Dsteosynthesis with long volar locking plates for meta- physeal-diaphyseal fractures of the distal radius

Objective：Metaphyseal-diaphyseal fractures of the distal radius are a major treatment dilemma and orthopaedic surgeons have to pay due consideration to restoration of anatomy of distal radius together with rotation of the radial shaft and maintenance of radial bow and interosseous *** performed this study to evaluate the clinic-radiological outcome of metaphyseal-diaphyseal fractures of the distal radius treated with long volar locking ***：This prospective study involved 27 patients （22 males and 5 females） with metaphyseal-diaphyseal fracture of the distal *** mean age was （30.12±11.48） years （range 19-52 years） and the follow-up was 26.8 months （range 22-34 months）.All patients underwent open reduction and internal fixation with a long volar locking *** to AO/OTA classification,there were 7 type A3,13 type C2 and 7 type C3 *** assessment was done based on the disabilities of the arm,shoulder and hand （DASH） *** evaluation was done by measuring grip strength and range of motion around the wrist and the radiological determinants included radial angle,radial length,volar angle and ulnar *** final assessment was done according to Gartland and Werley scoring ***：Postoperative radiological parameters were well maintained throughout the trial,and there was significant improvement in the functional parameters from 6 weeks to final *** average DASH scores improved from 37.5 at 6 weeks to 4.2 at final *** assessment using Gartland and Werley scoring system revealed 66.67%（n=l8） excellent and 33.33% （n=9） good *** was one case of superficial infection which responded to antibiotics and another carpel tunnel syndrome which was managed ***：Volar locking plate fixation for metaphyseal-diaphyseal fractures of distal radius is associated with excellent to good functional outcome,early rehabilitation and minimal complications.

Influence of volar margin of the lunate fossa fragment fixation on distal radius fracture outcomes:A retrospective series

BACKGROUND Distal radius fractures accompanied by the volar margin of the lunate fossa(VMLF)lesions are often overlooked or inadequately reduced in clinical *** To investigate the impact of VMLF fragment in distal radius fractures on the stability and function of the wrist *** This was a retrospective study of patients with distal radius fractures who underwent surgical treatment between January 2013 and December *** patients were divided into two groups according to whether the VMLF fragments were fixed or not.X-rays and computed tomography were performed before surgery,immediately postoperatively,and at 1,3,and 6 mo to measure the scapholunate angle,radiolunate angle,capitolunate angle,and effective radiolunate flexion(ERLF).The Mayo wrist score and disabilities of the arm,shoulder,and hand(DASH)score were determined at 1 *** Thirty-five patients were *** were 15 males and 20 *** mean age was 52.5±14.3(range:19-70)*** were 38 wrists(17 on the left side,15 on the right,and three bilateral;16 in the fixed group,and 22 in the unfixed group).The interval between trauma and surgery was from 1 h to 1 *** incidence of postoperative wrist instability in the unfixed group(86.4%)was higher than in the fixed group(25.0%)(P≤0.001).Ten patients had ERLF>25°in the unfixed group and none in the fixed group(P=0.019).The Mayo wrist score was 94±5.7 in the fixed group and 68±15.1 in the unfixed group(Pfractures,might damage the radial-lunar ligament,leading to postoperative wrist instability,sagittal force line imbalance,and poor recovery of wrist joint function.

Three-dimensional analysis of the gap space under forearm casts

Purpose:Secondary displacement represents a frequent complication of conservative treatment of fractures,particularly of the distal *** gap space between skin and cast may lead to a certain degree movements and this increased mobility might favor *** aim of this study was to develop a new 3D method,to measure the gap space in all 3 geometrical planes,and to validate this new technique in a clinical setting of distal radius ***:This study applies 3D imaging to measure the space between plaster and skin as a potential factor of secondary displacement and therefore the failure of conservative *** developed and validated a new methodology to analyze and compare different forearm casts made of plaster of Paris and *** unpaired t-test was performed to document differences between the investigated parameters between plaster of Paris and fiberglass *** significance level was set at p<***:In a series of 15 cases,we found the width of the gap space to average 4 mm,being slightly inferior on the radial *** the two different casting materials,plaster of Paris and fiberglass,we found a significantly larger variance of space under casts made of the first material(p=0.39).A roughness analysis showed also a markedly significantly higher irregularity of the undersurface of plaster of Paris as compared with ***:This study allows for a better understanding of the nature of the"gap space"between cast and skin and will contribute to develop and improve new immobilization techniques and materials.

