Pregnancy and the disease Recurrence of patients previously treated for differentiated thyroid cancer:A systematic review and meta analysis

Background:Differentiated thyroid cancer(DTC)is commonly diagnosed in women of child-bearing age,but whether pregnancy influences the prognosis of DTC remains controversial.This study aimed to summarize existing evidence regarding the association of pregnancy with Recurrence risk in patients previously treated for DTC.Methods:We searched PubMed,Embase,Web of Science,Cochrane,and Scopus based on the prespecified protocol registered at PROSPERO(CRD42022367896).After study selection,two researchers independently extracted data from the included studies.For quantitative data synthesis,we used random-effects meta-analysis models to pool the proportion of Recurrence(for pregnant women only)and odds ratio(OR;comparing the risk of Recurrence between the pregnancy group and the nonpregnancy group),respectively.Then we conducted subgroup analyses to explore whether risk of Recurrence differed by response to therapy status or duration of follow-up time.We also assessed quality of the included studies.Results:A total of ten studies were included.The sample size ranged from 8 to 235,with participants’age at pregnancy or delivery ranging from 28 to 35 years.The follow-up time varied from 0.1 to 36.0 years.The pooled proportion of Recurrence in all pregnant patients was 0.13(95%confidence intervals[CI]:0.06-0.25;I^(2):0.58).Among six included studies reporting response to therapy status before pregnancy,we observed a trend for increasingly higher risk of Recurrence from excellent,indeterminate,and biochemically incomplete to structurally incomplete response to therapy(P trendRecurrence in the pregnancy group showed no evidence of a significant difference from that in the nonpregnancy group(OR:0.75;95%CI:0.45-1.23;I^(2):0).The difference in follow-up time(below/above five years)was not associated with either the proportion of Recurrence in all pregnant patients(P>0.05)or the OR of Recurrence in studies with a comparison group(P>0.05).Two included studies that focused on patients with distant metastasis

Macular corneal dystrophy(MCD)is a progressive,bilateral stromal dystrophic disease that arises from mutations in carbohydrate sulfotransferase 6(CHST6).Corneal transplantation is the ultimate therapeutic solution for MCD patients.Unfortunately,postoperative Recurrence remains a significant challenge.We conducted a retrospective review of a clinical cohort comprising 102 MCD patients with 124 eyes that underwent either penetrating keratoplasty(PKP)or deep anterior lamellar keratoplasty(DALK).Our results revealed that the Recurrence rate was nearly three times higher in the DALK group(39.13%,9/23 eyes)compared with the PKP group(10.89%,11/101 eyes),suggesting that surgical replacement of the corneal endothelium for treating MCD is advisable to prevent postoperative Recurrence.Our experimental data confirmed the robust m RNA and protein expression of CHST6 in human corneal endothelium and the rodent homolog CHST5 in mouse endothelium.Selective knockdown of wild-type Chst5 in mouse corneal endothelium(AC^(siChst5)),but not in the corneal stroma,induced experimental MCD with similar extracellular matrix synthesis impairments and corneal thinning as observed in MCD patients.Mice carrying Chst5 point mutation also recapitulated clinical phenotypes of MCD,along with corneal endothelial abnormalities.Intracameral injection of wild-type Chst5 rescued the corneal impairments in AC^(siChst5)mice and retarded the disease progression in Chst5 mutant mice.Overall,our study provides new mechanistic insights and therapeutic approaches for MCD treatment by highlighting the role of corneal endothelium in MCD development.

Development and validation of a nomogram model for predicting the risk of gallstone Recurrence after gallbladder-preserving surgery

