Human bocavirus in children hospitalized for acute respiratory tract infection in Rome

Background The role of human bocavirus (HBoV) as a respiratory pathogen has not been fulfilled yet.We aimed to describe clinical and serological characteristics of children with HBoV hospitalized for acute respiratory tract infection and to evaluate whether differences occur between HBoV alone and in co-infection.Methods We retrospectively reviewed data from 60 children (median age of 6.2 months,range 0.6-70.9) hospitalized for acute respiratory symptoms,with HBoV detected from a respiratory sample,using a reverse transcriptase-PCR for 14 respiratory viruses (including respiratory syncytial virus (RSV),influenza virus A and B,human coronavirus OC43,229E,NL-63 and HUK1,adenovirus,rhinovirus,parainfluenza virus 1-3,and human metapneumovirus).Results HBoV was detected alone in 29 (48.3%) patients,while in co-infection with other viruses in 31 patients (51.7%),with a peak between December and January.Among the 60 patients,34 were bronchiolitis,19 wheezing,3 pneumonia,2 upper respiratory tract infection,and 2 whooping cough.Seven children (11.6%) required admission to the paediatric intensive care unit (PICU) for respiratory failure.No differences was observed in age,family history for atopy and/or asthma,clinical presentations,chest X-ray,or laboratory findings in children with HBoV alone vs.multiple viral detection.RSV was the most frequently co-detected virus (61.3%).When compared with HBoV detection alone,the co-detection of RSV and HBoV was associated with male sex (P=0.013),younger age (P=0.01),and lower blood neutrophil count (P=0.032).Conclusions HBoV can be detected alone and in co-infection respiratory samples of children with an acute respiratory tract infection.A cause-effect relationship between HBoV and respiratory infection is not clear,so further studies are needed to clarify this point.

BACKGROUND respiratory tract infections in the elderly are difficult to cure and can easily recur,thereby posing a great threat to patient prognosis and quality of life.AIM To investigate the therapeutic effects of different antibiotics in elderly patients with respiratory tract infection.METHODS Seventy-four elderly patients with respiratory tract infection were randomly allocated to a study(n=37;treated with cefoperazone sodium/sulbactam sodium)or control(n=37;treated with piperacillin sodium/tazobactam sodium on the basis of routine symptomatic support)group.Both groups were treated for 7 d.Time to symptom relief(leukocyte recovery;body temperature recovery;cough and sputum disappearance;and rale disappearance time),treatment effect,and laboratory indexes[procalcitonin(PCT),C-reactive protein(CRP),white blood cell count(WBC),and neutrophil percentage(NE)]before and 7 d after treatment and the incidence of adverse reactions were assessed.RESULTS In the study group,the time to WBC normalization(6.79±2.09 d),time to body temperature normalization(4.15±1.08 d),time to disappearance of cough and sputum(6.19±1.56 d),and time to disappearance of rales(6.68±1.43 d)were shorter than those of the control group(8.89±2.32 d,5.81±1.33 d,8.77±2.11 d,and 8.69±2.12 d,respectively;P=0.000).Total effective rate was higher in the study group(94.59%vs 75.68%,P=0.022).Serum PCT(12.89±3.96μg/L),CRP(19.62±6.44 mg/L),WBC(20.61±6.38×10^(9)/L),and NE(86.14±7.21%)levels of the study group before treatment were similar to those of the control group(14.05±4.11μg/L,18.79±5.96 mg/L,21.21±5.59×10^(9)/L,and 84.39±6.95%,respectively)with no significant differences(P=0.220,0.567,0.668,and 0.291,respectively).After 7 d of treatment,serum PCT,CRP,WBC,and NE levels in the two groups were lower than those before treatment.Serum PCT(2.01±0.56μg/L),CRP(3.11±1.02 mg/L),WBC(5.10±1.83×10^(9)/L),and NE(56.35±7.17%)levels were lower in the study group than in the control group(3.29±0.64μg/L,5.67±1.23 mg/L,8.13±3.01×10^(9)/L,and 64.22±8.08%,respectively;P=

Objective: To investigate the application of WBC, CRP and SAA combined detection in the early diagnosis of respiratory tract infection in children. Methods: Collect the literature reports on the early diagnosis of respiratory tract infection in children by the combined detection of WBC, CRP and SAA in recent years, and follow up the relevant literature reports from the selection of “new three routine” laboratory items for rapid diagnosis in pediatric outpatient department and the application of the combined detection of WBC, CRP and SAA in the early diagnosis of respiratory tract infection in children. Results: Many literature studies found that the combined detection of WBC, CRP and SAA has important clinical significance in the early diagnosis of respiratory tract infection in children. Conclusion: Through reviewing the relevant literature, we can understand the application of WBC, CRP and SAA combined detection in the early diagnosis of respiratory tract infection in children. To provide more accurate and reliable laboratory data for the early diagnosis and treatment of respiratory tract infection in children in the future.

