Traditional Chinese Medicine constitution among patients with allergic rhinitis and its correlation with anxiety and depression

OBJECTIVE:To preliminary investigate the distribution of Traditional Chinese Medicine(TCM)constitution among patients with allergic rhinitis(AR)and reveal the related factors with anxiety and depression.METHODS:Between July 2020 and June 2021,specialist doctor recruited AR patients in Beijing and Shanghai.A total of 630 patients admitted to participate in the survey,and 516(81.9%)(male 54.3%,female 45.7%)participants completed the questionnaires.Three scales including Constitution in Chinese Medicine Questionnaire(CCMQ),Generalized Anxiety Disorder Questionnaire(GAD-7)and The Patient Health Queationaire-9(PHQ-9)were applied.Theχ^(2)and the Fisher’s exact test were used to evaluate the classification data,multivariate logistic regression was used to explore the related factors of anxiety and depression.RESULTS:According to 516 AR patients,the most commonly first-constitution was Yang-deficiency(22.1%)and frequent of all constitutions was inherited-special(21.6%).One third of AR patients(33.5%)suffered from anxiety,nearly half of AR patients(46.5%)were found to be depressive.Inherited-special(27.7%)was the most common constitution in patients with anxiety whereas Yang-deficiency(26.7%)was most common constitution in patients with depression.Sex,duration of symptoms,balanced,Qi-deficiency,phlegm-dampness,Qi-stagnation,and inherited-special constitutions were related with anxiety;Sex,income,duration of symptoms,balanced,Qi-deficiency,Yang-deficiency,phlegm-dampness,Qi-stagnation,and inherited-special constitutions were related with depression.CONCLUSIONS:According to our study,the commonly constitution types of AR were Yang-deficiency and inherited-special.We found that the prevalence value of anxiety and depression were high.People with Qi-deficiency,phlegm-dampness,Qi-stagnation,and inherited-special constitutions were more likely to have mental problems than the others.Controlling these influential factors might be beneficial for clinical health management of AR patients,and the government should apply appropriate

OBJECTIVE: To use evidence-based medicine to explore the efficacy of acupoint application(AA) for allergic rhinitis(AR) at different time points and its safety. METHODS: We searched 7 databases(Pub Med, Cochrane, Embase, China National Knowledge Infrastructure, Wanfang Database, China Science and Technology Journal Database, and China Biology Medicine disc) as well as the international clinical trial registration platform from January 2010 to March 2020 for randomized controlled trials(RCTs) comparing the efficacy of AA versus placebo, Western Medicine or other alternative therapies on AR. Risk of bias was assessed according to the Cochrane handbook, and statistical analysis was performed using Rev Man 5.3. Outcomes included the total effective rate, recurrence rate, total nasal symptom score(TNSS), visual analogue scale(VAS), quality of life measured by the rhinitis Quality of Life Questionnaire(RQLQ) or Short Form-36(SF-36), adverse events, and biomarkers including immunoglobulin E(Ig E), peripheral blood eosinophil count(EOS), interleukin-4(IL-4), and interferon gamma(INF-γ). RESULTS: Twenty-eight RCTs involving 3282 participants were included. The short-term and long-term efficacy of AA was significantly better than placebo, including better total effective rate [RR: 3.05, 95%CI(1.84, 5.07), after treatment;RR: 9.29, 95%CI(2.57, 33.66), at 6 months], lower recurrence rate [RR: 0.55, 95%CI(0.45, 0.66), at 6 months;RR: 0.65, 95%CI(0.57, 0.74), at 1 year], lower TNSS [MD:-3.09, 95%CI(-3.58,-2.61), after treatment], and lower RQLQ [MD:-14.79, 95%CI(-21.49,-8.10), after treatment;MD:-11.92, 95%CI(-17.40,-6.45), at 4-6 months]. Compared with Western Medicine, AA had better long-term total effective rate [RR: 1.33, 95%CI(1.05, 1.69), at 3 months;RR: 1.49, 95%CI 1.22 to 1.81, at 1 year) and lower recurrence rate [RR: 0.48, 95%CI(0.39, 0.58), at 6 months;RR: 0.45, 95%CI(0.33, 0.60), at 1 year]. OBJECTIVE:To examine the efficacy of early intervention(4 weeks before pollen dispersal)with sphenopalatine ganglion(Xinwu acupoint)stimulation in patients with allergies after the onset of seasonal allergic rhinitis(SAR).METHODS:This is a prospective,randomized and unblinded half-open study.Forty-one SAR volunteers were randomly assigned to either the sphenopalatine ganglion(SPG)acupuncture plus supplementary acupuncture(SPG group)or the sham-SPG acupuncture plus supplementary acupuncture(SA group)stimulation 4 weeks before the onset of allergy season.The changes of the total nasal symptom score(TNSS)and the Rhinoconjunctivitis Quality of Life Questionnaire(RQLQ)scores were measured on the first week in the onset of allergy season.RESULTS:Four patients dropped out due to local hematoma and pain in the SPG and SA groups.The remaining 37 patients continued through to the end of the trial.After early intervention 4 weeks before the onset of allergy season,the sneezing,nasal congestion and itchiness scores in the first week of onset time were significantly lower in the SPG group than in the SA group patients(P<0.001).The RQLQ score obtained at the onset of symptoms indicated that symptoms were more significantly ameliorated in the SPG group than in the SA group(P<0.001).CONCLUSIONS:The results of this study confirmed that early intervention by sphenopalatine ganglion(Xinwu acupoint)stimulation can effectively improve the symptoms and the quality of SAR patients'daily lives.

