Cerebral venous sinus thrombosis following transsphenoidal surgery for craniopharyngioma:A case REPORT

BACKGROUND Cerebral venous sinus thrombosis(CVST)is a rare condition in patients with craniopharyngioma following transsphenoidal surgery.CASE SUMMARY A 56-year-old man who underwent transsphenoidal surgery for craniopharyngioma 26 d ago presented gradual headache and cerebrospinal fluid leakage while vomiting 5 d post-discharge and required readmission to our department of neurosurgery.After admission,head imaging examination showed a hyperdense shadow in the superior sagittal sinus and right transverse sinus,edema at the bilateral parietal lobe,and hemorrhage at the left parietal lobe and right occipital lobe;the venous phase of cerebral angiography revealed CVST.The patient was treated immediately by intravenous thrombolysis,endovascular thrombolysis,and mechanical thrombectomy after the definite diagnosis.However,the neurological status of the patient continued to deteriorate and he died on the fourth day after readmission.CONCLUSION For craniopharyngioma undergoing transsphenoidal surgery,it is vital to take an effective strategy to manage the postoperative complications,such as diabetes insipidus,severe electrolyte imbalance,and cerebrospinal fluid leakage.Additionally,the early differential diagnosis of CVST is essential when it develops clinical symptoms,especially in patients following transsphenoidal surgery with a high risk of CVST.Subsequently,the timely and effective treatment of the CVST is critical for preventing neurological deterioration.

BACKGROUND Spinal intraosseous schwannomas(SIS)are rare,and as yet have not been fully described in the literature.The first case of SIS was REPORTed in 1971,and 24 cases of SIS have been REPORTed so far.However,including the present case,there are only seven cases without spinal canal and neuroforamina involvement.CASE SUMMARY A 56-year-old man presented with a history of neck pain for 2 years.An obvious osteolytic destruction of the seventh cervical(C7)vertebra was observed on imaging examination.Magnetic resonance imaging of the cervical spine showed space-occupying lesions in the C7 vertebra,and destruction of the anterior cortex of the vertebra.The lesions had an exophytic component that extended from the C7 vertebra into the soft tissue on the front side.The foramen transversarium on both sides were intact.The patient underwent surgical biopsy and focal excision of the C7 lesion.The diagnosis of“schwannoma”was verified by postoperative pathological examinations.In a review of the literature,this is the seventh case of SIS without spinal canal and neuroforamina involvement,and the third REPORTed case of type VIII SIS.We discussed our case with respect to REPORTed classification characteristics of SIS.CONCLUSION SIS is a very rare tumor.We REPORT a rare case that may be important for further classification of osteo-schwannoma.The establishment of a complete disease classification is of high importance for the treatment and prognosis of this disease.Thus,more basic studies and retrospective analysis of related cases are necessary.

BACKGROUND Lymphoepithelioma-like carcinoma(LELC)is a non-keratinizing carcinoma with rich lymphocytic infiltration,which primarily originates from the nasopharynx.Primary lung LELC is a type of lung cancer with a relatively low incidence.Herein,we REPORT a rare case of lung LELC with expression of CD56.We also performed a literature review to summarize the epidemiological,clinical,and prognostic features of this disease.CASE SUMMARY A 51-year-old man was admitted to Cancer Hospital,Chinese Academy of Medical Sciences and Peking Union Medical College due to cough and chest pain lasting>2 mo and 1 wk,respectively.Positron emission tomography-computed tomography and magnetic resonance imaging examinations revealed the presence of a mass in the right upper lobe with enlargement of lymph nodes and multiple bone metastases.According to the results of bronchoscopy and cervical lymph node biopsy,a diagnosis of lung LELC with CD56-positive staining(CD56+lung LELC)was made.In the literature,458 cases of lung LELC have been REPORTed.However,only one other case of CD56+lung LELC has been REPORTed thus far.CONCLUSION The mechanism and potential role of CD56 expression in CD56+lung LELC require further investigation.

BACKGROUND Systemic sclerosis is a rare connective tissue disease characterized by localized ordiffuse skin thickening and fibrosis,which usually accumulates in various organsthroughout the body.Tachyarrhythmia is a common clinical manifestation ofcardiovascular damage in systemic sclerosis patients.However,few studies haveREPORTed the use of catheter ablation and an implantable cardioverter defibrillatorin patients with systemic sclerosis complicated by ventricular tachycardia.CASE SUMMARY A 39-year woman with an 11-year history of systemic sclerosis was referred toour hospital due to three syncopal episodes in the past 6 mo.The results of anelectrocardiogram and a transthoracic echocardiogram revealed ventriculartachycardia and left ventricular systolic and ventricular septum segmentalmotion abnormalities,respectively.The results of an electrocardiogram showed asinus rhythm with complete blockage of the left bundle branch.In light of theprogressive nature of systemic sclerosis,the presence of a left bundle branchblock,and the decreased ejection fraction,a cardiac resynchronization therapydefibrillatorwas implanted.The patient’s clinical conditions improved,and at the3-mo follow-up,the patient was free of ventricular tachycardia and all cardiacsymptoms.CONCLUSION We REPORT the first case of systemic sclerosis complicated by ventriculartachycardia that was successfully treated with a cardiac resynchronizationtherapy-defibrillator.

