Dural sinus septum:an underlying cause of cerebral venous sinus stenting failure and complications

Objectives The presence of dural sinus septum has long been identified anatomically but is often neglected for its clinical *** findings revealed the association of dural sinus septum with venous sinus stenting failure and complications supported by clinical *** This retrospective study included 185 consecutive patients treated with cerebral venous sinus stenting from January 2009 to May *** identified the dural sinus septa using digital subtraction angiography(DSA)and classified them into three types based on their *** septa at the transverse sinus were defined as typeⅠ,those at the junction between the transverse sinus and sigmoid sinus were defined as typeⅡand those at the sigmoid sinus were defined as typeⅢ.Based on the anatomic features and neuroimaging clues,we investigated the correlation of dural sinus septa with stenting failure and *** 32(17.1%)out of 185 patients(121 with idiopathic intracranial hypertension and 64 with venous pulsatile tinnitus)were identified with dural sinus septa by *** than half of the septa were typeⅠ(18/32,56.2%),followed by typeⅡ(11/32,34.4%)and typeⅢ(3/32,9.4%).The dural sinus septa caused three stenting failures and complications,including one case of venous sinus injury with subdural haemorrhage and two cases of incomplete stent *** analysis revealed that the presence of dural sinus septum(pseptum is a common structure in the cerebral venous *** found that the presence of dural sinus septa introduces uncertainties to cerebral venous sinus stenting and suggested precautions and ingenious skills in imaging and treatment.

Low grade chondrosarcoma of the nasal septum

Chondrosarcoma of the nasal septum is extremely rare. In some cases, it may be diff icult to preoperatively differentiate low grade chondrosarcoma from benign cartilaginous tumors such as chondroma. We report a case of low grade chondrosarcoma of the nasal septum with characteristic radiologic f indings. Characteristic radiologic f indings such as calcif ications on computed tomography scan and a ring-and-arc pattern on enhanced T1 weighted image were useful in the preoperative diagnosis of low grade chondrosarcoma of the septum. Awareness of radiologic f indings of low grade chondrosarcoma can help to make an accurate diagnosis and perform appropriate excision, leading to successful local control.

To the Editor:The septate uterus is the most common uterine anomaly and is associated with an increased risk of adverse reproductive *** resection of septum(TCRS)is widely performed in patients with adverse reproductive *** incidence of de novo intrauterine adhesions(IUAs)after TCRS is approximately 5%to 25%.[1]Various anti-adhesion methods are widely used in the clinical management of patients with septate uteri,including intrauterine devices,Foley balloons,estrogen,antibiotics,and hyaluronic acid ***,insufficient evidence supports or opposes any of these methods(Level C),[2]lacking high-grade evidence.

Donepezil rescues the medial septum cholinergic neurons via nicotinic ACh receptor stimulation in olfactory bulbectomized mice

Olfactory bulbectomy (OBX) causes cognitive dysfunction by degeneration of cholinergic neurons in the medial septum. Here, we define an involvement of nicotinic acetylcholine receptor (nAChR) in neuroprotective effect of donepezil in the septum neurons of OBX mice. Neuroprotective effects on the medial septal cholinergic neurons were assessed after chronic donepezil administration in OBX mice. We also measured Akt and ERK phosphorylation to define the neuroprotective mechanism of donepezil. We found that treatment with donepezil (1 - 3 mg/kg) for 15 consecutive days completely rescued cholinergic neurons in the OBX mice with concomitant improved memory. Reduction of both protein kinase B (Akt) and extracellular signal-regulated kinase (ERK) phosphorylation were restored by chronic donepezil administration (1 - 3 mg/kg) in OBX mouse medial septum. Both phosphorylated Akt and ERK immunoreactivities were localized in cell bodies of choline acetyltransferase (ChAT)-positive cholinergic cells in the medial septum. Enhancement of Akt and ERK phosphorylation seen following donepezil administration was totally blocked by pre-administration of mecamylamine (10 μM), a nicotinic acetylcholine receptor antagonist. Donepezil increases phosphorylation of Akt and ERK via nAChR stimulation in the medial septum cholinergic neurons. The Akt and ERK stimulation by donepezil is associated with its ability of neuroprotection in the medial septum and memory improvement.