Allergic rhinitis(AR)is a costly and highly prevalent chronic disease now.[1]More seriously,many patients with AR also have allergic asthma(AA).The presence of AR is a risk factor for the development of AA.[2]Moreover,AA is still a significant cause of mortality for all ages.[3]Therefore,predicting and preventing AA can reduce accidental death,severe health hazards,and the society's enormous economic burden.

To the Editor:Allergic asthma and allergic rhinitis(AR)are common diseases that often occur concomitantly.Approximately 80%of allergic asthmas suffer from AR symptoms,as the two diseases share common immunopathological mechanisms.Sensitizationand exposure to allergens are the risk factors that induce and aggravate allergic airway diseases such as allergic asthma and AR.

Background:In China, the prevalence of allergic rhinitis (AR) and asthma has increased in the past decade. As these two diseases frequently coexist, the Asia-Pacific Survey of Physicians on Asthma and Allergic rhinitis (ASPAIR) study aimed to assess physicians5 beliefs and treatment patterns of coexistent asthma-AR across six Asian countries. This analysis presents the results from China. Methods: The 200 hospital-based general physicians and pediatricians were interviewed from five cities in China. Physicians were questioned in-person about their knowledge, beliefs and management practices for patients with coexistent asthma-AR. Results: Approximately 70% of the physicians interviewed routinely evaluated their patients with asthma or AR for signs of coexistent disease. While the majority of physicians (>90% of physicians) recognized the increased burden of coexistent asthma-AR vs. one condition alone and that coexistent disease requires additional treatment, most physicians (96%) also believed that patients with coexistent asthma-AR were well managed if either condition alone improved. Similarly, although 71% of physicians selected a combination of intranasal and inhaled corticosteroids as their preferred treatment for coexistent asthma-AR, in line with treatment guidelines, two fifths of physicians indicated that treatment for coexistent disease requires too much medication and that their patients prefer oral medications and a third of physicians believed that corticosteroids should be delayed in children. Conclusions: This survey demonstrates that physicians interviewed in China have a broad understanding of coexistent asthma-AR and its impact on patients. A holistic approach to patient management with informed decisions regarding patients' overall treatment will benefit patients who suffer from coexistent disease.

Objective:To observe the clinical manifestations of allergic rhinitis mice and the expression changes of the eosinophils CCR3 and the granule protein rnRNA in the bone marrow,peripheral blood and nasal lavage fluid.Mctliods:Twenty-four BALB/c mice were randomly divided into the control group.PBS therapy group.siKNA therapy group and the CCR3 siRNA therapy group(n=6).Allergic rhinitis model were sensitized and stimulated by ovalbunfin,and CCR3 siKNA therapy group were administered with CCH3 transnasally before stimulated.The levels of the eosinophils CCR3.MBP.ECP and EPO in bone marrow,peripheral blood and nasal lavage fluid were detected by RT-PCR.Results:Compared to the control group and CCR3 siR.NA therapy group,the nasal mucosa of the PBS therapy group and siRNA therapy group developed epithalaxy.goblet cells hyperplasia,squamous epithelium metaplasia,epithelium necrosis,lamina propria and submucosa gland hyperplasia,vasodilatation,tissue edema,and the characterized eosinophil infiltration.RT-PCR indicated that the CCR3 rnRNA,MBP.ECP and EPC)expression in bone marrow,peripheral blood and nasal lavage fluid of the CCR3 siKNA therapy group was lower than the PBS therapy group and siR.NA therapy group(Prhinitis eosinophil,thus can reduce the effect of eosinophils and then reduce the inflammation effect of the allergic rhinitis.It may be a new treatment for respiratory tract allergic inflammation.

The therapeutic goals of the treatment of allergic rhinitis (AR) and chronic rhinosinusitis (CRS) are symptom relief, avoiding complications, and improving quality of life. In the treatment of AR and CRS, several limitations of currently prescribed medicines have been identified. Antihistamine administration (both oral and topical) together with intranasal corticosteroids bring relief to the majority of patients, but their dependency on the medications and a necessity to maintain strict compliance with regular medication regimes pose a challenge. Immunotherapeutic agents are an option in some patients, but polysensitized patients, the risk of anaphylaxis, and the need for daily administration for years are limiting it from becoming the main therapy modality. Immunotherapy in any form requires commitment by the patient, which renders adherence and compliance issues particularly relevant. The procedure involved are generally time-consuming and entail an associated risk of severe adverse reactions. The use of biologics could overcome the limitations of other therapeutic modalities. They could be used as a monotherapy or combined with pre-existing medications. The benefits of targeted therapy include less adverse effects and optimal efficacy. The aim of the present review was to investigate the collective literature to date pertaining to the role of biologics in managing children with AR and CRS.

Objective: To analyze the potential mechanism of Xinyi San in treating rhinitis through network pharmacology. Methods: In the database of Traditional Chinese Medicine Systems Pharmacology (TCMSP), chemical composition and potential targets of Xinyi San were got, and the target genes of rhinitis of Xinyi San were extracted from GeneCards databases. Then we constructed protein-protein interactions (PPI) network of target genes, and then analyzed the Key genes in GO analysis and KEGG analysis. Results: We got 97 components, 53 potential therapeutic targets, 1009 GO items and 92 pathways in our study. The main pathways included Lipid and atherosclerosis, Chemical carcinogenesis-receptor activation, PI3K-Akt signaling pathway, Human cytomegalovirus infection, Prostate cancer, etc. Conclusion: Xinyi San plays a role in treating rhinitis through multiple components, multiple targets and multiple pathways.