BACKGROUND Pancreatic acinar cell carcinoma (PACC) is a rare type of malignant pancreatic cancer that represents approximately 1%of all pancreatic neoplasms.Due to its very low incidence,only a few retrospective studies are available.Although surgery is the first choice for treatment,most patients experience recurrence(mainly in the liver) and there are no clear recommendations for patients with advanced disease.CASE SUMMARY We REPORT two patients with PACC treated with sturgery who experienced tumour recurrence in the liver.Patient 1 carried a germline mutation in the APC gene.Both patients were treated with gemcitabine plus oxaliplatin and gemcitabine plus capecitabine as first-and second-line therapies,respectively.After a favoturable response to chemotherapy,the patients underwent radiofrequency ablation of the remaining liver metastases.For patient 1,we documented a relapse in the liver after a disease-free period of 9 mo,and treatment with gemcitabine plus capecitabine was restarted.The patient achieved a complete response,and he remains alive without evidence of disease recurrence after six years.After radiofrequency ablation,patient 2 experienced disease-free survival for 21 mo,when peritoneal relapse was diagnosed and treated with chemotherapy.The patient achieved a stable disease state for nearly two years;nevertheless,further progressive disease was documented,and he died seven years after the first relapse.CONCLUSION PACC presents different biological behaviours than pancreatic adenocarcinoma.Multidisciplinary treatment involving local ablative therapies may be considered for PACC.

BACKGROUND Bruxism is a jaw-muscle activity characterized by the clenching or grinding of teeth. It can be divided into nocturnal bruxism and diurnal bruxism(DB). DB secondary to antidepressants is rare and refractory. REPORTs associated with antidepressant-induced DB are mostly anecdotal without long-term follow-up.The effect of drug intervention on antidepressant-induced DB is still contested.We herein REPORT the first case of successful treatment of venlafaxine-induced DB with an occlusal splint.CASE SUMMARY This case REPORT describes detailed 7-year follow-up of a patient with venlafaxineinduced DB treated with an occlusal splint. The patient who complained about involuntary daytime tooth grinding after taking venlafaxine for a period of 4 mo and was diagnosed with venlafaxine-induced DB. Subsequently, an occlusal splint with modified bilateral buccal-pterygoid pads was used to treat his tooth grinding and to protect the dental structures from tooth wearing. The patient REPORTed remission of symptoms after several months of treatment. His grinding activity was gradually and stably controlled after 2 years, with an almost complete recovery from DB after 6 years.CONCLUSION The maxillary buccal-pterygoid splint can be used as a noninvasive approach to treat venlafaxine-induced DB.

BACKGROUND Congenital radioulnar synostosis(CRUS)is a rare deformity of the upper extremity.It is characterized by loss of rotation of the involved forearm and functional limitations in daily activities.No studies on CRUS with osteoporosis have been REPORTed to date,and osteoporosis is usually recognized as an important dimension of genetic disorder in children.We discuss the possible relationship among this disorder,osteoporosis and fracture nonunion,investigate the strict surgical indications and recommended treatments.CASE SUMMARY A 14-year-old male patient with bilateral CRUS with osteoporosis,fragility fracture and nonunion of fractures in ulna and radius presented our institution for further treatment,complaining of limitation in rotation.The bone mineral density of the hip and lumbar spine was 0.687 g/cm2 and 0.705 g/cm2,respectively,and the Z-score for both was-2.1,which revealed osteoporosis and a high risk of fracture.Tow serum bone turnover markers indicated an imbalance of bone metabolism.Reoperation for ulna fracture with autogenous bone grafting and a postoperative physiotherapy program were adopted rather than the separation of pathological synostosis.Radiological examination,observational posture assessment and limb function scale were evaluated before and 1 year after surgery.At 1 year,the fracture nonunion had almost recovered,forearm movement function on the fracture side was restored,and function on the healthy side was significantly improved compared with that before rehabilitation.CONCLUSION Surgical indications for CRUS vary from person to person.Surgery should not be the first choice of treatment,and physiotherapy is not inferior to surgical treatment.

BACKGROUND Antiviral drugs are widely used in populations with viral infection caused by immunologic inadequacy.Because these drugs are mainly metabolized by the kidneys,patients with renal failure undergoing renal replacement therapy are prone to drug adverse effects and poisoning.Severe neurotoxicity caused by antiviral drugs is a rare but life-threatening complication.CASE SUMMARY This study REPORTed one male patient on peritoneal dialysis who suffered from severe mental disorders after receiving an overdose of acyclovir and valacyclovir for the treatment of herpes zoster.The literature review suggested that hemodialysis is better than peritoneal dialysis to clear acyclovir from the circulation.The patient died after his consciousness deteriorated despite peritoneal dialysis and continuous blood purification.CONCLUSION This case emphasizes cautiousness when using anti-retroviral drugs in patients with uremia.Hemodialysis is optimal method to remove the drugs.

BACKGROUND Pancreatic arteriovenous malformation(AVM)is a rare disease with a number of different REPORTed treatment methods,but there are as yet no established or definite treatments for the disease.CASE SUMMARY A 43-year-old man visited the hospital due to periumbilical pain.The patient underwent imaging study and laboratory testing for evaluation of cause.Pancreatic AVM associated with pancreatitis was suspected on computed tomography and magnetic resonance imaging.The patient was diagnosed with pancreatic AVM with pancreatitis on imaging study and angiography.Transcatheter arterial embolization with various embolic materials was performed.Follow-up computed tomography scan revealed progressive regression of AVM and improvement of pancreatitis.At two-year follow-up,the patient showed no recurrence of symptom or pancreatitis.CONCLUSION Transcatheter arterial embolization can be considered an effective treatment modality for selective cases of pancreatic AVM.