Background:The high incidence of gallstone Recurrence was a major concern for laparoscopic gallbladderpreserving surgery.This study aimed to investigate the risk factors for gallstone Recurrence after gallbladder-preserving surgery and to establish an individualized nomogram model to predict the risk of gallstone Recurrence.Methods:The clinicopathological and follow-up data of 183 patients who were initially diagnosed with gallstones and treated with gallbladder-preserving surgery at our hospital from January 2012 to January 2019 were retrospectively collected.The independent predictive factors for gallstone Recurrence following gallbladder-preserving surgery were identified by multivariate logistic regression analysis.A nomogram model for the prediction of gallstone Recurrence was constructed based on the selected variables.The C-index,receiver operating characteristic(ROC)curve and calibration curve were used to evaluate the predictive power of the nomogram model for gallstone Recurrence.Results:During the follow-up period,a total of 65 patients experienced gallstone Recurrence,and the Recurrence rate was 35.5%.Multivariate logistic regression analysis revealed that the course of gallstones>2 years[odds ratio(OR)=2.567,95%confidence interval(CI):1.270-5.187,P=0.009],symptomatic gallstones(OR=2.589,95%CI:1.059-6.329,P=0.037),multiple gallstones(OR=2.436,95%CI:1.133-5.237,P=0.023),history of acute cholecystitis(OR=2.778,95%CI:1.178-6.549,P=0.020)and a greasy diet(OR=2.319,95%CI:1.186-4.535,P=0.014)were independent risk factors for gallstone Recurrence after gallbladder-preserving surgery.A nomogram model for predicting the Recurrence of gallstones was established based on the above five variables.The results showed that the C-index of the nomogram model was 0.692,suggesting it was valuable to predict gallstone Recurrence.Moreover,the calibration curve showed good consistency between the predicted probability and actual probability.Conclusions:The nomogram model for the prediction of gallstone Recurrence might hel

Objective:To evaluate the efficacy and safety of Cidan Capsule combined with adjuvant transarterial chemoembolization(TACE) in patients with a high risk of early Recurrence after curative resection of hepatocellular carcinoma(HCC).Methods:A multicenter,randomized controlled trial was conducted in patients with high-risk Recurrence factors after curative resection of HCC from 9 medical centers between July 2014 and July 2018.Totally249 patients were randomly assigned to TACE with or without Cidan Capsule administration groups by stratified block in a 1:1 ratio.Postoperative adjuvant TACE was given 4-5 weeks after hepatic resection in both groups.Additionally,125 patients in the TACE plus Cidan group were administrated Cidan Capsule(0.27 g/capsule,5 capsules every time,4 times a day) for 6 months with a 24-month follow-up.Primary endpoints included diseasefree survival(DFS) and tumor Recurrence rate(TRR).Secondary endpoint was overall survival(OS).Any drugrelated adverse events(AEs) were observed and recorded.Results:As the data cutoff on July 9th,2018,the median DFS was not reached in the TACE plus Cidan group and 234.0 days in the TACE group(hazard ratio,0.420,95%confidence interval,0.290-0.608;PRecurrence in HCC patients at high-risk of Recurrence after radical hepatectomy and may be an appropriate option in postoperative anti-Recurrence treatment.(Registration No.NCT02253511)

The high risk of postoperative mortality in lung adenocarcinoma(LUAD)patients is principally driven by cancer Recurrence and low response rates to adjuvant treatment.Here,A combined cohort containing 1,026 stageⅠ-Ⅲpatients was divided into the learning(n Z 678)and validation datasets(n Z 348).The former was used to establish a 16-mRNA risk signature for Recurrence prediction with multiple statistical algorithms,which was verified in the valida-tion set.Univariate and multivariate analyses confirmed it as an independent indicator for both Recurrence-free survival(RFS)and overall survival(OS).Distinct molecular characteristics between the two groups including genomic alterations,and hallmark pathways were compre-hensively analyzed.Remarkably,the classifier was tightly linked to immune infiltrations,high-lighting the critical role of immune surveillance in prolonging survival for LUAD.Moreover,the classifier was a valuable predictor for therapeutic responses in patients,and the low-risk group was more likely to yield clinical benefits from immunotherapy.A transcription factor regulato-ry proteineprotein interaction network(TF-PPI-network)was constructed via weighted gene co-expression network analysis(WGCNA)concerning the hub genes of the signature.The con-structed multidimensional nomogram dramatically increased the predictive accuracy.There-fore,our signature provides a forceful basis for individualized LUAD management with promising potential implications.

