Electrochemotherapy and heart function:Treatment in a patient with implantable cardioverter defibrillator/pacemaker

Electrochemotherapy(ECT) is a recently described therapy that relies on the permeation of cancer cell membranes by electrical pulses to enhance cytotoxic drug penetration. It has been successfully used in the treatment of primary and metastatic skin cancer. Systemic chemotherapy is the most commonly used therapeutic strategy, and the prevailing orientation calls for the administration of the maximum tolerated dose; however, considerable limitations exist including toxicities to healthy tissues and low achievable drug concentrations at tumor sites. We reported a case of an 83-years-old patient with a laterocervical metastasis of a squamous epidermoidal lip cancer. The patient had a complex medical history and an implantable cardioverter defibrillator(ICD)/pace-maker. The lesion was localized in the supraclavicular right side with a distance from the pace-maker/ICD about 5 cm, but the nodule was not deeply located. The ECT was performed un-der general anesthesia and particular attention we put on the interference with the functioning of the heart. The synchronization algorithm currently implemented in Clinoporator Vitae device coupled with the external triggering device Accu Sync proved to be effective in preventing external stimulation of the heart during the so-called vulnerable period of the ventricles. As a result all electroporation pulses in our study were delivered outside the vulnerable period and no heart arrhythmias or any other pathological morphological changes were observed. The safety of treatment was demonstrated also by absence of side effects during and after ECT.

Photodynamic therapy with topical aminolevulinic acid

Photodynamic therapy(PDT) is a relatively new therapy in dermatology that uses the topical application of a porphyrin derivative to selectively destroy a cutaneous target. The action is implemented by the application of a specific light frequency. The ability of porphyrin to selectively target tumor tissue has been known since the 1960 s. In the late 1970 s, the underlying mechanism was defined, and Dougherty's discovery of the first chromophore led to the production and commercialization of Photofrin. Many other chromophores that can act as photosensitizers have been studied since then, with aminolevulinic acid currently the most commonly used chromophore in clinical practice. PDT is simple, minimally invasive and can be administered on an outpatient basis. The efficacy of PDT has been proven for actinic keratosis, Bowen's disease and basal cell carcinoma; another of its well-known applications is the treatment of photoaging. Indications for its use are continuously increasing, and promising results are reported for various skin diseases. In this paper we report the mechanism of action of PDT with aminolevulinic acid, the literature concerning the most common diseases treated with PDT and the subsequent level of evidence.

Cutaneous Melanoma:A Single Center Experience in Brazil

The aim is to evaluate the profile of cutaneous melanoma in rural and urban areas of the State of Espírito Santo and compare the results between *** information collected included gender and age at diagnosis,city of residence,tumor morphology,anatomic site,histological subtype,level of invasion,tumor thickness and staging.A retrospective observational study was conducted,using the Pathology Department’s database of Hospital Universitário Cassiano Antonio de Moraes(HUCAM).Statistical analysis was performed using SPSS Statistics.A total of 258 histopathological reports of Cutaneous Melanomas were identified between 2003 and *** skin cancer was more common among women,in the age group from 50 to 79 *** majority of lesions was localized in the malar region of the face,usually classified as in situ or with superficial invasion and presented Breslow’s thickness less than 1 *** the diagnosis,the male population was older and had more aggressive melanomas with higher rates of Clark’s levels IV and V and Breslow’s thickness when compared to the female *** was a great concentration of patients in the central mesoregion(173 cases—67.06%),probably because this is the most populous regional area of Espírito *** prevalence of melanoma between 2003-2016 was higher in the rural area(21.52 cases/100,000 habitants)when compared to the urban area(4.15 cases/100,000 habitants).

