Bioline HAT).Seropositives underwent parasitological examination(reference test)to confirm HAT and their DBS were tested in indirect enzyme-linked immunoassay(ELISA)/Trypanosoma brucei gambiense,trypanolysis,Loopamp Trypanosoma brucei Detection kit(LAMP)and m18S quantitative PCR(qPCR).Multivariable regression analysis assessed association of clinical presentation with HAT.Sensitivity,Specificity,positive and negative predictive values of key clinical presentations,of the RDTs and of the DBS tests for HAT diagnosis were determined.Results The HAT prevalence,as confirmed parasitologically,was 2.0%(48/2345,95%CI:1.5–2.7%).Odds ratios(OR)for HAT were increased for participants with swollen lymph nodes(OR=96.7,95%CI:20.7–452.0),important weight loss(OR=20.4,95%CI:7.05–58.9),severe itching(OR=45.9,95%CI:7.3–288.7)or motor disorders(OR=4.5,95%CI:0.89–22.5).Presence of at least one of these clinical presentations was 75.6%(95%CI:73.8–77.4%)specific and 97.9%(95%CI:88.9–99.9%)sensitive for HAT.HAT Sero-K-Set,rHAT Sero-Strip,and SD Bioline HAT were respectively 97.5%(95%CI:96.8–98.1%),99.4%(95%CI:99.0–99.7%)and 97.9%(95%CI:97.2–98.4%)specific,and 100%(95%CI:92.5–100.0%),59.6%(95%CI:44.3–73.3%)and 93.8%(95%CI:82.8–98.7%)sensitive for HAT.The RDT’s positive and negative predictive values ranged from 45.2–66.7%and 99.2–100%respectively.All DBS tests had specificities≥92.9%.While LAMP and m18S qPCR sensitivities were below 50%,trypanolysis and ELISA/T.b.gambiense had sensitivities of 85.3%(95

Background: Bithermal caloric irrigation, video head impulse test(vHIT), and rotational testing are commonly used to assess peripheral vestibular function, but the relative clinical utility of each test in differentiating patients with peripheral vestibulopathy is debated.Objectives: To determine whether(1) the combination of two or more vestibular tests enhances diagnostic utility over a single test;(2) abnormal test results on vestibular tests correlate with one another.Methods: Retrospective analysis of data collected from multidisciplinary vestibular clinics at two academic medical centers from 2016 to 2022.Results: 150 patients(54.10 ± 15.09 years, 88 females) were included. No individual test was significantly better at predicting the presence of peripheral vestibular damage(p > 0.05). vHIT test results improved significantly when combined with either the caloric test(p = 0.007) or rotary chair test(p = 0.039). Caloric and rotational testing had high sensitivity(74.65% and 76.06%, respectively) and Specificity(83.54% and 78.48%, respectively). vHIT demonstrated excellent Specificity(89.87%) but poor sensitivity(47.89%). Caloric, vHIT, and rotary chair tests results did not correlate with one another(p > 0.05).Conclusions: Vestibular function tests have comparable diagnostic utility, yet each offers unique advantages. Caloric and rotational testing may be best suited for screening peripheral damage and vHIT may function ideally as a confirmatory test.

Classification of electroencephalogram(EEG)signals for humans can be achieved via artificial intelligence(AI)techniques.Especially,the EEG signals associated with seizure epilepsy can be detected to distinguish between epileptic and non-epileptic regions.From this perspective,an automated AI technique with a digital processing method can be used to improve these signals.This paper proposes two classifiers:long short-term memory(LSTM)and support vector machine(SVM)for the classification of seizure and non-seizure EEG signals.These classifiers are applied to a public dataset,namely the University of Bonn,which consists of 2 classes–seizure and non-seizure.In addition,a fast Walsh-Hadamard Transform(FWHT)technique is implemented to analyze the EEG signals within the recurrence space of the brain.Thus,Hadamard coefficients of the EEG signals are obtained via the FWHT.Moreover,the FWHT is contributed to generate an efficient derivation of seizure EEG recordings from non-seizure EEG recordings.Also,a k-fold cross-validation technique is applied to validate the performance of the proposed classifiers.The LSTM classifier provides the best performance,with a testing accuracy of 99.00%.The training and testing loss rates for the LSTM are 0.0029 and 0.0602,respectively,while the weighted average precision,recall,and F1-score for the LSTM are 99.00%.The results of the SVM classifier in terms of accuracy,sensitivity,and Specificity reached 91%,93.52%,and 91.3%,respectively.The computational time consumed for the training of the LSTM and SVM is 2000 and 2500 s,respectively.The results show that the LSTM classifier provides better performance than SVM in the classification of EEG signals.Eventually,the proposed classifiers provide high classification accuracy compared to previously published classifiers.

