BACKGROUND As a significantly important part of clinical practice,the professional nursing process can be advanced in many ways.Despite the fact that case reports are regarded to be of a lower quality grade in the hierarchy of evidence,one of the principles of evidence-based medicine is that decision-making should be based on a systematic summary of evidence.However,the evidence on the reporting characteristics of case reports in the nursing field is deficient.AIM To use the CARE guidelines to assess reporting quality and factors influencing the quality of case reports in the nursing field.METHODS Nursing science citation indexed(SCI-indexed)journals were identified from the professional website.Each of the identified journals was searched on their website for articles published before December 2017.Twenty-one sub-items on the CARE checklist were recorded as“YES”,“PARTLY”,or“NO”according to information reported by the included studies.The responses were assigned corresponding scores of 1,0.5,and 0,respectively.The overall score was the sum of the 21 sub-items and was defined as“high”(more than 15),“medium”(10.5 to 14.5),and“low”(less than 10).The means,standard deviations,odds ratios(OR),and the associated 95%confidence interval(CI)were determined using Stata 12.0 software.RESULTS Ultimately,184 case reports from 16 SCI-indexed journals were identified,with overall scores ranging from 6.5 to 18(mean=13.6±2.3).Of the included case reports,10.3%were regarded low-quality,52.7%were considered middle-quality,and 37%were regarded high-quality.There were statistical differences in the mean overall scores of the included case reports with funding versus those without funding(14.2±1.7 vs 13.6±2.4,respectively;P=0.4456)and journal impact factor<1.8 versus impact factor≥1.8(13.3±2.3 vs 13.6±2.4,respectively;P=0.4977).Five items from the CARE guidelines,5a(Patient),6(Clinical findings),8c(Diagnostic reasoning),9(Therapeutic intervention),and 11d(The main take-away lessons)were well-reported(Reporting rate more than 90%)in mo

Iatrogenic bile duct injuries during cholecystectomy can present as fulminant intra-abdominal sepsis which precludes immediate repair or biliary reconstruction.We report the case of a 29-year-old female patient who sustained a bile duct injury after an open cholecystectomy in a neighboring country.She presented to our institution 22 d after initial surgery with septic shock and multiple intra-abdominal collections.Endoscopic retrograde cholangiography revealed a large common hepatic duct defect corresponding to a Strasberg type D bile duct injury.Definitive reconstruction such as a hepaticojejunostomy cannot be performed due to the presence of dense adhesions with infected and friable tissues.She underwent a combination of endoscopic biliary stenting and pedicled omental patch repair of the bile duct to control bile leak and sepsis as a bridging procedure to definite hepaticojejunostomy three months later.

BACKGROUND The vein of Galen aneurismal malformations(VGAM) are rare arteriovenous malformations of the embryonic choroid plexus. They represent about 30% of paediatric neurovascular disorders and show diverse characteristics. The VGAM is constituted by a midline dilated venous structure that receives blood from abnormal macroscopic or microscopic arteriovenous shunting vessels. Two types of VGAM exist, the choroidal and the mural. The treatment represents a challenge with the therapeutic objective to preserve the normal brain development without creating new neurological deficits.CASE SUMMARY We present three cases of VGAM in the early postnatal period and their treatment. All patents were treated with the endovascular technique, which was successful. According to our experience, the endovascular technique is a safe and efficient mode of VGAM treatment.CONCLUSION The objective of treatment aims to child's normal neurological development. A proper selection of patients and a thorough diagnostic workup is of vital importance. When the endovascular treatment is performed, the primary aim is not a complete VGAM exclusion at one time, which could produce a sudden reversal of blood flow with consequent venous infarction and ischemia. The aim is therefore to occlude as much of the VGAM as needed to relieve the congestive cardiac failure, to gain time and to create the conditions for a normal maturation of the neurovascular system. With the use of endovascular techniques, which represent not only the first choice of treatment but also the only safe therapeutic modality, the natural history of VGAM and their risks may be avoided safely.

Severe alcoholic hepatitis(AH) has a high mortality,and it is associated with encephalopathy,acute renal failure,sepsis,gastrointestinal bleeding,and endotoxemia.The 28-d mortality remains poor(34%-40%),because no effective treatment has been established.Recently,corticosteroids(CS) have been considered effective for significantly improving the prognosis of those with AH,as it prevents the production of pro-inflammatory cytokines.However,CS are not always appropriate as an initial therapeutic option,such as in cases with an infection or resistance to CS.We describe a patient with severe AH complicated by a severe infection caused by the multidrug resistance bacteria(Pseudomonas aeruginosa),and was successfully treated with granulocytapheresis monotherapy without using CS.The experience of this case will provide understanding of the disease and information treating cases without using CS.

INTRODUCTION: Diabetic kidney disease(DKD) is one of the most common microvascular complications of diabetes mellitus and is the leading cause of end-stage renal disease. DKD seriously affects the quality of life of patients and brings heavy economic burden to the country. At present, the pathogenesis of DKD is not entirely clear, and clinical treatment is mainly to control blood glucose and lower blood pressure and urine protein. However,the clinical effect is not satisfactory.CASE PRESENTATION: A female patient, aged 72 years, first visited our hospital endocrinology clinic,and was diagnosed with DKD. She suffered from fatigue, cold sensation in the lower extremities, convulsion of the lower limbs and frequent urination at night. She had a dark purple tongue with white and yellow fur and sublingual varices. Accessory examinations showed that her fasting blood glucose was 5.7 mmol/L, serum creatinine 159 μmol/L,blood urea nitrogen 13.6 μmol/L, blood uric acid493 μmol/L, and blood pressure 138/65 mm Hg.The patient received Traditional Chinese Medicine treatment of the modified Shenzhuo formula[Huangqi(Radix Astragali Mongolici), Dahuang(Radix Et Rhizoma Rhei Palmati), Shuizhi(Hirudo) and Danshen(Radix Salviae Miltiorrhizae)] for 7 years and achieved good results.CONCLUSION: This case provides a specific treatment plan and an effective reference for clinical application of Traditional Chinese Medicine for treating DKD. This may be an alternative treatment for DKD.
