目的评价septic（sensing of phage—triggered ion cascade）技术在结核分枝杆菌快速检测中的应用价值。方法制备纳米阱微芯片，用于检测噬菌体感染细菌时导致细胞内离子释放发生的微范围内的电位变化。调整大肠杆菌、结核分枝杆菌、耻垢分枝杆菌及其相应噬菌体的浓度。将细菌和噬菌体等体积混合，37℃孵育1min。取5μl混合液滴于探针上，30S后开始采集数据，每2min采集一个数据文件，共采集10min，计算机分析采集结果。结果当结核分枝杆菌与分枝杆菌噬菌体D29混合反应时，在0—8min内显示了非常明确的1／f功率谱特征。而且功率谱强度明显高于阴性反应的功率谱，并出现了明确的电压超出±4盯范围的波形，持续约0．2S的时间，可认为发生了一次细菌被噬菌体感染的事件，表明探针明确地捕获了细菌被噬菌体感染的事件。结论septic技术能在较短时间内检测、鉴定出活菌的存在，有可能为结核分枝杆菌的检测提供一种简便、快捷的方法。

Sepsis is among the most common reason for admission to intensive care units throughout the world. In the US and most Western nations sepsis is largely a disease of the elderly. Management of elderly patients with severe sepsis is challenging. Early recognition of this syndrome, together with the early administration of appropriate antibiotics and cautious fluid resuscitation is the cornerstone of therapy. Echocardiography together with non-invasive or invasive hemodynamic monitoring is recommended in patients who have responded poorly to fluids or have significant underlying cardiac disease. This paper reviews the hemodynamic changes that characterize sepsis, particularly as they apply to elderly patients and provides recommendations for the management of these patients.

The vagus nerve can control inflammatory response through a ＇ cholinergic anti-inflammatory pathway＇, which is mediated by the α7-nicotinic acetylcholine receptor （α7nAChR） on macrophages. However, the intracel- lular mechanisms that link α7nAChR activation and pro-inflammatory cytokine production remain not well under- stood. In this study, we found that miR-124 is upregulated by cholinergic agonists in LPS-exposed cells and mice. Utilizing miR-124 mimic and siRNA knockdown, we demonstrated that miR-124 is a critical mediator for the cho- linergic anti-inflammatory action. Furthermore, our data indicated that miR-124 modulates LPS-induced cytokine production by targeting signal transducer and activator of transcription 3 （STAT3） to decrease IL-6 production and TNF-α converting enzyme （TACE） to reduce TNF-ot release. These results also indicate that miR-124 is a potential therapeutic target for the treatment of inflammatory diseases.

Liver cirrhosis is a major cause of mortality worldwide,often with severe sepsis as the terminal event.Over the last two decades,several studies have reported that in septic patients the adrenal glands respond inappropriately to stimulation,and that the treatment with corticosteroids decreases mortality in such patients.Both cirrhosis and septic shock share many hemodynamic abnormalities such as hyperdynamic circulatory failure,decreased peripheral vascular resistance,increased cardiac output,hypo-responsiveness to vasopressors,increased levels of proinflammatory cytokines [interleukine(IL)-1,IL-6,tumor necrosis factor-alpha] and it has,consequently,been reported that adrenal insufficiency(AI) is common in critically ill cirrhotic patients.AI may also be present in patients with stable cirrhosis without sepsis and in those undergoing liver transplantation.The term hepato-adrenal syndrome defines AI in patients with advanced liver disease with sepsis and/or other complications,and it suggests that it could be a feature of liver disease per se,with a dif-ferent pathogenesis from that of septic shock.Relative AI is the term given to inadequate cortisol response to stress.More recently,another term is used,namely "critical illness related corticosteroid insufficiency" to define "an inadequate cellular corticosteroid activity for the severity of the patient's illness".The mechanisms of AI in liver cirrhosis are not completely understood,although decreased levels of high-density lipoprotein cholesterol and high levels of proinflammatory cytokines and circulatory endotoxin have been suggested.The prevalence of AI in cirrhotic patients varies widely according to the stage of the liver disease(compensated or decompensated,with or without sepsis),the diagnostic criteria defining AI and the methodology used.The effects of corticosteroid therapy on cirrhotic patients with septic shock and AI are controversial.This review aims to summarize the existing published information regarding AI in patients with liver cirrhosis.