Although nanozymes have been widely developed,directly utilizing light to drive catalytic reactions like natural photoenzymes still remains challenging.Herein,we propose that photonanozymes(PNZs),as a novel kind of nanozyme,exclusively possess enzyme-mimicking activity under illumination.Only in the presence of visible light,the as-synthesized TiO_(2) proposed in this contribution shows excellent Specificity of peroxidase-like without any oxidase-or catalase-like activity.The driving force of the light-empowered peroxidase-like photonanozymatic activity is explicated in terms of the photogenerated hot charge carriers in TiO_(2) PNZs and the accompanied reactive oxygen species.The co-substrates for photonanozymatic reaction over TiO_(2) PNZs facilitate the formation of the precarious and reactive peroxo-oxygen bridge between TiO_(2) and H_(2)O_(2),enabling the catalytic Specificity.With the TiO_(2) PNZ-based biosensing platform for visual glucose detection exemplifying the concept of the application of PNZs,this work may evoke more inspirations to explore strategies for enlarging the scope of photoenzyme mimics.

Nonalcoholic fatty liver disease(NAFLD)is the leading cause of chronic liver disease worldwide among children and adolescents.It encompasses a spectrum of disease,from its mildest form of isolated steatosis,to nonalcoholic steatohepatitis(NASH)to liver fibrosis and cirrhosis,or end-stage liver disease.The early diagnosis of pediatric NAFLD is crucial in preventing disease progression and in improving outcomes.Currently,liver biopsy is the gold standard for diagnosing NAFLD.However,given its invasive nature,there has been significant interest in developing noninvasive methods that can be used as accurate alternatives.Here,we review noninvasive biomarkers in pediatric NAFLD,focusing primarily on the diagnostic accuracy of various biomarkers as measured by their area under the receiver operating characteristic,sensitivity,and Specificity.We examine two major approaches to noninvasive biomarkers in children with NAFLD.First,the biological approach that quantifies serological biomarkers.This includes the study of individual circulating molecules as biomarkers as well as the use of composite algorithms derived from combinations of biomarkers.The second is a more physical approach that examines data measured through imaging techniques as noninvasive biomarkers for pediatric NAFLD.Each of these approaches was applied to children with NAFLD,NASH,and NAFLD with fibrosis.Finally,we suggest possible areas for future research based on current gaps in knowledge.

The present paper is based on the observations that 1) there is reported variation in the specificities according to the type of tumor targeted (target) by FDG PET and 2) that while one can posit that the sensitivity of the tracer depends on the avidity for glucose and the plasma supply of the target, even so that the targeting cannot influence the avidity of unrelated tissues or lesions. The hypothesis to be tested is twofold: 1) patients imaged for different types of lesions could have a different prevalence of FDG avid tissues or lesions different from the target and 2) that the target lesions could be generally located in body location (sites) more likely to contain unrelated foci of increased uptake. Variance analysis shows that the sensitivity varies according to the target (p = 0.022), but not according to the location (p = 0.34);the Specificity varies with the location (p = 0.0012) and the target (p = 0.05). Specificities are significantly different in different primary targets and target locations. The former is assumed to be due to different comorbidities in patients with different targets, the latter to the different locations of unrelated glucose avid organs or structures. Conclusion: When specificities are recorded or defined, the patient population characteristics and the organ or pathology of the false positives should also be described.

To the Editor:Many physical examination tests are used to diagnose femoroacetabular impingement(FAI).The sensitivity and Specificity of the flexion,abduction,and external rotation(FABER)test and the flexion,adduction,and internal rotation(FADIR)test for FAI diagnosis are quite different.Therefore,based on clinical practice,we proposed the possibility of adding a physical examination test to improve the accuracy of the traditional physical examinations.

The CNS and cranial nerves are rarely involved except in children.[1,2]Spinal cord MRI abnormal long T1 and T2 signs in the posterior funiculus has Specificity for the diagnosis.[3]There have been many case reports about SCD induced by nitrous oxide anaesthesia or entertainment.

To the Editor:Biliary atresia(BA)is a rare obliterative cholangiopathy that occurs during infancy in association with destructive liver inflammation and hepatic fibrosis.Although recent studies have shown that transforming growth factor-b(TGF-b)plays an important role in liver fibrosis and inflammation in BA,disease mechanisms remain poorly understood.Circular RNAs(circRNAs)are a recently discovered unique class of regulatory RNA molecules with high Specificity.As previous evidence links circRNAs to the regulation of the TGF-b inflammatory signaling pathway,we further investigated the potential regulatory roles of circRNAs in BA and found novel insights into BA pathogenesis.
