Impact of COVID-19 infection during pregnancy on maternal and fetal outcomes

Objectives:The objective of this study is to assess the impact of coronavirus disease 2019(COVID-19)infection during pregnancy on maternal and fetal outcome in relation to gestational age.Methods:This retrospective study was conducted between May 2020 and July 2021.Sixty-five women infected with severe acute respiratory syndrome coronavirus 2(SARS-CoV-2)and hospitalized at a quarantine hospital were included.Thirty-four women received prenatal care at the hospital until delivery,while 31 dropped out from the study due to COVID-19 recovery and discharge before delivery.Results:COVID-19 was found in 83%of the pregnant women after 20 weeks of pregnancy.The most common symptoms were cough and fever.Medical complications included severe pneumonia and thrombosis.The maternal mortality rate was 9.2%.Premature rupture of membranes and severe hypertension during labor affected nearly 9%of the pregnant women who completed prenatal care until delivery at the hospital.Preterm labor occurred at a rate of 38.2%.There were seven(20.8%)stillbirths and two cases of infant mortality.Conclusion:Hospitalized pregnant women who have coronavirus infection,lymphopenia,and a high C-reactive protein level were at a higher risk of developing severe illness,which can lead to maternal and neonatal complications.

Social distancing(SD)to prevent SARS-CoV-2(COVID-19)transmission reduced practices of physical activities(PA)and changed emotional conditions,but the relationship between PA and the emotions has to be further studied.This study aimed to analyze PA and the intensity of basic emotions during a period of social distancing in the State of S~ao Paulo,Brazil.Data collection was performed using the snowball system and an online survey with questions about the minimum weekly frequency of 30-min moderate and/or vigorous activities(MVPA),intensity of basic emotions,and sociodemographic profile.A total of 704 volunteers participated;mean age 38.113.4 years,67.9%female,82.0%insufficiently practiced MVPA,and 37.9%of the group reported no weekly PA.Anxiety/fear was the emotion with the highest intensity in the study period.A significant association was observed between PA and intensity of happiness(rho=0.125;p<0.01)and a negative association between PA practice and intensity of anxiety/fear(rho=-0.090;p<0.05),sadness(rho=-0.134;p<0.01),and anger(rho?0.109;p<0.01).Also,an association was observed between anxiety/fear and social isolation(rho=0.082;p<0.05).These results suggest that public policies offering PA programs and psychological care are required to improve the quality of life of the population.

Global health concerns about the health of the global population and the concepts of global health are gradually being accepted by all of the countries in the world.China health and quarantine at points of entry(POEs) is an integral part of global health,and its duty of prevention and control of the communicable diseases in POEs is consistent with the duty of global health to promote the health of global population.Under the principle of "prevention first",health and quarantine organ carries out the work of disease prevention and control and international travel health care,the reinforcement of core capacity at POEs,and focuses on human rights protection.All the same,there are some gaps with the requirements of global health,such as the inadequate of health promotion planning capacity,ethics awareness and legal protection.The future work of health and quarantine should change ideas on communicable diseases prevention at POEs,focus more on ethical consideration,and should optimize system architecture and human resources,strengthen cooperation,maintain core capacity at POEs,thus to build points of entry public health system with Chinese characteristics,meanwhile to apply the concepts of global health deeply into every aspects of health and quarantine at POEs.

Severe acute respiratory syndrome coronavirus 2(SARS-CoV-2)infection has affected the entire world,causing the coronavirus disease 2019(COVID-19)pandemic since it was first discovered in Wuhan,China in December 2019.Among the clinical presentation of the disease,in addition to fever,fatigue,cough,dyspnea,diarrhea,nausea,vomiting,and abdominal pain,infected patients may also experience neurological and psychiatric repercussions during the course of the disease and as a post-COVID-19 sequelae.Thus,headache,dizziness,olfactory and gustatory dysfunction,cerebrovascular disorders,neuromuscular abnormalities,anxiety,depression,and post-traumatic stress disorder can occur both from the infection itself and from social distancing and quarantine.According to current evidence about this infection,the virus has the ability to infect the central nervous system(CNS)via angiotensin-converting enzyme 2(ACE2)receptors on host cells.Several studies have shown the presence of ACE2 in nerve cells and nasal mucosa,as well as transmembrane serine protease 2,key points for interaction with the viral Spike glycoprotein and entry into the CNS,being olfactory tract and blood-brain barrier,through hematogenous dissemination,potential pathways.Thus,the presence of SARS-CoV-2 in the CNS supports the development of neuropsychiatric symptoms.The management of these manifestations seems more complex,given that the dense parenchyma and impermeability of brain tissue,despite protecting the brain from the infectious process,may hinder virus elimination.Still,some alternatives used in non-COVID-19 situations may lead to worse prognosis of acute respiratory syndrome,requiring caution.Therefore,the aim of this review is to bring more current points related to this infection in the CNS,as well as the repercussions of the isolation involved by the pandemic and to present perspectives on interventions in this scenario.

