Acceptance of American society for gastrointestinal endoscopy’s guidelines for endoscopy reports

Background: Several studies indicate that endoscopy reports lack uniform content and terminology. Thus ASGE, ESGE, and WEO have prepared guidelines to improve the quality of endoscopy reports. However, the acceptance of such recommendations in the community of endoscopists has not been assessed. Objective: The aim of the present study was to determine Non-US endoscopists’ agreement to current ASGE guidelines for endoscopy reporting and ESGE recommendations for image documentation. Design: 137 endoscopists were invited to participate in this internet survey, covering 34 items regarding the content of the endoscopy reports. Non-responders received three e-mail reminders before the study was closed. Settings: A web-based survey tool developed at the University of Oslo was used to perform this study (https://***/as/WebObjects/***). Results: Eighty (60%) of the 137 endoscopists responded, to the survey. Their agreement to the various items of the ASGE guidelines for text content ranged from 21% - 100%. Only 8.9% (95% CI, 4.4% - 17.2%) considered it necessary to perform routine image documentation according to the ESGE guidelines. Limitations: The response rate of 68%, is on the lower limit of acceptable. Conclusions: The cohort of endoscopists agrees partially to the ASGE guidelines. However, they do not consider systematic image documentation

In this paper we derive the optimal link quality predictor (LQPR) whose parameters are estimated from signal power and node speed samples. We propose a fast estimator for these parameters whose computational complexity is three orders lower than that of the optimal estimator with only a slight loss in accuracy thus enabling real- time execution. We show that using the most recent local mean of the signal as a predictor of future signal strength is also a very close approximation to the optimal predictor. This is the central result of this paper. It obviates the need for complex and/or computationally intensive link quality predictors for 802.11 in urban microcells and has the advantage of not requiring node speed information. The LQPRs are evaluated against the lower error bound. We show that the LQPR based on the most recent local mean of the signal predicts the packet reception probability for pedestrians in urban microcells on average with a mean absolute error of 13.47%, 16.54%, 18.21% and 19.38% for 1 s, 2 s, 3 s and 4 s into the future respectively. This LQP accuracy resembles closely the lower error bound with, for example, a difference of only 2.47% at 2 s into the future.

Background: An essential condition to improve patient safety is considered to ensure a supportive patient safety culture. Measuring the culture of patient safety in all health care institutions may be a first step to target improvements. Creating a culture of safety requires eliminating the culture of blame. In order to formulate actions for improvement, it is important for hospitals to assess their baseline scores for the existing safety culture and to determine the areas of priority. Aim: The aim of this study was first to measure the use, translation in Albanian and adaptation of the Hospital Survey on Patient Safety Culture (HSOPSC) assessment as a tool for improving patient safety in Kosovo Hospitals. The second aim was to measure the level of patient safety culture in Kosovo, in seven hospitals and one University Clinical Center (hospitals with over 50 beds, including psychiatric hospitals). Method: The questionnaire (HSOPSC) was translated into Albanian for use in the Kosovo. It was used forward-backward translation: the questions were translated into Albanian by one translator and then translated back into English by an independent translator who was blinded to the original questionnaire. Results: In the eight-factor model, the internal consistency of the factors and the construct validity of the HSOPSC questionnaire were mostly satisfactory. The construct validity was sufficient for all subscales, except for the 4 other subscale regarding intention to report incidents which correlated poorly with other subscales. In total, HSOPSC has 12 dimensions. Cronbach’s α showed that in Kosovarian society, we could use only 8 dimensions model. Conclusion: The hypothesis that HSOPSC would be a suitable instrument to provide important indicators for the improvement of patient safety culture was tested and it was confirmed, that HSOPSC could be used as 8 dimension model. In Democratic Republic of the Congo (DRC), the laboratory TAT is significantly very long and do not comply with either international standards or the suggestions of customers. However, there is neither a national nor a local strategy to improve the laboratory TAT. The aim of the present study is to develop practical management strategies to shorten clinical laboratory tests’ TAT. This was a qualitative study conducted in Kinshasa. Focus groups and Lean tools were used respectively to generate a wide range of views from a variety of laboratory staff and to eliminate several form of waste in the laboratory flow process. Based on the identified root causes of delay, focus groups participants reported that there is a lot of scope for the improvement of TAT in DRC. Consistent attendance and punctuality are essential. The hospital management should implement the Laboratory Information Systems (LIS) and install Middleware. Total laboratory automation, inventory system for all reagents and supplies used in the laboratory, expansion of the sampling area, sufficient number of high-power machine and a clear job description are indispensable. LIS, 3.5 mL BD vacutainer BarricorTM tube and point-of-care testing (POCT) are necessary for workflow improvement. A reduction of 312 minutes was achieved by eliminating or decreasing non-value-added activities. Applying the suggested key strategies, and particularly the new workflow process, is a basis for improving the laboratory tests’ TAT. The algorithm presented can be easily implemented in other laboratories that face this type of problem.