Assessment of Prevalence Group A Streptococci (GAS) Associated with Upper respiratory tract infection among Hajj and Umrah Pilgrims

Background: respiratory tract infection (RTI) is a prevalent infection across the world and is a major health concern linked to the crowding that occurs during the Hajj season. Objectives: To identify how prevalent Streptococcus pyogenes is among Umrah visitors and pilgrims by comparing its presence before and after the visitors perform Umrah and hajj. Methods: A cross-sectional study was carried out on Umrah visitors/pilgrims, for which their throat swabs were taken for assessing the upper respiratory tract infection’s carriage rate, particularly of Group A streptococci (GAS), after they arrived in Saudi Arabia for the Hajj and Umrah rituals and befor their leaving. After collecting the swabs, they were inoculated on Columbia agar using 5% sheep blood and then incubated at 37˚C. VITEK 2 systems were used to identify the isolates. Results: Of the 979/613 Umrah visitors/pilgrims involved, before performing Umrah, their Streptococcus pyogenes percentage was 0.2% and it increased to 0.7 after performing Umrah. Further, the Streptococcus pyogenes percentage before and after performing Hajj was 0.3% and 1.1%, respectively. No statistical significance was found in the difference between the positive Streptococcus pyogenes found before and after performing Hajj and Umrah among Umrah visitors/pilgrims (p-value = 0.2). All bacterial isolates were found to be sensitive to Cefotaxime and Ceftriaxone antibiotics (100%). Conclusion: The Streptococcus pyogeneshigh resistance to antibiotics combined with the lack of a vaccine poses a serious risk concerning the resulting infection spread and emphasizes the necessity of ensuring greater global surveillance of such epidemics.

The Prevalence of Human Metapneumovirus among Children with Acute respiratory tract infection in North Kordofan State, Sudan

Objective: To determine the prevalence of human metapneumovirus (hMPV) among children with acute respiratory tract infection and the correlation between (hMPV) and age group. Methods: This prospective, descriptive cross-sectional hospital-based study is carried out from January to May 2019 among children from under five years old who were admitted to El-Obeid pediatric teaching Hospital, North kordofan, Sudan with acute respiratory tract infection. Results: Fifty hospitalized children with acute respiratory tract infection were enrolled in the study. Two of these (4%) were tested positive for (hMPV) in the age group (7 months - 2 years) old. Moreover, the study showed no significant correlation between infection with (hMPV) and age.

New Progress in the Cause Analysis and Nursing of respiratory tract infection after Abdominal Surgery under General Anesthesia

This article provides a review of the causes of respiratory tract infection after abdominal surgery. These causes include general anesthesia, intubation factors, factors inherent to the patient, surgical factors, the injudicious use of antimicrobial agents, and the environmental factors of the ward. The perioperative management of the respiratory tract should be strengthened. Health education, respiratory function training, oral nursing intervention,atomization inhalation, and personalized expectoration methods should receive more attention to decrease the complications and promote the early rehabilitation of patients after abdominal surgery.

Objective We investigated changes in the intestinal flora of children with Mycoplasma pneumoniae pneumonia(MPP).Methods Between September 2019 and November 2019,stool samples from 14 children with MPP from The Fourth Hospital of Baotou city,Inner Mongolia Autonomous Region,were collected and divided into general treatment(AF)and probiotic(AFY)groups,according to the treatment of“combined Bifidobacterium,Lactobacillus,Enterococcus,and Bacillus cereus tablets live”.Highthroughput 16S rDNA sequencing was used to identify intestinal flora.Results Intestinal flora abundance and diversity in children with MPP were decreased.Both Shannon and Simpson indices were lower in the AF group when compared with healthy controls(P<.05).When compared with healthy controls,the proportion of Enterorhabdus was lower in the AF group,while the proportion of Lachnoclostridium was higher(P<0.05).The proportion of Bifidobacteria and Akkermansia was lower in the AFY group but Enterococcus,Lachnoclostridium,Roseburia,and Erysipelatoclostridium proportions were higher.The proportion of Escherichia coli-Shigella in the AFY group after treatment was decreased(P<0.05).Conclusions The intestinal flora of children with MPP is disturbed,manifested as decreased abundance and diversity,and decreased Bifidobacteria.Our probiotic mixture partly improved intestinal flora disorders.