Evaluation of the Effect of Sigmoid-Shaped Interventricular septum on Left Ventricular Systolic Function in Patients with Essential Hypertension by Two-Dimensional Speckle Tracking Echocardiography

Objective: To evaluate left ventricular regional and global systolic function by measuring left ventricular longitudinal strain (LS) in hypertensive patients with sigmoid-shaped interventricular septum (SIS) by two-dimensional speckle tracking (2D-STE);in order to explore whether the sigmoid-shaped interventricular septum affects the left ventricular systolic function in patients with hypertension. Methods: Routine echocardiographic parameters were measured in 30 hypertensive patients with SIS (SIS group) and 30 hypertensive patients without SIS (non-SIS group). The left ventricular segments and global LS were measured by 2D-STE, and the two sets of parameters were compared. Results: The value of the thickness of the basal segment of the interventricular septum (IVSBT), the thickness of the middle segment of the interventricular septum (IVSMT) and the ratio of the basal segment of the ventricular septum to the middle segment of the interventricular septum (IVSBT/IVSMT) in SIS group was higher than that in non-SIS group. However, the value of left ventricular outflow tract diameter (LVOTD) in SIS group was lower than that in non-SIS group. There was a significant difference between the two groups (all P Conclusion: SIS affects left ventricular regional systolic function of patients with hypertension. 2D-STE can early evaluate left ventricular longitudinal systolic function in hypertensive patients with SIS.

Exogenous Urocortin 1 Alters the Respiratory Exchange Ratio after Administration into the Lateral septum

Previous reports have suggested that hypothalamic urocortin 1 (Ucn1) exerts inhibitory control on energy metabolism as direct paraventricular nucleus injections dose-dependently decrease the respiratory energy exchange ratio (RER). Other evidence indicates that Ucn1 injections into the lateral septum may alter metabolic function. Consequently, the present study was designed to further characterize the effects of lateral septal Ucn1 signaling on eating and energy metabolism of adult Sprague-Dawley rats. Ucn1 was infused at the onset of the nocturnal cycle at doses of 10 - 100 pmol. In both females and males the peptide elicited a reliable suppression of food intake and significantly lowered RER over a 4 h postinjection period. The decrease in RER is consistent with enhanced lipid oxidation. Overall these findings suggest that, similar to the paraventricular nucleus, the lateral septum is a critical site of action in mediating the effects of Ucn1 on food intake and energy substrate utilization.

Clinical Study on Deviated Nasal septum and Its Associated Pathology

Deviated nasal septum is a frequently occurring condition that can cause nasal obstruction in an individual. It may result in permanent changes in the nasal and sinus mucosa because of altered ventilation of the nasal cavity. The present study makes an attempt to review its incidence and its associated pathology. Our study involves two groups of patients with 100 patients in each group. The first group consists of patients who are having nasal obstruction as a chief complaint and have deviated nasal septum. The second group consists of patients with either ear or throat complaints without any nasal symptoms, but were noted to have deviation of nasal septum on examination. An analysis was made to determine whether any particular type of deviation is more prone to develop pathology than other types. Our study shows that an “S” shaped deviation is more prone to be associated with ear, nose and throat pathology.

Transcatheter aortic valve replacement in membranous interventricular septum aneurysm with left ventricular outflow tract extension

We report a challenging case of a 81-year-old male with history of severe calcific aortic valve stenosis and aneurysmal membranous interventricular septum. The presence of anomalies in the sub-annular area can lead to valve malpositioning and its consequences. Transcatheter aortic valve implantation(TAVR) in patients with aneurysm of the perimembranous interventricular septum extending into the left ventricular outflow tract has not been previously reported. This case describes a successful transfemoral TAVR with an Edwards SAPIEN XT valve(Edwards Lifesciences, Irvine, CA, United States) with such anomaly.
