OBJECTIVE To evaluate the Quality of Life of the patients who received a total gastrectomy with different kinds of reconstruction methods. METHODS Patients who received a total gastrectomy between May 1999 to May 2003 were followed-up via questionnaires. Fifty-nine who were alive more than 2 years with no sigh of recurrence were enrolled in this study. The patients were divided into 2 groups as follows： a group treated with reconstruction with a duodenal passage after total gastrectomy （gastric substitute, GS）; and a group receiving reconstruction without duodenal passage after total gastrectomy （Jejenal pouch, JP）. Follow-up feedback data of 14 items from the patients were analyzed, comparing the Quality of Life （QOL） between the 2 groups. RESULTS Six months after operation, the most common symptoms of all patients were reflux and loss of body weight, but there was no statistically significant differences in the 14 items related to the special post-operation symptoms between the 2 groups. At 12 months after the operations, there were significant differences between the 2 groups in body weight （P=0.01）, eating time （P=0.034〈0.05）, and frequency of food intake （P= 0.040〈0.05）; At 24 months after the operations, the only difference between the 2 groups was body weight gain （P=0.025〈0.05）. The JP group was better than the GS group. CONCLUSION The JP reconstruction pattern is superior to a simple GS in gain of body weight, volume of food intake and frequency of food intake, soon after the operation.

Vegetation succession and environmental changes in Central India during Early Cenozoic

In order to infer the climate and the floral changes during the early Cenozoic of central India, a critical analysis has been made of the generic composition of Deccan Intertrappean series of India, which is now known to a greater extent. Most of the information regarding Deccan Intertrappean flora is derived from the assemblages from Vidharbha-Chhindwara region and Mandla District in central India. The Vidharbha-Chhindwara region constituted by taxa representing all major groups of plant kingdom belonging to different habitats like(i) marine(3 genera of doubtful marine alga)(ii) estuarine(27 species of palms, Nypa and Viracorpon like Pandanaceous fruits, Sonneratia, Cocos like plants)(iii) fresh water(6 genera of fresh water algae, water ferns, Eichhornia, Nymphea like hydrophytic angiosperms)(iv) marshy(5 genera of bryophytes, water ferns, Selaginella and Equisetum like pteridophytes and many angiospermic plants showing aerenchymatous tissue and few parts infected by fungal spores) and(v) terrestrial(2 families of gymnosperms and 15 families of arborescent angiosperms). Mandla-Dindori District mainly comprise angiospermous flora. These constitute 15 species of palm woods and woods of 25 dicot families indicating terrestrial dry habitat. The fossil flora so far recovered from these beds indicate that the tropical evergreen to semi-evergreen type forests with some moist deciduous plants, similar to the present day forests of Western Ghats, and northeast India, were flourshing around Vidharbha, Chhindwara and Mandla area of Central India during Upper Cretaceous-Lower Tertiary period, in contrast to the present day dry deciduous to moist deciduous types of forest. This flora was well represented in India in the past but has become restricted to Burma and Andaman-Nicobar Islands due to change in the climatic conditions. Background: Periodontal disease is a bacterial infection that causes bone resorption of bone supporting teeth and leads to change in the normal architecture of the alveolar process. There are instances where the technique sensitive nature of restorative procedures or the faulty restorative margins may inadvertently lead to conditions which could bring about periodontal disease/ destruction. Therefore, this study was designed to determine the impact of dental restorations type on marginal bone among some patients being treated for chronic periodontitis. Material and Methods: Three hundred patients from the periodontics clinics in college of dentistry, King Khalid university were participated in our study. A total of 292 patients completed the study, 152 males and 140 female. They were divided according to dental reconstructions into three groups: Group I was without dental reconstructions (control group), group II patients received amalgam class II fillings and group III received fixed bridge denture. Plaque index (PLI) gingival index (GI), clinical attachment loss (CAL) and marginal bone loss (MBL) were recorded. All data were collected and were analyzed by ANOVA test. Results: In the present study, the restored teeth revealed significantly higher mean values for PLI, GI, CAL and MBL than the non-restored teeth (p-value Conclusion: Although the limitations of the present study, the patients in group II had the highest clinical attachment loss and value of marginal bone loss which can explain the more extension of amalgam fillings into subgingival direction, leading to increased plaque accumulation and increased periodontal destruction.

