Background:Following initiation of China’s National Malaria Elimination Action Plan(NMEAP)in 2010,the'1-3-7’approach was developed and rolled out in China to facilitate the malaria elimination programme and accelerate malaria elimination.This study aims to summarize and condense these experiences through a Retrospective analysis in Jiangsu Province,which could be adapted and applied in other malaria elimination settings worldwide.Methods:A Retrospective analysis of imported malaria cases into China identified through an improved surveillance and response system in Jiangsu Province was carried out for the period of 2001-2020.To improve the malaria surveillance and response system,Centers for Diseases Control and Prevention from the prefectures and counties in Jiangsu province conducted population-level health education to improve healthcare seeking behavior,strengthened capacity of health facilities to improve performance of malaria diagnosis and treatment,and raised the capacity of public health providers to improve implementation of the'1-3-7’approach.Categorical variables were carried out by Chi square tests with Fisher’s exact correction.Results:From 2001 to 2020,a total of 9,879 malaria cases were reported in Jiangsu Province.Since 2012,no indigenous malaria cases have been reported in Jiangsu Province.However,in recent years,there has been a substantial increase of imported falciparum malaria cases.Between 2012 and 2020,an estimated 61.57 million individuals have benefited from population-level health education in Jiangsu Province.For healthcare-seeking services among the 2,423 imported malaria cases,687(28.4%)and 1,104(45.6%)cases visited hospitals on the first day and the second day from symptom onset,respectively.A total of 1,502(61.9%)cases were diagnosed on the first day at medical facilities.Jiangsu Province achieved 100%,99.4%and 98.3%completion rate in terms of case detection and notification(within one day),case investigation(within three days)and foci response and disposition(within seven days),res

Background:Ketogenic diet(KD)therapy is one of the main treatments for drug-resistant epilepsy.However,the KD therapy has been applied in only a small number of infantile spasm cases.In this large multicenter study,we investigated the efficacy of KD therapy in the treatment of infantile spasms.Methods:In this Retrospective,multicenter cohort study,clinical data from main epilepsy centers were analyzed.Patients were classified into different groups according to age,type of drug and whether glucocorticoid was used before initiation of KD.Results:From October 2014 to March 2020,481 patients(308 males and 173 females)with infantile spasms were treated with the KD therapy.The age of the patients ranged from 2 months to 20 years,with a mean age of 1 year and 10 months.The number of anti-seizure medications(ASMs)used before KD initiation ranged 0-6,with a median of 3.In different time from initiation(1,3,6,and 12 months),the rates of seizure freedom after KD were 6.9,11.6;16.0 and 16.8%,respectively(x^(2)=27.1772,P0.05).Two hundred and eighteen patients(45.3%)failed to respond to c

Safety and efficacy of a docetaxel-5FU-oxaliplatin regimen with or without trastuzumab in neoadjuvant treatment of localized gastric or gastroesophageal junction cancer: A Retrospective study

BACKGROUND Triplet chemotherapy,with docetaxel-5FU-oxaliplatin FLOT regimen recently became the standard perioperative treatment for localized gastric cancer(GC).An adapted regimen called TeFOX was recently tested in metastatic setting and gave promising results.AIM To determine safety and efficacy of TeFOX perioperative regimen.METHODS This monocentric Retrospective study aims to test efficacy and safety of the perioperative TeFOX regimen given alone or in combination with trastuzumab in patients with localized GC.TeFOX consist in docetaxel(50 mg/m^2)with oxaliplatin 85 mg/m^2 and and leucovorin(400mg/m^2^)5 FU bolus(400mg/m^2)on day 1,followed by continuous infusion of 5FU for 46h(2400mg/m^2)every 2 wk.RESULTS Thirty-three consecutive patients were included in this Retrospective study.Eighteen patients have a gastroesophageal junction cancer and 11 have a GC.Median follow-up of surviving patients was 32 mo.R0 resection was obtained in 30(91)patients.Twelve patients(36)had a pathological complete response and 8(24)patients a nearly complete pathological response.Median OS and PFS were not reached at data base lock.We have observed 6 metastatic relapses and 1 localized relapse.No relapse was observed in patients with pathological complete responses.The most common grade 3-4 adverse events were peripheral neuropathy(21)and asthenia(20).CONCLUSION TeFOX regimen could be safely administrated in perioperative treatment of localized GC.TeFOX and the FLOT regimen have comparable efficacy and safety profiles.