BACKGROUND Indentifying predictive factors for postoperative Recurrence of hepatocellular carcinoma(HCC)has great significance for patient prognosis.AIM To explore the value of gadolinium ethoxybenzyl diethylenetriamine pentaacetic acid(Gd-EOB-DTPA)enhanced magnetic resonance imaging(MRI)combined with clinical features in predicting early Recurrence of HCC after resection.METHODS A total of 161 patients with pathologically confirmed HCC were enrolled.The patients were divided into early Recurrence and non-early Recurrence group based on the follow-up results.The clinical,laboratory,pathological results and Gd-EOB-DTPA enhanced MRI imaging features were analyzed.RESULTS Of 161 patients,73 had early Recurrence and 88 were had non-early Recurrence.Univariate analysis showed that patient age,gender,serum alpha-fetoprotein level,the Barcelona Clinic Liver Cancer stage,China liver cancer(CNLC)stage,microvascular invasion(MVI),pathological satellite focus,tumor size,tumor number,tumor boundary,tumor capsule,intratumoral necrosis,portal vein tumor thrombus,large vessel invasion,nonperipheral washout,peritumoral enhancement,hepatobiliary phase(HBP)/tumor signal intensity(SI)/peritumoral SI,HBP peritumoral low signal and peritumoral delay enhancement were significantly associated with early Recurrence of HCC after operation.Multivariate logistic regression analysis showed that patient age,MVI,CNLC stage,tumor boundary and large vessel invasion were independent predictive factors.External data validation indicated that the area under the curve of the combined predictors was 0.861,suggesting that multivariate logistic regression was a reasonable predictive model for early Recurrence of HCC.CONCLUSION Gd-EOB-DTPA enhanced MRI combined with clinical features would help predicting the early Recurrence of HCC after operation.

Assessing recent Recurrence after hepatectomy for hepatitis Brelated hepatocellular carcinoma by a predictive model based on sarcopenia

BACKGROUND Sarcopenia may be associated with hepatocellular carcinoma(HCC)following hepatectomy.But traditional single clinical variables are still insufficient to predict Recurrence.We still lack effective prediction models for recent Recurrence(time to RecurrenceRecurrence-free survival(RFS)after hepatectomy for HCC based on sarcopenia.METHODS We retrospectively analyzed 283 hepatitis B-related HCC patients who underwent curative hepatectomy for the first time,and the skeletal muscle index at the third lumbar spine was measured by preoperative computed tomography.94 of these patients were enrolled for external validation.Cox multivariate analysis was per-formed to identify the risk factors of postoperative Recurrence in training cohort.A nomogram model was developed to predict the RFS of HCC patients,and its predictive performance was validated.The predictive efficacy of this model was evaluated using the receiver operating characteristic curve.RESULTS Multivariate analysis showed that sarcopenia[Hazard ratio(HR)=1.767,95%CI:1.166-2.678,P5 cm(HR=2.222,95%CI:1.285-3.842,PRecurrence of HCC.Based on the sarcopenia to assess the RFS model of hepatectomy with hepatitis B-related liver cancer disease(SAMD)was established combined with other the above risk factors.The area under the curve of the SAMD model was 0.782(95%CI:0.705-0.858)in the training cohort(sensitivity 81%,specificity 63%)and 0.773(95%CI:0.707-0.838)in the validation cohort.Besides,a SAMD score≥110 was better to distinguish the high-risk group of postoperative Recurrence of HCC.CONCLUSION Sarcopenia is associated with recent Recurrence after hepatectomy for hepatitis B-related HCC.A nutritional status-based prediction m

Background:Hepatectomy for hepatocellular carcinoma(HCC)beyond the Milan criteria is shown to be beneficial.However,a high rate of post-operative HCC Recurrence hinders the long-term survival of the patients.This study aimed to investigate and compare the impacts of tenofovir(TDF)and entecavir(ETV)on the Recurrence of hepatitis B viral(HBV)-related HCC beyond the Milan criteria.Methods:Data pertaining to 1532 patients who underwent hepatectomy and received antiviral therapy between January 2014 and January 2019 were collected from five centers.Recurrence-free survival(RFS)analysis was performed using the Kaplan-Meier method.Cox proportional hazards regression analysis was performed to determine prognostic factors for HCC Recurrence.Results:The analysis incorporates 595 HBV-related HCC patients.The overall 5-year RFS was 21.3%.Among them,533 and 62 patients received ETV and TDF treatment,respectively.The 1-,3-,and 5-year RFS rates were 46.3%,27.4%,and 19.6%,respectively,in the ETV group compared with 65.1%,41.8%,and 37.2%,respectively,in the TDF group(PRecurrence after hepatectomy.Compared to the ETV therapy,TDF administration significantly lowered the risk of HCC Recurrence.