The pathogenesis of Chlamydia pneumoniae-type pneumonitis in mice

Objective To evaluate mice as experimental animals for Chlamydia pneumoniae (C.pneumoniae) infection and investigate the pathogenesis of C.pneumoniae derived pneumonitis.Methods Icr mice were inoculated with the C.pneumoniae strain, CWL-029, either intranasally or intravenously. After a single dose inoculation, mice were killed on the 1st, 3rd, 7th, 14th, 21st, 28th and 60th days. The pathological changes in lung tissue were analyzed.Results The Icr mice were shown to be susceptible to C.pneumoniae. Inoculation into mice with C.pneumoniae induced a prolonged course of lung infection, as demonstrated by persistence of lung pathology (up to 60 days). Via intranasal inoculation of mice, lung pathology was characterized by patchy interstitial pneumonitis with predominately neutrophil leukocyte infiltration early (within the first 7 days) and lymphocyte infiltration in the later stages (14 days later) of infection. After intravenous inoculation, a similarly developed interstitial pneumonitis was observed, but it was milder and patchier, especially in early stages. C.pneumoniae DNA was detected by polymerase chain reaction (PCR) intermittently in the lung tissue. Inoculated mice developed serum IgG antibody responses.Conclusion The Icr mice were susceptible to C.pneumoniae, resulting in a pulmonary infection characterized by interstitial pneumonitis, occurring most strongly via intranasal inoculation.

Level of serum neuron-specific enolase and brain damage in children with febrile seizures

BACKGROUND ： Febrile seizure （FS） has good prognosis in the majority of cases. But there is an ongoing debate on the relationship between complicated febrile seizure （CFS） and later development of temporal lobe epilepsy （TLE） due to hippocampal sclerosis.OBJECTIVE： To evaluate the level of serum neuron-specific enolase （S-NSE） of children with simple febrile seizure （SFS） and complicated febrile seizure and compare with children with non-FS respiratory tract infection. DESIGN： Contrast observation.SETTING ： Department of Pediatrics and Department of Laboratory, Fujian Provincial Hospita. PARTICIPANTS： Forty-nine patients who were admitted to Department of Pediatrics of Fujian Provincial Hospital from June 2002 to September 2003 with FS were enrolled in this study. There were 28 boys and 21 girls aged from 5 to 72 months. All children were divided into 2 groups based on frequency and duration. Thirty-two children whose FS occurred within 24 hours （lasting shorter than single and twice durations and also shorter than 10 minutes） were regarded as SFS group; meanwhile, 17 children whose FS occurred within 24 hours （lasting longer than single and twice durations and also longer than 10 minutes） were regarded as CFS group. Another 23 patients who were admitted to our hospital with respiratory tract infection in the same period, without the history and positive symptoms/features of neurological dysfunction, were enrolled as control group. There were 13 boys and 10 girls aged from 5 months to 86 months. All parents were told the facts. METHODS： 2 mL venous blood was collected from FS children within 24 hours after the seizures. Meanwhile, 2 mL venous blood was also collected from children in the control group. Level of S-NSE was measured with enzyme immunoassay procedure, and differences among groups were compared with ttest. MAIN OUTCOME MEASURES： Level of S-NSE in each group RESULTS： A total of 49 children in FS group and 23 ones in control group were involved in the final analysis without any loss. Levels

Effects of erythropoietin and its receptor on nervous system

OBJECTIVE： To investigate the effects of erythropoietin （EPO） and its receptor （EPOR） on nervous system and its possible mechanism. DATA SOURCES ： By inputting the key words ＂erythropoietin, nervous system＂, we performed a search of Medline for English articles, which were published during September 1996 to August 2006, about EPO and EPOR in nervous system. STUDY SELECTION ： The materials were selected firstly, literatures were chosen for treatment group and control group and those obviously non-randomized studies were excluded. The full texts of the left literatures were searched. Inclusive criteria： ①Randomized controlled study. ②Experimental or clinical studies （parallel control group included）. ③Treatment group was recombinant human erythropoietin（rHuEPO）-treated group. Exclusive cdteda： repetitive study. DATA EXtractION： A number of 380 randomized or non-randomized articles about the effect of EPO on nervous system were collected, and 49 experiments or clinical trials met the inclusive criteria. Among 331 exclusive articles, 237 were non-randomized or repetitive studies and 94 were review articles. DATA SYNTHESIS ： Forty-nine experiments or clinical tnals confirmed that EPO and EPOR were expressed in the central nervous system （CNS） and peripheral nervous system（PNS） of gnawer, primate and human being; rHuEPO had obvious neuroprotective effects on brain hypoxia, brain ischemia, experimental intracranial hemorrhage, brain trauma, experimental autoimmune encephalomyelitis, human immunodeficiency virus （HIV）-related sensory neuropathy, distal axonopathy, experimental diabetic neuropathy and acute spinal injury models. Its mechanism maybe involve anti-excitatory toxicity, preventing the production of nitnc oxide （NO）, lessening inflammatory reaction, resisting apoptosis, maintaining vascular integrity, promoting angiogenesis, promoting the proliferation and differentiation of neural stem cells and progenitor cells and so on.