Palmar approach with Kirschner-wire fixation in the treatment of children＇s distal radius extension type fracture

Purpose： To explore the advantages of palmar approach with Kirschner-wire （K-wire） fixation in the treatment of children＇s distal radius extension type fracture, Methods： Thirty patients, average age of 8.5 years ranging from 5 to 13 years, with distal radius extension type fracture and undergoing a failed manual reposition in our hospital were included, and treated by palmar approach with K-wire fixation between May 2014 and December 2017. Among these patients （21 male and 9 female）, 5 patients had chronic injuries over 10 days, and 6 patients had fracture of the distal radius epiphysis. The time between injury and treatment ranged from 1 to 30 days. Among them, 11 patients with right-sided fractures and 19 patients with left-sided fractures were operated via the palmar longitudinal incision approach. Results： The results were evaluated after an average of 18 months ranging from 5 to 36 months after operation. The recovery time of fracture was from 4 to 8 weeks and all incisions were primary healing with an average of 6 weeks. Nonunion, delayed union, early closure of distal radial epiphysis, and wrist varus/valgus deformity were not found in all the cases. Based on Gartland and Wereley wrist score assessment undertaken three months after operation, excellent scores were achieved in 24 cases, good scores in 3 cases, acceptable scores in 3 cases. Conclusion： The palmar approach with K-wire fixation via a front longitudinal incision in the treatment of children＇s distal radius extension type fracture has following advantages：（1） easy to reposition for both fresh and old fractures; （2） less damage to surrounding tissues and epiphysis; （3） quick recovery. It is suitable to treat children＇s distal radius extension type fracture.

Isolated trochlea fracture in adults is a rare surgical entity as compared to its capitellar *** has been only mentioned sporadically in the literature as case *** of the trochlea is accompanied by other elbow injuries like elbow dislocation,capitellum fracture,ulnar fracture and extraarticular condylar *** we report a unique case of isolated displaced trochlea fracture associated with fractures of the lateral end clavicle and the distal end *** propose a unique mechanism for this rare combination of injuries：typical triad of injury,*** of the distal end radius with trochlea and fracture of the lateral end of the *** treatment is recommended for undisplaced humeral trochlea fractures;but for displaced ones,anatomical reduction and internal fixation are essential to maintain the congruous trochleacoronoid articulation and hence to maintain the intrinsic stability of the elbow.

A Galeazzi-variant type fracture-dislocation in adults

Objective：Fracture of either radius or ulna with a dislocation either at the proximal or distal radioulnar joint （DRUJ） is not a common injury and is inherently *** we report a case series,with both-bone forearm fractures associated with dislocation of DRUJ,as a Galeazzi-variant type fracture-dislocation,and try to analyze this injury ***：The study was based on 6 patients having Galeazzi-variant type fracture-dislocation of different age （20 to 45 years）.All fractures were closed *** fractures involved the same level and three fractures were at different levels of radius and ulna *** thorough examination and investigations they were treated with limited contact dynamic compression plate without additional fixation for ***：All cases were followed up for 24 *** maximum incidence occurred in age group between 31 and 40 *** the fractures of both radius and ulna were united in average time of 12 *** of motion of wrist and elbow,supination and pronation at final follow-up were *** was no subsequent re-subluxation or dislocation of the DRUJ in any of the ***：Galeazzi variant in adult is a new undescribed pattern of forearm with wrist *** open reduction and internal fixation of both-bone forearm fractures is mandatory,followed by 3 to 4 weeks of immobilization in a cast for the healing of disrupted DRUJ.

Treatment of displaced radial head fractures by internal fixation with absorbable pins

Objective： To study the effect of internal fixation with absorbable pins on treatment of displaced radial head fractures. Methods： From May 1999 to May 2004, 16 patients with displaced radial head fractures （Mason types Ⅱ and Ⅲ） were treated with internal fixation by absorbable pins. The duration of follow-up averaged 22.6 months （12-58 months）. The outcome was assessed on the basis of elbow motion, radiographic findings and the functional rating score delineated by Broberg and Morrey. Results： All fractures healed within 10 months without avascular necrosis of radial head. The mean elbow flexion loss was 15°（0°-35°） , and pronation and supination decreased by 10°（ 0°-30°） on average compared with those of the contralateral elbow. Five patients had an excellent result, 6 a good result, and 3 a fair result according to the criteria of Borberg and Morrey. Conclusions： Internal fixation with absorbable pins is an effective method in treating displaced radial head fractures. It can maintain the biomechanical stability of forearm, improve the elbow function and avoid second operation.

Palmar plate fixation is becoming a well recognized treatment for distal radial fracture. Tendon ruptures or irritations have been reported after this procedure. Inadvertent retention of angled drill guides useful for easy placement of locking screws in proper direction in volar plating can represent a high risk of secondary tendon rupture. In this case, we presented our experience in one patient in whom drill guides removal could be delayed for one year and summarized our techniques to prevent the aforementioned complications.

PURPOSE: To observe the effects of acupuncture on the pituitary-thyroid axis in rabbits with fracture. METHOD: 36 male rabbits with a 3-mm width experimental defect in the middle segment of the left radius were randomly divided into the treatment group and control group, and the changes in serum T3, T4 and TSH contents in different weeks were investigated.