High-dose ruthenium-106 plaque therapy for circumscribed choroidal hemangioma: a Retrospective study of 25 Chinese patients

AIM:To evaluate the efficacy and safety of ruthenium-106(106Ru)plaque radiotherapy at a dose(>50 Gy)higher than recommended(29-50 Gy)for treatment of circumscribed choroidal hemangioma(CCH)in Chinese patients.METHODS:This Retrospective study included 25symptomatic CCH patients undergoing 106Ru plaque brachytherapy involving 25 eyes between January 2005and August 2016.Ophthalmic examination was performed at the baseline and at each post-treatment follow-up visit,using best-corrected visual acuity(BCVA),dilated fundus examination,and B-scan ultrasonography.The primary efficacy outcome measures included the changes in BCVA and hemangioma dimensions at the last followup visit from the baseline.RESULTS:The mean follow-up duration was 28.0±26.6(range,12-110)mo.All the hemangiomas were located in the posterior pole except for two involving the fovea.The mean apex dose of 106Ru plaque radiotherapy was84.4±19.7 Gy.The mean BCVA improved from 41.4±29.3(0-97)at the baseline to 53.0±33.8(0-97)ETDRS letters at the last visit(P=0.01).The mean hemangioma height declined from 3.98±0.88(2.40-5.50)mm to 0.84±1.63(0-6.47)mm(P≤0.001),and the greatest linear diameter(GLD)reduced from 9.36±2.23(6.80-15.00)to 7.40±2.45(0-13.00)mm(P≤0.001).Hemangioma size increased in one(4%)eye with a worsened vision,and subretinal fluid completely resolved in all but one patient(4%).Radiationrelated retinopathy was observed in two patients at posttreatment 9 and 11mo,respectively.CONCLUSION:106Ru plaque brachytherapy at a dose(>50 Gy)higher than recommended(29-50 Gy)is an effective treatment regimen for symptomatic CCH associated with significantly improved visual acuity and a favorable safety profile in Chinese patients.

OBJECTIVE:To evaluate the clinical efficacy of nourishing Yin and tonifying Yang sequential therapy(NYTYST)in combination with the Western Medicine Climen in treating diminished ovarian reserve(DOR).METHODS:From September 2014 to September2016,46 patients with DOR who received NYTYST and Climen(treatment group)and 51 patients who received Climen only(control group)at the Jiangsu Provincial Traditional Chinese Medicine Hospital(Nanjing,China)were Retrospectively reviewed.Before and at 3 months after treatment,serum levels of follicle-stimulating hormone(FSH),FSH/luteinizing hormone(LH),estradiol(E2),and anti-Mullerian hormone(AMH),the antral follicle count(AFC),stromal peak systolic velocity(PSV),symptom scores,and the clinical effectiveness rate were measured and compared between the two groups.RESULTS:After treatment,serum levels of FSH,FSH/LH and E2,and symptom scores were significantly decreased compared with before treatment(all P<0.05)in the two groups.AMH levels,AFC,and PSV were significantly increased after treatment compared with before treatment in both groups(all P<0.05).Serum levels of FSH,FSH/LH,and E2 after treatment were significantly lower in the treatment group than in the control group(all P<0.05).However,AMH levels and the AFC were significantly higher after treatment in the treatment group than in the control group(both P<0.05).The clinical effectiveness rate was significantly higher in the treatment group than in the control group(87%vs 68.6%,P<0.05).CONCLUSION:NYTYST combined with Climen may be more effective in treating DOR than Climen alone.

Primary tumor location and survival in colorectal cancer: A Retrospective cohort study

BACKGROUND Primary tumor location is a prognostic factor for metastatic colorectal cancer(m CRC).Post hoc analyses of m CRC clinical trials,including FIRE-3,CALGB/SWOG 80405,suggest that primary tumor location is also predictive of survival benefit with cetuximab or bevacizumab in combination with 5-fluorouracil-based chemotherapy.AIM Evaluate prognostic/predictive roles of primary tumor location in real-world m CRC patients treated with cetuximab or bevacizumab plus 5-fluorouracil-based chemotherapy.METHODS This Retrospective cohort study selected patients with KRAS wild-type m CRC who initiated first-line therapy with cetuximab or bevacizumab in combination with 5-fluorouracil/leucovorin/irinotecan(FOLFIRI)or 5-fluorouracil/leucovorin/oxaliplatin(FOLFOX)between January 2013 and April 2017 from the Flatiron Health electronic health record-derived database of de-identified patientlevel data in the United States.Primary tumor location was abstracted from patients’charts.Left-sided primary tumor location(LPTL)was defined as tumors that originated in the splenic flexure,descending colon,sigmoid colon,or rectum;right-sided primary tumor location(RPTL)was defined as tumors that originated from the appendix,cecum,ascending colon,hepatic flexure,or transverse colon.Propensity score matching was used to balance the baseline demographic and clinical characteristics between patients treated with cetuximab and patientstreated with bevacizumab.Kaplan-Meier and Cox regression methods were used for survival analyses.RESULTS A total of 1312 patients met the selection criteria.Of 248 cetuximab plus FOLFIRI or FOLFOX patients,164 had LPTL and 84 had RPTL;of 1064 bevacizumab plus FOLFIRI or FOLFOX patients,679 had LPTL and 385 had RPTL.Cetuximab LPTL and RPTL patients were more likely to receive FOLFIRI vs bevacizumab patients(LPTL:64.0%vs 24.3%;RPTL:76.2%vs 24.9%,P<0.001).Stage at initial diagnosis was different between cetuximab RPTL vs bevacizumab RPTL patients(P<0.001);cetuximab RPTL patients were more likely to have stage III

Gutuo Jiejiu decoction improves survival of patients with severe alcoholic hepatitis: A Retrospective cohort study