The formation of glycohemoglobin, especially the hemoglobin A1c(Hb_(A1c)) fraction, occurs when glucose becomes coupled with the amino acid valine in the β-chain of Hb; this reaction is dependent on the plasma concentration of glucose. Since the early 1970 s it has been known that diabetics display higher values of Hb_(A1c) because they have elevated blood glucose concentrations. Thus Hb_(A1c) has acquired a very important role in the treatment and diagnosis of diabetes mellitus. After the introduction of the first quantitative measurement of Hb_(A1c), numerous methods for glycohemoglobin have been introduced with different assay principles: From a simple minicolumn technique to the very accurate automated highpressure chromatography and lastly to many automated immunochemical or enzymatic assays. In early days, the results of the quality control reports for Hb_(A1c) varied extensively between laboratories, therefore in United States and Canada working groups(WG) of the Diabetes Controls and Complications Trial(DCCT) were set up to standardize the Hb_(A1c) assays against the DCCT/National Glycohemoglobin Standardization Program reference method based on liquid chromatography. In the 1990 s, the International Federation of Clinical Chemistry and Laboratory Medicine(IFCC) appointed a new WG to plan a reference preparation and method for the Hb_(A1c) measurement. When the reference procedureswere established, in 2004 IFCC recommended that all manufacturers for equipment used in Hb_(A1c) assays should calibrate their methods to their proposals. This led to an improvement in the coefficient of variation(CV%) associated with the assay. In this review, we describe the glycation of Hb, methods, standardization of the Hb_(A1c) assays, analytical problems, problems with the units in which Hb_(A1c) values are expressed, reference values, quality control aspects, target requirements for Hb_(A1c), and the relationship of the plasma glucose values to Hb_(A1c) concentrations. [Objectives]To determine the aerobic plate count(APC)in the milk samples,evaluate the uncertainty of the test results,and to provide a scientific basis for the quality control of the testing process.[Methods]In compliance with the national food safety standard Food Microbiological Examination:Aerobic Plate Count(GB 4789.2-2016),the aerobic plate count in the milk samples was *** source of the uncertainty of the test result was analyzed and a mathematical model was established in accordance with Evaluation and Expression of Uncertainty in Measurement(JJF 1059.1-2012).Then,the introduced uncertainty components were evaluated to determine the uncertainty of the final combined aerobic plate count.[Results]The expanded uncertainty of the test result of the aerobic plate count in the milk samples was 0.0434,and the logarithmic value interval of the results was(3.924,4.010),and the antilogarithm was taken to get the aerobic plate count in the sample to be 8395-10233 CFU/mL.[Conclusions]This method can effectively evaluate the uncertainty of the aerobic plate count,and ensure the accurate and scientific laboratory test data.

The article deals with special questions of some aspects of food safety and application of methods of food physics in food processing and also in food quality control and quality assurance. The role of food safety has developed significantly in the last 2 decades, so today the production and processing of safe and quality food is of primary importance. Modem food production is based on principles of GAP and GMP （including GHP） and different methods and tools for quality control and assurance （IFS, HACCP, ISO-9000, ISO-9000/2010, ISO-22000, TQM）. There are different techniques and methods to produce safe food and to control the quality of the foodstuffs. The modem, up-to-date technologies and measurement techniques involve the application of many physical methods - high pressure, pulsing electrical field, microwave, non-destructive techniques （e.g., INAA, NMR, NIR-NIT, PAS）, radiation treatments as well. Using radiation technologies （of course not only nuclear ones） it is possible to fulfil some very important expectations of modem food processing, e.g., decrease of microbial contamination, improvement of sensory properties, increase of storability of food products.

BACKGROUND:The Joint Commission accreditation manual contains standards in improving organization performance related to report and review of patient care issues causing unexpected *** spite of regulations mandating reporting,it remains inconsistent,varying by provider type and *** purpose was to determine current attitudes,knowledge,and practice of error reporting among emergency department(ED) ***:We administered a survey assessing ED staff practice regarding error *** involved reporting of errors in which the practitioner was directly involved,errors the practitioner observed,and general awareness of reporting *** also questioned individuals regarding fear of repercussions for ***:Fifty-two surveys were *** most errors,providers were more likely to tell their supervisor about the issue than to tell the *** percent of respondents did not think that referring errors for review was their *** 31%of respondents were aware of standardized institution-wide pathways to report *** respondent who was aware of the institution-wide pathway also felt responsibility for error ***-three percent of the respondents were concerned about negative repercussions from reporting *** querying the hospital reporting system,263 cases were referred for quality issues over the previous year,51%of them were referred by nurses,27%by medical technicians(MTs),2%by mid-level providers(MLPs),1%by physicians,and 19%by other ***:Although most of the ED staff are responsible for patient safety,most are not aware of systems available to assist in reporting,and even many do not utilize those systems.
