Culture and Sensitivity Patterns of Various Antibiotics Used for the Treatment of Pediatric Infectious Diarrhea in Children under 5 Years of Age: A Tertiary Care Experience from Karachi

Background and objective: Infectious diarrhea is one of the most common infections and a significant cause of morbidity and mortality in children under 5 years of age. Frequent and irrational use of antibiotics has resulted in increased bacterial resistance. The aim of our study was to determine the culture and sensitivity patterns of antibiotics used for the treatment of diarrhea in children less than 5 years of age in a tertiary care hospital of Karachi, Pakistan. Methodology: This cross sectional study was conducted for a period of six months in the children of ages ranging between 6 months and 5 years. stool samples were obtained from the patients presented with signs and symptoms of diarrhea in OPD or being referred to microbiology department for stool C/S (culture and sensitivity). Data were analyzed on SPSS version 19.0. Results: A total number of 325 stool samples were collected, out of which 152 samples were positive for pathogens. The most common pathogen isolated was E. coli 92 (60%), followed by Klebsiella 56 (37%) and Salmonella 4 (3%). All 152 isolates were highly resistant to majority of the standard antibiotics. Conclusion: Infectious diarrhea is highly prevalent among children under 5 years of age. E. coli was found to be the most frequent pathogen isolated in stool samples of the patients presenting with diarrhea and was highly resistant to many of the commonly used standard antibiotics in our clinical set up. Male children were predominantly affected by infectious diarrhea as compared to female children. Therefore, injudicious use of antibiotics should be stopped as well as should not be prescribed empirically for the treatment of all cases of pediatric diarrhea.

A Case Report of Intestinal Myiasis in a Japanese Man

We recently experienced a case of intestinal myiasis caused by maggots of fly, Sarcophagidae. A 61-year-old Japanese man presented in August 2015 with a 2-day his-tory of diarrhea and white maggots in his stool. Two days before his presentation, he took Chinese noodles and lettuce. At his presentation, we observed a number of discharged insect bodies in his stool. The insect bodies were identified as the larvae of the 3rd instar flesh fly Sarcophagidae based on macroscopic and microscopic examinations. We finally diagnosed as intestinal myiasis due to the fact that the patient had the insect bodies mixed in his diet, lettuce. A few days after the treatment with antibiotics, his symptoms disappeared. Accurate diagnosis of intestinal myiasis in developed countries is necessary to avoid ineffective treatment.

Comparison of in Clinic-Based Fecal Microbiome Collection Techniques for Increase in Study Participation and Utilization of Microbiome Analysis

We compared the collection techniques of fecal specimens for DNA extraction and fecal microbiome analysis by utilizing the glove from a standard-of-care digital rectal exam (DRE) and the rectal swab from a pre-prostate biopsy bacterial rectal culture collected in clinical care settings. DNA yield from the swab technique compared to the glove technique yielded similar amounts of DNA (18.1 vs. 13.1 ng/μL, p = 0.06), slightly favoring the swab technique. However, utilizing DNA yield cutoffs of 15 ng/μL (37% vs. 29%, p = 0.18) and 30 ng/μL (15% and 9%, p = 0.16), we identified no differences in yield between the swab versus glove technique, respectively. Absorbance values for overall DNA quality were significantly different in favor of the glove technique (mean 1.6 vs. 2.0, p < 0.001). Using an absorbance value of 1.5 as an indication of DNA quality, only 26% (19/91) met the cutoff value using the swab group compared to 47.3% (53/112) if the glove technique was used (p < 0.001). Similar results occurred for the RNA quality with an absorbance value cutoff of 2.0 (2.2% vs. 30.4%, p < 0.001). To increase sampling feasibility and improve population sampling, gloves used from a DRE may be utilized as a consistent and efficient fecal DNA collection technique for fecal microbiome analysis. DNA yield and quality from the glove technique are comparable to—if not better than—rectal swab collection.

To observe the effect of acupuncture on acute bacillary dysentery,wetreated 326 cases of the disease respectively with acupuncture and *** resultindicated that acupuncture could relieve obviously symptoms and signs of the patient and theshort term curative rate reached 79.01%,similar to furazolidone(P>0.05).Thoughacupuncture was not better than pharmacotherapy in killing bacillus dysenteriae,it could ef-fectively cured acute bacillary dysentery without side—effect and drug allergy.