AIM To verify the value of Gutuo Jiejiu decoction in improving the survival of patients with severe alcoholic hepatitis(SAH).METHODS We performed a Retrospective cohort study in consecutive patients diagnosed with SAH at the Teaching Hospital of Chengdu University of Traditional Chinese Medicine and Shuguang Hospital, Shanghai University of Traditional Chinese Medicine. The traditional Chinese medicine formula Gutuo Jiejiu decoction was employed as an exposure factor. Patients from the Teaching Hospital of Chengdu University of Traditional Chinese Medicine who had been treated with Gutuo Jiejiu decoction + prednisone were assigned to an observation group, and patients from Shuguang Hospital, Shanghai University of Traditional Chinese Medicine who had been treated with prednisone alone were selected as a control group. A Retrospective analysis was performed by comparing age, alcohol intake, and clinical parameters of liver injury beforeand after treatment. Additionally, the 3-and 12-mo survival rates and the occurrence of complications were analyzed.RESULTS One hundred and twenty-eight eligible patients were selected from 175 cases with SAH, of which 68 were assigned to the observation group and the other 60 to the control group. No significant difference was found in the patients' baseline characteristics(P > 0.05). However, significant improvements of 90-d survival rate [56/68(82.4%) vs 27/60(45.0%), P = 0.0000] and 365-d survival rate [48/68(70.6%) vs 13/60(21.7%), P = 00000] were observed in the observation group after treatment. After the first 3 mo of treatment, more improvements in the clinical parameters and scoring systems related to liver injury occurred in the observation group than in the control group(P < 0.05). After treatment for 12 mo, the differences in the clinical parameters and scoring systems related to liver injury between the two groups were more significant(P < 0.05). BACKGROUND Neoadjuvant chemoradiotherapy(nCRT)followed by resection and postoperative multi-agent chemotherapy(maChT)is the standard of care for locally advanced rectal cancer.Using this approach,maChT administration can be delayed for several months,leading to concern for distant metastases.To counteract this,a novel treatment approach known as total neoadjuvant therapy(TNT)has gained popularity,in which patients receive both maChT and nCRT prior to resection.We utilized the National Cancer Database to examine temporal trends in TNT usage,and any potential effect on survival.AIM To study the temporal trends in the usage of TNT and evaluate its efficacy compared to neoadjuvant chemoradiation.METHODS We queried the National Cancer Database for patients with locally advanced rectal cancer,Stage II-III,from 2004-2015 treated with nCRT or TNT.TNT was defined as maChT initiated≥90 d prior to nCRT initiation.Overall survival was calculated from the date of diagnosis to the date of last contact or death using Kaplan-Meier curves to present the cumulative probability of survival,with logrank statistics to assess significance.Multivariable cox regression was used to identify predictors of survival and propensity score analysis accounted for bias.RESULTS We identified 9066 eligible patients,with 8812 and 254 patients receiving neoadjuvant chemoradiation followed by maChT and TNT,respectively.Nodal involvement,stage III disease,and treatment in recent years were predictive of TNT use.There was greater use of TNT with more advanced stage,specifically>1 node involved(odds ratio[OR]=2.88,95%confidence interval[CI]:2.11-3.93,P<0.01)and stage III disease(OR=2.88,95%CI:2.11-3.93,P<0.01).From 2010 to 2012 the use of TNT increased(OR=2.41,95%CI:1.27-4.56,P<0.01)with a greater increase from 2013 to 2015(OR=6.62,95%CI:3.57-12.25,P<0.01).Both the TNT and neoadjuvant chemoradiation arms had a similar 5-year survival at 76%and 78%respectively.Multivariable analysis with propensity score demonstrated that increased age,high comorbidity s

A Retrospective Analysis of Intracranial Hemorrhage in Children with Hemophilia A

To investigate the incidence,risk factors,clinical manifestations and prognosis of intracranial hemorrhage (ICH)in children with hemophilia A in a center of China, we conducted a Retrospective analysis of 126 children with hemophilia A at our hospital in recent 4 years.Thirty-six children with hemophilia A (including 19 severe cases,and 17 moderate cases complicated with joint diseases)received low dose factor Ⅷ (FⅧ) prophylaxis,and none of them had ICH.However,13 cases of hemophilia A not given prophylaxis were complicated with ICH (12 severe cases,and 1 moderate case)and demonstrated an incidence of 10.3%(13/126)in all patients,and 28.6%(12/42)in severe cases.Of the 13 cases,9 severe ICH cases had a definite history of head injury,accounting for 69.2%.Headache was common in children >3 years,but somnolence,irritability, gaze or convulsions in children <3 years.The most common findings of cranial CT scan included intracranial hematoma (9/13),and less commonly observed were subependymal hemorrhage and intraventricular hemorrhage.After administration of FⅧ,all patients survived.Hematoma of 6 cases was observed during CT reexamination after 1-3 months. During the follow-up period,only one case had slight activity limitation on one side of the limb,but steadily recovered.Besides the decreased concentration of FⅧ,trauma is the most common risk factor of ICH in children with hemophilia A.The active treatment can improve the prognosis of ICH in children with hemophilia A.