Grafting Propagation Techniques of Red Sandalwood(Pterocarpus santalinus),a Precious Hongmu Tree Species

[Objectives] This article aimed to study the grafting propagation techniques of red sandalwood ( Pterocarpus santalinus ), so as to provide technical support for the effective propagation of seedlings of this precious Hongmu species.[Methods] Based on the analysis and comparison of the growth performance of the Hongmu tree species introduced to China, taking 1 to 2-year-old Pterocarpus indicus seedlings as rootstocks and single-bud stems of P. santalinus as scions, grafting was carried out from January to April using three grafting propagation methods, cut grafting propagation, skin grafting propagation and oblique grafting propagation.[Results] The results of several years of experiments show that in early spring, P. santalinus could be propagated well using the grafting propagation methods of skin grafting propagation and oblique grafting propagation, with survival rate over 80%. For rootstocks with young age or thin stems, the joint unions of the oblique grafting propagation healed well, without apparent protrusions, unlike those of skin grafting propagation. The survival rate of oblique grafting propagation that is fast and reliable was higher than that of cut grafting propagation.[Conclusions] It is feasible to use the grafting propagation methods to propagate P. santalinus seedlings. This is a traditional and innovative technology path in the propagation of high-quality seedlings of precious Hongmu trees, and can be promoted and applied in industrial practice.

Pigmented hidroacanthoma simplex:A diagnostic pitfall

Hidroacanthoma simplex(HAS) is a rare skin appendage tumor and is recognized as an intraepidermal variant of poroma. Pigmented HAS is an extremely rare variant with dendritic melanocytes and/or melanin pigment in the tumor cells and a few cases of pigmented HAS have been reported in the English literature. We herein report a case of pigmented HAS and discuss the clinicopathological features of pigmented HAS and the pitfall in the diagnosis of black lesions.

Teledermatology update:Mobile teledermatology

Mobile teledermatology is a relatively recent modification of teledermatology, which involves using mobile platforms like cellular phones to transmit images and data for the purpose of teleconsultations. With the rapidly improving quality of smart phone cameras combined with easier access to mobile internet, mobile teledermatology is emerging as a feasible and costeffective method for teledermatology practice. Mobile teledermatology has shown good results in concordance studies comparing it to face-to-face consultations. Mobile teledermatology can be used for most types of clinical dermatology cases. Mobile teledermatology has been found to be useful in diagnosis, screening and triage of skin lesions including skin cancers. It is also useful as a tool to follow up patients with chronic dermatological problems like psoriasis and chronic wounds. The obvious advantage of mobile teledermatology is it's cost-effectiveness and the fact that access to expert dermatology care is made easier for patients especially in remote areas. Further research is however required to standardize protocols for mobile teledermatology. Collaborative research among people working in this field would be very useful in this standardization and would help in optimizing the opportunities provided by this interesting tool. This article gives a brief overview of mobile teledermatology including definitions, tools involved, indications, limitations and future applications.

From the outside-in: Epidermal targeting as a paradigm for atopic disease therapy

Atopic dermatitis(AD) is a chronic inflammatory skin disorder which can precede asthma and allergic rhinitis in a disease trajectory known as the atopic march. The pathophysiology of AD includes cutaneous inflammation, disrupted epidermal barrier function, xerosis and propensity to secondary infections. AD had previously been thought to arise from the systemic atopic immune response and therapies are therefore directed towards ameliorating Th2-mediated inflammation. However in recent years the focus has shifted towards primary defects in the skin barrier as an initiating event in AD. Links between loss-of-function variants in the gene encoding filaggrin and disrupted activity of epidermal serine proteases and AD have been reported. Based on these observations, a mechanism has been described by which epidermal barrier dysfunction may lead to inflammation and allergic sensitization. Exogenous and endogenous stressors can further exacerbate inherited barrier abnormalities to promote disease activity. Pathways underlying progression of the atopic march remain unclear, but recent findings implicate thymic stromal lymphopoietin as a factor linking AD to subsequent airway inflammation in asthma. This new appreciation of the epidermis in the development of AD should lead to deployment of more specific strategies to restore barrier function in atopic patients and potentially halt the atopic march.