The outbreak of coronavirus disease 2019(COVID-19)has aroused a global alert.To release social panic and guide future schedules,this article proposes a novel mathematical model,the Delay Differential Epidemic Analyzer(D2EA),to analyze the dynamics of epidemic and forecast its future trends.Based on the traditional Susceptible-Exposed-Infectious-Recovered(SEIR)model,the D2EA model innovatively introduces a set of quarantine states and applies both ordinary differential equations and delay differential equations to describe the transition between two states.Potential variations of practical factors are further considered to reveal the true epidemic picture.In the experiment part,we use the D^2EA model to simulate the epidemic in Hubei Province.Fitting to the collected real data as non-linear optimization,the D^2EA model forecasts that the accumulated confirmed infected cases in Hubei Province will reach the peak at the end of February and then steady down.We also evaluate the effectiveness of the quarantine measures and schedule the date to reopen Hubei Province.

The outbreak of coronavirus disease 2019(COVID-19)has caused more than 80813 confirmed cases in all provinces of China,and 21110 cases reported in 93 countries of six continents as of 7 March 2020 since middle December 2019.Due to biological nature of the novel coronavirus,named severe acute respiratory syndrome coronavirus 2(SARS-CoV-2)with faster spreading and unknown transmission pattern,it makes us in a difficulty position to contain the disease transmission globally.To date,we have found it is one of the greatest challenges to human beings in fighting against COVID-19 in the history,because SARS-CoV-2 is different from SARS-CoV and MERS-CoV in terms of biological features and transmissibility,and also found the containment strategies including the non-pharmaceutical public health measures implemented in China are effective and successful.In order to prevent a potential pandemic-level outbreak of COVID-19,we,as a community of shared future for mankind,recommend for all international leaders to support preparedness in low and middle income countries especially,take strong global interventions by using old approaches or new tools,mobilize global resources to equip hospital facilities and supplies to protect noisome infections and to provide personal protective tools such as facemask to general population,and quickly initiate research projects on drug and vaccine development.We also recommend for the international community to develop better coordination,cooperation,and strong solidarity in the joint efforts of fighting against COVID-19 spreading recommended by the joint mission report of the WHO-China experts,against violating the International Health Regulation(WHO,2005),and against stigmatization,in order to eventually win the battle against our common enemy—COVID-19.

The ongoing coronavirus disease 2019(COVID-19)pandemic is a global public health crisis,causing social and economic disasters in many countries.In China,two-consecutive negative results of nucleic acid tests for SARS-CoV-2 from the respiratory samples are required to end the quarantine of COVID-19 patients.However,clinicians face a dilemma in case of patients with long-term viral shedding.This report described an unusual COVID-19 case who had persistent viral RNA positivity for more than 4 months after initial illness in the presence of low neutralizing antibodies,but without prolonged clinical symptoms.Multiple anti-viral drug treatments had no impact and there was no evidence of re-infection.When the patient was self-quarantined at home,no infection occurred to the three family members living with her for 15 to 19 days.Sputum viral culture in BSL-3 laboratory on the 102nd day after symptom onset was negative.From the 129th day on,8 continuous nucleic acid tests of sputum samples showed negative results.The patient was discharged on 137th days since symptom onset.In conclusion,viral RNA shedding in the sputum of the COVID-19 patient may last over 4 months.As no evidence shows the existence of infectious virus,two-consecutive negative nucleic acid tests may not be the prerequisite for ending quarantine of COVID-19 patients with prolonged viral shedding.