Objective:To delineate the onset and Recurrence characteristics of noncardiogenic ischemic stroke patients in China.Methods:A prospective,multicenter and registry study was carried out in 2,558 patients at 7 representative clinical sub-centers during November 3,2016 to February 17,2019.A questionnaire was used to collect information of patients regarding CM syndromes and constitutions and associated risk factors.Additionally,stroke Recurrence was defined as a primary outcome indicator.Results:A total of 327(12.78%)patients endured Recurrence events,1,681(65.72%)patients were men,and the average age was 63.33±9.45 years.Totally 1,741(68.06%)patients suffered first-ever ischemic stroke,1,772(69.27%)patients reported to have hypertension,and1,640(64.11%)of them reported dyslipidemia,1,595(62.35%)patients exhibited small-artery occlusion by The Trial of Org 10172 in Acute Stroke Treatment(TOAST)classification.Specifically,1,271(49.69%)patients were considered as qi-deficient constitution,and 1,227(47.97%)patients were determined as stagnant blood constitution.There were 1,303(50.94%)patients diagnosed as blood stasis syndrome,1,280(50.04%)patients exhibited phlegm and dampness syndrome and 1,012(39.56%)patients demonstrated qi deficiency syndrome.And 1,033(40.38%)patients declared intracranial artery stenosis,and 478(18.69%)patients reported carotid artery stenosis.The plaque in 1,508(41.36%)patients were of mixed.Particularly,41.09%of them demonstrated abnormal levels of glycated hemoglobin(HbA1 c)levels.Conclusions:Recurrence in minor and small-artery stroke cannot be ignored.Hypertension,dyslipidemia,abnormal HbA1 c,intracranial artery stenosis and carotid plaque were more common in stroke patients.Particularly,phlegm-dampness and blood stasis syndromes,as well as qi deficiency and blood stasis constitutions,were still the main manifestations of stroke.(Trial registration at ClinicalTrials.gov,No.NCT03174535)

Background:Gestational diabetes mellitus(GDM)brings health issues for both mothers and offspring,and GDM prevention is as important as GDM management.It was shown that a history of GDM was significantly associated with a higher maternal risk for GDM Recurrence.The incidence of GDM Recurrence was unclear because of the incidence of second-child was low before 2016 in China.We aim to investigate the prevalence of GDM Recurrence and its associated high-risk factors which may be useful for the prediction of GDM Recurrence in China.Methods:A retrospective study was conducted which enrolled participants who underwent regular prenatal examination and delivered twice in the same hospital of 18 research centers.All participants were enrolled from January 2018 to October 2018,where they delivered the second baby during this period.A total of 6204 women were enrolled in this study,and 1002 women with a history of GDM were analyzed further.All participants enrolled in the study had an oral glucose tolerance test(OGTT)result at 24 to 28 weeks and were diagnosed as GDM in the first pregnancy according to the OGTT value(when any one of the following values is met or exceeded to the 75-g OGTT:0 h[fasting],≥5.10 mmol/L;1 h,≥10.00 mmol/L;and 2 h,≥8.50 mmol/L).The prevalence of GDM Recurrence and development of type 2 diabetes mellitus were calculated,and its related risk factors were analyzed.Results:In 6204 participants,there are 1002 women(1002/6204,16.15%)with a history of GDM and 5202 women(5202/6204,83.85%)without a history of GDM.There are significant differences in age(32.43±4.03 years vs.33.00±3.34 years vs.32.19±3.37 years,PRecurrence of GDM,and normal women.Moreover,it seems so important in the degree of abnormal glucose