Onsite wastewater systems dispose of primary treated effluent by utilizing the soil for final recycling and renovation of wastewater into the environment. Soil and site limitations have become a challenge to design a wastewater system and dispose of onsite wastewater using a conventional pipe and gravel design. Using secondary-treated effluent from an advanced treatment unit applied to a reduced disposal area offers an additional alternative when developing an onsite wastewater system. The objective of this study was to determine the feasibility of hydraulically loading limiting soils with secondary-treated effluent in a reduced disposal area. A reduced disposal area was constructed at six existing residences within the same subdivision that had shallow redoximorphic features that precluded using a conventional pipe and gravel wastewater design. Each residence had an existing advanced treatment unit with a surface discharge of secondary-treated effluent. Flows were diverted from the surface discharge to the reduced disposal area. Wastewater flows were recorded at regular intervals, along with ponding depths in the disposal area and fluctuations in the seasonal water table over a 12-month period (March 2017 to March 2018). The disposal areas were hydraulically loaded at 2 to 3.8 times the rate recommended for secondary-treated effluent. Wastewater effluent was sampled throughout the study and resulted in a mean of -1 total suspended solids, -1 biochemical oxygen demand, and >6.3 mg·L-1 dissolved oxygen, all of which met or exceeded the minimum water quality criteria for surface discharges of secondary-treated effluent. Three of the six sites showed ponding depths between 0 and 4 cm in the trenches during the study period. The remaining three sites showed ponding between 0 and 35 cm in the trenches during the study period. Based on the results of this study, a reduced disposal area utilizing secondary-treated effluent appears to be a feasible option to surface discharging.

Fulminant sepsis after liver biopsy:A long forgotten complication?

We report on a 74-year-old patient with recurrent cholangitis and a large juxtapapillary duodenal diverticulum. Despite drainage of the common bile duct by an endoscopically placed stent, the elevated liver enzymes normalized only partially. To rule out other possible causes of liver injury, a percutaneous liver biopsy was done. After the liver biopsy the patient developed fulminant septic shock and died within 24 h. We discuss the possible causes of the septic shock following percutaneous liver biopsy in our patient and give a concise overview of the literature.

AIM: To determine the predictors of 50 d in-hospital mortality in decompensated cirrhosis patients with spontaneous bacterial peritonitis(SBP).METHODS: Two hundred and eighteen patients admitted to an intensive care unit in a tertiary care hospital between June 2013 and June 2014 with the diagnosis of SBP(during hospitalization) and cirrhosis were retrospectively analysed. SBP was diagnosed by abdominal paracentesis in the presence of polymorphonuclear cell count ≥ 250 cells/mm3 in the peritoneal fluid. Student's t test, multivariate logistic regression, cox proportional hazard ratio(HR), receiver operating characteristics(ROC) curves and Kaplan-Meier survival analysis were utilized for statistical analysis. Predictive abilities of several variables identified by multivariate analysis were compared using the area under ROC curve. P septic shock, serum bilirubin, international normalized ratio, aspartate transaminase, and model for end-stage liver disease- sodium(MELD-Na) were significantly associated with in- hospital mortality in patients with SBP(P ≤ 0.001). Multivariate coxproportional regression analysis showed AKI(HR = 2.16, 95%CI: 1.36-3.42, P = 0.001) septic shock(HR = 1.73, 95%CI: 1.05-2.83, P = 0.029) MELD-Na(HR = 1.06, 95%CI: 1.02-1.09, P ≤ 0.001) was significantly associated with 50 d in-hospital mortality. The prognostic accuracy for AKI, MELD-Na and septic shock was 77%, 74% and 71% respectively associated with 50 d inhospital mortality in SBP patients.CONCLUSION: AKI, MELD-Na and septic shock were predictors of 50 d in-hospital mortality in decompensated cirrhosis patients with SBP.