Significance of an Advanced Image-Based Virtual Monoenergetic Reconstruction of Dual Source Dual-Energy CT Data at Low keV Increases Image Quality for Portal Vein System of Pancreatic Cancer Patients

Purpose: To explore the significance of dual-source computed tomography (DECT) virtual monoenergetic reconstructions technology in improving image quality for portal vein system of pancreatic cancer patients. Materials and methods: 47 patients with clinically suspected pancreatic cancer (all confirmed by pathology) were collected. Routine plain scan was performed with Siemens Force dual-source dual-energy CT followed by 3 scans respectively carried out in arterial phase, portal phase and delayed phase. Traditional virtual monoenergetic reconstructions (Mono_E) and new generation of virtual monoenergetic reconstructions (Mono+) were respectively performed on portal vein images to obtain virtual single energy images including Mono_ E70 keV, Mono_E 55 keV and Mono+ 70 keV and Mono+ 55 keV. The signal-to-noise ratio (SNR) and noise of portal vein, normal pancreatic tissues and pancreatic lesions of 100 kV, Mono_E and Mono+ images were compared. In addition, the contrast noise ratio of portal vein and lesions as well as pancreatic tissues and lesions (CNR PV, CNRtumor) were also compared. At the same time, two imaging physicians with rich clinical experiences read the films and scored the images of each group by using the 5-point scoring method. Results: Mono+ 55 keV images including SNRpv, SNRpanc, SNRtumor, Noise, CNRpv, CNRtumor were statistically different from 100 KV images and Mono_E images (P < 0.05). As for the subjective score, Mono+ 55 keV image score also had the highest score, which had statistical significance (P < 0.05). The results showed that Mono+ 55 keV images had the best quality. Conclusion: The new generation of virtual Mono+ post-treatment can reduce image noise. Low energy Mono+ images can improve the contrast between pancreatic cancer lesions and portal of pancreatic cancer patients.

In this work a complete approach for estimation of the spatial resolution for the gamma camera imaging based on the [1] is analyzed considering where the body distance is detected (close or far way). The organ of interest most of the times is not well defined, so in that case it is appropriate to use elliptical camera detection instead of circular. The image reconstruction is presented which allows spatially varying amounts of local smoothing. An inhomogeneous Markov random field (M.r.f.) model is described which allows spatially varying degrees of smoothing in the reconstructions and a re-parameterization is proposed which implicitly introduces a local correlation structure in the smoothing parameters using a modified maximum likelihood estimation (MLE) denoted as one step late (OSL) introduced by [2].

Materials and methods: 68 patients with clinically suspected esophageal cancer (all confirmed by pathology) were collected. Routine plain scan was performed with SIEMENS Force dual-energy CT and then dual-phase scans were performed. The venous phase images were respectively subjected to traditional virtual monoenergetic reconstructions (Mono_E) and new generation virtual monoenergetic reconstructions (Mono+). Mono_E 55 keV and Mono+ 55 keV virtual single-energy images were obtained respectively. The signal-to-noise ratio (SNR) of normal esophageal tissue and esophageal cancer lesions, noise and contrast noise ratio (CNR tumor) of normal esophageal tissue and esophageal cancer lesions were compared among 100 kV images, Mono_E images and Mono+ images. At the same time, two imaging physicians read the films and scored the images of each group by using a 5-point scoring method. Results: Mono+ 55 keV images, SNR, SNRtumor, noise and CNRtumor were statistically different from those of 100 kV images and Mono_E images (P reconstructions post-processing (Mono+) could reduce image noise and improve the contrast between esophageal cancer lesions and normal esophageal tissues, which was of great significance to improve the imaging quality of esophageal cancer patients and improve the early detection rate of esophageal cancer.
