Eighty-one patients with suspected renovascular hypertension (RVH) were studied using 99m Tc-MAG3 renography before and after administration of 25 mg captopril (CAP). Among the 81 cases, 22 were proven by renal arteriography or DSA. The results showed that by using CAP, the sensitivity and specificity of renography in RVH diagnosis increased from 66% to 72% and from 33% to 100%, respectively. We conclude that renography is a simple, noninvasive . sensitive and specific method for screening RVH and is suitable for use in hospitals with no Y-cameras.

At the cellular level, reduced kidney perfusion in atherosclerotic renal arthery disease (ARVD), induces hypoxia, activation of the renin-angiotensin-aldosterone system (RAAS) and cytokine activation. Impaired blood flow in the kidneys creates a microenvironment triggering significant cytokine production, contributing to vascular damage and endothelial disfunction. Interactions between cytokines and endothelial, glomerular, and tubular cells often result in increased vessel permeability, and fibrosis, and contribute to the development of chronic kidney disease (CKD). Molecules such as endothelins, prostaglandins, and nitric oxide play a crucial role at the molecular level. The imbalance between vasoconstrictor and vasodilator factors contributes to vascular dysfunction. Oxidative stress and inflammatory processes at the cellular level contribute to endothelial damage and structural changes in blood vessels. Mineralocorticoid receptor antagonists (MRAs) therapy in the context of ARVD holds promise in reducing fibrosis, promoting angiogenesis and enhancing overall outcomes in patients with this pathology. Recent data also indicates the antioxidative, anti-inflammatory, and antifibrotic effects of SGLT2 inhibitors. They reduce oxidative stress caused by hypoxic conditions and enhance renal perfusion, contributing to the preservation of cellular function. Studies employing Blood Oxygen Level-Dependent (BOLD) imaging have identified adaptations to reduced blood flow, volume, and glomerular filtration rate in post-stenotic kidneys that preserve oxygenation in the medulla and cortex during medical therapy. Data from the literature indicate that despite the partial recovery of renal hypoxia and restoration of blood flow after revascularization, inflammatory cytokines and injury biomarkers remain elevated, and the glomerular filtration rate (GFR) does not recover in ARVD. Aim:The aim of the study was to investigate the central mechanism for modulation of chronic intermittent hypobaric hypoxia（CIHH）on baroreflex in renovascular hypertension（RVH）***:Male adult Sprague-Dawley rats were randomly divided into four groups:Sham-operated（Sham）group,renovascular hypertension(group,CIHH treatment（CIHH）group,RVH and CIHH treatment（RVH+CIHH）***-kidney-1-clip method was used to make RVH *** rats were accepted 28 days hypobaric hypoxia simulating 5000m altitude,6 hours daily in hypobaric ***+CIHH rats were treated with CIHH after *** renal sympathetic nerve activity（RSNA）,arterial blood pressure（BP）and heart rate（HR）were recorded under rats ***:*** in RVH+CIHH rats wassignificant lower than that in RVH rats,and there was no difference of BP between RVH+CIHH and Sham *** with Sham rats,the RSNA was increased significantly in RVH rats,but not in RVH+CIHH *** with rats in other groups,the baroreflex function curve was shifted down and left in RVH *** BP,HR and RSNA of all rats were decreased after microinjection of L–NAME,a inhibitor of nitric oxide（NO）synthase,in the nucleus of solitary tract（NTS）.The decrease of BP,HR and RSNA was lower in RVH rats than that in other *** BP and RSNA of rats were increased after microinjection of EDA/NO,a NO donor,in the *** increase of BP and RSNA was lower in RVH rats than that in other *** HR of the RVH rats were decreased after microinjection of EDA/NO in the NTS,but the HR of the other rats were increased after microinjection of EDA/NO in the ***:CIHH can antagonized the damage of baroreflex-modulated sympathetic outcome in RVH rats through increasing NO production in NTS,which might be a mechanism of CIHH anti-hypertensive effect.

From 1976 to 1981,9 cases of renovas,cular hypertension in children were treated *** etiology of renal artery steno sis in this series consisted of congenital abhor real renal artery(2 cases),fibromuscular dyspla sin(1 case)and Takayasu's arteritis(6 cases).The diagnosis was made by the aorto-reno gram in each case and was proved by the intervention and the pathological *** those 3 cases,the others were treated by nephrectomy,of which one case died from unknown cause one year after operation,6 cases had undergone the renal autotransplan *** authors reviewed 13 pediatric cases(including this series)reported in Chinese literature(＜12 years old),who were treated by renal autotransplantation with cure rate 84.6%(normotension on no medication).The authors indicated that the results of the renal autotrancplantation were much better than that of the revascularization techni ques with the following advantages：(1)the success rate was higher;(2)it provided excellent exposure for the vascular anastomosis in the iliac fossa;(3)the blood pressure in internal itiac artery was generally higher than that of the renal artery so that the blood pressure in transplanted kidney could be elevated after operation,and(4)it seemed more profitable in cases with renal artery stenosis by Takayasu's arteritis,because of no apparent stenosis：in the internal iliac arteries,which was observed from the arteriograms in the papers reported by others and us.

Objective To study the character of vascular morphological changes and the preventive effect of tetrandrine in renovas-cular hypertensive(RH) rats. Mehtods: renovascular hypertension was induced by two kidney one clip(2K1G) operation. Morphometumeasurements were performed in caudal artery, renal arterioles, and coronary arterioles. Results: The wet weight of aorta, caudalartery and femoral artery of RH rats(12 weeks after 2K1G operation) were greater than those of sham-operated rats. The parietalthickness, lumen caliber, and the ratio of parietal thickness to lumen caliber of abdomen aorta, caudal artery, coronary arterioles, and re-nal arterioles were significantly increased. There were no significant changes in the number of vascular smooth muscle cells(VSMC) inmost vessels, except the renal arterioles, where the number of VSMC was significantly increased.
